    Théâtre et migration : le rôle des acteur.rice.s (non-/professionnel·le·s) dans les formes de théâtres participatifs en Allemagne et en France

    Die Forschungsarbeit analysiert aus einer deutsch-französischen Perspektive Formen des partizipativen Theaters, an denen Darsteller:innen beteiligt sind, die selbst oder deren Familien eine Zuwanderungsgeschichte haben. Der Schwerpunkt liegt auf Produktionen, die professionelle und nicht-professionelle Teilnehmende in den Kreationsprozess einbeziehen, in welchem Inszenierung und Theatertext entstehen. Anhand von drei aktuellen Beispielen („Désobéir“, „Nos morts dorment entre les racines et se dispersent dans la pluie“, „Ich bin Muslima – Haben Sie Fragen?“) verbindet die Untersuchung den Ansatz der Genèse théâtrale mit der Analyse der Re-Adressierung. Ziel ist es, zu verstehen, wie Subjektivationsprozesse und die Entstehung von Rollen innerhalb institutioneller Rahmenbedingungen ablaufen, und dabei ästhetische, soziale und politische Kontraste zwischen dem deutschen und dem französischen Kontext herauszuarbeiten.Hans-Böckler-Stiftun

    Excimer laser-assisted keratoplasty is successful in the management of complex cases of intracorneal ring segments in keratoconus

    Ziel: Analyse des Visus, der Keratometrie und der Aberrometrie (Koma) nach ICRS Implantation bei Keratokonus (KK) bzw. der Visusentwicklung nach sequenziell notwendiger Excimerlaser-assistierter perforierender Keratoplastik (PKP). Patienten und Methoden: Retrospektive Analyse einer Fallserie von 7 Patienten aus dem Homburger Keratokonus Center (HKC), die 3,2 ± 3,0 Jahre nach ICRS Implantation bei nicht zufriedenstellendem Visus eine Excimerlaser-PKP erhielten. Neben dem unkorrigierten (UCVA) und bestkorrigierten (BCVA) Fernvisus in logMAR wurden die flache (K1), steile (K2), mittlere Keratometrie (Kmean), die Higher Order Aberrations (HOA, Koma) sowie die Regularität des Astigmatismus mittels Tomographie analysiert. Die Daten der PKP-Gruppe (PG) wurden mit einer Kontrollgruppe (KG) von 7 altersadaptierten, zufriedenen ICRS-Patienten ohne PKP-Notwendigkeit verglichen. Die Datenerhebung erfolgte vor und 7,4 ± 4,5 Monate nach PKP. Ergebnisse: Vor PKP zeigte sich eine signifikant schlechtere UCVA von 0,94 ± 0,54 und BCVA von 0,49 ± 0,07 in der PG im Vergleich zur KG (UCVA 0,40 ± 0,35; BCVA 0,06 ± 0,05, p< 0,01). K1 war in der PG vor PKP signifikant höher (47,1 ± 3,1 vs. 43,0 ± 2,7 D) als in der KG (p= 0,02), K2 (p= 0,86) und Kmean (p= 0,12) nicht. Die Regularität des peripheren (p= 0,54) und zentralen Astigmatismus (p= 0,69) sowie die Koma (p= 0,39) unterschieden sich präoperativ zwischen den Gruppen nicht. Die Excimerlaser-PKP war bei allen 7 Augen wie gewohnt möglich. Sieben Monate nach komplikationsloser PKP trotz liegender ICRS zeigte sich schon bei liegenden Fäden ein signifikanter Visusanstieg (BCVA) von 0,49 ± 0,07 auf 0,33 ± 0,1 (p< 0,001). Auch K1 verringerte sich nach PKP signifikant (p= 0,002). Schlussfolgerungen: Patienten nach erfolgloser ICRS-Implantation zeigten neben schlechtem Visus eine höhere K1 bei ähnlichen Werten für K2, Kmean und Koma. Diese Patienten profitieren von einer Excimerlaser-PKP, die trotz liegender ICRS problemlos möglich ist.Introduction: Analysis of visual acuity, keratometry and aberrometry (coma) after intrastromal corneal ring segment (ICRS) implantation in keratoconus (KC) and the evolution of visual acuity after subsequent excimer laser-assisted penetrating keratoplasty (PKP). Patients and methods: Retrospective analysis of a case series of 7 patients from the Homburg Keratoconus Center (HKC), who received excimer laser PKP for unsatisfactory visual acuity 3.2 ± 3.0 years after ICRS implantation. Uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA) in logMAR, flat (K1), steep (K2) and mean keratometry, (Kmean), higher order aberrations (HOA, coma) and regularity of astigmatism were analyzed via tomography. Data of the PKP group (PG) were compared to a control group (CG) of 7 age-adapted satisfied patients after ICRS implantation without the need of PKP. Data analysis was performed before and 7.4 ± 4.5 months after PKP. Results: Prior to PKP, data analysis showed a significantly lower UCVA of 0.94 ± 0.54 and BCVA of 0.49 ± 0.07 in the PG compared to the CG (UCVA 0.40 ± 0.35; CDVA 0.06 ± 0.05, p< 0.01). The K1 was significantly higher in the PG prior to PKP compared to the CG (47.1 ± 3.1 vs. 43.0 ± 2.7 D, p= 0.02) but K2 (p= 0.86) and Kmean (p= 0.12) were not. Regularity of peripheral (p= 0.54) and central (p= 0.69) astigmatism as well as coma (p= 0.39) did not show significant differences between the PG and KG prior to PKP. Excimer laser-assisted PKP was possible without any obstacles in all 7 eyes. In addition, BCVA increased significantly from 0.49 ± 0.07 to 0.33 ± 0.1 (p< 0.001) 6 months after PKP. The K1 decreased significantly after PKP (p= 0.002). Conclusion: After unsuccessful ICRS implantation, patients seem to present with lower visual acuity and higher K1 despite similar values for K2, Kmean and coma. These patients may profit from an excimer laser assisted PKP, which can be performed without any obstacles despite the ICRS in place

