Development of a novel instrument to measure Japanese psychiatric nurses’ technological competency as caring in nursing
Background: To effectively advance person-centered care (PCC) practice, it is important to equip healthcare providers with person-centered values and beliefs while simultaneously transforming their work environment to align with PCC. Thus, instruments to measure caring practice status in nursing competency for psychiatric-specific behavioral limitations, ethico-moral behavior, technology use, and PCC need to be developed.
Objective: This study developed the Technological Competency as Caring in Psychiatric Nursing Instrument (TCCNPNI) to measure practice status and test its content and construct validity.
Methods: Five different phases were followed: 1) Literature Review; 2) Operational definition of the construct and development of items; 3) Two-round Delphi method; 4) Validity measure; and 5) Reliability measure. The online survey was conducted in 2024.
Results: The developed instrument comprises 22 items with a 4-factor structure: competency to practice caring and person-centered care (Factor 1); competency to recognize and respond to ethical issues in psychiatry (Factor 2); competency to utilize technology in psychiatry (Factor 3); and competence to practice care for the preservation of human dignity and shared decision making (Factor 4). Cronbach’s alpha for the entire scale was 0.864, while that for factors 1-4 was 0.911, 0.814, 0.773, and 0.64, respectively. Cumulatively, these four factors contributed 49.6% and explained nearly 50% of the total data. Item-total correlation values were 0.6 or higher among factors 1-3. However, factor 4, for which items were Q30, Q33, Q34, and Q35 (r = 0.03, 0.04, 0.21, 0.11, respectively), were inverted items and had low I-T correlation values. These low correlations suggest that these items capture different concepts. The developed TCCNPNI allows for the measurement of the practice of nursing as caring in psychiatry, the state of ethico-moral behavior, and the practice status of technological competency as caring in psychiatric nursing.
Conclusion: This study demonstrated satisfactorily and efficiently evaluated the practice status of technological competency in psychiatric nurses’ caring. Measuring technological competency as caring in psychiatric nursing can be an important adjunct for in-service education in psychiatric hospitals or formalized nursing education in nursing universities.doctoral thesi

温存術後20年目に同側乳房に非浸潤性乳癌を伴って発症した乳房Paget病
A case of Paget’s disease coexisting with ductal carcinoma in situ (DCIS) is reported. 60s-year-old women was visited at our hospital because of increasing tendency of erosion and vanishing of the right nipple, who had undergone breast conserving surgery and axillar dissection for breast cancer (pT1cN0M0) in 20 years ago. No evidence of disease except the nipple-areola lesion was detected by MR imaging or ultrasonograhy in bilateral breast and axilla. As Paget’s disease of the was considered, a biopsy of the lesion was conducted soon. According a diagnosis of Paget’s disease, arc shaped partial mastectomy was performed. After surgery, all cut-slices of specimen was microscopically assessed. Paget’s lesion and two DCIS were independently observed and margin was free for carcinoma. Pathologic diagnosis of the surgical specimen is important to perform breast conserving surgery for Paget’s disease.journal articl

本稿の目的は「勤勉性を獲得できていない大学生の進路選択セルフ・エフィカシー(Career Decision-Making Self-Efficacy;以下,CDMSE)を高めるために,社会的スキルは鍵となるのか」について確認することである。CDMSE とは進路選択に関する意志決定の効力を表す概念である。1つの先行研究によると,Erikson のライフサイクル論で議論される「勤勉性」の獲得(児童期の発達課題)が大学生の CDMSE に影響することが示されているものの,勤勉性の獲得に失敗した大学生のCDMSE 向上の方策は明らかになっていない。そこで,そのような学生の CDMSE を高めるにはどうすればよいのかを検討するために質問調査を実施したところ,社会的スキルが重要であることが分かった。The purpose of this paper is to examine whether social skills are the key to improving career decision-making self-efficacy (CDMSE) in students who have not yet acquired a sense of industry, a feeling of confidence in one’s abilities. CDMSE is a concept that expresses a level of self-confidence in one’s career choices. Although a previous study has shown that the acquisition of a sense of industry, discussed in Erik Erikson’s theory of development, affects CDMSE, no strategies have been identified to improve CDMSE among students without a sense of industry. Effect of contact force on diode characteristics of Rh/epitaxial graphene/n-SiC
The contact force dependence of the electrical characteristics of metal/epitaxial graphene/n-SiC diodes was investigated using conductive contact-mode scanning probe microscopy (SPM) with a Rh-coated conductive nano probe. Graphene nano-islands formed by the thermal decomposition method were used. The measured electrical characteristics showed excellent rectification behavior. The on-off ratio improved with increasing contact force. The estimated Schottky barrier height decreased with increasing contact force. In the low contact force regime (< 100 nN), Fowler-Nordheim tunneling (FNT) phenomena were observed, as the carbon layers (graphene and buffer layer) acted as a tunneling barrier.journal articl
Reliability of Emotion Analysis from Human Facial Expressions Using Multi-task Cascaded Convolutional Neural Networks
Life support robots in care settings must be able to read a person’s emotions from facial expressions to achieve empathic communication. This study aims to determine the degree of agreement between Multi-task Cascaded Convolutional Neural Networks (MTCNN) results and human subjective emotion analysis as a function to be installed in this type of robot. Forty university students talked with PALRO robot for 10 minutes. Thirteen area of interest videos were used to assess the validity identified by MTCNN was facial expression was happy or combination of happy and other emotions. Twenty university students and 20 medical professionals identified which of the 7 emotions (angry, disgust, fear, happy, sad, surprise, neutral) were present. Fleiss’ kappa coefficient was calculated. Kappa coefficients of the emotion analysis for seven emotions ranged from 0.21 to 0.28. Kappa coefficient for “Happy” was the highest (0.52 to 0.57) with moderate agreement. Among female university students, only “Surprise” had a moderate agreement with Fleiss’ kappa coefficient of 0.48. MTCNN emotion analysis and human emotion analysis were in moderate agreement for the identification of “Happy” emotions. The comparison of the agreement between the results of emotion analysis from facial expressions using non-contact MTCNN and subjective human facial expression analysis suggested that the use of MTCNN may be effective in understanding subjects’ happy feelings.doctoral thesi
