    Il presente progetto di ricerca si propone di analizzare modi e tempi in cui il sistema commerciale altoadriatico si è trasformato tra IV e VI sec. d.C. Esso muove i propri passi a partire da questa domanda storica: “Quale impatto ebbero gli eventi politici, militari e ambientali verificatisi tra Tardantichità e Altomedioevo sulla realtà del tempo?” Per farlo si è scelto di utilizzare come lente di osservazione un sistema commerciale inteso come insieme di elementi impegnati nel trasferire le merci dal produttore al consumatore. Pertanto, alla sua definizione concorrono sia le infrastrutture deputate allo spostamento delle merci che i prodotti stessi. Dal momento che la funzione principale di un sistema commerciale è quella di rispondere ai bisogni di una comunità, esso risulta essere molto sensibile rispetto agli eventi esterni (politici, culturali e ambientali) che, di conseguenza, ne influenzano l’evoluzione . Nello specifico è stato scelto come caso studio il sistema commerciale del comprensorio altoadriatico, inteso come spazio esteso tra Ravenna e le foci del Timavo, con particolare attenzione ad Aquileia e al territorio circostante. Tale scelta è dovuta, sostanzialmente, a due ragioni: 1. Il coinvolgimento che questo comprensorio ha avuto nelle dinamiche politiche e militari che interessarono l’Impero Romano tra la fine dell’età tardoantica e l’inizio dell’Altomedioevo . 2. Il ruolo di primo piano che questa zona e in particolare Aquileia ebbero negli itinerari commerciali dell’Adriatico settentrionale in età romana e in particolare tardoantica. Si tratta di temi ampiamente noti in area altoadriatica ma che sono sempre stati affrontati in modo separato, focalizzandosi di volta in volta sulle infrastrutture oppure sui commerci. A partire da queste basi la presente ricerca ha proposto di affrontare la problematica integrando i due aspetti, l’archeologia delle strutture portuali, di stoccaggio e di vendita con quella dei commerci al fine di: - proporre una ricostruzione dei modi e tempi con cui si è sviluppato il sistema commerciale di quest’area tra IV e VI sec. d.C.; - offrire un quadro generale circa i rapporti commerciali a lungo, medio e corto raggio che hanno interessato questo territorio tra la Tardantichità e l’Altomedioevo; Premesse fondamentali per poter raggiungere questi obiettivi principali sono state: - l’individuazione di tutte le infrastrutture che compongono il sistema commerciale che fa capo ad Aquileia tra IV e VI sec. d.C.; - la definizione di un quadro di sintesi del materiale ceramico che caratterizza il comprensorio in esame tra IV e VI sec. d.C.; Il primo passo è quindi stato quello di procedere al censimento di tutti i contesti archeologici presenti nel comprensorio geografico esaminato che abbiano restituito stratigrafie inquadrabili tra IV e VI sec. d.C. L’attenzione è stata inizialmente rivolta ai luoghi del sistema commerciale (approdi, edifici di stoccaggio, luoghi di vendita), per poi allargarsi a contesti di altra natura. Per ogni sito sono state raccolte informazioni sulla destinazione d’uso, il vasellame ceramico rinvenuto e il periodo cronologico di utilizzo. La ricognizione del dato edito è stata affiancata dallo studio di due contesti inediti del centro di Aquileia, entrambi con funzioni commerciali: il complesso mercantile tardoantico dell’ex Fondo Pasqualis e le botteghe sul lato orientale del Foro. Per entrambi si è proceduto a definire i tempi di costruzione, frequentazione e abbandono attraverso lo studio tipo- cronologico del vasellame ceramico ivi rinvenuto. I dati raccolti sono stati organizzati in un database appositamente realizzato che è, quindi, stato ancorato a un sistema GIS

    Suicide risk in patients undergoing hemodialysis: a systematic review and meta-analysis of prevalence

    BackgroundKidney failure is strongly associated with psychological disorders, including anxiety, depression, and suicidal ideation. However, the prevalence of suicide risk in patients undergoing dialysis has not been fully established.MethodsWe performed a systematic review and meta-analysis aimed at assessing the prevalence of suicide risk in adult patients on dialysis and any correlation with demographic and clinical parameters.ResultsTwelve unique cross-sectional studies involving 1259 adult patients on dialysis were included. All patients were receiving hemodialysis treatment. In univariate meta-regression analysis, no significant effect of age, sex, and percentage of patients with diabetes mellitus on the prevalence of suicide risk was found. Overall, the prevalence of suicide risk was 20.6%. It varied widely, ranging from 4.0% to 57.3%, depending on the tools used (i.e. clinical interviews or self-report questionnaires). Among studies with more or less than 40% of patients on dialysis with anxiety and depression, the prevalence of suicide risk was very similar (21.2% vs 20.4%, respectively). The prevalence of suicide risk was more than double in lower-income than in higher-income countries (33.5% vs 15.3%, respectively).ConclusionsThe present systematic review and meta-analysis is a pioneering effort to identify the prevalence of suicide risk in patients on dialysis. It revealed a significant prevalence of suicide risk among hemodialysis patients, with a rate notably higher than that observed in the general population. This elevated risk is influenced by a complex combination of biological, psychological, and socio-economic factors

