    Developing guidance on necessary & avoidable care transitions in senior citizens

    The doctoral dissertation is based on two articles. The first reports on a Delphi study aimed to deliver a new consensus-based definition of “avoidable care transitions”. The second publication reports on a systematic literature review aimed to identify and comprehensively describe the assessment tools addressing avoidable care transitions that can support interest-holders´ decisions on older adults. As a result, the new definition encompasses three dimensions: balance of benefit and harm, resource consumption, notion of patient´s preferences. The systematic literature search identified 58 relevant studies on 48 assessment tools. All of the reviewed tools are not comprehensive with respect to the dimensions covered. The new definition can enhance the understanding of avoidable care transitions. Review findings are systematically summarised in a clinically accessible website, and can support informed decision-making when choosing the right tool addressing avoidable care transitions

    Dieser Artikel analysiert, wie sich Diskurse über sozialräumliche Qualität früher Bildung lokal etablieren und welche Herausforderungen sich für verschiedene Akteursgruppen vor Ort ergeben. In ländlichen Gebieten in Sachsen-Anhalt, die im Rahmen des vom BMBF finanzierten Forschungsprojektes NeOBi „(Neu‑)Ordnungen von Bildungslandschaften reflexiv gestalten. Zur riskanten gesellschaftlichen und schulischen Teilhabe von Kindern in peripheren Sozialräumen“ als kommunale Bildungs- und Betreuungslandschaften untersucht werden, bearbeiten die Akteur*innen aus Politik, Verwaltung, Wissenschaft und frühkindlicher Bildung explizite und implizite normative Qualitätsansprüche und übersetzen diese in ihre alltägliche Praxis. Dabei übernehmen sie nicht nur die staatlich-regulativen Vorgaben, sondern interpretieren diese neu und bringen, je nach Standort und Perspektive, Qualität als normative Erwartung, aber auch in ihrer Praxis hervor. Vor allem zwischen der frühkindlichen Bildungspraxis und der Kommunalpolitik entfaltet sich in diesen Prozessen ein Spannungsverhältnis zwischen Qualitätsansprüchen, die in Form eines New Public Managements im Zuge der Gebietsreform in Sachsen-Anhalt Einzug hielten und den Möglichkeiten ihrer praktischen Umsetzung. Die darin enthaltenden Ambivalenzen, die mit unterschiedlichen politischen, ökonomischen und frühpädagogischen Perspektiven auf sozialräumliche Qualität verbunden sind, müssen von den lokalen Akteur*innen bearbeitet und miteinander verhandelt werden

    Transabdominal ultrasound for the characterization and follow-up of cystic pancreatic lesions

    Cystic pancreatic lesions (CPL) pose a diagnostic challenge due to their morphological diversity and malignant potential. Given the limited study data, transabdominal ultrasound (TAUS) is currently not established for either primary diagnostics or CPL monitoring. This study compared the diagnostic accuracy of TAUS in the assessment of CPL to that of the reference method, endoscopic ultrasound (EUS), to identify patient subgroups suitable for TAUS monitoring. In a monocentric, retrospective analysis, patients with CPL who underwent EUS and TAUS within six months from 01/2016 to 06/2022 were included. Univariate and multiple logistic regression analyses were used to identify determinants for the detection of CPL via TAUS. Cross-method morphological assessments were analysed, and a patient-specific algorithm for selecting the appropriate monitoring method was developed. Among 105 patients, CPL were detected by both EUS and TAUS in 90 patients (86%). Patients with “TAUS negative” CPL (n = 15) exhibited greater body mass indices (BMI, p = 0.002) and smaller CPL diameters (p = 0.043). The final multivariate model (BMI, age, CPL diameter) yielded an 85% accuracy in predicting CPL detectability by TAUS. TAUS could be a cost-effective and patient-friendly imaging method for the surveillance of CPL in selected patients

    Long-lasting plutonism during orogenic cycles : incremental emplacement of the Variscan Bassiès Pluton (Central Pyrenees, France)