    Dural repair after intraoperative CSF leakage in endoscopic endonasal skull base surgery without pedicled nasoseptal flap: is it a safe surgical technique?

    In this study we evaluated the outcome and efficacy of a multilayer closure technique of the sellar floor after transsphenoidal endoscopic approaches for sellar pathologies in correlation to the intraoperative grading of cerebrospinal fluid (CSF) leak. Overall 280 patients were reviewed retrospectively who underwent endonasal transsphenoidal endoscopic surgery for sellar pathologies from January 2011 to April 2020. Among them, 87 patients had an intraoperative cerebrospinal fluid leak and were included in this study. The intraoperative cerebrospinal fluid grading was based on the classification by Esposito. Clinical outcomes were evaluated based on postoperative complications and development of postoperative cerebrospinal fluid leak in correlation to the used closure technique and material of dural and skull base reconstruction. Their association with the intraoperative cerebrospinal fluid leak grade, underlying pathology and the tumor size were analyzed as well. From the 87 patients, there was 54 women (62%) and 33 men (38%). The mean age was 56.3 ± 14.8. The most frequent histological diagnosis was non-secreting adenoma in 40 cases (45%), followed by the secreting adenomas with 16 cases (18%). Eight cases of meningioma (9%), six cases of Rathke’s cleft cyst (7%), six cases of craniopharyngioma (7%), four cases of colloid cysts (4%), four chordomas (4%) were observed. Of the 87 patients with intraoperative cerebrospinal fluid leaks, 20 cases were classified as grade 1, 37 cases as grade 2 and 30 cases as grade 3. The materials used for the closure of grade 1 were primarily sealant sponge Tachosil ® (70%), followed by fibrin glue (55%) and bone (55%). For grade 2, Tachosil ® (78%), bone (57%) and autologous fat graft (54%). In grade 3 dural leaks, the combination of lumbar drains (90%), autologous fat graft (87%) and fascia lata (60%) were mainly used. Only nine patients developed a new postoperative cerebrospinal fluid leak in this series, and were treated conservatively with lumbar drains, but only three of them discontinued by this method. The remaining six patients with persistent postoperative cerebrospinal fluid leak underwent revision surgery. The analyzed multilayer closure technique correlated to the intraoperative CSF leakage for endoscopic transsphenoidal surgery has been shown to be safe. This study shows a lower intraoperative cerebrospinal fluid leak rate compared to other studies and similar rate of postoperative cerebrospinal fluid leak (3.1% vs. 3.9%) compared to the pedicled vascular flap and other techniques in the literature. Based on these results, the multilayer closure represents a real alternative to nasoseptal flap application in selected cases. However, in large defects and complex reconstruction of anterior skull base defects, use of lumbar drain and pedicled nasoseptal flap should remain the gold standard of treatment

    Extrakorporale Lungenunterstützung: Analyse und vergleichende Betrachtung dreier Verfahren hinsichtlich ihrer Effektivität und Auswirkungen auf das Gerinnungssystem und das Überleben

