    Expanding the scope of PSMA-RLT: evaluating treatment in challenging mCRPC patients with poor performance status (ECOG 3)

    Purpose Given the increasing inclusion of ECOG 3 patients in oncology practice, data on this subgroup in the context of prostate-specific membrane antigen (PSMA)-targeted radioligand therapy (RLT) remain limited. This study evaluates the safety and outcome of PSMA-RLT in metastatic castration-resistant prostate cancer (mCRPC) patients with ECOG perfor mance status 3. Methods In this analysis, a cohort of 18 mCRPC patients with ECOG performance status 3 who received PSMA-RLT was examined. The median number of treatment cycles was 2 (range: 1–10), with a mean cumulative administered activity of 21.5±15.0 GBq (range: 2.7–62.6 GBq) of [177Lu]Lu-PSMA-617. Outcome and adverse events including hematologic and renal toxicities, fatigue, and xerostomia were analyzed. Results 50% of patients achieved either stable disease or a partial biochemical response. Median progression-free survival and overall survival were 1.3 months and 2.8 months, respectively. Severe adverse events were uncommon, occurring in three patients: one developed grade 3 leukopenia, another experienced grade 3 thrombocytopenia, and one patient had pan cytopenia of grade 3/4. No higher RLT-induced grade of renal toxicity and xerostomia were observed, whilst symptoms of fatigue improved in the cohort. Conclusion This study indicates that PSMA-RLT is a feasible and overall well-tolerated treatment for mCRPC ECOG 3 patients with manageable toxicity profile. Despite limited survival outcomes, ECOG 3 status may be considered not to be a categorical exclusion criterion for RLT. Future prospective studies should further investigate the role of PSMA-RLT in this challenging subgroup

    Understanding noise correlations in generative models for graphics content generation and editing

    Graphics content plays an essential role in our daily lives, spanning diverse media forms such as images, videos, textures, point patterns, and 3D objects. Despite their ubiquity, limited research has investigated how the concept of noise correlation across these varied representations can have an impact on controllable graphics generation and editing aligned with user intent. This thesis explores how noise correlations can inform and simplify the process of generating and editing visual content. We focus in particular on the synthesis and editing of point patterns, images, and stereo videos. Our primary contributions lie in developing novel pipelines that leverage noise correlations to improve both fidelity and controllability in these domains. Specifically, we propose: (1) a training-free holistic feature extraction pipeline inspired by noise correlation for point pattern synthesis, (2) a decoupled representation and correlation embedding space for user-friendly point pattern editing, (3) a new perspective on integrating blue noise correlations into the training and sampling of diffusion models, and (4) a unified pipeline with noisy degraded data augmentation for fine-tuning diffusion models to enable simultaneous stereo video generation and restoration. Our methods demonstrate the capability of outperforming existing state-of-the-art techniques and highlight the untapped potential of noise structure as a guiding principle in visual synthesis. These findings open new avenues for controllable and high-quality graphics content generation and editing.Grafische Inhalte spielen eine zentrale Rolle in unserem Alltag und erscheinen in vielfältigen Medienformen wie Bildern, Videos, Texturen, Punktmustern und 3D-Objekten. Trotz ihrer Allgegenwärtigkeit wurde bisher kaum erforscht, wie das Konzept der Rauschkorrelationen über diese unterschiedlichen Repräsentationen hinweg die kontrollierbare Generierung und Bearbeitung grafischer Inhalte im Einklang mit Benutzerabsichten beeinflussen kann. Diese Dissertation untersucht, wie Rauschkorrelationen den Prozess der Generierung und Bearbeitung visueller Inhalte vereinfachen und verbessern können. Im Fokus stehen dabei insbesondere die Synthese und Bearbeitung von Punktmustern, Bildern und Stereovideos. Der zentrale Beitrag liegt in der Entwicklung neuartiger Pipelines, die Rauschkorrelationen nutzen, um sowohl die Qualität als auch die Steuerbarkeit in diesen Bereichen zu erhöhen. Im Einzelnen schlagen wir vor: (1) eine trainingsfreie Pipeline zur holistischen Merkmalsextraktion, inspiriert von Rauschkorrelationen, für die Synthese von Punktmustern, (2) eine entkoppelte Repräsentation und einen Korrelations-Embedding-Raum für benutzerfreundliche Punktmusterbearbeitung, (3) eine neue Perspektive zur Integration von Blue-Noise-Korrelationen in das Training und Sampling von Diffusionsmodellen sowie (4) eine einheitliche Pipeline mit augmentierten, verrauschten Daten zur Feinabstimmung von Diffusionsmodellen für die gleichzeitige Generierung und Restaurierung von Stereovideos. Unsere Methoden zeigen eine Überlegenheit gegenüber aktuellen State-of-the-Art-Techniken und unterstreichen das bisher ungenutzte Potenzial von Rauschstrukturen als richtungsweisendes Prinzip in der visuellen Synthese.     Optimization of dielectric elastomer actuators: Advanced manufacturing, electrical failure analysis, and innovative repair method