    Laser ablation U–Pb single zircon geochronology was applied to four peraluminous granite and granodiorite samples from the Bassiès pluton in the Central Pyrenees (France) yielding a wide range of concordant ages from early Carboniferous (Tournaisian, 351 Ma) to early Permian (Artinskian, 285 Ma). Emplacement of the Bassiès pluton occurred incrementally during the main Variscan deformation phase, with increased activities every 7–15 Myr and a peak at around 321 Ma. Evolution of the Bassiès pluton is more complex than the previously published single age of 312 Ma implied, underscoring the importance of synorogenic Variscan magmatism within the Axial Zone. Supported by geochronologic data from other plutons and gneiss domes, the notion that magmatism in the Axial Zone was confined to the late- to post-orogenic phase (315–295 Ma) is refuted. A possible thermal source for the long-lasting widespread plutonism in the Variscan crust is the TUZO mantle plume

    Background: Vascular restenosis resulting from neointima formation significantly limits the efficacy of percutaneous interventional therapies compared with bypass surgery. The adventitial layer is involved in neointima formation, but the detailed pathophysiological interplay of the different cell types in this process is still unclear. Methods: We analyzed the correlation between adventitial and neointimal tissue size in human postmortem restenotic lesions after angioplasty. In porcine and mouse models of vascular injury, we examined early proliferation of fibroblasts and adventitial expansion. Using anti‐CD45 antibodies, we identified recruited leukocytes as the source of fibroblast activation following vascular injury in mice. A time‐course experiment on neointima formation demonstrated that adventitial activation precedes the proliferation of medial and neointimal smooth muscle cells (SMCs). To further investigate this process, we developed a mouse model enabling the surgical removal and transplantation of adventitial tissue. Results: We observed that activated adventitial fibroblasts release interleukin 6 and other cytokines, which strongly induce SMC proliferation and migration in vitro. In interleukin 6 knockout mice, supernatants from activated adventitia grafts failed to stimulate SMC proliferation and migration. Furthermore, transplantation of adventitial grafts from interleukin 6 knockout mice did not induce neointima formation. Cell fate tracking experiments using double transgenic reporter mice demonstrated that resident adventitial cells do not directly contribute to the neointimal cellular mass. Instead, medial SMCs were identified as the primary source of neointimal cells. Conclusions: We show that the release of interleukin 6 by adventitial fibroblasts induces the subsequent proliferation and migration of medial SMC in the process of neointima formation. Thus, we propose a new paradigm for adventitial fibroblasts in this process as a paracrine inflammatory engine. Anti‐inflammatory targeting of the vascular adventitia might thus be promising to limit neointima formation

    Uncertainty in blacklisting potential Pacific plant invaders using species distribution models

    Invasive alien species pose a growing threat to global biodiversity, underscoring the need for evidence-based prevention strategies. Species distribution models (SDMs) are a widely used tool to estimate the potential distribution of alien species and to inform blacklists based on establishment risk. Yet, data limitations and modelling decisions can introduce uncertainty in these predictions. Here, we aim to quantify the contribution of four key sources of uncertainty in SDM-based blacklists: species occurrence data, environmental predictors, SDM algorithms and thresholding methods for binarising predictions. Focusing on 82 of the most invasive plant species on the Hawaiian Islands, we built SDMs to quantify their establishment potential in the Pacific region. To assess uncertainty, we systematically varied four modelling components: species occurrence data (native vs. global), environmental predictors (climatic vs. edapho-climatic), four SDM algorithms and three thresholding methods. From these models, we derived blacklists using three alternative blacklisting definitions and quantified the variance in establishment risk scores and resulting species rankings attributable to each source of uncertainty. SDMs showed fair predictive performance overall. Among the sources of uncertainty, the thresholding method had the strongest and most consistent influence on risk scores across all three blacklist definitions but resulted in only minor changes in blacklist rankings. Algorithm choice had the most pronounced effect on blacklist rankings, followed by smaller but important effects of species occurrence data and environmental predictors. Notably, models based only on native occurrences often underestimated establishment potential. Synthesis and applications. SDMs can provide valuable support for planning the preventive management of alien species. However, our findings show that blacklist outcomes are highly sensitive to modelling decisions. While ensemble modelling across multiple algorithms is a recommended best practice, our results reinforce the importance of incorporating global occurrence data when available and carefully evaluating the trade-offs of including additional environmental predictors. Given the strong influence of thresholding on risk scores, we emphasise the need for transparent, context-specific threshold selection. More broadly, explicitly assessing uncertainty in SDM outputs can improve the robustness of blacklists and support scientifically informed, precautionary decision-making, particularly in data-limited situations where pragmatic modelling choices must be taken
