Febuxostat attenuates ulcerative colitis by the inhibition of NF-κB,proinflammatory cytokines, and oxidative stress in mice
Ulcerative colitis is a chronic inflammatory disorder characterized by oxidative stress and upregulation of
proinflammatory mediators in colonic tissue. Febuxostat, a xanthine oxidase inhibitor has been shown to exert
anti-inflammatory and antioxidant effects. The aim of this study was to investigate the protective effect of
febuxostat against ulcerative colitis, and to elucidate the potential mechanisms involved. Experimental colitis
was induced in mice by intrarectal administration of 5% acetic acid. Mice were treated with febuxostat (10 and
20 mg/kg/day, orally) for three days. Results showed that body weight loss, colon shortening, macroscopic
damage and histopathological changes of colonic mucosa were reduced in mice treated with febuxostat.
Treatment of mice with febuxostat significantly increased the levels of glutathione (GSH) and superoxide dismutase
(SOD), and decreased the levels of malondialdehyde (MDA), carbonyl protein, xanthine oxidase, nitric
oxide (NO) and myeloperoxidase (MPO) activity of colon tissue compared with those in the acetic acid-induced
colitis group. The expression of nuclear factor kappa B (NF-κB) as a key regulator of inflammation in the colonic
tissue was decreased by febuxostat. Furthermore treatment with febuxostat significantly reduced the levels of
proinflammatory cytokines tumor necrosis factor (TNF)-α, interleukin (IL)-1β, IL-6 and interferon (IFN)-γ, while
increased the levels of IL-10 compared with the colitis group. These results suggest that febuxostat is able to
decrease the severity of acetic acid-induced colitis by inhibition of oxidative stress and inflammatory responses
through NF-κB pathway
Investigation of the association between SNPs in the microRNAs binding site in the MTHFR gene and preterm labor in Ardebil province
Background: Preterm labor is the mostcommon cause of neonatal mortality, while infant mortality rates are acriterion for assessing the quality and comparisons of health systems incommunities. Evidence suggests that MTHFR polymorphism is currently involved inearly delivery.
Methods: In this case-control study, 50 pregnant womenwith preterm labor (less than 37 weeks) as case group and 50 pregnant womenwith a delivery of more than 37 weeks were selected as control group. From eachindividual, 2.5 cc of blood was collected in EDTA tubes after extraction of DNAfrom the blood, the pcr reaction was performed and the results were analyzed onagarose gel. The polymorphism of pcr products was investigated by Revers DotBlot Technique. Data were analyzed by SPSS software usingt-test and Mann-Whitney test.
Results: The prevalence of C677T polymorphism amongwomen with preterm delivery and control was 50% and 60%respectily. Forty-four percent of women with preterm labor and 60% of control group wereheterozygote for A1298C polymorphism and 50% of women with preterm deliverywere homozygous for this polymorphism, while in control group 40% of thehomozygotes were observed. The prevalence of A1298C polymorphism among womenwith preterm delivery and control was 80% and 70%respectily. 66% of women withpreterm delivery and 70% of control group were polymorphisms of A1298C geneheterozygote and 14% of women with preterm delivery were homozygous for thispolymorphism, while in the control group women did not have a consensus.
Conclusion: In this study, there was a significant relationship between C677T andA1298C polymorphisms of MTHFR gene with preterm labor
Epidemiological Study of Brucellosis in Ardabil
Brucellosis is a chronic and infectious zoonosis disease which is reported in many developing countries, including Iran. The aim of this study was to investigate the prevalence and incidence rate of brucellosis in the Ardabil County. In this descriptive study, all patients with brucellosis who were identified and treated were studied during the year 2013-2017. Individual information and some epidemiologic parameters of the patient were collected and analyzed by using SPSS 23 software. 372 cases in Ardabil county was identified and treated that 56.2 % were mail and 43.8 % female. 64 % of patients located in rural and 36 % in urban areas. The highest morbidity rate was observed in animal husbandry and farmer (32%) and followed by housekeeping (20.4%). In this study, the incidence of brucellosis was 2-33 /100,000 for over 5 years that increased from 2013 to 2017. According to the results of this study, the incidence of disease in the Ardebil County was moderate and in rural areas with husbandry and farmer, more was more than urban areas, therefore, it has to be planned in the field of education for the prevention of disease in rural area
Evaluation of mandibular and maxillary first molars anatomy by using CBCT archive in Ardabil population in 2015-2019
Background & Objective: The endodontic treatment is successful when root canal system is purified completely and shaped well. So the clinician should be aware of anatomical variations of root and root canal system. But whereas the genes are responsible in formation of root canal system, so this system can vary in different ethnic groups. Because of that, this study designed to evaluate the morphology of first maxillary and mandibular molars in Ardabil population by using CBCT.
Methods: In this study 196 images containing bilateral molars and including criteria, evaluated. These images assessed in NNT software and in three planes; axial, sagittal and coronal. The collected data evaluated in SPSS software.