    Over the past twenty years, dielectric elastomers (DE) have been identified as a highly promising concept for next-generation actuators that could replace conventional electro-magnetic, pneumatic, and hydraulic systems. Their self-sensing capability, high energy efficiency, and suitability for soft robotics make them particularly attractive. To advance from lab-scale demonstrators toward industrial applications, cost-efficient and scalable manufacturing methods and improved reliability are essential. This thesis presents the first systematic investigation of screen printing parameters for applying electrodes to silicone thin films, aiming to characterize and optimize an industry-grade process with respect to key electro-mechanical performance. It further provides a detailed study of electrical breakdown to determine operational limits of currently used silicone thin films, focusing on the influence of temperature, humidity, and pre-stretch. The analyses revealed a significant effect of electrode materials on breakdown strength, motivating an additional comparative study of various electrodes reported in literature. All three subjects resulted in peer-reviewed journal publications integrated into this thesis in a cumulative way. A fourth, patent-pending contribution introduces a novel test and repair method for DEs during fabrication, enabling reliable operation at a defined voltage and significantly improving breakdown behavior and operability of next-generation DE actuators.Dielektrische Elastomere (DE) wurden in den letzten zwanzig Jahren als vielversprechende Technologie für neuartige Aktorsysteme identifiziert, die konventionelle elektromagnetische und hydraulische Technologien ersetzen könnten. Ihre hohe Energieeffizienz, integrierte Sensorsignale und das Potenzial für „Soft Robotics“ wurden bereits in diversen Forschungsprojekten demonstriert. Für den Schritt vom Labormaßstab zur industriellen Anwendung werden skalierbare, wirtschaftliche Herstellungsverfahren und eine optimierte Betriebszuverlässigkeit erforderlich. Diese Dissertation präsentiert erstmals eine systematische Untersuchung des Siebdrucks zur Applikation von Elektroden auf Silikon-Dünnfilme und charakterisiert das Verfahren im Hinblick auf relevante elektro-mechanische Leistungskennwerte. Darauf aufbauend wird das elektrische Durchschlagverhalten unter Variation von Temperatur, Luftfeuchtigkeit und Vordehnung analysiert, um die Betriebsgrenzen aktuell verwendeter Silikon-Dünnfilme zu bestimmen. Der dabei erkannte starke Einfluss der Elektroden führte zu einer erweiterten Studie verschiedener Elektrodenmaterialien und Herstellungsverfahren. Alle drei Themenbereiche führten zu wissenschaftlich begutachteten Publikationen, die in kumulativer Form in diese Dissertation eingebunden sind. Ergänzend stellt die Arbeit eine patentangemeldete Test- und Reparaturmethode vor, die die Fertigung zuverlässiger DE ermöglicht und die Einsatzgrenzen künftiger Aktorsysteme deutlich erweitert

    Comparison of postpartum antibody levels in umbilical cord blood following pertussis vaccination during pregnancy