    Fragestellung: Bei den extrakorporalen Lungenersatzverfahren sind pumpenlose von pumpengetriebenen sowie Systeme im Hochflussbereich von denen im Niedrigflussbereich zu unterscheiden. Drei dieser Systeme wurden hinsichtlich Effektivität, Auswirkungen auf Gerinnungs- und Hämolyseparameter, Hämoglobinwert, Thrombozytenkonzentration und Überlebensrate verglichen: die arterio-venöse extrakorporale Kohlenstoffdioxidentfernung (avECCO2R) (Niedrigflussbereich, pumpenlos), die veno-venöse extrakorporale Kohlenstoffdioxidentfernung (vvECCO2R) (Niedrigflussbereich, pumpengetrieben) und die veno-venöse extrakorporale Membranoxygenierung (vvECMO) (Hochflussbereich, pumpengetrieben). Methoden: In dieser retrospektiven Single-Center Studie wurden insgesamt 115 Patienten, hiervon 71 Patienten, die mittels vvECMO und je 22 Patienten, die mittels avECCO2R und vvECCO2R behandelt wurden, verglichen. Die Effektivität wurde anhand der Blutgasparameter und des Horowitz-Index (PaO2/FiO2) vor sowie vier bis sechs Stunden nach Etablierung des Lungenersatzverfahrens analysiert. Hinsichtlich des Hämoglobinwertes, der Thrombozytenkonzentration und der Auswirkungen auf die Gerinnungsparameter wurden die letzten Werte vor mit den Werten der ersten acht Tage der jeweiligen Therapieform verglichen. Für die Überlebenszeitanalyse wurden die Zeitpunkte des Therapiebeginns und des Todes bzw. des letzten Zeitpunktes, zu dem der Patient nachweisbar am Leben war, herangezogen. Wesentliche Ergebnisse: Die analysierten Systeme bewirkten eine statistisch signifikante Erhöhung des pH-Wertes (avECCO2R und vvECMO: p<0,001, vvECCO2R: p= 0,002) und Verringerung des arteriellen Kohlenstoffdioxidpartialdruckes (jeweils p<0,001). Bezüglich des arteriellen Sauerstoffpartialdruckes und des Horowitz-Index ergab sich bei der vvECMO-Gruppe eine statistisch signifikante Erhöhung (p=0,008 und p<0,001), bei der vvECCO2R-Gruppe keine Veränderung und bei der avECCO2R-Gruppe eine statistisch signifikante Verringerung (p<0,001 und p=0,008). Bei den pumpengetriebenen Systemen fielen Hämoglobinwert und Thrombozytenkonzentration stärker ab als im pumpenlosen System, die Hämolyse überwog ebenfalls. Der Fibrinogenwert fiel bei allen drei Systemen ab. Antithrombin III sowie INR blieben bei allen drei Systemen stabil. Die Überlebensrate beim Einsatz des pumpenlosen Systems stellte sich am niedrigsten dar. Schlussfolgerungen: Zur suffizienten Kohlenstoffdioxidentfernung sind auch niedrige Blutflüsse ausreichend, da der Effekt vor allem auf dem Gasfluss beruht. Die Oxygenierung hängt hauptsächlich vom Blutfluss ab. Der Kontakt des Blutes mit Fremdoberflächen führt allgemein zu einer Aktivierung des Gerinnungssystems. Diese fällt bei den pumpengetriebenen Systemen stärker aus. Ursächlich ist hauptsächlich ein größerer Scherstress, der durch die Pumpe verursacht wird. Durch den Scherstress kommt es außerdem zu einem deutlicheren Absinken der Thrombozytenkonzentration sowie einer stärker ausgeprägten Hämolyse mit resultierender Reduktion des Hämoglobinwertes. Eine Verbesserung der Biokompatibilität sowie die Entwicklung von Pumpen mit geringerer Induktion von Scherstress ist ein Ziel für die Zukunft. Die Überlebensrate hängt von vielen unterschiedlichen Faktoren ab und ist bei pumpenlosen Systemen am niedrigsten.Introduction: In extracorporeal respiratory support different systems are in use: pumpless and pump-driven systems and those with high flow and low flow rate. Three of these were compared regarding effectiveness, effects on coagulation system, hemolysis, hemoglobin level, platelet count and survival rate: arterio-venous extracorporeal carbon dioxide removal (avECCO2R) (low-flow, pumpless), veno-venous extracorporeal carbon dioxide removal (vvECCO2R) (low-flow, pump-driven) and veno-venous extracorporeal membrane oxygenation (vvECMO) (high-flow, pump-driven). Methods: In this retrospective single-center study, a total of 115 patients, including 71 patients treated by vvECMO, 22 patients treated by avECCO2R and 22 patients treated by vvECCO2R were compared with each other. The effectiveness was measured on the basis of blood gas analysis and Horowitz index (PaO2/FiO2) before and six hours after starting the extracorporeal support. Hemoglobin levels, platelet count and effects on the coagulation system were analyzed based on the last values before and those from day one to day eight after starting the device. For the survival analysis, the start of treatment and death of the patient or the last time at which the patients were ascertained alive was used. Results: All devices caused a statistically significant increase in pH (avECCO2R and vvECMO: p<0,001, vvECCO2R: p= 0,002) and reduction in arterial partial pressure of carbon dioxide (each p<0,001). Regarding the arterial partial pressure of oxygen and Horowitz index, there was a statistically significant increase in the vvECMO group (p=0,008 and p<0,001), no statistically significant change in the vvECCO2R group and a statistically significant decrease in the avECCO2R group (p<0,001 and p=0,008). In the pump-driven systems, hemoglobin levels and platelet count dropped more than in the pumpless system, hemolysis also prevailed. Fibrinogen levels decreased in all three devices. Antithrombin III and INR remained stable. The survival rate was lowest using the pumpless device. Conclusion: Low blood flows are sufficient for adequate carbon dioxide removal, since the effect is mainly based on gas flow. Oxygenation mainly depends on blood flow. Contact of blood with foreign surfaces generally leads to an activation of the coagulation system. This is more pronounced with pump-driven systems. This is mainly due to greater shear stress caused by the pump. The shear stress also results in a significant reduction in platelet concentration and greater hemolysis resulting in reduction in hemoglobin levels. An improvement in biocompatibility and the development of pumps with lower induction of shear stress is a goal for the future. Survival rate depends on many different factors and is lowest using the pumpless system

