Repair versus Replacement in mitral valve papillary muscle rupture: a multicenter study
Objectives: Papillary muscle rupture (PMR) is a rare but potentially fatal mechanical complication after acute myocardial infarction (AMI). Despite surgery is considered the gold-standard treatment for post-AMI PMR, the optimal surgical strategy remains unclear. Methods: Data from post-AMI PMR patients submitted to MVR or MVr surgery in the period between 2001 and 2019, from 20 international centers, were collected in the CAUTION study database. In-hospital, and long-term post-discharge mortality were the end-points. Multivariable logistic regression model was used to determine mortality independent factors. Results: The patient cohort available included 218 patients. MVR was the most frequent type of surgery (81.6%). Complete PMR was more common in MVR group (71.9%, p = 0.008), while partial PMR was more frequent in MVr patients (75%, p = 0.008). In-hospital mortality rate was 25.8% in MVR subgroup and 20% in MVr subjects (p = 0.440). In MVR subgroup, concomitant CABG was associated with lower in-hospital mortality (n = 20/96, 21%) than no concomitant CABG (31.7%, p = 0.035). Survival at 1,3,5 and 10 years was 59.3%, 55.9%,53.1%,46.9% in MVR group and 59.9%,56.8%,54.1% and 43.2% in MVr patients, respectively, with no statistical differences (p = 0.474). Patients underwent MVr surgery 1,3,5 and 10 years survival was 79.8%, 75.4%, 68.5% and 37.5% when CABG revascularization is performed, while no CABG survival was 16.7%, 16.7%, 8.3% and 8.3% (p < 0.001). Conclusions: MVR is the mostly performed in complete post-AMI PMR and MVr in partial PMR. No differences were observed regarding in-hospital and long-term mortality in the two surgical groups, and no independent factors were associated with overall mortality. Concomitant CABG was associated to higher in-hospital survival. Clinical registration number: Clinicaltrials.gov, NCT03848429
NEL SALOTTO DELLA SIGNORA VON F. TRE NOVELLE DI PAUL HEYSE: FRAU VON F. (1878), DIE SCHWARZE JAKOBE (1883) E NINON (1900)
The article explores the role of the setting and atmosphere that forms the backdrop to the oral
narrative act of three framed novellas by Paul Heyse (1830-1914): Frau von F. (1878), Die schwarze
Jakobe (1883) and Ninon (1900). Frau von F.’s living room, both in the Swiss hotel (Frau von F.) and in her
Berlin home (Die schwarze Jakobe; Ninon), is as an intimate space that facilitates confessions and
reflections. The open French window in Frau von F. connects the inside with the outside, thus creating a
contrast with the symbolic closure typical of places where confessions of intimate and personal experiences
are staged. The lamp with the green lampshade becomes a recurring element, a symbol of intimacy and a
narrative guide, encouraging the transition from emotional tension to narration. In Die schwarze Jakobe,
Frau von F. recounts the tragic life of the peasant girl Jakobe in an indoor evening setting, a mirror of her
introspection. In Ninon, the scene shifts around a tea table, where the university professor narrates the story
of the sculptress Ninon, offering a reflection on the female condition between independence and
conformism. The three novellas show how the scenic space, with its theatricality gathered in the word,
amplifies the emotional dynamics, transforming the act of telling into a ritual of listening and intimate
revelation

Decentralized Edge Intelligence: A Big Picture on Integrating Blockchain and Federated Learning for IoT Security
Compromising in cyber security, especially regarding data leaks and single points of failure, pose a problem to the rapid growth of the Internet of Things (IoT). A solution for these issues is Blockchain and Federated Learning (FL) integration, which offer a more private and decentralized method for ensuring the security of IoT devices. This integration improves the integrity of data autonomously, while authentication and secure model aggregation block any attempts to tamper. Moreover, FL allows Artificial Intelligence (AI) to be trained on devices, making the exposure of data less likely. In this paper, we analyse the attack surface and emerging vulnerabilities of Blockchain-FL based IoT systems while constructing a taxonomy of the threats posed. They focus on novel security frameworks aimed at constructing next generation IoT system infrastructure, which is scalable, robust, intelligent and above all, secure
Leveraging intellectual capital for sustainable innovation: a spatial analysis of resilience in the olive oil sector
