    A systematic review on the surgical management of acquired rectourethral fistula

    Recto-urethral fistulas represent a rare and challenging condition, occurring either congenitally or due to various prostatic and pelvic interventions such as radical prostatectomy and radiation therapies. This condition often manifests with symptoms such as pneumaturia, fecaluria, and urinary tract infections. Despite its rarity, this condition presents significant clinical management challenges due to the lack of consensus on standardized treatment protocols, particularly in patients with a history of irradiation. This study aims to systematically review the available literature on the efficacy and safety of primary surgical repair of adult-acquired recto-urethral fistulas. We conducted a comprehensive bibliographic search of MEDLINE, Scopus, and Web of Science Core Collection databases in August 2024, following the PRISMA guidelines. A total of 10 studies, covering over 500 patients, were included in our review. The most common surgical approach used was the transperineal technique with gracilis muscle flap interposition. Findings indicate higher complication rates and diminished healing in irradiated patients compared to non-irradiated counterparts. Although transperineal approaches generally resulted in high success rates irradiated individuals frequently required additional surgeries or definitive urinary diversion. Further research is required to establish evidence-based guidelines for optimal management, particularly for complex cases involving radiation-induced recto-urethral fistulas

    La pronuncia della Cassazione civile n. 22922/2024 in esame offre un'importante occasione di chiarimento in merito alla nozione di ''lavoro effettivo'' ai fini del riconoscimento della NASpl, con specifico riferimento alla sua configurazione normativa nel periodo compreso tra il 6 marzo 2015 e il 22 marzo 2021. La Corte di Cassazione, confermando la decisione della Corte d'Appello, ha ritenuto che anche i periodi di ferie retribuite debbano essere inclusi nel computo delle 30 giornate di lavoro effettivo richieste dall'art. 3, comma 1, lett. c), del D.Lgs. n. 22/2015. Attraverso un richiamo sistematico a precedenti giuri­ sprudenziali e ad altre discipline previdenziali affini, la Suprema Corte ha valorizzato un'interpretazione analogica del concetto di "lavoro effettivo", fondandola sul legame inscindibile tra obbligazione retributiva, contributiva e rilevanza giuridica della presta­ zione lavorativa, anche nelle sue fisiologiche sospensioni. Tale approccio, in linea con l'art. 12 delle preleggi, consente dì superare rigidità letterali potenzialmente discriminatorie, promuovendo una lettura unitaria e sostanziale della nozione in esame. Nonostante la successiva abrogazione del requisito normativo, la decisione si inserisce in un più ampio orientamento volto alla definizione omogenea di ''lavoro effettivo'' nel sistema delle tutele previdenziali, confermando la legittimità dell'inclusione delle pause retribuite tra i periodi rilevanti ai fini dell'accesso agli ammortizzatori sociali

    Maternal Immunization: Current Evidence, Progress, and Challenges

    Maternal immunization is a key strategy for protecting pregnant individuals and newborns from infectious diseases. This review examines the mechanisms and benefits of maternal immunization, with a focus on transplacental IgG transfer and immune system interactions. We provide an overview of current recommendations and the safety and efficacy profiles of maternal vaccines, including influenza, tetanus–diphtheria–acellular pertussis (Tdap), respiratory syncytial virus (RSV), COVID-19, and hepatitis B. Additionally, we analyze the barriers to maternal immunization, such as misinformation, vaccine hesitancy, and disparities in healthcare access, while exploring potential strategies to overcome these challenges through targeted educational initiatives, improved provider communication, and policy-driven interventions aimed at increasing vaccine confidence and accessibility. Finally, this review highlights recent innovations and future directions in maternal immunization, including emerging vaccines for Group B Streptococcus and cytomegalovirus. Expanding immunization programs and advancing research on maternal–fetal immunity are essential to optimizing vaccination strategies, improving public health outcomes, and reducing the global burden of infectious diseases

    This study explores the application of fuzzy soft classification techniques combined with vegetation indices to address spectral overlap and heterogeneity in agricultural image processing. The methodology focuses on the integration of three key vegetation indices: Soil-Adjusted Vegetation Index (SAVI), Modified Soil-Adjusted Vegetation Index (MSAVI), and Modified Chlorophyll Absorption in Reflectance Index (MCARI), with Modified Possibilistic C-Means (MPCM) clustering. The analysis involves preprocessing the image data, calculating the vegetation indices, and applying the MPCM algorithm to perform soft classification, allowing pixels to belong to multiple classes with varying degrees of membership. A quantitative assessment is conducted to evaluate the accuracy of the classification results. Methodological approach: • Integrating advanced image processing techniques and vegetative band ratios with the fuzzy classification method MPCM to handle the inherent complexities in agricultural image analysis, such as spectral overlap and mixed boundaries. • Quantitative assessment of classification accuracy using Fuzzy Error Matrices (FERM). This approach provides a robust framework for analyzing spectral overlaps among the crops and weeds and improving the accuracy of crop classification, particularly in heterogeneous environments

    A Multi-agent EV Simulator Supporting the Positioning and Sizing of Charging Stations

    The rapid spread of electric vehicles and the growing attention towards environmental sustainability is not always accompanied by adequate charging infrastructures, capable of meeting the growing demand and offering energy produced from renewable sources. The main challenges concern the planning, sizing and monitoring of charging stations to ensure an effective distribution of charging facilities capable of reducing waiting times and meeting the needs of different types of users. To address these challenges, it is essential to have tools capable of simulating different real-world scenarios and providing useful data for efficient planning of charging infrastructures. This work describes the design and implementation of a multi-agent simulator, developed in Python, which, by reproducing the individual behavior of single EV users in different scenarios, provides valuable information both to electric mobility service providers for the optimization of charging station management, and to policy makers as a support to local policy choices to improve urban mobility and encourage eco-sustainable behaviors of citizens
