Elevation of Parathyroid Hormone Levels after Surgery in Childhood-onset Primary Hyperparathyroidism : A Case of an 11-year-old Boy
Full text linkPrimary hyperparathyroidism (PHPT) is rare in children. The endocrinological management after parathyroidectomy in children is not well established. The patient reported here was an 11-year-old boy who underwent parathyroidectomy for PHPT. He started taking calcium lactate hydrate and alfacalcidol 6 days after surgery to prevent postoperative hypocalcemia. His intact parathyroid hormone (PTH) level subsequently increased over the 2 months after surgery, while his serum calcium levels were stable. Although we were concerned about recurrence, his intact PTH level subsequently decreased, and his bone mineral density (BMD) improved postoperatively. Three years after parathyroidectomy, his intact PTH level and BMD are normal, and he is growing normally. It was thought that his transient intact PTH elevation had been caused secondarily by insufficient calcium supplementation after surgery, as suggested by low urine calcium levels at that time. Fortunately, in our case, growth and BMD were not affected postoperatively. Nevertheless, in pediatric cases of PHPT, following up the postoperative endocrinological course is thought to be necessary to obtain healthy growth in terms of stature and bone.journal articl
青年の「むなしさ」体験による肯定的変化のプロセスに関する研究
Full text link本研究では，「むなしさ」を経験した結果生じる，個人内の肯定的変化のプロセスについて質的に検討することを目的とし，研究Ⅰでは質問紙調査，研究Ⅱでは，個人の自由表現による語りを収集・分析した。M-GTA による分析の結果，「むなしさ」は【理想と現実のギャップ】の実感が起点となって生じるが，【視野が広がる】経験を通じて，体験への【新たな意味づけ】がなされるという過程が示され，「むなしさ」は個人内の肯定的変化や，精神的健康に関連していることが示唆された。departmental bulletin pape
Minute Pulmonary Meningothelial-like Nodules : A Case Report
Full text linkMinute pulmonary meningothelial-like nodules (MPMNs) are traditionally identified incidentally as solitary or multiple nodules with a size of 0.1-3 mm in autopsied or resected lungs. Recently, there have been several reports of MPMNs identified as ground-glass nodules (GGNs) on computed tomography (CT), leading to surgical intervention. Here, we present a case of MPMN with a brief overview of the literature. A 73-year-old female patient underwent CT for the evaluation of aspergilloma, and multiple GGNs were identified in the lower lobes of both lungs. One of the GGNs, as well as the aspergilloma, was resected for pathological examination and was subsequently diagnosed as MPMN. Differential diagnoses for multiple GGNs include atypical adenomatous hyperplasia, multifocal micronodular pneumocyte hyperplasia, and metastatic lung tumors. In MPMN, GGNs exhibiting ring-like structures with a central lucency are sometimes observed, which may be a characteristic feature to differentiate MPMN from other diseases.journal articl