Purpose: This study, which emphasizes the olive oil business, analyses how Intellectual Capital (IC) operates in the agri-food sector, highlighting its valuable contribution to innovation. It also explores how consortium membership and proximity can impact the longevity of firms. Thus, strategic partnerships for Geographical Indications (GIs) in the olive oil sector could be vital for surviving continuing climate risks and achieving the Sustainable Development Goals (SDGs). Design/methodology/approach: Over nine years, 1,827 Italian olive oil firms underwent spatial analysis to evaluate the impact of physical proximity on forming strategic partnerships and sharing climatic risks within the same area. The spatial analysis and the map of climatic events were juxtaposed to assess the significance of collaboration among neighboring enterprises. Findings: The study reveals that membership in the Consortium for the Protection of GIs shows spatial dependency, highlighting its significance for resilience in the agri-food sector. Overlapping climate risk areas with company clusters indicates that olive-growing is especially vulnerable to climate events. Collaboration among neighboring firms, supported by local authorities, is crucial for risk mitigation and economic sustainability. Originality/value: This study broadens IC and sustainability research by exploring the relationship between IC and strategic alliances in the agri-food industry. It presents innovative perspectives on how IC can foster the sustainable growth of rural economies through spatial analysis, highlighting cooperation as a crucial aspect of resilience and innovation in the olive oil sector
First record of the non-indigenous Myrianida pachycera (Augener, 1913) (Annelida: Syllidae) in the Mediterranean Sea revealed by integrative taxonomy
The non-indigenous syllid polychaete Myrianida pachycera (Augener, 1913), originally described from Australia, is reported for the first time in the Mediterranean Sea, based on individuals sampled in port environments in the Tyrrhenian and Ionian Sea. The identity of the specimens was investigated through morphological identification and DNA barcoding of two individuals, resulting in COI sequences with almost 100% identity with an individual from California. Myrianida pachycera must be considered non-indigenous for the Mediterranean Sea. The distribution of this species seems to be relatively wide in the basin, but its detection has possibly been hindered by the loss of the majority of the diagnostic characters following fixation. Conversely, due to their striking live colour pattern, live individuals might be successfully tracked via citizen science
The ‘underdog’ informal Vietnamese enterprises: R&D, innovation and productivity performances
This paper analyzes the different performances of formal vs. informal firms in terms of R&D, Innovation, and Productivity. Using data from the 2011, 2013, and 2015 results of the Micro, Small, and Medium Enterprise Survey in Vietnam, we also evaluate the effects of the transition from an informal to a formal status on firms’ innovativeness and productivity. By accounting for the under-reporting of R&D investments, we show that (i) informal firms invest less in R&D, and (ii) those who switch to a formal status have
a higher probability of introducing innovations. However, our study provides evidence of hump-shaped R&D-innovation and innovation-productivity relationships. Also, it identifies the existence of an innovation probability threshold, beyond which informal firms outperform formal ones in productivity. Although the transition to formal status exerts a positive direct effect on productivity, the formalization’s benefits diminish as innovation probability intensifies. Hence, these findings underscore an interesting and hidden trade-off in the linkage between firms’ flexibility and productivity. Even though formal firms have a higher probability of introducing innovations, our results also reveal that—against all odds—(iii) the’underdog’ informal Vietnamese firms may have a higher level of productivity, the more likely they are to introduce radical or incremental innovations. Results also hold when we control for a reverse causality effect of firms’ productivity that may result in a driven factor of their formalizations. Thus, our outcomes challenge one-size-fits-all approaches to formalization, and policymakers should recognize that formalization’s benefits also depend on the innovative profile of firms