    Bordetella pertussis ist ein hochinfektiöses, gramnegatives Bakterium, das die Infektionskrankheit Keuchhusten verursacht. Trotz umfassender Impfprogrammen treten in Deutschland jährlich viele Keuchhustenfälle auf. Besonders gefährdet sind dabei Säuglinge durch schwere und komplikationsreiche Verläufe. Seit 2020 empfiehlt die Ständige Impfkommission die Impfung aller Schwangeren im dritten Trimenon. Der dadurch entstehende Nestschutz wird durch transplazentare Übertragung von Immunglobulin G ab der 17. Schwangerschaftswoche aufgebaut. Eine Antibiotikatherapie und eine antenatale Kortikosteroidtherapie können die Impfreaktion abschwächen. Ziel der Studie ist es, den Einfluss dieser Therapien auf die Wirksamkeit der Pertussis-Impfung und den Nestschutz zu untersuchen und potenzielle Korrelationen zwischen maternalen und kindlichen Antikörperleveln sowie weiteren Einflussfaktoren zu identifizieren. Insgesamt wurden im Rahmen einer prospektiven monozentrischen Querschnittsstudie die Antikörperlevel im Blut von 50 Schwangeren und 55 Kindern nach Tdap-Auffrischimpfung während der Schwangerschaft untersucht. Postpartal wurden venöse Blutproben aus der Nabelschnur und einer Armvene der Mutter gewonnen. Die Proben wurden mittels Chemilumineszenz-Immunoassay auf Immunglobulin G Titer gegen den Virulenzfaktor Pertussis Toxin analysiert. Im Rahmen unserer Analyse wurde ein signifikanter Zusammenhang zwischen maternalen und kindlichen Antikörperleveln (p < 2,2 × 10–16) mit einem prozentuale plazentare Antikörpertransfer von 48,5% bis 404,2% festgestellt. Bei mütterlichen Antikörperspiegeln von über 100 IU/ml wurden die höchsten Antikörperwerte im Nabelschnurblut gemessen. Die Proben mit einer antenatalen Kortikosteroidtherapie zeigten einen signifikant höheren Mittelwert der maternalen Antikörper (p = 0,028) von 61,12 IU/ml und der kindlichen (p = 9,974 x 10-16) von 73,97 IU/ml , während ohne diese Therapie ein niedrigerer Mittelwert von 34,79 IU/ml und 49,82 IU/ml gemessen wurde. Ebenso war die Antibiotikatherapie mit erhöhten maternalen (55,79 IU/ml gegenüber 34,97 IU/ml; p = 0,027) und kindlichen (73,97 IU/ml gegenüber 49,82 IU/ml; p = 9,974 × 10–16) Antikörperspiegeln assoziiert. Eine hohe Rate an Doppeltherapien mit Antibiotika und Kortison erschwerte die eindeutige Zuordnung des Einflusses. Die Impfung erfolgte im Mittel in der 30. Schwangerschaftswoche, wobei ein Abstand von 45 bis 90 Tagen zwischen Impfung und Entbindung signifikant zu den höchsten kindlichen Antikörperspiegeln führte (p < 2,2 × 10–16). Zusätzlich beeinflussten auch das maternale Alter signifikant die Antikörperlevel von Mutter (p = 0,002) und Kind (p = 7,76 x 10-16), sowie maternales Übergewicht (p = 0,032) die des Kindes. Zusammenfassend konnten wir einen signifikanten Einfluss von Antibiotikatherapie, antenataler Steroidtherapie, Impfzeitpunkt und Abstand zwischen Impfung und Geburt auf die Antikörperspiegel nachweisen. Zudem wirken sich maternales Alter und Übergewicht negativ auf die Antikörperwerte im Nabelschnurblut aus. Unsere Ergebnisse legen nahe, dass diese Faktoren während der Schwangerschaft bedacht und individuell optimiert werden sollen, um den