    A multi-dimensional view on the etiology of Parkinson's disease

    Parkinson’s disease (PD) has been proposed to be a predominantly genetic versus a mainly environmental disease. We suggest to consider PD rather as a disease caused by multi-dimensional factors, including genetic and environmental, but also epigenetic and metabolic effects. This view is supported by very recently published data on the role of cytochromes P450 (P450s) as a model for a participation of different metabolic pathways to PD. P450s are involved in a broad variety of physiological reactions in the human body and, in addition, play a fundamental role in the detoxification of environmental compounds. Therefore, the majority of PD cases is likely based on a complex interplay between disturbances in genes and the corresponding proteins, which catalyze various metabolic pathways, as well as epigenetic, physiological and environmental effects contributing to PD symptoms. In this context, PD can be caused by one gene mutation (in rare cases), by a combination of variants in different genes, and by an association of environmental factors (e.g. pesticides) with such variants. Characterization of these factors in each individual patient opens up new avenues for a personalized therapy

    Effect of red blood cell morphology and suspension heterogeneity on microcirculation dynamics

    The morphology and microcirculatory behavior of the red blood cell represent a dynamic interplay that essentially underlies the pathophysiology of various blood disorders. The abnormal shapes, as seen in spherocytosis and ellipsocytosis, affect RBC deformability significantly and, hence, the capacity to pass through narrow capillaries. This can lead to reduced blood flow and subsequent tissue ischemia, culminating in organ dysfunction. Numerical simulations using the Lattice-Boltzmann Method are conducted in the present work to explore how changes in RBC shapes-from healthy bi-concave shapes to spherocytes-affect the flow properties, such as flow rate, cell-free layer dynamics, and hydrodynamic diffusion. Non-trivial non-monotonic behaviors in flow rate versus asphericity are unveiled, which depend on channel width and point out optimal shapes for given geometries. We further discuss the consequences of heterogeneity within RBC suspension and show that the variation in concentration of abnormal shapes is crucially important to determine blood viscosity and flow dynamics. Interestingly, as few as 15% to 20% of spherocytes can reduce RBC flux by about 50% while doubling blood viscosity, whereas other abnormal shapes have relatively weaker effects. We also discuss the phenomenon of the "memory" of RBCs, proposing that their previous mechanical interactions determine their subsequent behavior and distribution, particularly at points of bifurcation, where lingering and partitioning happen.Die Morphologie und das mikrozirkulatorische Verhalten der roten Blutkörperchen stellen ein dynamisches Zusammenspiel dar, das im Wesentlichen der Pathophysiologie verschiedener Bluterkrankungen zugrunde liegt. Die abnormen Zellformen, wie sie bei Sphärozytose und Elliptozytose auftreten, beeinträchtigen die Verformbarkeit der roten Blutkörperchen erheblich und damit auch ihre Fähigkeit, enge Kapillaren zu passieren. Dies kann zu einer verminderten Durchblutung und nachfolgender Gewebeischämie führen, was schließlich eine Organfunktionsstörung zur Folge haben kann. In der vorliegenden Arbeit werden numerische Simulationen unter Verwendung der Lattice-Boltzmann-Methode durchgeführt, um zu untersuchen, wie Formveränderungen der roten Blutkörperchen – von gesunden bikonkaven Formen hin zu Sphärozyten – die Strömungseigenschaften wie Durchflussrate, Dynamik der zellfreien Schicht und hydrodynamische Diffusion beeinflussen. Dabei werden nicht-triviale, nicht-monotone Verhaltensweisen der Durchflussrate in Abhängigkeit von der Asphärizität aufgedeckt, die von der Kanalbreite abhängen und auf optimale Zellformen für bestimmte Geometrien hinweisen. Darüber hinaus werden die Auswirkungen von Heterogenität innerhalb einer Suspension roter Blutkörperchen diskutiert und gezeigt, dass die Konzentrationsvariation abnormaler Zellformen entscheidend für die Bestimmung der Blutviskosität und der Strömungsdynamik ist. Interessanterweise können bereits 15% bis 20% Sphärozyten den Fluss roter Blutkörperchen um etwa 50% reduzieren und gleichzeitig die Blutviskosität verdoppeln, während andere abnorme Zellformen vergleichsweise schwächere Effekte aufweisen. Zudem wird das Phänomen des „Gedächtnisses“ roter Blutkörperchen diskutiert: Frühere mechanische Wechselwirkungen bestimmen ihr späteres Verhalten und ihre Verteilung, insbesondere an Verzweigungspunkten, an denen Verweilen und Aufteilung auftreten

