    タンポンの長期留置によってトキシックショック症候群をきたした一例

    Toxic shock syndrome is a condition that rapidly causes circulatory and multi-organ failure due to toxin-producing strains of Staphylococcus aureus and Streptococcus species. We report a case of toxic shock syndrome caused by long-term use of a tampon. The patient was a 48-year-old woman who was hospitalized and treated for schizophrenia at another hospital. The day before admission, she had a fever of 40°C and an increased inflammatory response ; therefore, antibiotic treatment was initiated. The next day, she was transported to our hospital by ambulance because of loss of consciousness and hypotension. After transport, a tampon with a foul odor was found in her vagina, the duration of retention of which was unknown. Other tests did not reveal any obvious infectious foci, and toxic shock syndrome was suspected. Shock management was initiated with massive fluid infusion and vasopressor administration, as well as the administration of broad-spectrum antibiotics and anti-methicillin-resistant Staphylococcus aureus drugs. The shock subsided on the third day of admission, and MRSA was detected in the culture of the vaginal secretion sample obtained at the time of her admission. The patient’s general condition improved and inflammation rapidly resolved thereafter. When toxic shock syndrome is suspected based on clinical findings, MRSA should also be considered as a possible causative bacterium and treatment should be initiated promptly.journal articl

    Initiative of an e-Learning Course Distribution for Educating Physicians on Perioperative Oral Function Management

    Purpose: Perioperative oral function management is undertaken based on request from physicians. However, the awareness among physicians on perioperative oral management (POM) may be limited. In this study, physicians were educated on POM using an online tool. We then reported the initiative of employing an online tool that is efficient and allows for data dissemination to all participants while maintaining data confidentiality. Methods: We conducted a questionnaire survey with physicians from each department using an online tool to investigate the level of awareness and utilization of e-learning. The POM overview video included information on the effectiveness of POM, medical fees, and current and future initiatives, and served as an announcement to encourage requests. The original POM video which limited the target audience to the patients undergoing radiotherapy/chemotherapy or palliative care related to cancer (PRCP) was emphasizing POM in PRCP and supportive care for oral mucositis. Result: According to the questionnaire survey, physicians acknowledged the effectiveness of POM. However, despite the presence of these patients, approximately one-third of the physicians did not initiate the request. The awareness on POM increased through e-learning. Furthermore, the number of requests for POM increased during the period of physician education, compared to a similar period prior to the education. Conclusion: Active dental intervention during the perioperative period necessitates continuous education for physicians and collaboration between medicine and dentistry departments.     大学における起業家教育の効果検証に向けた試論 : 選択・必修科目に注目して

    本稿では,大学生の起業意思と企業家的志向性がどのように形成されるのかについて,起業家 教育の選択・必修科目に焦点を当てながら分析を行った。分析の結果,明らかになったのは以下の二点である。第一に,起業行動に必要と言える企業家的志向性の形成において,起業家教育の選択科目 が部分的に正の効果を持つことが確認され,必修科目の場合には部分的に負の効果,統計的に有意で ないことが明らかになった。必修科目の場合,起業に興味がある学生とそうでない学生が混在しており,選択科目の場合には,学生は起業に対する興味関心と学習意欲が高いという可能性が示された。 第二に,起業家教育の必修科目と選択科目は,起業意思の形成に正の効果を持つことが明らかになった。以上のことから,起業家教育の履修は受講者の起業意思を高めるが,必ずしも起業に必要な能力 や特性を身に付けるとは限らないことが示された。This study analyzes how university students develop entrepreneurial intentions and individual entrepreneurial orientation, with a specific focus on elective and compulsory courses in entrepreneurship education. The results revealed that elective courses in entrepreneurship education positively affect the formation of individual entrepreneurial orientation necessary for entrepreneurial behavior. Conversely, the effect is partially negative and statistically insignificant for compulsory courses. These results suggest that the students enrolled in compulsory courses may include not only students interested in entrepreneurship but also those with less motivation in the subject. In contrast, elective courses are likely to attract students who are genuinely interested in entrepreneurship and eager to learn. Furthermore, compulsory and elective entrepreneurship education courses positively affect the formation of entrepreneurial intentions. Hence, these results demonstrate that entrepreneurship education may increase participants’ intention to start a business but may not equip them with the skills and traits essential for entrepreneurial success.departmental bulletin pape

    Association Between Physical Function and Edentulism in Older Adults : Findings from the Indonesian Family Life Survey 2014

    This study aimed to determine the association between physical function and edentulism among older adults. The fifth wave of Indonesian Family Life Survey (IFLS-5) data was used. Physical function was evaluated through physical performance, physical capability, and appendicular skeletal muscle mass (ASM). Edentulous was found in 10.96% of 2554 older adults. Low physical performance (OR 2.02, 95% CI 1.32-3.09; P = .001) was shown to be associated with edentulism in the 60 to 69 age group. In the 70 to 79 age group, both low physical performance (OR 1.62, 95% CI 1.04-2.53; P = .033) and 1 dependency in ADL/IADL (OR 1.75, 95% CI 1.02-2.98; P = .04) were significantly associated with edentulism. Two or more dependencies in ADL/IADL (OR 4.02, 95% CI 1.15-13.99; P = .029) demonstrated significant association with edentulism in older adults ≥80 years.     Effects of the COVID-19 Pandemic on the Lifestyle and Physical Activity of School-aged Children : Survey of Students at Japanese Language Supplementary School in the United States