bestmöglichen Nestschutz zu gewährleisten.Bordetella pertussis is a highly infectious, Gram-negative bacterium that causes whooping cough. Despite extensive vaccination programs, many cases of whooping cough occur in Germany every year. Infants are particularly at risk due to severe and complicated disease courses. Since 2020, the Standing Committee on Vaccination has recommended that all pregnant women be vaccinated during the third trimester to protect their newborns. Maternal passive immunity is established through transplacental transfer of immunoglobulin G from the 17th week of pregnancy onwards. However, antibiotic treatments and antenatal corticosteroid therapies may impair the immune response to vaccination. The aim of this study was to explore how these therapies affect the effectiveness of pertussis vaccination and the resulting passive immunity. We also sought to identify potential correlations between maternal and infant antibody levels, as well as other influencing factors. In a prospective, single-center, cross-sectional study, we measured antibody levels in the blood of 50 women and 55 infants following Tdap booster vaccination during pregnancy. Postpartum, venous blood samples were taken from the umbilical cord and the mother. The samples were analyzed and antibody titers against the pertussis toxin were quantified using a chemiluminescence immunoassay. Our analysis revealed a significant correlation between maternal and infant antibody levels (p < 2.2 × 10–16) with a placental antibody transfer of 48.5% to 404.2%. The highest antibody concentrations in the umbilical cord blood were measured when maternal antibody levels exceeded 100 IU/ml. The samples with antenatal corticosteroid therapy showed a significantly higher mean level of maternal (61.12 IU/ml versus 34.79 IU/ml; p = 0.028) and fetal antibodies (73.97 IU/ml versus 49.82 IU/ml; p = 9.974 x 10-16) than without this therapy. Similarly, antibiotic therapy was also associated with elevated maternal (55.79 IU/ml versus 34.97 IU/ml; p = 0.027) and fetal (73.97 IU/ml versus 49.82 IU/ml; p = 9.974 × 10–16) antibody levels. Due to the high proportion of cases involving combined antibiotic and corticosteroid treatment, it was challenging to clearly isolate the effects of each therapy. On average, the vaccination was given in the 30th week of pregnancy. An interval of 45 to 90 days between vaccination and delivery was linked to significantly higher infant antibody levels (p < 2.2 × 10–16). Additionally, maternal age had a significant impact on antibody levels in both mothers (p = 0.002) and newborns (p = 7.76 x 10-16), while maternal obesity affected only the antibodies of the umbilical cord blood (p = 0.032). In summary, our findings highlight that antibiotic therapy, antenatal corticosteroid treatment, timing of vaccination, and the interval between vaccination and birth all significantly influence antibody levels. Moreover, maternal age and obesity appear to negatively affect the transfer of antibodies to the infant. These results suggest that such factors should be carefully considered and managed during pregnancy to maximize neonatal protection against pertussis