Results: As the results most of the mandibular molars had 2 roots and 3 canals, and most of the maxillary molars were 3-rooted and 4-canal. Most frequent Vertucci configuration of mandibular molar roots was type IV and I (mesial and distal root, respectively), and most frequent Vertucci configuration of maxillary molar roots was type II, I, I (mesiobuccal, distobuccal and palatal root respectively). Roots were straight in most of the roots, except mesiobuccal root that had distal deviation in most of the cases, and in 3.9% of distobuccal roots was in S-shape. The incidence of MB2 canal was 80.6% and no MM canal was found in this study. Also there was approximately 90% bilateral symmetry in both of the groups.
Conclusion: The morphology of root and root canal system are based on genetics and it varies based on different ethnics
The Relationship between the Severity of Airway Obstruction and COPD Diagnostic Questionnaire Score in COPD Patients
Background & objectives: Chronic obstructive pulmonary disease (COPD) is characterized by irreversible airflow limitation and systemic inflammation which is also associated with pulmonary and extra-pulmonary complications. COPD Diagnostic Questionnaire (CDQ) is a valid questionnaire that is used to identify patients with COPD. The purpose of this study was to determine the relationship between the severity of airflow limitation and the CDQ score in
COPD patients.
Methods: In this study, 100 patients with COPD and smoking history were included. The diagnosis of COPD was established based on American Thoracic Society (ATS) guidelines:
cough and expulsion of phlegm, chronic dyspnea, obstructive spirometric pattern (forced expiratory volume in one second (FEV1)/forced vital capacity (FVC) <70%). Disease severity
was classified according to GOLD criteria. After obtaining consent, demographic data, history of smoking, the exacerbation rate in the past year, arterial oxygen saturation (Spo2) and spirometer variables were recorded. Then, CDQ and Modified Medical Research Council (MMRC) dyspnea score were completed. Also, the associations of the CDQ score with FEV1% predicted and clinical parameters of patients were tested.
Results: All studied patients were male. There was a significant relationship between CDQ score with FEV1% (p<0.001), GOLD stage (p<0.01), Spo2 (p<0.01), exacerbation rate (p<0.01) and severity of dyspnea according to MMRC dyspnea scale (p<0.01).
Conclusion: The CDQ can be used to determine airflow limitation, severity of dyspnea and exacerbation rate in COPD patients. Therefore, it is recommended that this questionnaire can be used in primary health centers for identifying and referring of COPD patients
Causes of Maternal Mortality and Associated Risk Factors in Ardebil, Iran, from 2006 to 2016.
introduction and purpose: Maternal death during pregnancy or delivery and 42 days after the termination of pregnancy for any reason other than an accident is called maternal mortality. Maternal mortality due to the complications of pregnancy and childbirth is a sensitive indicator of women status in society, access to care, efficiency and quality of healthcare, capacity of the service system to meet the needs of women and demonstrates the economic and social conditions of each society. In addition, mortality due to pregnancy and delivery complications is one of the most important indicators that shows the condition of developing countries. In this regard, this study was conducted to investigate the causes of maternal mortality and related risk factors in pregnant women in Ardebil, Iran, from 2006 to 2016. Methods: In this cross-sectional study, registered information were extracted regarding the mortality of pregnant women, including demographic characteristics, indicators related to pregnancy, childbirth, and causes of death from 2006 to 2016 from the Family Health Unit of Provincial Health Deputy. Finally, the collected data were analyzed. Results: According to the obtained results, there were 39 cases of maternal mortality from 2006 to 2016. However; there was no maternal mortality in 2008 and 2015. The mean age of dead mothers was 30±1.8 years (age range: 15-44 years). Considering the mortality stage, 10 patients died during pregnancy (25.6%), one patient died during labor (2.5%), 12 patients died 24 h after delivery (30.7%), and 16 patients died after 24 h. On the other hand, 11 (28.2%) mothers had normal vaginal birth (NVD) and 28 cases (71.8%) had a cesarean section. Moreover, 31 women (79.48%) were at risk. There was a positive relationship between the type of delivery (cesarean section or NVD), parity (the first and second pregnancy), and risk factors during pregnancy with maternal mortality. Hemorrhage was the most common cause of maternal mortality. It is worth mentioning that there was no significant relationship between the causes of maternal mortality. Conclusion: According to the obtained results, there was a significant relationship between the number of pregnancies and type of delivery with maternal mortality. Moreover, hemorrhage was the most common cause of maternal mortality. Regarding the high levels of cesarean section effects on maternal mortality, it is recommended to carry out vaginal delivery. In this regard, it is necessary to take measures to conduct vaginal delivery without pain to reduce cesarean section rates. In addition, due to the high mortality rate in middle-aged mothers with a high number of pregnancies, it is suggested to use contraceptive methods, such as oral contraceptives, condoms, and birth control
Awareness of General Dentists of Ardabil City about Prescribed Drugs in Dental Root Procedures
Introduction: A dentist must be aware about the medications that he/she prescribes, particularly those taken by patients with systemic disorders. The present study focused on the level of knowledge of general dentists in Ardabil concerning the medications prescribed during root canal treatment.
