    Die X-chromosomal gebundene Myotubuläre Myopathie (MTM) ist eine seltene neuromuskuläre Erkrankung, die vorwiegend männliche Neugeborene betrifft und durch generalisierte Muskelhypotonie charakterisiert ist. Ziel dieser Arbeit war die Analyse physiotherapeutischer Ansätze bei Kindern mit MTM, um Förderstrategien für die physische, motorische und sozial-emotionale Entwicklung zu identifizieren. Auf Grundlage von neun Experteninterviews mit Physiotherapeuten wurde mittels qualitativer Inhaltsanalyse ein Kategoriensystem entwickelt, das zentrale Themen wie Therapiegestaltung, interdisziplinäre Zusammenarbeit und emotionale Unterstützung der Familien umfasst. Die Ergebnisse zeigen, dass Physiotherapeuten eine Schlüsselrolle in der ganzheitlichen Versorgung einnehmen und durch gezielte Maßnahmen motorische Fähigkeiten fördern sowie die Lebensqualität der Betroffenen verbessern können. Weitere Forschung und Vernetzung sind erforderlich

    Generalizability of clinical prediction models in mental health

    Concerns about the generalizability of machine learning models in mental health arise, partly due to sampling effects and data disparities between research cohorts and real-world populations. We aimed to investigate whether a machine learning model trained solely on easily accessible and low-cost clinical data can predict depressive symptom severity in unseen, independent datasets from various research and real-world clinical contexts. This observational multi-cohort study included 3021 participants (62.03% females, MAge = 36.27 years, range 15–81) from ten European research and clinical settings, all diagnosed with an affective disorder. We firstly compared research and real-world inpatients from the same treatment center using 76 clinical and sociodemographic variables. An elastic net algorithm with ten-fold cross-validation was then applied to develop a sparse machine learning model for predicting depression severity based on the top five features (global functioning, extraversion, neuroticism, emotional abuse in childhood, and somatization). Model generalizability was tested across nine external samples. The model reliably predicted depression severity across all samples (r = 0.60, SD = 0.089, p < 0.0001) and in each individual external sample, ranging in performance from r = 0.48 in a real-world general population sample to r = 0.73 in real-world inpatients. These results suggest that machine learning models trained on sparse clinical data have the potential to predict illness severity across diverse settings, offering insights that could inform the development of more generalizable tools for use in routine psychiatric data analysis

    Health disparities and selection bias in obtaining broad consent in a general practitioner setting

    Background: For research with electronic health records in the outpatient setting, obtaining Broad Consent (BC) is increasingly important. However, the presence of potential selection bias in this context remains unclear. Since 2020, the BeoNet-Halle outpatient database collects patient data from participating general and specialty ambulatory practices’ management systems in Germany, whereby data is obtained anonymously or pseudonymously via BC. For clarity, anonymized datasets are routinely extracted for descriptive analyzes, whereas pseudonymized datasets are available only when patients provide BC (details in Methods and Ethics). The primary objective of this study is to compare health related parameters between patients who provided BC and the general practice population. Methods: This is a single-center, cross-sectional study. From February 2021 to May 2023, patients were asked by a general practitioner or a specially trained member of the joint practice to provide BC. Within the yearly contact group of 2022, we compared patients who provided BC with the reference population (RP) of patients with at least one physician–patient contact during that period in a joint practice including eight eneral ractitioners. Data pertaining to health, morbidity, and health utilization were extracted from the BeoNet-Halle database. Results: A total of 5,034 patients were analyzed (BC-group: 439 vs. RP group: 4,595). Sex was similar distributed between the groups. In the BC group, patients were slightly older (56.2 vs. 54.1 years), had more physician contacts (15.0 vs. 9.2) and more often at least one chronic condition (76.1 vs. 51.6%) than patients from RP group. Patients were more likely to be referred to at least one other specialist (74.0 vs. 44.4%) and to get at least one drug prescription (89.5 vs. 69.6%). Conclusion: Differences between BC and the reference population (older age, higher multimorbidity, more contacts, referrals, and drug use) indicate selection processes at the point of consent. Given the single-practice design and descriptive analysis, generalizability beyond similar German group practices is limited and requires validation in multi-site studies

    Optimizing safety and efficacy of intravenous Vancomycin therapy in orthopedic inpatients through a standardized dosing protocol : a pre-post cohort study

    Background: Intravenous vancomycin remains a key agent in the treatment of complex orthopedic infections, particularly those involving methicillin-resistant Staphylococcus aureus (MRSA). However, its use is associated with significant risks, most notably nephrotoxicity. Despite guideline recommendations, standardized dosing and monitoring protocols are often absent in orthopedic settings, leading to inconsistent therapeutic drug exposure and preventable adverse events. This study evaluated the clinical impact of implementing a structured standard operating procedure (SOP) for intravenous vancomycin therapy in orthopedic inpatients. Methods: We conducted a single-center, pre-post cohort study at a university orthopedic department. The intervention consisted of a standard operating procedure (SOP) for intravenous vancomycin therapy, which mandated weight-based loading doses, renal function-adjusted maintenance dosing, trough level monitoring, and defined dose adjustments. Patients treated before SOP implementation (n = 58) formed the control group; those treated under the SOP (n = 56) were prospectively included. The primary outcome was the incidence of vancomycin-associated acute kidney injury (VA-AKI) defined by KDIGO Stage 1 criteria. Secondary outcomes included therapeutic trough level attainment and infusion-related or ototoxic adverse events. Results: All patients in the post-SOP group received a loading dose (100% vs. 31% pre-SOP, p < 0.001). The range of measured vancomycin trough levels narrowed substantially after SOP implementation (7.1–36.2 mg/L vs. 4.0–80.0 mg/L). The proportion of patients reaching therapeutic trough levels increased, although this was not statistically significant. Most notably, VA-AKI occurred in 17.2% of patients in the control group, but in none of the patients after SOP implementation (0%, p = 0.0013). No cases of ototoxicity were observed in either group. Infusion-related reactions decreased after the implementation of the SOP, though not significantly. Conclusions: The introduction of a structured vancomycin protocol significantly reduced adverse drug events and improved dosing control in orthopedic inpatients. Incorporating such protocols into routine practice represents a feasible and effective strategy to strengthen antibiotic stewardship and clinical quality in surgical disciplines

    Annual level changes of serum neuronal and glial biomarkers in a German professional football club

    Background: Professional football players (PFP) experience repeated mild traumatic brain injuries (TBI) and have an increased long-term dementia risk. We aimed to assess annual level changes of blood neuronal (neurofilament light chain, NfL) and astroglial (glial fibrillary acidic protein, GFAP) biomarkers in PFPs over 2 years. Methods: We measured with commercial immunoassays NfL and GFAP concentrations n = 129 serum samples obtained from n = 43 male PFPs playing for a German professional football team. Samples were collected at five time points over 2 years and before/after an index match. Associations between blood markers and potential sources of neuronal damage, such as intense physical activity, injuries, and headers, were tested. Results: Serum NfL and GFAP concentrations in PFPs were significantly different at repeated measurements (p < 0.001) but were not associated with metrics of physical activity, total time of physical activity, total number of headers, and headers-per-match. After injuries with mild TBI, serum NfL and GFAP increased and returned to normal levels within few days. Before and after an index match, serum levels of NfL and GFAP were not significantly different, nor they were significantly associated with physical activity and headers. Discussion: Serum NfL and GFAP may be used to monitor PFP over time. Repeated headers and intense physical activity in PFPs seem to be safe on a neurochemical level
