    Diagnostic value of biological parameters in biopsy-confirmed thrombotic microangiopathy-MATRIX consortium group

    International audienceIntroductionThe diagnosis of thrombotic microangiopathy (TMA) relies on common biological parameters which diagnostic value is unknown.MethodsThe presence of common biological parameters was assessed in 967 patients with TMA during the 2009-2023 period (ClinicalTrials.gov: NCT05991245).ResultsMedian age was 49 [36-64] and 53.2% were male. All TMA causes were represented (atypical hemolytic uremic syndrome (HUS): 41.6%; drugs: 24.9%; malignancy: 21.4%; auto-immune: 18.8%; infection: 7.6%, complement-mediated HUS (C-HUS): 6.8%, organ transplantation: 5.8%, pregnancy: 3.8%, bone marrow transplantation (BMT): 2.9%, shigatoxin (STEC-HUS): 0.6%, thrombotic thrombocytopenic purpura (TTP) (0.6%).The presence of TMA-related parameters concerned virtually all patients with TTP but varied widely for the other patients: anemia 81.7% (anemia (81.7%), high lactate deshydrogenase (LDH) (75.4%), low haptoglobin (53.7%), thrombocytopenia (40.3%). Their diagnostic performance was accurate only for TTP. Eleven distinct ways were used for schistocyte metrics and reporting. Relying on schistocytes presence as the single diagnostic criterion would lead to missed diagnosis in 23.8% (shigatoxin HUS) to 86.4% (bone marrow transplantation) of patients (for anemia: 8.2% to 22.3%; thrombocytopenia: 31.8% to 67.9%; high LDH: 10.0% to 40.7%, low haptoglobin: 0% to 70.4%, according to the causes of TMA). The overall risk of missed diagnosis using these parameters was ≥ 50% in all TMA, except in TTP.The best diagnostic performances were obtained when fibrinogen levels were <5 g/L, creatinine ≥300 μmol/L, prothrombin time (PT) <90% and when TMA causes were TTP, STEC-HUS, infection or complement-mediated HUS.ConclusionsCommon biological parameters miss the diagnosis in more than 50% of TMA except when fibrinogen is <5 g/L, creatinine ≥300 μmol/L and prothrombin time (PT) <90%. Schistocyte reporting is heterogenous, and its results are usually deceptive in TMA

    International audienceJean Rodolphe Sinner (1730-1787) ran the State Library (now the Burgher Library) of Bern for a quarter of a century, from 1748 to 1776. He can be described as a “library reformer” who transformed a book depository into a public place, oriented towards the Republic of Letters and the questionsof the contemporary society. Recognized as a scholar, he published a good number of catalogues raisonnés which marked the culmination of his activities for the promotion of the library. What is more, Sinner had completely understood that the growing reputation of the City of Bern’s library was first of all founded on book donations. The great French humanist Jacques Bongars (1554-1612) had assembled an illustrious private library which was given as a donation to the Bernese, in 1632. In the same way, an important collection of English Enlightenment works was transferred to Bern, as a gift of a generous British donor, Thomas Hollis (1720-1774), who had great sympathies for Switzerland and especially for the canton of Bern. The paper is particularly interested in this form of book transfer and in Sinner’s strategy to increase the prestige and splendour of the library, tackling the wide-spread prejudice that the Bernese Republic was a state disdainful of the arts and sciences

L'auteur y étudie le rôle que joue le schème de la confession dans les dispositifs contemporains de représentations de la sexualité, même si elles ont quitté les boudoirs libertins du siècle des Lumières. Il souligne la logique de surenchère qu’il crée dans les productions vidéos pornographiques, comme les paradoxes de la représentation de l’intimité sexuelle. Plus généralement il relie la question de la confession aux controverses qui agitent les études consacrées à ces sujets dans les études culturelles : définition même de la pornographie, discussion sur sa nature aliénante ou émancipatrice, son invariance ou son historicité