    Giudizio amministrativo e tutela delle future generazioni in materia ambientale: riflessioni sull’accesso alla giustizia tra ius conditum e ius condendum

    Negli ultimi decenni, la dimensione “intergenerazionale” della tutela dell’ambiente, oggi riconosciuta anche dalla Costituzione, ha portato la dottrina a domandarsi non solo quali obblighi da essa discendano per le generazioni attuali, ma anche quali forme di tutela, anche processuale, possano essere approntate per quelle future. Quest’ultimo punto è oggi soggetto ad una particolare attenzione per la sua capacità di mettere in discussione le categorie giuridiche tradizionali: ci si chiede, infatti, non solo quale possa essere l’oggetto di una tutela processuale “intergenerazionale”, ma anche chi sia legittimato a richiederla. Il contributo affronta tale tematica dal punto di vista del giudizio amministrativo, chiedendosi se e come gli interessi “delle future generazioni” possano accedere a tale giudizio, costruito per tutelare, nell’impostazione tradizionale, interessi "attuali", di cui i ricorrenti sono titolari

    Objectives: To investigate predictors and clinical impact of delayed cerebral infarction (CI) in a national multicentric poor grade aneurysmal subarachnoid hemorrhage (aSAH) population. Methods: Retrospective analysis of prospectively collected data from the multi-centric POGASH (Poor Grade aSAH) registry of consecutive patients treated from January 1st, 2015, to June 30th, 2023. Poor grade was defined according to the worst-pretreatment WFNS scale. CI was defined as a new ischemic lesion/s within 6 weeks of aSAH onset, not present on CT acquired within 48hrs and not related to treatment or hematoma. Clinical outcome was assessed by the modified Rankin Scale. Results: Among 532 consecutive WFNS grades IV-V aSAH patients, CI occurred in 106 (19.9%). CI (adjusted OR 0.59 0.35-0.98; p 0.045) independently predicted increased disability. Volume of SAH (aOR 1.02, 95% C.I. 1.00-1.04; p 0.015), anterior communicating aneurysm, ACoA, (aOR 6.2, 95% C.I. 1.4-27.3; p 0.015) and need of angiographic treatment (aOR 2.2, 95% C.I. 1.3-3.8; p 0.002) resulted independently predictive of CI occurrence. CI volume emerged as the strongest predictor of increased disability (aOR 1.03 95% C.I. 1.01-1.05; p < .001) and mortality (aOR 1.009 95% CI 1.002-1.02; P 0.018) in the CI affected population. Conclusions: ACoA, SAH volume and need for DSA treatment predicted CI, whose detrimental role on outcome is mainly driven by its volumetric extension. CI volume resulted independently predictive of mortality and increased disability in early brain injury survivors

    Clinical diagnosis and communication in functional neurological disorders: a survey among Italian neurologists

    IntroductionFunctional neurological symptom disorder (FND), including functional motor disorders (FMD) and psychogenic non-epileptic seizures (PNES), is a common yet under-recognized condition, often associated with diagnostic uncertainty. This national survey investigated how Italian neurologists apply validated clinical signs and communication strategies in FND diagnosis.MethodsA cross-sectional survey was distributed to approximately 2,700 members of three Italian neurological societies. The questionnaire assessed demographic and professional characteristics, the frequency of use of specific clinical signs for FMD and PNES, and strategies for communicating the diagnosis. Responses were rated on a five-point frequency scale and analyzed using ordinal generalized linear models and Spearman's correlations, adjusting for demographic covariates.ResultsA total of 245 neurologists (43.3% general neurologists, 35.9% movement disorder specialists, 20.8% epileptologists) completed the survey. Movement disorder specialists more frequently assessed tremor entrainment and fixed dystonia in FMD, while epileptologists were more likely to assess signs such as eye closure and irregular jerks in PNES. Clinicians with greater experience more frequently evaluated established signs for both FMD and PNES and employed broader communication strategies. Communication practices were largely consistent across subspecialties. Written information was rarely provided across all groups.ConclusionThe application of validated clinical signs and communication strategies for FND varies by subspecialty and clinical experience. While epileptologists and movement disorder specialists demonstrate greater familiarity with signs relevant to their subspecialty, key diagnostic tools for FMD remain underutilized. These findings highlight the need for targeted educational interventions to improve diagnostic accuracy, enhance communication, and optimize care for people with FND

    Single bladder instillation of gemcitabine versus mitomycin C after minimally invasive radical nephroureterectomy: A propensity-score match analysis from the ROBUUST 2.0 collaborative group