    Genetic Features of Staphylococcus aureus from Tanzania: Strain-Specific Phenotypic Behaviors Typing of S. aureus within the African-German StaphNet Consortium

    The Gram-positive bacterium Staphylococcus aureus is a common colonizer of mammals and is regularly found in humans, particularly in the nasopharynx, on the skin, and in the intestine. At the same time, this bacterium is a feared cause of a large number of different infectious diseases that occur both in the community and in the hospital environment. The bacterial species exhibits a high degree of genetic heterogeneity and differs in the repertoire of genes that code for virulence factors. On the genetic level, the species S. aureus can be divided into different clonal complexes (CCs) or sequence types (STs) using modern typing methods. While the genetic composition of CCs/STs circulating in society or the hospital environment is well studied for the Western world, our knowledge of the genetic composition of CCs/STs circulating in the community/healthcare system is quite fragmentary for many regions of Africa. Similarly, very few phenotypic studies exist on the virulence characteristics of S. aureus isolates circulating on this continent. Therefore, one of the primary objectives of this PhD thesis was to collect information on the genetic composition of S. aureus strains circulating in Tanzania and their virulence potential. A second aim of this dissertation was to investigate whether there are differences in the composition and virulence potential of S. aureus isolates circulating asymptomatically in the community or causing infections in Tanzania, and which antibiotic resistances can be found in these isolates. For this purpose, I collected S. aureus isolates that on the one hand led to community-acquired infections in the Bagamoyo area and on the other hand originated from asymptomatic (nasal) carriers. These isolates were subsequently genotypically characterized using DNA microarray technology, and a representative selection of these isolates were phenotypically examined using various test methods with regard to their virulence potential and antibiotic resistance profile. For the latter investigations, a selection of isolates collected and genotypically characterized by the African-German StaphNet Consortium was added. These investigations revealed that the S. aureus strain repertoire circulating in the Bagamoyo area of Tanzania is very heterogeneous on the genotypic level. At the CC level, CCs 152, 121, 8, 15, 88, and 5 were the most commonly found CCs in this region, each accounting for between 13 and 9%. Interestingly, the two most frequently found CCs, CC152 (13%) and CC121 (12%), were preferentially isolated from infection, while isolates of CCs 15 and 8 were mainly obtained from asymptomatic carriers. Concerning the virulence factor repertoire, larger differences could be identified between the individual isolates, but also at the CC level, which were particularly striking with regard to the repertoire of cell-damaging toxins such as hemolysins and leukocidins. For example, the majority of isolates of CCs 152, 121, 88, 80 and 30 harbored the lukF-PV and lukS-PV genes coding for the Panton-Valentine leukocidin (PVL) subunits F and S, respectively, whose gene products are thought to play an important role in the development of necrotic S. aureus infections. Moreover, unlike almost all other isolates of this strain collection, CC152 isolates harbored an intact hlb gene, whose gene product, -hemolysin, plays an important role in S. aureus biofilm formation and host tissue damage, while the gene seb, which codes for enterotoxin B, was preferentially found in isolates of CC121. Interestingly, all four virulence genes were detected significantly more frequently in infection-associated isolates than in isolates obtained from nasal swabs. The resistance gene profiles of S. aureus isolates circulating in Bagamoyo, Tanzania, showed a low prevalence for the methicillin-resistance mediating gene mecA (2.7%), but a very high prevalence for the penicillinase gene blaZ (99.6%) and fosB (74%), encoding a metallothiol transferase conferring resistance to fosfomycin. In addition, increased rates of the resistance determinants ermC (56%, mediating