Alcuni aspetti ambientali come la crisi climatica, l’inquinamento atmosferico, e, più in generale, il degrado ecologico sono stati annoverati dal mondo scientifico e da parte di organismi internazionali, quali l’Organizzazione Mondiale della Sanità e l’Unicef, fra le emergenze di sanità pubblica.1-3 Questi aspetti sono ampiamente rappresentati negli obiettivi dell’Agenda 2030 per lo sviluppo sostenibile in cui i bambini – i futuri adulti – devono essere al centro.4
I professionisti sanitari che lavorano a stretto contatto con bambini, ragazzi e famiglie devono, perciò, conoscere e sapere affrontare anche gli aspetti ambientali.
Nell’ambito delle proprie attività educative, la Società Italiana di Pediatria (SIP) ha istituito tre anni fa una Commissione (ora Gruppo di Studio) su Ambiente e Salute del Bambino e dell’Adolescente con lo scopo di informare ed educare pediatri, famiglie, bambini e adolescenti su temi ambientali, affiancando le altre iniziative di educazione e comunicazione che SIP svolge per tematiche più strettamente sanitarie
Evaluating the learning curve in robot-assisted laparoscopic total hysterectomy: single-port versus multi-port Da Vinci platforms
The aim of this study is to assess and compare the learning curves of the Da Vinci S Multi-port (MP) and Da Vinci Single-Port (SP) platforms for total hysterectomy, with particular attention to how prior MP experience influences SP performance. This is a retrospective comparative study on patients undergoing robot-assisted total hysterectomy (benign or early endometrial cancer indication) with the MP and SP systems. All operations were performed by the same surgeon, and cases performed with the SP system were performed after the MP system. Operating time was used as the main indicator of the learning curve, analyzed by the CUSUM method. Data were processed with R software, and differences between groups were evaluated with t-test and Chi-squared test (significance p < 0.05). A total of 147 patients were analyzed (73 MP, 74 SP). Mean operating times were similar between the two groups (117 min MP vs. 114 min SP). Hemoglobin drop was significantly lower in the SP group (- 1.18 g/dL vs. - 2.07 g/dL, p < 0.05). CUSUM analysis showed that the learning curve of the MP system reaches the mastery phase after 50 cases, while 13 cases are sufficient for the SP system when initiated after having mastered the MP system. The learning rate was - 0.3 min/case for MP and - 0.009 min/case for SP. In conclusion much of the experience gained with the Da Vinci Multiport System is transferable to the SP platform, allowing for a shorter learning curve and rapid achievement of surgical mastery with similar learning phases

Approach to Cystic Pancreatic Lesions
Pancreatic cystic lesions (PCLs) have different morphological and histological features, characterized by peculiar imaging findings. Prevalence is about 2.6% and increases with age. These lesions may be inflammatory or proliferative, ranging from tumors with benign to malignant behavior. In most cases, the diagnosis of PCLs is incidental due to the increasing use of cross-sectional imaging and the improvement of imaging quality. Given the malignant potential of some of these lesions, an accurate and reproducible multidisciplinary approach is essential. As the first step, it is important to analyze the clinical history of the patient to exclude an inflammatory process. After clinical evaluation, imaging studies allow us to identify the morphological features and stratify the risk of malignant potential. Non-invasive imaging findings will guide the decision to eventually perform endoscopic ultra-sonography with fine-needle aspiration (EUS-FNA) when appropriate. Regarding management, patients with benign lesions should undergo surveillance, while surgical treatment is recommended for lesions with signs of malignant degeneration
Redox-dependent activity and thioredoxin interaction of cyclophilin TgCyp21 from Toxoplasma gondii
Cyclophilins (Cyps) are ubiquitous cytosolic proteins with peptidyl-prolyl cis-trans isomerase (PPIase) activity and the ability to bind the immunosuppressant cyclosporin A (CsA). The genome of Toxoplasma gondii, the parasite responsible for toxoplasmosis, encodes multiple putative Cyps, whose specific functions remain largely unexplored. Here, we characterize TgCyp21, a predicted Cyp from T. gondii. TgCyp21 displays PPIase activity and is inhibited by CsA in vitro. Importantly, its activity decreases markedly under oxidizing conditions but is partially restored by reducing agents, including dithiothreitol (DTT) and the parasite endogenous thioredoxin (TgTrx). TgCyp21 contains four cysteines, with Cys87 and Cys141 predicted to be spatially close based on structural modeling. Substitution of both residues significantly reduced PPIase activity, with Cys87 emerging as the main contributor to this loss. Structural modeling further indicates that Cys87 and Cys141 are suitably oriented to interact with the conserved active-site cysteines of TgTrx. This interaction is supported experimentally by mixed disulfide trapping, which identifies a stable disulfide-linked intermediate between TgCyp21 and TgTrx, consistent with a thiol-disulfide exchange mechanism. Small-angle X-ray scattering (SAXS) and nuclear magnetic resonance (NMR) spectroscopy further confirm the formation of the complex. Taken together, our data indicate that TgCyp21 behaves in vitro as a redox-responsive Cyp and a substrate for Trx, suggesting a potential involvement in Trx-mediated redox processes in T. gondii
The effects of different multidirectional plyometric sequences on shooting, balance, and neuromuscular performance in professional male basketball players
Objective Repeated multidirectional jumps are highly specific movements in basketball, which may be important to integrate the training routine. Accordingly, this study aimed to assess the effects of three different multidirectional plyometric jumping sequences on shooting, balance, and neuromuscular performance in professional, male basketball players. Methods Eighteen players underwent familiarization and baseline performance assessments in an initial session. Players then completed 21 visits in a randomized, counterbalanced manner. Each visit involving 10 repetitions of one of three different multidirectional plyometric protocols followed by a single performance test (countermovement jump (CMJ), squat jump (SJ), five jump (FJT), change-of-direction T-test (CoD T), Stork balance (SBT), Y-balance (YBT), three-point shooting (3pts) tests). The first plyometric protocol (P1) consisted of a combination of vertical and horizontal jumps, the second protocol (P2) consisted of a combination of drop, lateral, and vertical jumps, and the third protocol (P3) consisted of a combination of drop, single-leg step-up, and horizontal jumps. Each session was separated by at least 48 h of rest and recovery time between repetitions was 20 s. Results Neuromuscular (CMJ, SJ, FJT and CoD T), body balance (SBT and YBT), and Shooting performances were significantly worse following each plyometric protocol compared to baseline (p < 0.001; d = 0.23-2.21). In addition, the majority of the measured performances were significantly lower following P3 compared to P1 and P2. Conclusions Basketball-specific performance was adversely influenced by repeated multidirectional plyometric routines. Incorporating these particular strategies into the training regimen to counteract the induced tiredness will be intriguing
