不登校児童生徒が語る教育支援センターの安心感とその要因
Get PDF近年，不登校児童生徒数が増加する中，教育支援センターが果たす役割への期待が高まっている。本研究では，甲信越地方の教育支援センターに所属する小中学生11 名を対象に半構造化面接を実施し，修正版グラウンデッド・セオリー・アプローチを用い，対象者の経験から教育支援センターにおける安心感の要因とその形成プロセスを構築した。分析の結果，安心感を得るプロセスは，【関係性を築くきっかけ】から始まり，【自由と主体性の確保】，【安全とリラックスできる空間】，【スタッフのサポートと受容】を経て，【仲間とのつながり】へと向かう流れが明らかになり，安心感の要因をあらゆる場面に広げていく必要性を指摘した。departmental bulletin pape

Measuring the Effectiveness of Gatekeeper Training for Suicide Prevention: Focusing on the Self-Efficacy of Municipal Employees
Get PDF目的：A 自治体の特性をふまえたゲートキーパー（以下、GK）研修の有効性について、ゲートキーパー自己効力感尺度（以下、GKSES）を用いて評価を行うこと。方法：A 自治体職員64 名を対象に90 分のGK 研修を実施した。研修は講義60 分、A 自治体の保健師による自殺の実態および取り組みの説明10 分、動画視聴15 分で構成された。研修においては、A 自治体の特性をふまえ「学習の準備状態の促進」、「受講者の当事者性への配慮」、「GK 行動の実践促進」を目的とした工夫点を内容の中に取り入れた。効果測定の指標にはGKSES を用い、研修受講前後で回答を求めた。また、年代、性別、自殺予防に関する研修受講経験、自殺の可能性のある人への支援経験の有無を聴取した。研修の効果測定を目的に、研修受講前・受講後のGKSES の合計得点について対応のあるt 検定を、そして受講前・受講後のGKSES の各項目得点についてウィルコクソンの符号付順位和検定を実施した。結果：61 名から回答を得た（回収率95.3%）。GKSES の合計得点、および各項目の得点が、研修受講後に有意に上昇したことが示された。研修受講経験、支援経験がある者は受講前のGKSES 合計得点が30 点前後と比較的高い数値となっており、受講後の得点は有意に上昇しているものの、上昇得点は他の属性と比べ、小さいことが示された。考察：本研修がGK の役割遂行に関する包括的な自己効力感の向上に寄与したことが示唆された。「学習の準備状態の促進」と「受講者の当事者性への配慮」を目的とした工夫点が、学習に関する効果的な姿勢や環境の形成に、「GK 行動の実践促進」を目的とした工夫点が、GK を支えるフォロー体制の認識や、具体的な役割遂行のイメージ獲得を促し、結果として、自己効力感が向上した可能性があると考えられる。journal articl
A Case of Severe Mitral Stenosis with Very Small Left Ventricular Diameter Treated with 2-stage Mitral Valve Replacement after Percutaneous Transseptal Mitral Commissurotomy
No full textA 73-year-old woman was diagnosed with severe mitral stenosis five years ago but had not been to a hospital since then. Last year, she was referred to our cardiology department due to dyspnea on exertion caused by heart failure.An echocardiogram showed a mitral valve area of 0.37 cm2 and TR-PG of 86.2 mmHg, and she was diagnosed with severe mitral stenosis. Because of the extremely small left ventricular diameter (LVDd 29.8 mm, LVDs 20.9mm) we scheduled the patient for mitral valve replacement after performing PTMC and confirming that the left ventricular diameter had expanded.Three months after PTMC, echocardiography confirmed left ventricular diameter enlargement (LVDd 43.7mm, LVDs 28.2 mm, TR-PG).