macrolide/linkosamide/streptogramin resistance) and tetK (47%, conveying tetracycline resistance) were detected in CC152 isolates. Phenotypic antimicrobial susceptibility testing confirmed the high penicillin-resistance rate (>90%), and revealed considerable frequencies of resistance to the commonly prescribed antibiotics erythromycin (29%), clindamycin (24%), and tetracycline (19%). My strain-specific phenotypic behavior typing studies of S. aureus isolates circulating in Tanzania and from the African-German StaphNet Consortium revealed that isolates of CC152 had a higher virulence potential than isolates of other CCs in almost all test procedures. Treatment of human erythrocytes with supernatants of CC152 isolates resulted in hemolysis titers that were higher than those induced by isolates of other CCs tested. Similarly, treatment of human keratinocytes with supernatants of CC152 isolates resulted in significantly more cell damage than with other CCs tested. However, when the virulence potentials of the isolates were correlated with their origin, in most of the assays carried out, no clear difference was found with regard to whether the isolates were obtained from infection or isolated from asymptomatic donors. Taken together, these studies indicate that infections caused by CC152 isolates represent an increased risk for the patient to develop a more severe course of disease. However, the lack of clear differences in virulence potential between isolates recovered from clinical infection and those recovered from asymptomatic donors suggests that commensal isolates have a similar potential to infect humans as infection-related isolates. The very high frequencies of blaZ and fosB in Tanzanian S. aureus isolates indicate that antibiotics such as penicillin and fosfomycin are widely used on the community level in this geographical region.Das Gram-positive Bakterium Staphylococcus aureus ist ein häufiger Kolonisierer von Säugetieren und beim Menschen insbesondere im Nasen-Rachenraum, auf der Haut und im Darm vorzufinden. Gleichzeit ist dieses Bakterium ein gefürchteter Verursacher einer Vielzahl von verschiedenen Infektionskrankheiten, die sowohl in der Gesellschaft als auch im Krankenhausumfeld auftreten. Die Bakterienart selbst weißt eine hohe genetische Heterogenität auf und unterscheidet sich insbesondere in Hinblick auf das Repertoire an Genen, die für Virulenzfaktoren codieren. Die Spezies S. aureus lässt sich mit modernen Typisierungsverfahren in verschiedene klonale Komplexe (CCs) bzw. Sequenztypen (STs) unterteilen. Während für die westliche Welt die genetische Zusammensetzung der in der Gesellschaft oder im Krankenhausumfeld kursierenden CCs/STs gut untersucht ist, ist unser Wissen um die genetische Zusammensetzung der in vielen Regionen Afrikas in der Gesellschaft und im Gesundheitswesen kursierenden CCs/STs noch sehr fragmentarisch. Ebenso existieren nur sehr wenige phänotypische Untersuchungen zu den Virulenzeigenschaften der auf diesem Kontinent kursierenden S. aureus Isolate. Daher war es eines der vorrangigen Ziele dieser Doktorarbeit, Informationen über die genetische Zusammensetzung der in Tansania kursierenden S. aureus Stämme und deren Virulenzpotential zu sammeln. Des Weiteren sollte im Rahmen dieser Dissertation untersucht werden, ob es Unterschiede in der Zusammensetzung und im Virulenzpotential der in Tansania in der Gesellschaft asymptomatisch kursierenden bzw. infektionsauslösenden S. aureus Isolate gibt und welche Antibiotikaresistenzen in diesen Isolaten vorzufinden sind. Um diese Ziele zu erreichen, wurden von mir S. aureus Isolate gesammelt, die zum einen in der Region Bagamoyo zu aus der Gesellschaft erworbenen Infektionen führten und zum anderen von asymptomatischen Trägern stammten. Diese wurden nachfolgend mithilfe der DNA-Microarray-Technologie genotypisch charakterisiert und eine repräsentative Auswahl dieser Isolate mittels verschiedener Untersuchungsmethoden phänotypisch in Hinblick auf ihr Virulenzpotential und ihr Resistenzprofil hin untersucht. Für letztere Untersuchungen wurde des Weiteren auch auf eine Auswahl von Isolaten zurückgegriffen, die über das Afrikanisch-Deutsche StaphNet-Konsortium gesammelt und genotypisch charakterisiert wurden. Diese Untersuchungen zeigten, dass das in