    Morphological and functional aspects and quality of life in patients with achromatopsia

    Purpose: Achromatopsia is a very rare disease encompassing complete colour blindness, severe glare sensitivity and visual impairment. The natural course of the disease and impact on life are still not completely understood to this date. Here, we aimed to assess the morphological, functional characteristics, and quality of life in a large sample size of patients with achromatopsia. Methods: A total of 94 people with achromatopsia were included in this retrospective cohort study. Sixty-four were patients of the Department of Ophthalmology, Saarland University Medical Centre in Homburg/Saar, Germany, between 2008 and 2021. Thirty further participants with achromatopsia from the German national support group (https://www.achromatopsie.de) were included using an online questionnaire. Statistical analysis was performed using SPSS Version 25. Results: The 94 patients (37 males (39.4%) and 57 females (60.6%)) showed a mean age of 24.23 ± 18.53 years. Visual acuity was stable (1.0 logMAR ± 0.2) over a time of observation from 2008 to 2021. Refractive error was comparable to healthy subjects, with the mean spherical refraction being +1.83 diopters ± 3.76 and the mean astigmatic refraction being -1.86 diopters ± 0.88. However, it did not correlate with visual acuity. Pathological findings were seen in the anterior segments and the optic nerve head only in later adult years. The mean age of diagnosis established was 6.49 ± 1.50 years. The type of school visited (mainstream versus specialized low vision schooling) did not influence academic achievements. Edge filter glasses are the most used optical aids while reading glasses are the most used low vision aids. No genotype-phenotype correlation was found when both, functional and anatomical features were taken into account. Conclusion: Our findings give a first insight into describing the natural development and the quality of life of achromatopsia. The results demonstrate that achromatopsia is a predominantly stationary disease with small morphological changes over time. Academic performance does not seem to be affected by achromatopsia. The individual prescription of edge filters and low-vision aids is essential following a personalised fitting.Ziel: Achromatopsie ist eine seltene Krankheit, deren natürlicher Verlauf und Auswirkungen auf das Leben bis zum heutigen Datum noch weitgehend unbekannt sind. Hier werden wir die morphologischen, funktionellen Merkmale und die Lebensqualität bei einer großen Stichprobe von Patienten mit Achromatopsie analysieren. Methoden: In diese retrospektive Kohortenstudie wurden insgesamt 94 Personen mit Achromatopsie aufgenommen. 64 davon waren Patienten der Klinik für Augenheilkunde des Universitätsklinikums des Saarlandes in Homburg/Saar, Deutschland, zwischen 2008 und 2021. Weitere 30 Teilnehmer mit Achromatopsie aus der nationalen Selbsthilfegruppe wurden mithilfe eines Online-Fragebogens einbezogen. Die statistische Analyse wurde mit SPSS-Version 25 durchgeführt. Ergebnisse: Die 94 Patienten (37 Männer (39,4%) und 57 Frauen (60,6%)) zeigten ein Durchschnittsalter von 24,23 ± 18,53 Jahren. Die Sehschärfe war über einen Beobachtungszeitraum von 2008 bis 2021 stabil (1,0 logMAR ± 0.2). Eine Refraktionsanomalie war häufig, korrelierte jedoch nicht mit der Sehschärfe. Pathologische Befunde wurden nur in fortgeschrittenem Alter in den vorderen Augenabschnitten und am Sehnervenkopf beobachtet. Das Durchschnittsalter bei der Diagnosestellung betrug 6,49 ± 1,50 Jahre. Die besuchte Schulart (Regelschule versus Schule mit Förderbedarf Sehen) beeinflusste nicht die schulischen Leistungen. Kantenfilterbrillen sind die am häufigsten verwendeten optischen Hilfsmittel, während Lesebrillen die am häufigsten verwendeten Sehhilfen sind. Es wurde keine Genotyp-Phänotyp-Korrelation, sowohl hinsichtlich funktionaler als auch anatomischer Merkmale, gefunden. Diskussion: Unsere Ergebnisse geben erstmals Einblick in die Beschreibung des natürlichen Verlaufs und der Lebensqualität bei Achromatopsie. Die Ergebnisse zeigen, dass es sich bei Achromatopsie um eine überwiegend stationäre Erkrankung mit geringen morphologischen Veränderungen im Laufe der Zeit handelt. Die schulische Leistung scheint von Achromatopsie nicht beeinflusst zu werden. Die individuelle Verschreibung von Kantenfiltergläsern oder -kontaktlinsen und Refraktionskorrekturen, sowie Sehhilfenverordnungen ist nach einer personalisierten Anpassung unerlässlich

