Relation between methods of determining histological type and Lymph Node Metastasis of colorectal carcinoma with submucosal invasion
新潟大学Niigata University博士(医学)本邦の大腸癌治療ガイドラインや米国の National Comprehesive Cancer Network (NCCN )ガイドラインでは,内視鏡で完全切除された大腸粘膜下層浸潤癌 (以下,大腸pT1(SM)癌)のリンパ節郭清を伴う追加腸切除の必要性を決定する因子の一つとして,癌の組織型(以下,組織型)が挙げられている.本研究ではリンパ節郭清がなされた外科切除大腸pTl (SM)癌466例を対象に,組織型を大腸癌取扱い規約第9版に従い面積的に最も優勢な組織型で評価する「主組織型」,WHO分類第4版に基づき癌全体に占める腺管形成割合により分化度を決定する 「histological grading」, 粘膜下層浸潤部(以下,SM浸潤部)において対物40倍1視野以上の領域性をもって存在する低分化腺癌•印環細胞癌・粘液癌の有無を評価する 「SM浸潤部最低分化度」,3つの評価法で判定し,いずれがリンパ節転移予測因子として最も有用か を検討した.その他の臨床病理学的因子としで性別,年齢,病変部位,腫瘍径,癌の肉眼型,SM浸潤距離,脈管侵襲(リンパ管侵襲・静脈侵襲),族出を評価した.単変量解析では SM浸潤距離,リンパ管侵襲,静脈侵襲,族出,histological grading (grade 2/3), SM浸潤部最低分化度(低分化腺癌成分の存在),SM浸潤部最低分化度(低分化腺癌•印環細胞癌・粘液癌いずれかの存在)がリンパ節転移と有意に相関していた.多変量解析では,SM浸潤部最低分化度(低分化腺癌成分の存在),リンパ管侵襲,族出が独立したリンパ節転移リスク因子であった.以上のことからSM浸潤部最低分化度(低分化腺癌成分の存在)が主組織型やhistological gradingよりもリンパ節転移のリスク因子として意義の大きい組織型である可能性が示唆された.新大院博(医)第1243

Osteoporosis significantly predisposes patients to fragility fractures and a reduced quality of life. Therefore, osteoporosis prevention plays an important role in extending healthy life expectancy. The purpose of this study was to identify whether physical functional status was associated with low bone mineral density, and to determine cut-off values of physical status indicators for osteoporosis.
Methods
This cross-sectional study evaluated 343 women aged 60 years or older who were able to walk independently. The measured variables were the body mass index, lumbar and total hip bone mineral density, grip strength, 5-m normal walking speed, one-leg standing time, timed up-and-go test, and skeletal muscle mass using bioelectrical impedance analysis. The associations between physical status indicators and low bone mineral density were analyzed and the cut-off values for detecting osteoporosis were calculated using receiver operating characteristic curve analyses.
Results
The prevalence of osteoporosis was 29.2 %. All measured variables significantly differed between the osteoporotic and non-osteoporotic groups (p < 0.05). Multivariate logistic regression analysis showed that the factors associated with osteoporosis were the skeletal muscle mass index, walking speed, and body mass index. In the receiver operating characteristic curve analysis, the cut-off values of the skeletal muscle mass index, walking speed, and body mass index associated with osteoporosis were 6.31 kg/m2, 1.29 m/s, and 22.6 kg/m2, respectively.
Conclusions
Older women with low bone mineral density have lower skeletal muscle mass, slower walking speed, and lower body mass index. ヒト胎盤抽出物はマクロファージと老化細胞の調節を介してマウスの肝硬変を改善する
新潟大学博士(医学)Introduction: Cirrhosis is a disease with poor prognosis that requires the development of a novel therapeutic approach alternative to liver transplantation. In this study, we focused on the placenta and aimed to clarify the effects of human placental extract (HPE) on cirrhosis.
Methods: A mouse model of carbon tetrachloride-induced cirrhosis was used to evaluate the effect of HPE administration subcutaneously and compared with the control group (n ¼ 8 for each group). In vitro and in vivo, real time-PCR and immunostaining were performed for HPE mechanistic analysis. Spatial transcriptomics was also performed for detailed analysis of the effect of HPE on cirrhosis.
Results: HPE administration improved serum ALT levels compared to control mice. Furthermore, there was a decrease in the number of senescent cells in the liver and the mRNA levels of secrete senescenceassociated secretory phenotype factors and Cdkn2a (p16). In vitro, HPE induced macrophage polarization to the anti-inflammatory M2 phenotype. Spatial transcriptomics was also performed to analyze the underlying anti-inflammatory mechanism. The results showed that HPE strongly polarized macrophages to the M2 phenotype, especially in macrophage-rich regions in the liver. Gene expression pathway analysis using spatial transcriptomics also revealed the possibility of improving senescent cell-derived inflammation via mitochondrial function.
Conclusions: HPE improves serum ALT levels via anti-inflammatory mechanisms in macrophages and senescent cells. HPE serves as a novel agent for cirrhosis treatment.