der Region Bagamoyo, Tansania, kursierende S. aureus Kollektiv, genotypisch betrachtet, insgesamt sehr heterogen ist. Auf klonaler Ebene waren in dieser Region vor allem die CCs 152, 121, 8, 15, 88 und 5 vorzufinden, deren Anteil jeweils zwischen 13 und 9% betrug. Interessanterweise wurden die beiden am häufigsten vorgefundenen CCs, CC152 (13%) und CC121 (12%), bevorzugt aus dem Infektionsgeschehen heraus isoliert, während Isolate der CCs 15 und 8 mehrheitlich von asymptomatischen Trägern gewonnen wurden. Auch in Hinblick auf das Virulenzfaktor-Repertoire konnte zwischen den einzelnen Isolaten, aber auch auf CC-Ebene, zum Teil große Unterschiede identifiziert werden, die insbesondere in Hinblick auf das Repertoire an zellschädigenden Toxinen wie Hämolysine und Leukozidine auffällig waren. So wiesen Isolate der CCs 152, 121, 88, 80 und 30 mehrheitlich die für die Panton-Valentine Leukozidin Untereinheiten F und S kodierenden Gene lukF-PV and lukS-PV auf, deren Genprodukte eine wichtige Rolle bei der Entstehung nekrotischer S. aureus Infektionen zugeschrieben wird. Isolate des CCs 152 wiesen zudem, anders als nahezu alle anderen Isolate dieses Stammsets, ein intaktes hlb Gen auf, dessen Genprodukt, das -Hämolysin, wichtige Funktion bei der Biofilmbildung von S. aureus und der Schädigung von Wirtsgewebe übernimmt, während das Gen seb, welches für das Enterotoxin B kodiert, bevorzugt in Isolaten des CCs 121 vorgefunden wurde. Spannenderweise wurden alle vier Gene deutlich häufiger in Infektions-assoziierten Isolaten detektiert als in Isolaten, die aus Nasenabstrichen gewonnen wurden. In Hinblick auf die Resistenzgenprofile der in Bagamoyo, Tansania, kursierenden S. aureus Isolate zeigte sich eine niedrige Prävalenz für das Methicillinresistenz-vermittelnde Gen mecA (2.7%), aber sehr hohe Prävalenzen für das Penicillinase-Gen blaZ (99.6%) und für fosB (74%), welches für eine Metallothiol-Transferase kodiert, die eine Fosfomycinresistenz vermittelt. In CC152 Isolaten wurden des Weiteren erhöhte Raten der Resistenzdeterminanten ermC (56 %, vermittelt eine Resistenz gegenüber Makrolid/Linkosamid/Streptograminantibiotika) und tetK (47 %, vermittelt eine Resistenz gegenüber Tetracyclinen) festgestellt. In meinen phänotypischen Untersuchungen der in Tansania kursierenden S. aureus Isolate zeigte sich, dass Isolate des CCs 152 in nahezu allen Testverfahren ein höheres Virulenzpotential aufwiesen als die Isolate anderer CCs. So führte die Behandlung von menschlichen Erythrozyten mit Überständen von CC152 Isolaten zu einem Hämolyse-Titer, der sich mehrheitlich signifikant von den Hämolyse-Titern unterschied, die mit den Isolaten der anderen CCs ermittelt wurden. Des Weiteren führte die Behandlung von humanen Keratinozyten mit Überständen der CC152 Isolaten zu einer deutlich stärkeren Zellschädigung, wenn mit der Zellschädigung anderer CCs verglichen. Wurden hingegen die Virulenzpotentiale der Isolate mit deren Ursprung korreliert, so zeigte sich in der überwiegenden Mehrheit der Untersuchungen keine klaren Unterschiede in Hinblick darauf, ob die Isolate aus dem Infektionsgeschehen heraus isoliert wurden oder von asymptomatischen Spendern gewonnen wurden. Die antimikrobiellen Empfindlichkeitstestungen bestätigten die hohe Penicillinresistenzrate (>90%) und brachten eine erhöhte Häufigkeit von Resistenzen gegenüber den in Tansania häufig im Klinikalltag genutzten Antibiotika Erythromycin (29%), Clindamycin (24%) und Tetracyclin (19%) zum Vorschein. Zusammengenommen weisen diese Untersuchungen darauf hin, dass Infektionen, die durch CC152 Isolate verursacht werden, ein erhöhtes Risiko für den Patienten darstellen, einen schwereren Krankheitsverlauf zu entwickeln. Das Fehlen eindeutiger Unterschiede im Virulenzpotenzial zwischen aus klinischer Infektion gewonnener Isolate und aus der Nase von asymptomatischen Spendern gewonnener Isolate lässt jedoch darauf schließen, dass kommensale Isolate ein vergleichbares Infektionspotenzial für den Menschen aufweisen wie klinische Isolate. Das häufige Vorkommen von blaZ und fosB in den von mir untersuchten Stämmen aus der Region Bagamoyo, Tansania, weist darauf hin, dass Antibiotika wie Penicillin und Fosfomycin in dieser geographischen Region im Haushaltsgebrauch sehr häufig Verwendung finden

