    Risk of Inflammatory Bowel Disease in Patients With Psoriasis and Psoriatic Arthritis/Ankylosing Spondylitis Initiating Interleukin-17 Inhibitors: A Nationwide Population-Based Study Using the French National Health Data System

    International audienceObjective: To investigate whether the initiation of treatment with an interleukin-17 inhibitor (IL-17i) in real life is associated with a higher risk of inflammatory bowel disease (IBD) in patients who had both psoriasis (PsO) and psoriatic arthritis (PsA)/ankylosing spondylitis (AS). Methods: This nationwide cohort study was conducted using the French National Health Data System database. All adult patients with PsO and PsA/AS who were identified as having newly initiated treatment with an IL-17i during 2016–2019 were included. As controls, patients with PsO and PsA/AS who had newly initiated either 1) apremilast or 2) etanercept (ETN) during this period but had not received IL-17i were included. The follow-up end date was September 30, 2019. The primary end point was the risk of occurrence of IBD associated with exposure to an IL-17i compared to exposure to the other treatments, as determined in a time-to-event analysis with propensity score–weighted Cox and Fine-Gray proportional hazards models. Results: The study included a total of 16,793 new IL-17i users (mean ± SD age 48.4 ± 13 years, 45% men), 20,556 new apremilast users (age 52.6 ± 15 years, 54% men), and 10,294 new ETN users (age 46.3 ± 15 years, 44% men). New IL-17i users and new ETN users had received more biologics for their underlying disease compared to new apremilast users. IBD occurred in 132 patients: 72 new IL-17i users (0.43%), 11 new apremilast users (0.05%), and 49 new ETN users (0.48%). Most IBD cases occurred after 6 months of exposure (82%, 55%, and 76%, respectively). After propensity score weighting, the risk of IBD was significantly greater among patients initiating an IL-17i compared to those initiating apremilast (weighted hazard ratio [HR] 3.8 [95% confidence interval (95% CI) 2.1–6.8]). No difference in the risk of IBD between new IL-17i users and new ETN users was observed (weighted HR 0.8 [95% CI 0.5–1.2]). Conclusion: Patients with PsO and PsA/AS who initiate treatment with an IL-17i do not have a higher risk of developing IBD when compared to patients initiating ETN who display the same severity of underlying disease. These results need to be confirmed in other large studies of patients with PsO and PsA/AS

    International trade and face-to-face diplomacy

    International audienceThis paper examines the effects of foreign visits by public officials on international trade. Using an original database that gathers more than 13,000 visits of French officials abroad and of foreign officials in France over the 1977–2007 period, it is shown that a bilateral visit to a foreign country is associated with an increase in French exports by 8% on average. This average effect hides considerable heterogeneity across world regions. The paper provides some evidence on the underlying mechanisms and finds that face-to-face visits mitigate trade costs related to contractual enforcement, insecurity and information. Indeed, analysis at the sectoral level reveals that visits increase French exports of a large variety of products, but that their effect is larger for differentiated products, i.e., goods highly exposed to contractual risks. Furthermore, bilateral visits reinforce trade, particularly in countries characterized by weak contract enforcement, corruption and lower political proximity with France, as well as by a smaller number of French migrants

    Rain, Wind, and Dust Connections in the Sahel

    International audienceThe Sahel is a dust source region where dust emission could be drastically modified in the future due to climatic and land use changes. Based on observations of meteorological parameters and dust concentration for about 1,000 rain events, we investigated the processes leading to dust emission during the rainy season when Mesoscale Convective Systems (MCSs) regularly cross the Sahel. We show that the highest wind speed is strongly linked to the MCS cold pool intensity, which is characterized by a drop in surface temperature. This is observed during the premonsoon period (∼May to June) when the midtroposphere is still sufficiently dry to allow intense evaporation of raindrops. Because this coincides with the time of the year that the surface is the least protected by the vegetative residue, the premonsoon wind speed leads to the highest observed dust concentration in our record. Most of the highest wind speed occur before or just at the beginning of a rainy event allowing a large part of the dust raised to be transported ahead the rain limiting dust removal by wet scavenging. Finally, we show that the number of 5-min dust concentration higher than 5,000 μg m−3 is almost only occurring during the rainy season. These results suggest that until the dust models fail to correctly resolve MCS, it will be difficulty to obtain reliable estimates of dust emission from the Sahel for the present or future scenarios

    Estimation for dynamical systems using a population-based Kalman filter – Applications in computational biology

    International audienceEstimation of dynamical systems - in particular, identification of their parameters - is fundamental in computational biology, e.g., pharmacology, virology, or epidemiology, to reconcile model runs with available measurements. Unfortunately, the mean and variance priorities of the parameters must be chosen very appropriately to balance our distrust of the measurements when the data are sparse or corrupted by noise. Otherwise, the identification procedure fails. One option is to use repeated measurements collected in configurations with common priorities - for example, with multiple subjects in a clinical trial or clusters in an epidemiological investigation. This shared information is beneficial and is typically modeled in statistics using nonlinear mixed-effects models. In this paper, we present a data assimilation method that is compatible with such a mixed-effects strategy without being compromised by the potential curse of dimensionality. We define population-based estimators through maximum likelihood estimation. We then develop an equivalent robust sequential estimator for large populations based on filtering theory that sequentially integrates data. Finally, we limit the computational complexity by defining a reduced-order version of this population-based Kalman filter that clusters subpopulations with common observational backgrounds. The performance of the resulting algorithm is evaluated against classical pharmacokinetics benchmarks. Finally, the versatility of the proposed method is tested in an epidemiological study using real data on the hospitalisation of COVID-19 patients in the regions and departments of France

    The Teaching Profession in France since the Late Nineteenth Century: Greater Integration or Reinforced Segmentation?

    International audienceThe emergence, in the mid-twentieth century, of an integrated French educational system that embraces all levels and tracks within a `unique' organised structure of compulsory school education for children between 6 and 16 years of age came with a renewal of the distinctions between the professional identities, trajectories and careers of the various categories of teachers. While this system has helped to redefine, and to some extent blur, the divisions between teaching groups, their professional identities and administrative statuses have remained quite distinct, mainly because of persisting forms of institutional differentiation. In addition, some new lines of division can be observed. Our argument draws on a review of the socio-historical research conducted in relation to three teaching groups (primary school teachers, agrégés and technical and vocational education teachers) and on a particular case study of the contract teachers (contractuels) whose rise marks a new form of segmentation among teachers in France
