Feasibility of Using Standardized Outcome Measures in the Remote Assessment of Patients with Chronic Stroke: A Case Series
Get PDFIntroduction: With the increase of telerehabilitation utilization worldwide after the COVID-19 pandemic, a review of the current literature shows a concurrent increase in telerehabilitation studies focusing on interventions and delivery models. There is a gap in research on valid and reliable tests and measures that can be used in the physical therapy examination of people with stroke via telerehabilitation. Current reports on psychometric properties of most standardized outcome measures are only applicable in an in-person setting and may not be directly applicable in a remote setting where various challenges contribute to the reliability of results. This report describes the feasibility of administering selected standardized outcome measures in the remote assessment of three patients with chronic stroke with different contexts.
Methods: Three patients with chronic stroke underwent physical therapy examination and intervention sessions via telerehabilitation with varying contexts.
Results: Various factors affected the results of the assessment including patient and caregiver’s (if present) ability to follow instructions, availability of material and spatial resources, and stability of internet connection. Strategies to improve reliability of assessment results can also be employed such as performing repeat trials, reviewing recordings for accuracy and confirmation, using highly visible markers, and confirming measurement accuracy on video.
Discussion: Physical therapy outcome measures that can feasibly be administered remotely are the Timed Up and Go Test, the 10-Meter Walk Test, the 6-Minute Walk Test, the 5-Times Sit to Stand Test, and the Stroke Impact Scale. The Mini Balance Evaluation Systems Test could not be administered remotely as close to standard protocol as possible due to the difficulty of administering the reactive postural component with patient anxiety and safety considerations and materials required

Occupational Therapy’s Opportunity to Impact Care at the End of Life
Get PDFNavigating and managing the end of life can be difficult. The focus of occupational therapy in care at the end of life is to aid the patient and caregiver through these challenges by facilitating improved safety, comfort, and quality of life through the participation in meaningful occupations. Despite the ways that occupational therapy benefits these patients, occupational therapy involvement in care at the end of life is misunderstood, under-researched, and underutilized. There is a general lack of awareness regarding this topic and a stigma that occupational therapy focuses only on rehabilitation and therefore does not belong in care at the end of life. To combat these issues, there needs to be additional education and training for all healthcare professionals, including occupational therapy practitioners, about occupational therapy’s role in care at the end of life. Occupational therapy professionals in this area of practice must strive to educate others and advocate for their role to improve the understanding of this practice area and thus positively impact more patients and families
A Guidebook for The Utility of MSK Ultrasound in Occupational Therapy Practice: An Educational Guidebook Series
Get PDFMusculoskeletal (MSK) ultrasound is expanding to non-physician rehabilitation providers, and occupational therapy has a clear role in using and facilitating MSK ultrasound that will drastically shape treatment plans and client satisfaction. With increased knowledge of impeding factors, occupational therapists can enforce their role by teaching new skills and techniques to enhance functional mobility and simultaneously assist in completing activities of daily living (ADLs). This capstone aims to understand, develop, and enhance the role of occupational therapy in MSK ultrasound to increase early detection of orthopedic conditions impacting ADLs, which will decrease hospital readmission rates. MSK ultrasound is a safe imaging modality with high sensitivity and specificity for detecting traumatic, inflammatory, and degenerative soft tissue MSK conditions but is operator-dependent (Warning et al., 2021). Occupational Therapy for People With Dementia who Have Occupational Performance Challenges due to Depression
Get PDFIndividuals with dementia experience a multitude of barriers and challenges that impact their daily engagement throughout the environment. In addition, although the pandemic has concluded, noxious effects are still influencing people\u27s lives. Residential care facilities have noticed continual acts of isolation that are presenting as barriers in mental health. Depression has known to be a common comorbidity in the ailment of dementia. Occupational therapy (OT) practitioners have the unique and valuable role in working with this population to target beyond the biomechanical aspects of a resident. Identifying barriers for mastery in leisure pursuits throughout someone’s day is crucial. When residents do not have successful experiences throughout their day they are not living through an encouraging environment to allow for the opportunity of prolonging cognition. Based on the literature review and the capstone experience site OT practitioners show the need for a program consisting of therapeutic activities that target depression in those with dementia. The purpose of the project is to develop an occupation-based OT program to treat people with dementia who had occupational performance challenges due to depression. This program is structured around the Model of Human Occupation (MOHO), Behavioral Activation (BA), and the Allen Cognitive Level Screen (ACLS). The program consists of eight therapeutic activities that are broken down into three different ways of executing, based off the ACLS levels of three through five. Evidence Based Practice Change to Incorporate a Standardized Process Checklist for Fecal Microbiota Therapy (FMT) to Increase Staff Adherence
Get PDFPractice Problem: Staff adherence to new procedures is crucial for ensuring patient safety, maintaining quality care, reducing risks and liabilities, and fostering a positive organizational culture that supports continuous improvement. Having a procedure done safely for patients infected with C.diff provides an overall benefit to the healthcare system.
PICOT: In an endoscopy lab staffed with nurses and surgical technicians (P), does the implementation of a new standardized process of administering FDA approved Fecal Microbiota Transplant Therapy (I) compared to the current process (C), increase staff adherence to the process (O) over 12 weeks(T)?
Evidence: Leadership, supervision, and organizational culture are crucial for promoting adherence to guidelines, while factors such as lack of knowledge, lack of confidence, organizational climate, and forgetfulness contribute to non-adherence. Direct observation proved invaluable in bridging the gap between perceived and actual actions, while workplace culture holds sway over compliance with procedures.
Intervention: Fecal microbiota therapy is recommended for preventing Clostridioides difficile infection recurrence, administered as an enema without sedation or bowel prep, unlike traditional options. A stepwise process was established to ensure proper execution, including creating a checklist to aid endoscopy unit staff who will assist in the procedures.
Outcome: Nurses generally adhered closely to the checklist, with one nurse at 75% adherence. Feedback was provided to improve infusion pace for optimal results, and overall staff adherence exceeded the 70% threshold, with 74% adherence. Even though no statistical significance was calculated, clinically, the checklist was an important tool to utilize when training nurses for the first time or when refreshing someone who hasn’t done a procedure in some time.
Conclusion: This DNP project aimed to establish a standardized process with a checklist for treating CDI, emphasizing the crucial role of nursing staff adherence in delivering safe, effective, and high-quality patient care
Social Participation Program for Older Adults with Dementia Residing in Long Term Care Facilities
Get PDFSocial participation is characterized by activities that involve social interaction with others and supports social interdependence; individuals engage in this occupation through a variety of contexts including with their community, family, friends, and peers (American Occupational Therapy Association, 2020). Social participation has been discovered to influence general health and well-being, cognitive health, risk of dementia, and stress (Sommerlad et al., 2023). Dementia is a prevalent disease affecting approximately 50 million individuals worldwide (Sommerlad et al., 2023) causing increased dependence and disability among the older adult population and is shown to be growing as the aging population increases in number (Wang et al., 2023). The aim of this capstone project is to create a program to promote social participation between older adults with dementia in long term care (LTC) settings to improve occupational engagement and quality of life. Through the implementation of this capstone program, older adults with dementia residing in LTC settings will engage in structured group activities in familiar social contexts to improve their occupational engagement and further improve their quality of life through this experience