    International audienceFormée dans les années 1970 à l’université de Vincennes, familière des théories structuralistes comme du concept derridien de déconstruction, Michèle Métail envisage la langue comme un système à déconstruire. Ses pratiques poétiques, qu’elles soient visuelles ou sonores, sont fondées sur « la notion de système de contrainte ». Prenant la langue au pied de la lettre comme objet de réflexion, elles abolissent par là même sa transparence. Cette approche littérale et systémique du langage est au fondement même des matérialités du poème, tant en termes d’écriture que de publication. C’est ce double aspect que la communication se propose d’aborder, pour montrer notamment quels sont les enjeux de la publication envisagée comme médiation matérielle du poème

    International audienceA new green methodology has been developed for the synthesis of a series of substituted 5-arylidene-2,4-thiazolidinediones (ATZDs) 3a-q through Knoevenagel condensation, using barium hydroxide as an efficient heterogeneous catalyst. All compounds were characterized by H-1 NMR, C-13 NMR, and HRMS spectroscopy. Additionally, seven compounds were analyzed by X-ray diffraction (XRD). Furthermore, some selected TZD derivatives were screened for their antioxidant activity using oxygen radical absorbance capacity (ORAC) assays. The results showed that five compounds exhibited excellent antioxidant properties, with ORAC values ranging from 4.73 TE to 6.31 TE, surpassing the reference antioxidants ferulic acid (3.70 TE) and melatonin (2.45 TE). Computational investigations based on the DFT method revealed that these compounds exert their antioxidant activity primarily through the HAT mechanism, preferentially from the OH bond rather than the NH bond. This study highlights the advantages of this methodology, providing products of high purity, with good yields and short reaction times, and proposes 5-arylidene-thiazolidine-2,4-dione derivatives as promising antioxidants

    International audienceThe use of biochar produced from the rachis of date palm leaves could help fight soil salinization while using under-utilized crop residues. To improve the specific surface area of the biochars produced and therefore their adsorption capacity, the addition of oxygen (3 and 7%) during pyrolysis was tested. The amount of Na adsorbed was relatively low (between 15 and 20% of Na added) whatever are the biochar considered (produced with or without O2). The release of this adsorbed Na in contact with water was quite important, from 100% for the lowest concentrations of Na added to 30-40% for the highest concentrations added, highlighting a low retention of Na by the biochars studied and a limited impact of the presence of O2 during pyrolysis

    Exploring the Interactions between Drugs and Metabolic Disturbances in CKD

    International audienceBackground: The metabolic disturbances associated with chronic kidney disease might alter drug distribution, decrease albumin drug binding, and thus increase the free (unbound) drug concentrations. Uremic toxins can affect the pharmacokinetic or pharmacodynamic activity of certain drugs. Vitamin K antagonists (VKA) are interesting candidates for the evaluation of potential interactions between uremic toxins and drugs. The primary objective of the present study was to investigate the association between free VKA concentrations and the estimated glomerular filtration rate (eGFR). Furthermore, we sought to determine whether this relationship was mediated by protein carbamylation (measured by homocitrulline levels) and/or the accumulation of protein-bound uremic toxins (PBUTs). Methods: In this prospective cross-sectional study, 389 adult patients treated with VKA were included between May 2021 and June 2023. Levels of free VKAs, total VKAs, homocitrulline and PBUTs were assayed using liquid chromatography-tandem mass spectrometry. We used a linear regression model to explore the association between kidney function and free VKA levels and mediation analyses to determine whether the association between kidney function and free VKA levels was mediated (at least partly) by PBUTs and/or protein carbamylation. Results: Patients with an eGFR < 40 mL/min/1.73 m 2 or those on chronic hemodialysis had lower total VKA levels, higher free VKA levels and thus a higher of free/total VKA ratio than those with an eGFR ≥ 40 mL/min/1.73 m 2 . Kidney function was independently associated with free VKA levels (β 1 =0.31 [0.19; 0.42], p<0.001). Twenty one percent [95%CI, 1%-35%] of the association between kidney function and free VKA levels was mediated by homocitrulline, but not by PBUTs. Conclusions: Our results showed that a low kidney function was associated with an elevation in the free drug fraction. This association was independent of blood albumin levels and appeared to be partly mediated by protein carbamylation