    Objective: Radical nephroureterectomy (RNU) is considered the standard of care for patients with high-risk upper tract urothelial carcinoma. Current literature reveals a deficit in direct comparative studies evaluating the efficacy of different chemotherapeutic agents administered in single postoperative instillation following RNU. The primary aim of this study was to compare the bladder recurrence (BR) rates between patients receiving a single instillation of mitomycin C (MMC) versus gemcitabine (Gem) after RNU. Methods: The ROBUUST (ROBotic surgery for Upper tract Urothelial cancer STudy) 2.0 is an international, multicenter registry that aggregates data on patients who have undergone curative surgery for upper tract urothelial carcinoma across participating centers from January 2015 to December 2022. Data including primary baseline variables of the patients, characteristics of the tumors, surgical management, and definitive histopathological characterizations were collected and stratified based on the type of postoperative bladder instillation: MMC (the MMC group) and Gem (the Gem group). We selected variables correlated with our primary outcome to conduct a propensity-score match analysis. Results: One hundred patients in the MMC group were matched 1:1 with 100 patients in the Gem group. At 36 months of follow-up, 30 patients in the MMC group and 39 patients in the Gem group experienced BR, representing recurrence rates of 30% and 39%, respectively (p=0.2). The Cox proportional hazards model comparing BR between the groups revealed a hazard ratio of 1.58 (95% confidence interval: 0.98-2.55) with a non-statistically significant increased risk of BR in the Gem group compared with the MMC group (p=0.059). Conclusion: A single perioperative instillation of Gem or MMC seems to offer similar efficacy in reducing the risk of BR in patients undergoing RNU. Further research, ideally within the framework of prospective studies, is warranted to elucidate the optimal chemotherapeutic approach in this setting. 2025 Editorial Office of Asian Journal of Urology.     Risankizumab differentially modulates circulating T-cell populations in psoriasis according to autoreactivity status

    Background: In a recent paper, our group described that the presence of double autoreactivity to both LL37 and ADAMTSL5 autoantigens in psoriatic patients decreased the clinical responses to risankizumab, but how this influences the changes in the peripheral inflammatory T-cell populations is still unknown. Objective: This study aims to evaluate how risankizumab modulates the circulating inflammatory T-cell populations in psoriatic patients and, specifically, in autoreactive subjects. Methods: The presence of LL37- and ADAMTSL5-reactive circulating T-cells was assessed in a cohort of 142 psoriatic patients, and 87 demonstrated autoreactivity at baseline. Patients were treated with risankizumab for 52 weeks, and specific T-cell populations were analyzed at different timepoints. Results: The frequency of Ki67+ CD4+ , Ki67+ CD8+ T-cells, CD8+ IL-17+ and CD8+ IL-22+ T-cells showed a posi- tive correlation with baseline PASI and decreased with treatment. Notably, CD8+ IL-17+ T-cells decreased both in single-LL37 and single-ADAMTSL5-reactive subjects, but not in subjects that showed autoreactivity to both autoantigens. LL37 autoreactivity of CD4+ and CD8+ T-cells decreased with treatment, but not for CD4+ in double-reactive subjects. While Treg frequency negatively correlated with baseline PASI and in- creased within 16 weeks of treatment, significantly decreasing the IL-17+ CD4+ /Treg ratio over time, Treg modulation was not evident in double-reactive subjects. Interestingly, the subpopulations of CD8+ MAIT IL-17+ and CD3+ MAIT IL-22+ cells, involved also in psoriatic arthritis, decreased in treated subjects following IL-23 inhibition. Conclusion: Rizankizumab efficiently decreases the circulating inflammatory T-cell populations and modulates Tregs’ plasticity in single-LL37- or single-ADAMTSL5-reactive subjects, but not in double-reactive subjects. © 2025 Japanese Society for Investigative Dermatology.     A genetic approach for automatic AxC design exploration at RTL based on assertion mining and fault analysis

    In Approximate Computing (AxC), design exploration methods have been introduced to automatically identify approximation targets at the gate level. However, only some of them are applicable at at Register Transfer Level (RTL); furthermore, the benefits of combining information from assertions and fault analysis have not been fully explored. This paper proposes a novel methodology for guiding AxC design exploration at RTL considering two approximation techniques: bit-width reduction and statement reduction. Then, it employs fault injection to mimic the approximation effect on the design under approximation. To guide the designer while assessing the approximation choices, assertions, which formally capture the behaviors implemented in the design, are dynamically generated from the RTL simulation traces. Then, the impact of fault injections on the truth values of the assertions is employed as a proxy for measuring the functional accuracy of the corresponding approximations. Based on this evaluation, a genetic algorithm is finally used to rank and cluster the approximation targets, thus providing the designer with an efficient and effective way to automatically analyze AxC variants in terms of the trade-off between accuracy and performance. The experiments carried out on state-of-the-art benchmarks show that the proposed approach represents a promising solution for the automation of AxC design exploration at RTL
