Effectiveness of booster dose of tetanus and diphtheria toxoids (Td) vaccine in management of recurrent aphthous stomatitis: A prospective, randomized, triple-blind and placebo-controlled clinical trial
Background: Recurrent aphthous stomatitis (RAS) is the most common oral disease. The activation of the immune system by vaccines might reduce the interactions between oral mucosa and microorganisms.
Aim: To evaluate the effect of the tetanus-diphtheria toxoids (Td) vaccine in management of RAS.
Design and Setting: This prospective, randomized, triple-blind and placebo-controlled clinical trial study was conducted on 70 eligible patients with minor RAS at the dermatology outpatient clinic.
Method: Finally, a total of 66 participants (48 male, 18 female; mean age: 38.56 ± 10.98 years) completed the study in two groups, one in which colchicine and a single dose of vitamin B6 (placebo group) was treated and one in which colchicine and a single dose of Td vaccine (intervention group) was treated.
Results: After six months of follow-up, the patients were evaluated, which revealed significant effects of the Td vaccine on pain intensity, ulcer size, recovery time, and the interval between episodes. At the end of the six month follow-up, 27 patients (81.8%) in the intervention group and 13 patients (39.4%) in the placebo group showed partial or complete recovery, and there was statistically significant difference between the groups (p < 0.001). Recovery was not significantly associated with sex, education level, marital status and duration of RAS. However, occupation and positive family history of RAS had significant relations with recovery.
Conclusion: A booster dose of Td vaccine had relatively favorable effects on pain intensity and recurrence of RAS, but further research is needed to confirm its efficacy
Environmentally friendly ultrasound-assisted synthesis of magnetic zeolitic imidazolate framework - Graphene oxide nanocomposites and pollutant removal from water
Ultrasound-assisted procedure is widely used for synthesizing materials due to its low cost, environmental friendly, facile, and mild properties. Herein, ultrasound-assisted synthesis of magnetic zeolitic imidazolate framework-8 (ZIF-8)/cobalt ferrite (CoFe2O4)/graphene oxide (GO) nanocomposites (ZIF-8/CoFe2O4/GO: ZCG) with different amounts of CoFe2O4/GO (25 and 50 mg denoted as ZCG-25, and ZCG-50) were studied in details. The GO was prepared from graphite. The synthesized materials including GO, ZIF-8, CoFe2O4, CoFe2O4/GO, ZCG-25, and ZCG-50 were fully characterized using BET, TEM, XRD, SEM, FTIR, and VSM. The ultrasound-assisted pollutant removal ability of the synthesized materials was investigated using Malachite Green (MG) as a model contaminant. The surface area of GO, CoFe2O4, ZCG-25, and ZCG-50 was 46, 52, 1543, and 2490 m2/g, respectively. Pollutant removal obeyed the pseudo-second order and Langmuir models. The pollutant removal capacity of ZCG-50, ZCG-25, CoFe2O4/GO, ZIF-8, CoFe2O4 and GO was 2610, 1948, 212, 901, 100, and 118 mg/g, respectively
Translation and Psychometrics of Good Nursing Care Scale from Nurses & Patients' Viewpoints in Ardabil Educational Centers in 2018
Background: Identifying and evaluating the weaknesses and strengths of nursing care provided is emphasized to improve the quality of nursing care and its achievement requires the use of valid tools in this field. This study was conducted with the aim of translating and determining the psychometric properties of the Persian version of the Good Nursing Care Scale.
Method: The present study was a methodological study in which the psychometric aspects of nursing and patient version of the instrument of "good nursing care scale" were evaluated in 400 nurses and occupational patients admitted to hospitals affiliated to Ardabil University of Medical Sciences. The dimensions of psychology of both nursing and patient nursing instruments were evaluated by the "Good Nursing Care Scale". After translation, the validity and reliability of the tools were evaluated. In this study, SPSS version 16 and AMOS version 24 were used.