    Boosting the engraftment of subcutaneously transplanted pancreatic islets by nanofat

    Aims In the therapy of type 1 diabetes mellitus, the subcutaneous space has been suggested to be a clinically preferable transplantation site for pancreatic islets due to its easy accessibility. However, its poor vascularisation capacity and, thus, challenging environment typically result in islet engraftment failure. In the present proof-of-principle study, we demonstrate that this problem can be overcome by nanofat, an emulsified fat derivative already used in clinical practice. Materials and Methods The cellular composition of nanofat was assessed by immunohistochemistry. The angiogenic activity of the soluble and cellular nanofat fraction was analyzed by an angiogenic protein array, tube formation and spheroid sprouting assays. The viability and endocrine function of islets exposed to the nanofat fractions was investigated by flow cytometry, qRT-PCR and ELISA. In vivo, islets and nanofat were co-transplanted under the kidney capsule as well as into the subcutaneous space of diabetic animals. Results In a panel of in vitro assays, we showed that the soluble and cellular nanofat fraction improve the viability, hormone release, and angiogenic activity of islets. The beneficial effects of these two fractions were validated in vivo in the murine diabetic kidney capsule model, as indicated by an accelerated restoration of normoglycaemia. The co-transplantation of islets with nanofat resulted in successful islet engraftment within the subcutaneous space of diabetic mice. Conclusions These findings demonstrate that nanofat markedly boosts the vascularisation and endocrine function of islet grafts. Hence, its co-transplantation with pancreatic islets represents a simple, clinically feasible approach to make the subcutaneous space available for future islet transplantation

    Immune dysregulation as a key driver of peripartum cardiomyopathy - an exploratory advanced imaging and biomarker study

    Aims Peripartum cardiomyopathy (PPCM) is an idiopathic cardiomyopathy occurring in women in the late stages of pregnancy or in the postpartum period and is associated with significant morbidity, mortality, and persistent left ventricular dysfunction. PPCM pathogenesis involves multiple putative mechanisms including inflammation. We aimed to explore the acute inflammatory processes in PPCM using 18F-fluorodeoxyglucose positron emission tomography-computed tomography (18FDG-PET-CT), cardiovascular magnetic resonance (CMR), and inflammasome profiling. Methods and results Women with a new diagnosis of PPCM (n = 10, all within 3 months postpartum), five healthy postpartum controls (HPC), and five healthy non-postpartum controls (HNPC), underwent 18FDG-PET-CT, CMR, and serum inflammatory proteomic profiling. PPCM patients had a median age of 34 years (interquartile range [IQR] 30.3–38.5, similar in control groups), and a median parity of 3 (IQR 1–4). PPCM patients presented with severe, symptomatic heart failure (all New York Heart Association functional class III/IV), reduced median left ventricular ejection fraction of 35.5% (IQR 18.1–37.9). PPCM and HPC groups showed higher myocardial and splenic 18FDG uptake compared to HNPC. On CMR, myocardial interstitial fibrosis (elevated T1 time, extracellular volume, and late gadolinium enhancement mass) was solely present in PPCM. Inflammatory profiling showed pro-inflammatory cytokine dysregulation in PPCM (elevated neutrophil-to-lymphocyte ratio, C-reactive protein, interleukin-6, tumour necrosis factor-α, chemokine (C-C motif) ligand 3, hepatocyte growth factor, chemokine (C-X-C motif) ligand 10 and colony-stimulating factor-1) compared to controls. Conclusions Patients with PPCM exhibited a dysregulated immune response, associated with early myocardial interstitial fibrosis and adverse cardiac remodelling. This highlights the importance of rapid initiation of guideline-directed medical therapy especially with drugs documented to have anti-fibrotic effects

    Application of a 2D Hydrodynamic Numerical Model for Heavy Precipitation-Induced Soil Erosion

    Soil erosion, particularly when intensified by heavy precipitation, is a natural process and a persistent challenge in agricultural management. To date, this problem has been addressed using existing erosion models. However, these models often rely on simpli fied hydraulic approaches, whereas two-dimensional (2D) hydrodynamic numerical mod els are state-of-the-art for overland flow simulations. In this study, the combination of a 2D model and a soil erosion approach allowed for a more precise consideration of the hydraulics. The Govers approach was used with the 2D HydroAS model and evaluated using experimental plot data and naturally occurring field-scale erosion data, sourced from the literature. Results indicate that the combined model simulated sheet erosion and pro duced reasonable estimates for small rills using the Govers transport capacity approach. However, larger rills require calibration of this method. Additionally, the resolution of the digital elevation model (0.25 m) used as the basis for the simulation was of great importance to avoid overestimating smaller rills. Sensitivity analysis revealed that these smaller rills were particularly influenced by the input grain diameter and surface rough ness. Comparisons with other erosion models underscore that incorporating an improved hydraulic approach and adapting the topography at each simulation time step enhances estimation of the spatial distribution and quantity of erosion of the rills. Knowledge about the occurrence and quantification of rill erosion can help planners develop geoecological solutions for flooding and erosion
