Ugo La Pietra: ricerche in materiali plastici, dal quadro all'ambiente (1966-1970)
The work of artist and architect Ugo La Pietra in the second half of the 1960s is focused on the contacts between architectonical and urban research and the abstract use of plastic materials. Following ideas he developed as an assistant professor at the Milan Polytechnic, it is possible to trace the development of his work from an abstract “signic” painting to the use of an analogous language as a “reading” of a urban form. This reflection on the relationship between sign and field stays consistent in his “objectual” paintings in transparent methacrylate (Strutturazioni tissurali, 1966). The following step in his research is the use of the same materials and principles for architectural projects (Altre Cose shop, 1967, in Milan) and meta-architectural immersive spaces (Self-service audiovisivo, 1968 and Audio-Visual Environment, 1969). At the turn of the decade the artist gradually sets aside abstract forms and plastic materials in his more critically oriented research, without abandoning them in his painting practice
Parental satisfaction with diagnosis disclosure: A study on parents of children or adults with genetic syndromes
Objectives: The study aimed to examine parental satisfaction with the communication of the diagnosis in parents of children or adults diagnosed with a genetic syndrome. Several factors were considered, such as the time of ascertainment (prenatal, at birth or in childhood) and knowledge of the syndrome. Potential predictors of parental satisfaction were analysed by considering cognitive, affective and environmental factors. Methods: Participants in the study were 390 parents of individuals (aged between two months and 45 years) diagnosed with a genetic syndrome. Parents completed a specially developed online questionnaire to assess the perceived severity of their child's condition, their experience with the diagnosis communication process, and their satisfaction with that process. Results: Parents who received the communication at birth appeared less satisfied with the communication process. No significant differences emerged in the satisfaction of parents who received communication about a known genetic syndrome (i.e. if the parents knew broadly or exhaustively what it was) and those who received communication about an unknown syndrome (i.e. if the parents had never heard of it or only knew about the existence of the genetic condition). Cognitive factors (e.g. the amount of information provided), affective factors (e.g. empathy), and environmental factors (e.g. delivery of written documents) contribute to explaining individual variability in parental satisfaction. Conclusions: Many factors contribute to explaining parents’ satisfaction with the disclosure of the diagnosis of a genetic syndrome in their children. The contribution of individual parental factors, such as personality traits or well-being, remains to be investigated. Practice Implications: Great care needs to be taken when communicating the diagnosis of genetic syndromes, especially if it is made in the perinatal period. Some practical measures, such as the provision of written information, can improve the experience for parents
E. Zoccoli, Friedrich Nietzsche.

Cost-Benefit Analysis of Treating Sickle Cell Disease: Comparing Drug Therapy and Hospitalization in Agago and Gulu Districts, Northern Uganda
This study compares the cost-effectiveness of hydroxyurea therapy versus hospitalization for managing Sickle Cell Disease (SCD) in Uganda’s Agago and Gulu districts. Findings show that hydroxyurea reduces hospital admissions by up to 70%, lowers mortality, and lessens transfusion needs, while costing just €200–300 annually per patient, versus €750–2,500 for hospitalization. Despite its benefits, hydroxyurea use is hampered by drug stockouts, geographic barriers, and stigma. The study calls for decentralizing SCD care, subsidizing treatment, and investing in education campaigns to overcome systemic obstacles. Scaling up hydroxyurea to 75% coverage could save Uganda €1.2M in five years, providing a clear case for preventive, cost-efficient SCD management in resource-limited settings
Periodic solutions of differential systems with curvature-like principal part and unbounded nonlinearity
We consider a Lagrangian system with a curvature-like principal part. Under suitable assumptions, we prove the existence of a nonconstant periodic solution. For this purpose, nonsmooth variational methods are applied
Determinants of Prolonged Hospitalization in Children and Adolescents: A Retrospective Observational Study
