Fibromyalgia: Understanding, Diagnosis and Modern Approaches to Treatment
Fibromyalgia (FM) is a chronic condition characterized by generalized musculoskeletal pain associated with other symptoms, especially sleep and mood disorders, fatigue, and cognitive dysfunctions. The etiopathogenesis of FM is not sufficiently known, and regardless of numerous research, the clinical presentation is nonspecific, which makes it difficult to approve a timely diagnosis and, subsequently, an adequate therapeutic approach. Genetic, hormonal, immunological, and environmental factors are cited as potential factors in the development of this condition. Diagnosis is based on a clinical approach and known diagnostic criteria, while additional methods, such as radiographic, magnetic resonance, or laboratory analyses, can be useful to exclude other conditions. The heterogeneity of FM significantly impacts both diagnosis and treatment, as it presents a wide spectrum of symptoms that vary in severity, combinations, and underlying contributing factors. This variability is a challenge for clinicians and requires a holistic, comprehensive, multidisciplinary, patient-centered approach. According to The European League Against Rheumatism (EULAR) from 2016, treatment begins with patient education and involves the simultaneous application of pharmacological and nonpharmacological treatments. The application of only pharmacological or nonpharmacological treatment is most often not successful. Due to differences in pain threshold, psychological factors, and comorbidities, patients may respond differently to the same interventions. Although there is no universal treatment, this review brings up the fact that the timely recognition of symptoms and a tailored treatment with a patient-centered plan can significantly improve the quality of life of patients
Fuzzy group fixed-effects estimation with spatial clustering
The paper discusses the problem of estimating group heterogeneous fixed-effect panel data models under the assumption of fuzzy clustering, that is each unit belongs to all the clusters with a membership degree. To enhance spatial clustering, a spatio-temporal approach is considered. An iterative procedure, alternating panel data estimation and spatio-temporal clustering of the residuals, is proposed. The proposed method can be of relevance to researchers interested in using fuzzy group fixed-effect methods, but want to leverage spatial dimension for clustering units. Two empirical examples, the first on cigarette consumption in the US states and the second on non-life insurance demand in Italy, are presented to illustrate the performance of the proposed approach. The spatial fuzzy GFE model reveals important regional differences in both the US cigarette consumption and non-life insurance determinants in Italy. In the case of the US, we found a distinction in two main clusters, East and West. For the Italy provinces data, we find a distinction in North and South clusters. Regarding the regression results, for cigarette consumption data, different from the previous studies, we find that the smuggling effect is significant only in east regions, thus suggesting localised impacts of bootlegging. In the context of Italian non-life insurance demand, we find that while population density explains insurance consumption in northern provinces, the trust issues in the south explain the lower insurance demand
Antropocene e Terra dei Fuochi: prime note a margine della pronuncia della Corte EDU
L’articolo analizza la recente sentenza della Corte Europea dei Diritti dell’Uomo (CEDU) che ha condannato l’Italia per l’omessa tutela dei cittadini campani esposti ai rischi ambientali nella “Terra dei Fuochi”. La Corte ha rilevato che lo Stato non ha adottato misure adeguate nonostante fosse a conoscenza della grave situazione da anni. Viene evidenziata l’assenza di interventi tempestivi nella valutazione del rischio, nella bonifica dei suoli e nel monitoraggio sanitario. La sentenza richiama il principio di precauzione e impone nuove misure: una strategia coordinata, un sistema indipendente di valutazione e una piattaforma informativa aggiornata. L’analisi si estende al concetto di Antropocene e alla necessità di una rinnovata armonia tra tutela ambientale e sviluppo, in linea con la riforma dell’art. 9 della Costituzione. Abdominal lymphatic malformations in children: case series
Lymphatic Malformations (LMs) are benign congenital malformations of the lymphatic system that commonly involve the abdomen in children (mesentery of the small intestine and omentum). The management of these malformations is not unique. 7 children with different ages (range: newborn to 14 years), diagnosis was incidental in some cases, while in others for abdominal pain. All
patients underwent abdominal ultrasound and Magnetic Resonance
Imaging (MRI). Laparoscopy for diagnosis was useful in 4 cases.
Treatment was: conservative in 1 child, laparoscopic excision in 3
patients, laparotomic excision in 3 cases. At follow up we observed recurrence in a case that required integrated treatment and bowel occlusion after excision in 1 case. This benign malformations may not cause any symptoms to patient. The goal of treatment is to maintain organ function, preserve aesthetic integrity and complications control. Management of these patients could be varied: the best approach should be evaluated on the basis of the patient