    Ziel dieser Studie war es, die Auswirkung zweier verschiedener intraoperativer CO2 Gasdrücke während laparoskopischer Hysterektomien auf Grund benigner uteriner Pathologien, im Hinblick auf postoperative Bauch- und Schulterschmerzen, den Schmerzmittelverbrauch, vegetativen Veränderungen, den arteriellen pCO2, sowie die Sicherheit zu vergleichen. Eingeschlossen in die Studie wurden Patientinnen, die sich einer laparoskopischen Hysterektomie wegen gutartiger Gebärmuttererkrankungen unterziehen mussten. Die Patienten wurden randomisiert in zwei Gruppen, eine mit Standarddruck (15mmHg), und eine Gruppe mit Low-Pressure (8mmHg). Primär untersucht wurden die postoperativen Schulter- und Bauchschmerzintensitäten, gemessen über die numerische Analogskala. Des Weiteren wurden vegetative Parameter wie Müdigkeit, Übelkeit, Erbrechen, Meteorismus betrachtet, jeweils nach drei, 24, und 48 Stunden postoperativ. Sekundär wurde der Schmerzmittelverbrauch und der arterielle CO2 Partialdruck untersucht. Zusätzlich wurde die Sicherheit mittels Rate an intra- und postoperativen Komplikationen erfasst. Insgesamt nahmen an der Studie 187 Patientinnen teil. In der Low-Pressure Gruppe (LP) befanden sich 91 Patientinnen in der Standard-Pressure Gruppe (SP) 87 Patientinnen. Die LP-Gruppe zeigte signifikant geringere postoperative Bauch- und Schulterschmerzen (NAS 1) im Vergleich zu der SP-Gruppe ((NAS 4)(p≤ 0.01)). Hinsichtlich des Schmerzmittelverbrauches zeigten sich signifikant geringere Dipidolor Verbrauchsmengen in der LP-Gruppe (6mg) im Vergleich zur SP-Gruppe ((17mg) SP auf (6mg) LP (p≤ 0.01)). Es konnte eine Reduzierung der postoperativen stationären Aufenthaltsdauer (3 Tage [2−6] LP zu 3 Tagen [1−10] SP, p ≤ 0, 01) und geringere intra- und postoperative pCO2-Werte beobachtet werden. Ab 30 Minuten nach Operationsbeginn zeigten sich bereits signifikant geringer Werte in der LP-Gruppe im Vergleich zu der SP-Gruppe (40, 1mmHg [Min 34mmHg - Max 45mmHg] LP zu 41mmHg [Min 35, 3mmHg - Max 55, 3mmHg] SP; ≤ 0.01). Auch am OPEnde zeigten sich signifikant geringere Werte (40, 9mmHg [Min 35, 1mmHg - Max 45, 1mmHg] LP zu 46, 9mmHg [Min 34mmHg - Max 58, 8mmHg] SP; ≤ 0.01).. Bei den intra- oder postoperativen Komplikationen zeigten sich keine signifikanten Unterschiede zwischen den beiden Gruppen. Die LowPressure Laparoskopie scheint eine effektive und sichere Technik zur Verringerung von postoperativen Schmerzen und Laparoskopieinduzierten metabolischen und vegetativen Veränderungen zu sein.The aim of this study was to determine the effects of two different intraoperative CO2 gas pressures during laparoscopic hysterectomies due to benign uterine pathologies, with regard to postoperative abdominal and shoulder pain, pain medication consumption, vegetative changes, the arterial pCO2, and the Compare security. The study included female patients who underwent laparoscopic hysterectomy for benign uterine diseases. Patients were randomly divided into two groups, one with standard pressure (15mmHg) and one with low pressure (8mmHg). The postoperative shoulder and abdominal pain intensities were primarily examined, measured on the numerical analog scale. Vegetative parameters such as fatigue, nausea, vomiting and meteorism were also considered. After 3, 24 and 48 hours postoperatively. Pain medication consumption and arterial CO2 partial pressure were examined secondarily. In addition, further attention was paid to surgical parameters such as intra and postoperative complications. A total of 187 patients took part in the study. There were 91 patients in the Low-pressure group compared to the Standard-Pressure group, which included 87 patients. The LP group showed significantly less postoperative abdominal and shoulder pain. Also less vegetative symptoms and less consumption of pain relievers. There was also a reduction in postoperative inpatient stays and lower intra- and postoperative pCO2 values. No differences were observed in the intra- or postoperative complications. Lowpressure laparoscopy appears to be an effective and safe technique for reducing postoperative pain and laparoscopy-induced metabolic and vegetative changes

    The influence of job demands and job resources on the health of migrant domestic workers: a diary study with Ugandan housemaids in Saudi Arabia

    Migrant domestic workers are mostly employed under precarious work conditions, yet little is known about the impact of their work on their health and wellbeing. Due to economic hardships and their low social status, the workers may endure such harsh conditions in exchange for a liveable wage. This study therefore utilized the Job Demands and Resources model’s health impairment process to explore this impact using a sample of Ugandan female migrant domestic workers in Saudi Arabia. Data were collected before leaving Uganda and through a biweekly diary method across four waves which lasted 8 weeks (Nparticipants = 172, Nobservations = 366). Results showed that family demands positively predicted emotional exhaustion and somatic complaints. Unexpectedly, work pressure, sexual harassment, and work precarity did not predict either out come. Job autonomy negatively predicted both emotional exhaustion and somatic complaints, whereas social support had a positive effect on both emotional exhaustion and somatic com plaints. Furthermore, core self-evaluation attenuated the negative relationship between job autonomy and emotional exhaustion. This study provides a foundation for further research on migrant domestic workers’ well-being, a unique group often employed under precarious working conditions