Materials and Methods: The present cross sectional–descriptive study focused on the knowledge possessed by Ardabil general dentists concerning the medications prescribed during root canal treatment in 2016‒2017. To this end, we developed a questionnaire which was completed by as many as 80 general dentists. The questionnaire consisted of questions on demographic data and level of awareness of the dentists. Data were analyzed with SPSS 23, using descriptive statistics and one-way ANOVA (α = 0.05).
Results: The results suggested that the level of knowledge on the drugs prescribed during root canal treatment was moderate (14.56 ± 6.91). Furthermore, the results showed that the mean knowledge levels of the dentists concerning the medications prescribed during root canal treatment were significantly different in terms of age, experience and university of origin (p value 0.05).
Conclusion: The level of knowledge of the general dentists in Ardabil concerning medications prescribed during root canal treatment was moderate. The inverse relation between the experience and level of knowledge indicates that knowledge of the dentists is not updated relative their increasing experience and no serious measures have been taken to update their knowledge. This reveals the need for consideration of retraining courses for the dentists concerning the medications prescribed during root canal treatment

Evaluating the efficacy of duloxetine and gabapentin in pain reduction in patients with knee osteoarthritis
Introduction: Knee osteoarthritis (OA) is a common form of arthritis in elders which can lead to reduced daily activity and quality of life. It is important to administer a proper treatment with high efficacy and low side effects. In this study we evaluated the efficacy of duloxetine and gabapentin in patients with moderate to severe knee OA.
Methods: In this randomized clinical trial, 150 patients with moderate to severe knee OA were randomly allocated to receive duloxetine 30 mg (n=50), gabapentin 300 mg (n=50) or acetaminophen 1000 mg (n=50) all twice a day for 12 weeks. Pain severity was measured using visual analogue scale (VAS) and functional status with Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) before, 2 weeks, one and 3 months after intervention.
Results: Both gabapentin and duloxetine groups had significantly more reduction in pain severity compared to acetaminophen group (p<0.001 for both), with no significant difference between groups (p=0.33). WOMAC total and its subgroups score were significantly lower in duloxetine compared to gabapentin group in 2 weeks and 1 months after intervention, with no significant difference between groups at the end of third month. The percent of change in WOMAC and its subscales scores in both gabapentin and duloxetine groups were significantly higher than acetaminophen (p<0.001), with no significant difference between gabapentin and duloxetine groups.
Conclusion: Both gabapentin and duloxetine have similar and acceptable effects in pain reduction and improvement of functional status in patients with knee OA at the end of third month’s treatment. Duloxetine effects begin from the first weeks, while gabapentin effects begin gradually with the best at the end of third month
Effects of High-Dose Vitamin D Replacement on the Serum Levels of Systemic Inflammatory Biomarkers in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease
Chronic Obstructive Pulmonary Disease (COPD) is associated with increased inflammatory responses to noxious particles, which can be further enhanced during Acute Exacerbation of
COPD (AECOPD). Considering the important immunoregulatory function of vitamin D, high prevalence of Vitamin D Deficiency (VDD) in COPD patients and a negative link between vitamin D levels and inflammatory biomarkers, suggests the seemingly interesting mechanism of vitamin D
effects on inflammation resolution during the conventional treatment of AECOPD. The admitted AECOPD patients with VDD were recruited and randomly allocated to receive either 300,000 IU of
intramuscular vitamin D (n¼35) or placebo (n¼35). Primary outcomes included inflammation
resolution dynamics, which were assessed by monitoring the serum levels of IL-6, IL-8, and hs-CRP.
Symptom recovery was evaluated based on the modified Medical Research Council (mMRC) dyspnea scale on the 1st and 6th days of admission. Secondary outcomes included the length of hospital stay (LOS) and 30-day mortality rates. Inflammatory biomarkers were highest at Day 1.
Baseline vitamin D levels were 11.25 ± 3.09 and 10.59 ± 3.90 ng/ml (P¼0.45), which reached 11.35 ± 3.16 and 18.17 ± 4.24 by Day 6 (P<0.001) in the placebo and, vitamin-D groups, respectively.
IL-6 levels significantly decreased in the vitamin-D vs. placebo group on the 6th day (P¼0.02); however, no significant differences were observed in IL-8 (P¼0.15) and hs-CRP (P¼0.24) levels, mMRC scale (P¼0.45), LOS (P¼0.20), and mortality rates (P¼0.61). Vitamin D replacement as adjunctive therapy may accelerate inflammation resolution in hospitalized AECOPD patients. Further studies were needed to establish vitamin D exact role on inflammation resolution
in AECOPD