Introduction: Ensuring an appropriate length of stay (LOS) is a primary goal for hospitals, as prolonged LOS poses clinical risks and organizational challenges. Children and adolescents are particularly susceptible to prolonged LOS due to frequent hospitalizations and unique vulnerabilities, including developmental disabilities that may necessitate additional care and monitoring. This study aims to describe the LOS of children and adolescent patients and identify the sociodemographic, organizational, clinical, and nursing care factors contributing to prolonged LOS in this population. Design: Observational, retrospective, monocentric study. Methods: A sequential sampling approach was used to select the clinical records of 1538 children and adolescent patients admitted to an Italian university hospital in 2022. The study included all children and adolescents aged 3-18 who were hospitalized for a minimum of 2 days. Patients from outpatient units and those with LOS shorter than 2 days were excluded. The Neonatal Pediatric Professional Assessment Instrument (PAIped) and the Hospital Discharge Register were used to collect sociodemographic, organizational, clinical, and nursing care patient data, including nursing diagnoses (NDs) and nursing actions (NAs). A forward stepwise regression approach was used to identify predictors of LOS among the selected variables. A mediation analysis was conducted to explore the role of nursing predictors, identified in the stepwise regression, as mediators between the number of medical diagnoses and LOS. Results: Positive correlations between the number of medical diagnoses, NDs, NAs, and LOS were discovered (rs = 0.262, p = < 0.001; rs = 0.114, p = < 0.001; rs = 0.384, p = < 0.001, respectively). Longer hospital stays were associated with an increased number of medical diagnoses, NDs, and NAs. The number of NAs emerged as an independent predictor of LOS (β = 0.516; p < 0.001). Other significant determinants of LOS included a higher number of NAs and medical diagnoses, the presence of a medical DRG category, increased DRG weight, emergency admissions, residency in rural areas, and older age (F = 122.222, p < 0.001, R2 = 0.361, adjusted R2 = 0.358). The mediation analysis showed that the number of medical diagnoses positively predicted the number of NAs (β = 2.774, p < 0.001), which, in turn, positively affected LOS (β = 0.162, p < 0.001). A significant indirect effect of the number of medical diagnoses on LOS through NAs was observed (β = 0.448, 95% CI [0.34, 0.55]), along with a significant direct effect of medical diagnoses on LOS, even with the mediator in the model (β = 0.633, p < 0.001), indicating partial mediation (F = 321.6892; R2 = 0.295; p < 0.001). These results highlight the influence of medical diagnoses on LOS through the mediating role of NAs. Conclusions: Our study highlights the significant interplay between determinants of LOS in children and adolescent patients, emphasizing the need for targeted interventions, resource planning, and the integration of clinical nursing information systems to enhance care quality and support evidence-based practices. Clinical relevance: Optimizing resource distribution and implementing specific interventions for patients at risk of prolonged LOS could help mitigate this negative outcome and enhance the quality of care. Incorporating nursing data into DRG systems could improve reimbursement accuracy and benefit the nursing profession, which may result in better patient outcomes and lower hospital expanses
A single bout of physical exercise improves 1-hour post-load plasma glucose in healthy young adults
Purpose: Physical exercise is a key component in the treatment of type 2 diabetes and plays an important role in maintaining a healthy glucose metabolism even in healthy subjects. To date, no studies have investigated the effect of a single bout of aerobic physical exercise on glucose metabolism in young, moderately active, healthy adults. Methods: We performed an OGTT 7 days before and 24 h after a single bout of physical exercise, to evaluate 1-hour post-load plasma glucose and surrogate indexes of insulin sensitivity and insulin secretion. Results: Glucose levels were significantly reduced after exercise at baseline and one hour after glucose load; similarly, insulin was significantly lower 1 h after glucose load. We found a significant increase in the Matsuda index, confirmed by OGIS index, QUICKI index, and by significant reduction in HOMA-IR. Conversely, we observed a trend to increase in HOMA-B. Conclusion: This is the first study to evaluate the effect of a single bout of exercise on 1-hour glucose levels following OGTT. We found a significant reduction in 1-hour glucose levels following OGTT together with an increased insulin sensitivity. A single 30-minute bout of aerobic exercise also seemed to improve the insulin secretion pattern. Modifications in beta cell secretory capacity during exercise are likely secondary to an improvement in insulin action in insulin dependent tissues