    Coronavirus disease 2019 (COVID-19) and associated lockdowns affected many people regardless of location, age, or gender. This study aimed to focus on school-aged children studying at a Japanese Language Supplementary School (JLSS) in the southwestern region of the United States and assessed the impact of COVID-19 on their lifestyle and physical activity (PA). An online-based cross-sectional survey was administered to students (age range : 6-18) enrolled in a JLSS as of June 11, 2021. Data were collected from 151 students. The questionnaire consisted of lifestyle and PA questions. A paired and Student t-test was used to test the differences in changes before COVID-19 and during COVID-19 among children. Chi-square tests were used for comparisons with descriptive analyses of frequencies. Level of statistical significance was set at p <.05. Screen time was significantly longer on weekdays (t = 8.71, p<.001) and weekends (t = 5.94, p < .001) than before COVID-19. The number of children who went to bed after 10 pm was significantly higher (χ2 = 4.06, p < 0.05) than before the new coronavirus infection. The frequency of PA per week was significantly higher (χ2 = 4.01, p < 0.05) in the group using digital devices.     Enhanced photocatalytic activity of TiO2/Au/TiO2/Au stacked nanostructures synthesized via sputtering and subsequent annealing

    We synthesized TiO2/Au/TiO2/Au (IMIM) stacked nanostructure composed of TiO2 thin films (TFs) and Au nanoparticles (NPs). The TFs of TiO2 were deposited by sputtering Ti targets in Ar/O2 gas, followed by annealing to induce the anatase phase. The Au NPs were deposited by sputtering an Au target in Ar gas and then annealed to form the NPs. The nanostructures made of TiO2/Au/TiO2 (MIM) and TiO2/Au (IM) were synthesized for the purpose of comparison. The photocatalytic activity was evaluated by measuring organic decomposition and bacterial inactivation under ultraviolet and visible light irradiation, with the visible light wavelength matching the localized surface plasmon resonance (LSPR) of Au NPs. The photocatalytic performance of IMIM was more enhanced than TiO2 TF as the visible light intensity increased. The IMIM also outperformed the MIM and IM nanostructures. The improved photocatalytic activity would be attributed to the hot electron transfer from Au NPs to TiO2, originating from the visible-light-driven LSPR at the TiO2/Au interface at the top of IMIM. Additionally, the increased visible light absorption attributed to more oxygen vacancies in the top TiO2 layer due to Au NPs and a rougher surface due to the top TiO2 layer would enhance the photocatalytic activity.journal articl

    Pathogenesis, Diagnosis, and Management of Trigeminal Neuralgia : A Narrative Review

    Trigeminal neuralgia (TN) is an excruciating neurological disorder characterized by intense, stimulus-induced, and transient facial stabbing pain. The classification of TN has changed as a result of new discoveries in the last decade regarding its symptomatology, pathogenesis, and management. Because different types of facial pain have different clinical therapy and neuroimaging interpretations, a precise diagnosis is essential. Diagnosis should include magnetic resonance imaging with specific sequences to rule out secondary causes and to identify possible neurovascular contact. The purpose of demonstrating a neurovascular contact is to aid in surgical decision making, not to validate a diagnosis. Microvascular decompression is the first-line procedure for individuals who do not respond to medical management, whereas carbamazepine and oxcarbazepine are the preferred medications for long-term care. New developments in animal models and neuroimaging methods will shed more light on the biology and etiology of TN. This paper reviews the pathogenesis, the clinical features, the diagnosis, and the management of TN. Furthermore, the potential role of low-intensity pulsed ultrasound in neurological disorders is discussed.journal articl

In particular, the number of patients requesting tooth bleaching procedures for discolored teeth has been increasing. Tooth bleaching is a minimally invasive esthetic dentistry that can make patients smile. To meet the needs of these patients, Department of Cariology at the University of Tokushima Hospital has reestablished a system for patients to receive tooth bleaching treatment for discolored teeth.     インストラクショナル・デザイン(ID)の「態度の学習」における教授方略の提起 : 自己効力感理論に基づくオンデマンド動画教材の開発

    本研究の目的は,インストラクショナル・デザイン研究(以下:ID)における「態度の学習」について,自己効力感の観点からその教授方略を提起することである。そこで,本研究ではIDの代表的なモデルであるADDIEアプローチに自己効力感の観点を取り込み,それに沿ったオンデマンド動画教材の開発を行なった。具体的には,A短期大学におけるオンデマンド科目「生活に役立つ教養とSDGs」を対象に,自己効力感理論の情報源の一つである代理的経験に焦点を当てた教材設計を実施した。そこでは,SDGs課題の解決方法について事例から学べるようにし,「態度」を育めるようにした。また本研究では,開発した動画教材の評価を行うために,一般性自己効力感尺度の得点を従属変数にし,時期(事前,事後)を被験者内要因,群(SDGsを実践する具体的な方法への理解度)を被験者間要因とする2×2の2要因混合計画の分散分析を実施した。The objective of this study is to propose a pedagogical approach to the concept of 'attitude learning' within the context of instructional design (ID) research, with a particular emphasis on the role of self-efficacy. Accordingly, this study incorporates the self-efficacy perspective into the ADDIE approach, which is a representative model of ID, and develops on-demand video teaching materials in accordance with this approach. The teaching materials were designed for the on-demand course, entitled 'Liberal Arts for Life and SDGs', which is offered at Junior College A. The materials focus on vicarious experiences (observational learning), which is a source of information in self-efficacy theory. In this way, students can learn about solutions to the issues raised by the SDGs (vicarious experiences) and to promote the development of ‘attitudes’. In this study, to assess the efficacy of the developed on-demand video materials, an analysis of variance (ANOVA) was conducted, utilizing GSES scores as the dependent variable and two independent variables: timing and understanding of how to implement the SDGs.departmental bulletin pape
