    Neutral buffered electrolytes guarantee ideal band-edge pinning for semiconductor photoanodes

    The photoelectrochemical (PEC) processes occurring in semiconductor photoelectrodes have been regarded as similar to the physical processes occurring at the semiconductor/metal interfaces. In contrast, the physicochemical processes occurring in the electrolyte have been considered to be unaffected by the (photo)electrode materials that are employed. We found that these “ideal” situations are not always guaranteed during performing the actual PEC reaction. That is, the present study based on impedance and hydrodynamic voltammetry analyses proposes that the band diagram at the interface between a semiconductor photoanode and an electrolyte can be affected by transient physicochemical phenomena in the electrolyte during the PEC oxygen evolution reaction. Specifically, in the case that a neutral unbuffered electrolyte was employed, a local pH gradient was formed during the reaction and produced a positive shift in the flat-band potential. This means the breakdown of the ideal band bending at the Schottky-like junction. Meanwhile, a neutral buffered phosphate-based electrolyte suppressed the formation of this pH gradient and thus guaranteed ideal band-edge pinning at the photoanode/electrolyte interface. This study provides insights demonstrating that PEC water splitting occurring at the semiconductor/electrolyte interface are distinct from simple analogy to the conventional semiconductor physics and to the physicochemical processes in the electrolyte.journal articl

    本研究では、A病院における術後疼痛管理チームが硬膜外鎮痛中の患者に対して実施した介入の実態を調査した。研究対象は、2019年10月から2023年1月までに硬膜外鎮痛を併用し手術を受けた患者で、結果として術後疼痛管理チームの介入があった患者は延べ254名で、延べ324件の介入件数であった。介入内容は「薬理学的介入」「PCAに関する患者教育介入」「カテーテルトラブル介入」の三つのカテゴリに分類された。分類の中で薬理学的介入が最も多いことにより、薬理的介入の重要性が明らかとなり、外科医師、病棟看護師に術後鎮痛プロトコールの浸透が不十分な可能性が示唆された。PCAに関する患者教育としては術後直後から開始するのではなく、術前からの情報提供が求められることが明らかになった。カテーテルトラブルヘの迅速な対応には、術後疼痛管理チームと病棟看護師との連携が不可欠であることが明らかとなった。本研究から術後疼痛管理の質を向上させるためには多職種間の連携の重要性が強調された。今後は多職種連携を強化し、より効果的な疼痛管理を実現することが期待される。departmental bulletin pape

    Identification of potential transcriptomic and microRNA markers to assess benralizumab effectiveness

    Background: Benralizumab targets IL-5 receptors and eliminates eosinophils, demonstrating its efficacy in type 2 inflammation-predominant asthma. Real-world evidence indicates variability in patient responses, emphasizing the need to understand the pathologic characteristics that influence its effectiveness. Objective: We postulated that patients with high and low benralizumab response would exhibit distinct pathophysiologic characteristics. We aimed to identify specific genes and microRNAs (miRNAs) as potential indicators of effectiveness-related characteristics. Methods: This prospective observational study included 17 patients with severe asthma undergoing benralizumab treatment. T-cell transcriptomic analysis was conducted before and 24 weeks after treatment initiation. To identify the genes associated with treatment efficacy, we identified differentially expressed genes (DEGs) and used a weighted gene coexpression network analysis. Additionally, serum miRNAs were analyzed quantitatively by using PCR arrays, and their correlation with gene clusters associated with treatment efficacy was investigated. Results: Transcriptomic analysis revealed that 223 DEGs were included in the response to benralizumab treatment. Weighted gene coexpression network analysis identified 21 coexpressed gene clusters, with most DEGs enriched in the 2 modules. One of these modules, the magenta module, was associated with the effectiveness of benralizumab treatment. Serum miRNA profiling identified 5 miRNAs associated with both treatment effectiveness and the magenta module (miR-7-5p, miR-155-5p, miR-320b, miR-342-3p, and miR-484). Among these, miR-7-5p showed the highest predictive accuracy for benralizumab effectiveness. Conclusion: This study recognized specific gene expression patterns and serum miRNAs associated with the effectiveness of benralizumab in treating severe asthma. These findings contribute to our understanding of the asthma endotypes and may improve patient selection for benralizumab treatment.journal articl

    A Case of Primary Intestinal Tuberculosis

    A 57-year-old women, foreigner residing in Japan, admitted to our hospital because of right lower abdominal pain. Whole computed tomography revealed a mass in the terminal ileum and oral intestinal distention without any pulmonary abnormal findings. Endoscopic examination of the lower digestive tract showed an irregularshaped mass in the terminal ileum causing stenosis. The pathological findings revealed the epithelioid cell granuloma accompanied by caseous necrosis. Although smear, PCR and culture for tuberculosis using the tissue were negative, interferon-gamma releasing assay was positive. Thus, the therapeutic diagnosis of intestinal tuberculosis was made and anti-tuberculosis therapy was initiated. The therapy had continued for one year, resulted in the complete remission in terminal ileum. Primary intestinal tuberculosis is extremely rare, however, physicians should be aware of the presence even in subjects without pulmonary involvement and immunodeficiency.
