    Get PDF
Practice Problem: Within a 12-month period, high incidences of severe post-spinal surgical complications resulted in a host of concerns in this clinical site when patients either returned to the operating room for spine revision or increased the length of stay. PICOT: The PICOT question that guided this project was: In Medical Surgical (Med-Surg) nurses (P), how would a Competency-Based Orientation (CBO) protocol with the inclusions of four critical elements of postoperative spinal care [POSC] (I) compared to the usual postoperative practice (C) enhance competency level in Med-Surg nurses when delivering postoperative practice at an inpatient setting (O) over 10 weeks (T)?” Evidence: An extensive evidence literature review supported organizational reform in competence management and improves patient outcome with the development and implementation of a CBO assessment tool highlighting critical components on POSC. Intervention: The intervention was the implementation of POSC CBO assessment tool in an inpatient hospital setting. National Association of Orthopaedic Nurses guided the CBO assessment tool to focus on four crucial elements on POSC. Outcome: Statistical significance reflected the positive impact demonstrated in improved patient recovery in clinical practice after the implementation of a POSC CBO assessment tool compared to previous standard of post-surgical care in the Med-Surg Unit. The clinically significant findings were a reduction of post-spinal complication and improvement in patient outcomes. Conclusion: CBO assessment tool was designed to restructure the clinical site’s previous standardized nursing care in POSC. With a CBO assessment tool to clearly outline the nurses’ functional role to safely and competently deliver POSC, the rate of post-spinal complications significantly reduced and patient outcome improved nearly 90%

Clinical Practice Implementation to Address ASCVD Risk: A Practice Change in Primary Care

    Get PDF
Practice Problem: Heart disease stands as the leading cause of mortality in the United States. While healthcare providers strive to identify and optimize prevention strategies, particularly in high-risk patient populations, notable gaps in care persist, notably in the management of modifiable risk factors such as low-density lipoprotein cholesterol (LDL). By harnessing the power of artificial intelligence (AI) integrated software within clinical settings, we can revolutionize the landscape of this devastating chronic disease. PICOT: The PICOT question that guided this project was: In Primary Care Advanced Practice Providers (APP) caring for high-risk and/or very high-risk patients with atherosclerotic cardiovascular disease (ASCVD) (P), how do automated electronic alerts with guideline-based recommendations (I) compare to standard notification practice (C) affect referral initiation to cardiology or prompt medication change (O) within 10 weeks (T)? Evidence: In the realm of modern healthcare, it is crucial to recognize the impact of AI on Electronic Health Records (EHRs). This fusion of data analysis and health information technology provides an opportunity for healthcare treatments to become much more effective, resulting in better patient outcomes. Fifteen studies that matched the inclusion criteria were collected and used as substantiating evidence for this project. Intervention: AI software integrated into the EHR system computed comprehensive data analytics, consequently discovering a substantial cohort of patients with an elevated risk profile for ASCVD, accompanied by an LDL-C level that exceeded established clinical guidelines. Subsequently, an automated communication was sent to the APP, furnishing them with pertinent notifications and offering referral recommendations. Outcome: By integrating AI processes into the EHR, data management is streamlined and real-time disease prevention analysis is achieved. The primary goal was to identify high-risk ASCVD patient groups using AI within the EHR and assess the effectiveness of AI-generated electronic alerts with clinical guidance in encouraging behavior change. The clinical significance of this data collection and implementation was substantial. While the statistical analysis produced relevant metrics, it also exhibited applicability in the clinical context. The data exposed a patient population lacking aggressive medical management or referrals, a concern noted by APPs. Conclusion: Introducing AI-based tools can direct the pathway of care and bridge crucial gaps in care in high-risk populations. The result of this technology utilization and integration offers timely screening strategies, education, clinical decision support, and opportunities to address vital pathways for providers and health systems to address ASCVD treatment gaps

The Use of Alcohol Impregnated Port Catheters for Decreasing Central Line Infections in Acute Care