Finding: After determining the face and content validity quantitatively and qualitatively, all 40 items contained in both tool versions were maintained. According to the results of the confirmatory factor analysis, the seven dimensions that were introduced in the main tools were confirmed with acceptable values. The high correlation between the scores obtained from the main questionnaire and the criterion of the criterion of the validity of the test was confirmed. The results of Cronbach's alpha (0.907 in nurses' version and 0.885 in patient version) in the study of internal consistency and stability in repeat test (0/867 for nurse version and 0/894 for patient version) showed acceptable reliability of questionnaires Was.
Conclusion: Persian version of good nursing care scale has acceptable psychometric indices in the Iranian nurses and patients population and can be used as a valid tool in the fields of nursing care, nursing care and nursing care quality assessment
Food neophobia status in Ardabil children
Background: Food neophobia means not accepting a new food, which seems to be a protective mechanism against new foods. The aim of this study was to assess the food neophobia status in Ardabil children.
Methods: The present study was a cross-sectional study to assess the situation of children's disgust at two to six years of age in households in Ardabil. A questionnaire relating to five main food groups was used to
assess the type of food that was hated. A questionnaire containing four questions was completed to check the factors affecting food neophobia.
Results: The results of the study showed that 52.5% (95% confidence interval) of the children were suffering from food neophobia. The children in the study felt more disgusted about dairy products and vegetables.
Parents have stated that their children refuse to eat fruit and vegetables.
Conclusion: More than half of the children were suffering from food neophobia and the socioeconomic status of parents, parental employment and the type of child feeding before the age of two were among the factors influencing the disgust of children in the study
Central Application of Aliskiren, a Renin Inhibitor, Improves Outcome After Experimental Stroke Independent of Its Blood Pressure Lowering Effect
Epidemiological studies suggest that pharmacological reduction of systemic hypertension lowers incidence and severity of stroke. However, whether the reduction of blood pressure per se or the compounds used to reduce hypertension are responsible for this effect received little attention. In the current study we therefore aimed to investigate whether Aliskiren, a renin-inhibitor used to treat arterial hypertension, may improve outcome in a mouse model of ischemic stroke when applied centrally and in a dose not affecting blood pressure. Male C57BL/6 mice received 0.6, 2.0, or 6.0 µg Aliskiren or vehicle by intracerebroventricular injection as a pre-treatment and were then subjected to 60 min of middle cerebral artery occlusion (MCAo). Infarct volume, brain edema formation, mortality, antioxidant effects, and functional outcome were assessed up to seven days after MCAo. Central administration of Aliskiren (0.6 or 2.0 µg) had no effect on systemic blood pressure but significantly reduced infarct volume and brain edema formation, blunted mortality, and improved neurological outcome up to 1 week after MCAo. Due to the central and prophylactic administration of the compound, we cannot make any conclusions about the potency of Aliskiren for acute stroke treatment, however, our study clearly demonstrates, that in addition to lowering blood pressure Aliskiren seems to have a direct neuroprotective effect. Hence, renin-inhibitors may be an effective addition to prophylactic treatment regimens in stroke patients
Association between Molar Incisor Hypomineralization and Both Prenatal and Postnatal Factors in 8-10 year Old Children in Ardebil.
Introduction: Molecular Hypomineralization is a malformation with a systemic background which etiology is not well defined. The prevalence of these lesions varies in different countries, but few studies have been conducted on its prevalence in Iranian society. The aim of this study was to determine the prevalence of hypomineralization of molar Incisor among 8-10 year old children in Ardebil and its relationship with pre and postnatal-factors.
Materials and Methods: Participants in this study included 520 children from public schools of Ardebil. In first stage, the frequency of MIH was determined by a clinical examination based on the index introduced by Tourino et al. Then, they were evaluated in case and control groups to determine the causative factors. To collect information, a questionnaire including information about factors before and after birth was used. Data were analyzed using SPSS18 software. Chi-square and T-test and logistic regression were used to test the relationship between different factors.
Results: The prevalence of molar- incisor hypo mineralization in children aged 8-10 year old in Ardebil was estimated to be 24%; The results also show that there is a significant relationship between asthma / bronchitis variables, hospitalization history and fever above 38.5 degrees and MIH (P <0.05).
