Prostate cancer MRI methodological radiomics score: a EuSoMII radiomics auditing group initiative
Objectives: To evaluate the quality of radiomics research in prostate MRI for the evaluation of prostate cancer (PCa) through the assessment of METhodological RadiomICs (METRICS) score, a new scoring tool recently introduced with the goal of fostering further improvement in radiomics and machine learning methodology. Materials and methods: A literature search was conducted from July 1st, 2019, to November 30th, 2023, to identify original investigations assessing MRI-based radiomics in the setting of PCa. Seven readers with varying expertise underwent a quality assessment using METRICS. Subgroup analyses were performed to assess whether the quality score varied according to papers’ categories (diagnosis, staging, prognosis, technical) and quality ratings among these latter. Results: From a total of 1106 records, 185 manuscripts were available. Overall, the average METRICS total score was 52% ± 16%. ANOVA and chi-square tests revealed no statistically significant differences between subgroups. Items with the lowest positive scores were adherence to guidelines/checklists (4.9%), handling of confounding factors (14.1%), external testing (15.1%), and the availability of data (15.7%), code (4.3%), and models (1.6%). Conversely, most studies clearly defined patient selection criteria (86.5%), employed a high-quality reference standard (89.2%), and utilized a well-described (85.9%) and clinically applicable (87%) imaging protocol as a radiomics data source. Conclusion: The quality of MRI-based radiomics research for PCa in recent studies demonstrated good homogeneity and overall moderate quality. Splicing-based biomarkers define a robust multigene classifier for relapsing-remitting multiple sclerosis
Background: Alternative splicing (AS) is recognized as a key mechanism in multiple sclerosis (MS). We aimed to construct and validate a multivariate AS-based classifier (MS-Splicing Score, MS-SS) for the discrimination of relapsing-remitting MS (RRMS) patients from healthy controls. Methods: Three AS events (IFNAR2 exon-8 skipping, NFAT5 exon-2 skipping, PRKCA exon-3∗ inclusion) were selected based on functional and literature evidence. Isoforms were quantified via fluorescent-competitive RT-PCR in peripheral blood RNA from two independent cohorts (Italy: 37 RRMS, 50 controls; USA: 29 RRMS, 20 controls). A logistic regression model was trained to derive the MS-SS, followed by ROC analysis. Results: The MS-SS distinguished RRMS patients from controls in both cohorts (Italy: p = 0.00083, AUC = 0.71, 95 %CI = 0.59–0.82; USA: p = 0.00074, AUC = 0.77, 95 %CI = 0.63–0.90). In the pooled dataset, the score remained significantly elevated in MS (p = 5.9 × 10−6, AUC = 0.72, 95 %CI = 0.64–0.81), and a PCA-based refinement improved classification accuracy, yielding an AUC = 0.87 (95 %CI = 0.81–0.94). At the optimal cutoff (Youden's index), the score achieved a sensitivity of 80 % and specificity of 86 %. Supervised rule-based modeling using a logic-learning machine algorithm identified interpretable splicing thresholds and enabled clinical classification at the individual level. Conclusion: Our study introduces a novel, robust AS-based classifier for RRMS and proposes a strategy for transcriptome-based biomarker development in neuroimmunology. However, the relatively small sample sizes within each cohort may limit the generalizability of these findings, warranting larger validation studies to confirm the clinical utility of this biomarker
From Inner Sense to Outer Action: Interoception and Impulsive Compulsive Behavior Disorders in Parkinson's Disease
Background: Impulsive-compulsive behavior disorders (ICBDs) in Parkinson's disease (PD) include impulse control disorders (ICDs) and compulsive behaviors, often linked to dopaminergic treatment and altered reward processing. Interoception, the perception of internal bodily signals, plays a key role in emotional regulation and decision-making. Neuroimaging studies suggest that alterations in interoceptive and reward-related brain circuits may underlie ICDs in PD. However, the relationship between interoception and ICDs remains underexplored. Objective: To investigate the relationship between impulse control disorders (ICD) severity and interoceptive abilities—specifically interoceptive accuracy, insight, and self-reported interoception—in PD patients. Methods: Fifty-one participants were assessed using the Questionnaire for Impulsive-Compulsive Disorders in Parkinson's Disease Rating Scale (QUIP-RS) alongside measures of interoceptive performance, anxiety, depression, and apathy. Results: Higher ICD severity predicted increased interoceptive insight and sensibility. No significant link was found between ICD severity and interoceptive accuracy. Additionally, higher Levodopa Equivalent Daily Dose (LEDD) was related to lower interoceptive insight, suggesting a potential modulatory effect of dopaminergic therapy on meta-cognitive interoception. Conclusions: These findings highlight the complex interplay between ICDs and interoception, suggesting that heightened self-reported interoception and interoceptive insight may reinforce impulsive behaviors in PD via enhanced bodily signal salience. This study contributes to understanding the characteristics of ICDs in PD, implicating reward-related circuits such as the insula and anterior cingulate cortex