    Get PDF
Practice Problem: Lack of proper sterile protocol for central lines was noted as a problem in the acute care units, causing CLASBI rates to be higher than the national average at the hospital in question. PICOT: In hospitalized acute care adult patients 18 years of age and older with central lines, how does the use of alcohol impregnated port protectors (AIPPs) compared to current central line sanitation practices affect central-line associated infections (CLASBI) within 10 weeks? Evidence: The literature evidence suggests that Alcohol Impregnated Port Protectors (AIPPs) decrease CLASBI rates and provide better outcomes for acute care patients with central lines. Alcohol Impregnated Port Protector (AIPPs) caps disinfect in one minute and provide a physical barrier to contamination and last up to seven days on central lines. Intervention: The Evidence-Based Practice Project established use of Alcohol Impregnated Port Protectors (AIPPs) vs standard of care for central lines (70% isopropyl alcohol prep pads on the central lines for an undefined period) on all patients in the acute care units with central lines (Emergency Room and Intensive Care) for 10 weeks. The use of direct observation and return demonstration was used to measure compliance. Weekly CLASBI rates from Infection Control were used to track the on-going rates during implementation. Outcome: Overall, this Evidence-Based Practice Project was clinically significant as it was able incorporate the use AIPPs vs lack of proper sterile protocol for central lines. The statistical data showed a decrease in CLASBI rates (8.02%) in as little as ten weeks. Conclusion: Central Line Infections are a great challenge in the healthcare system and AIPPs are a good way of curtailing those challenges. AIPPs promote evidence-based practice and uphold the safety standards by reducing CLASBI rates which ultimately reduce morbidity and mortality in acute care patients

Advocating and Educating the Role of Occupational Therapy for Adaptive Swim Lessons Among Children with Autism

    Get PDF
Drowning is the number one cause of death for children ages 1-4 and the second leading cause of death for children ages 5-14 (Centers for Disease Control and Prevention [CDC], 2022). Children with autism are 160x more likely to drown than their neurotypical peers (Autism Society of Florida, n.d.). Many parents and caregivers put protective layers in place such as locks, gates, and fencing. However, children with autism are known to wander from safe spaces. According to the Autism Society of Florida (n.d.) children with autism are naturally drawn to water due to its ability to ease sensory needs. Brenner et al. (2009) state that swim lessons can reduce drownings by 88%. Purpose: The purpose of this capstone project was to promote and educate individuals on adaptive swim programs from the perspective of occupational therapy. This was accomplished by creating a comprehensive resource to address enrollment, layered protection, and financial burdens.https://soar.usa.edu/otdcapstonesfall2023/1017/thumbnail.jp

    Improving Access to Cardiac Catheterization

    Get PDF
The American Heart Association lists cardiovascular disease as the underlying cause of death for 874,613 people who lost their lives in 2019 (Tsao et al., 2022). On average, someone in the United States has a myocardial infarction every 40 seconds, and someone dies of a stroke every 3 minutes and 30 seconds. Studies support coronary artery disease as a significant contributor to cardiac arrest in a non-hospital setting (Yannopoulos et al., 2019). The authors provide evidence supporting the importance of early cardiac catheterization. There is a 10-15% survival rate with a higher level of functioning compared to delayed or no access to a cardiac catheterization lab

Hypertension Education to Enhance Health Literacy

    Get PDF
Practice Problem: The health of patients suffers greatly when their health literacy is not addressed through evidence-based education material. PICOT: The PICOT question that guided this project was, in adult patients with hypertension (P), how does evidence-based education material (I) compared to the current state of verbal instructions provided (C) affect health literacy rates based on the high blood pressure health literacy scale (O) within 12-weeks (T)? Evidence: The literature evidence revealed three strong themes, which included information on who is most at risk for low health literacy levels, education delivery methods, and specific education to be used. Intervention: This virtual intervention included a hypertension education format from the American Heart Association in both English and Spanish, when appropriate. The educational intervention was completed by the director at the clinic. Outcome: The results demonstrated a change in health literacy following education delivery. The two-tailed Wilcoxon signed rank test was not significant based on an alpha value of .05, V = 0.00, z = -1.83, p = .068 for the full high blood pressure-health literacy assessment for the four participants; however, the clinical significance was meaningful. Conclusion: The project was conducted in a free clinic for those who are marginalized. Four patients qualified based on the inclusion criteria, and each participant was given the health literacy assessment before and after receiving the evidence-based education material specific to hypertension