Conclusion: Due to the high prevalence of MIH in Ardebil, preventive measures, informing the parents and periodic examination of the teeth in early years of permanent teeth are recommended
Clinical and Paraclinical Findings of Dead Patients Due to Poisoning with Aluminium phosphide in Ardabil Province during 2009-2017
Background and Objective: Aluminum phosphide or rice tablets are one of the most common pesticides and insecticides used in agriculture to protect crops, cereals and rice. This substance was introduced in Year 2 as an ideal pesticide. Research has shown that, in vitro, phosphine gas inhibits cytochrome oxidase C in mitochondria and induces cellular hypoxia and extensive tissue destruction. The dominant clinical feature was severe hypotension that was resistant to dopamine. Exposure to aluminum phosphide poisoning, rapid transfer to the treatment center, no gastric lavage with paraffin, rapid analysis of arterial blood gas, laboratory tests to evaluate sodium and potassium, calcium, magnesium, electrocardiography and such Antacid administration can be one of the main actions. The aim of this study was to evaluate the clinical and paraclinical findings of patients who died due to rice poisoning in Ardabil province.
Methods: In this study, the clinical records of all deceased patients (97 patients) from rice poisoning during 2009 to 2016 were studied and clinical and paraclinical findings of the deceased hospitalized records were studied. Data were analyzed using descriptive and analytical statistics in SPSS software version 19.
Results: The mean age of patients was 34.3 ± 15.8. The mean age of men was 35.6 ± 16.2 and the mean age of women was 30.1 ± 13.9%. 77.3% of patients were men and 22.7% women. The most important symptoms were nausea and vomiting seen in Males (92%) and females (100%). Patients had a mean recovery time of 5.89 hours, 22 patients received 2 tablets and 75 patients received 1 tablet, which increased the mortality rate with increasing dose. 92.7% of the patients had taken rice tablets orally. 67% of patients complained of nausea and vomiting. Clinical and laboratory findings, decreased GCS, systolic blood pressure, PHC, and increased heart rate, and white blood cell count, were associated with an increased mortality rate. The mean hospital stay was 49 hours. Women had significantly lower blood pressure. Women had significantly lower pH and higher Po2.
Conclusion: Increased heart rate, white blood cell count and GCS decrease, systolic blood pressure, and carbohydrate pH are associated with increased mortality in aluminum phosphide poisoning patient
Iranian Registry of Crohn’s and Colitis: study profile of first nation-wide inflammatory bowel disease registry in Middle East
Background/Aims: A recent study revealed increasing incidence and prevalence of inflammatory bowel disease (IBD) in Iran. The Iranian Registry of Crohn’s and Colitis (IRCC) was designed recently to answer the needs. We reported the design, methods of data collection, and aims of IRCC in this paper. Methods: IRCC is a multicenter prospective registry, which is established with collaboration of more than 100 gastroenterologists from different provinces of Iran. Minimum data set for IRCC was defined according to an international consensus on standard set of outcomes for IBD. A pilot feasibility study was performed on 553 IBD patients with a web-based questionnaire. The reliability of questionnaire evaluated by Cronbach’s α. Results: All sections of questionnaire had Cronbach’s α of more than 0.6. In pilot study, 312 of participants (56.4%) were male and mean age was 38 years (standard deviation=12.8) and 378 patients (68.35%) had ulcerative colitis, 303 subjects (54,7%) had college education and 358 patients (64.74%) were of Fars ethnicity. We found that 68 (12.3%), 44 (7.9%), and 13 (2.3%) of participants were smokers, hookah and opium users, respectively. History of appendectomy was reported in 58 of patients (10.48%). The most common medication was 5-aminosalicylate (94.39%). Conclusions: To the best of our knowledge, IRCC is the first national IBD registry in the Middle East and could become a reliable infrastructure for national and international research on IBD. IRCC will improve the quality of care of IBD patients and provide national information for policy makers to better plan for controlling IBD in Iran. (Intest Res 2019;17:330-339
