Prevalence of neuropathic pain and factors affecting sleep quality in women with breast cancer after radiotherapy
Introduction: Neuropathic pain can be one of the main causes of decreased sleep quality, and undesirable sleep quality has negative effects on quality of life and treatment continuation. This study was performed with aim to evaluate neuropathic pain and factors affecting sleep quality in women with breast cancer after radiotherapy.
Methods: This descriptive-correlational study was performed from 21March 2017 to 20February 2019 on women with breast cancer undergoing radiotherapy in Imam Reza Educational Center, Tabriz University of Medical Sciences. The tools included demographic characteristics, VAS pain questionnaire, and Pittsburgh Sleep Disorders Questionnaire which measured neuropathic pain and sleep status. Data was analyzed by SPSS software (version 20) and multivariate regression test. P <0.05 was considered statistically significant.
Results: Mean of pain severity in patients participating in the study was 06.55±1.03 and mean score of sleep was 12.29±3.20. According to regression results, the variables of age (P=0.012), pain intensity (P=0.001), mastectomy (P=0.011), lymph nodes removal (P=0.03), number of radiotherapy sessions (P=0.003), history of chemotherapy (P=0.002) and stage of disease (P=0.01) significantly predicted sleep disorder.
Conclusion: Prevalence of neuropathic pain after radiotherapy in women with breast cancer was high in this study, and this pain was the predictor of sleep disorder in these women
Primary Sclerosing Cholangitis Associated with Severe Ulcerative Colitis in a Young Woman: Case Report
Background: Primary sclerosing cholangitis is a rare and progressive cholestatic disease
characterized by narrowing of the biliary tract. Despite high levels of ulcerative colitis, early
diagnosis of primary sclerosing cholangitis before the diagnosis of inflammatory bowel
disease is rarely reported. Case history: A 28-year-old woman with no history of medical disease visited our clinic
because of a mild generalized pruritus. In initial blood laboratory tests with elevated liver
enzymes was observed. In Magnetic resonance cholangiopancreatography (MRCP) was
introduced primary sclerosing cholangitis to the patient. Therefore, the patient was under
colonoscopy and extensive colitis with terminal ileum involvement was detected and the
diagnosis of ulcerative colitis was confirmed in the pathology. Conclusion: Patients diagnosed with primary sclerosing cholangitis should undergo biopsy
colonoscopy to rule out inflammatory bowel disease, even if they have no digestive tract

The AP‐1 pathway; A key regulator of cellular transformation modulated by oncogenic viruses
Cancer progression is critically associated with modulation of host cell signaling pathways. Activator protein‐1 (AP‐1) signaling is one such pathway whose deregulation renders the host more susceptible to cancer development. Oncogenic viruses, including hepatitis B virus, hepatitis C virus, human papilloma virus, Epstein‐Barr virus, human T‐cell lymphotropic virus type 1, and Kaposi's sarcoma‐associated herpes virus, are common causes of cancer. This review discusses how these oncoviruses by acting through various aspects of the host cell signaling machinery such as the AP‐1 pathway might affect oncoviral tumorigenesis, replication, and pathogenesis. The review also briefly considers how the pathway might be targeted during infections with these oncogenic viruses
The Effect of Microchimerism on Frequency and Severity of Organ Involvement in Iranian Scleroderma Patients
Microchimerism is defined as the presence of non-self and circulating cells in a host. The current study aimed to assess the effect of microchimerism on scleroderma major organ involvements. This cross-sectional study was conducted on 56 scleroderma patients registered in a tertiary rheumatology center of Shiraz University of Medical Sciences. Information on the patients’ demographics and disease complications was gathered through a review of medical records. Skin score was applied to better assess skin thickening. High Resolution CT-scan as well as pulmonary function test (PFT) results were also used to investigate pulmonary involvement in patients. Y chromosome serum levels were measured using Phenol Chloroform Extraction protocol and following real-time PCR. Fifty-six scleroderma patients with a mean age of 46±10 years were recruited in this study (58.9% with diffuse scleroderma and 41.07% with limited scleroderma). Other than skin thickening, the most common clinical presentation among the patients was interestitial lung disease (67.8%). No significant difference was found between Y chromosome levels of patients with either lung, cardiac, renal, or gastrointestinal involvement and those who did not have these complications. Y chromosome serum levels based on the results of PFT were also shown to have no significant difference. Moreover, no association was demonstrated between serum Y chromosome and skin score. The serum level of chromosome Y has no impact on the severity and frequency of major organ involvement in Iranian scleroderma patients
Effectiveness of Cisplatin and Capecitabin regimen as the first line treatment compared to the DCF regimen in pationts referring to oncology clinic of Imam Khomeini hospital in Ardabil with advanced gastric cancer in 2017-2018
Background: Gastric cancer is the third leading cause of cancer death in the world. Ardabil province has the highest prevalence of this cancer in Iran. Chemotherapy is an essential part of treatment. Different chemotherapy regimens have been used, but the presence of toxicants is a limiting cause of treatment. Therefore, considering the importance of the subject and the side effects of the drugs used, we conducted a cross-sectional study to compare the two regimens of cisplatin, capecitabine, and DCF and effectiveness of the therapeutic response, complications, cost, and length of hospitalization.
Aim: The aim of this study was to introduce an effective chemotherapy regimen for patients with advanced gastric cancer with the cost and duration of hospitalization and complication were lower compared to conventional diets.
Materials and Methods: In this cross-sectional study, 53 patients with advanced gastric cancer were enrolled by census and retrospective sampling from October 2017 until the end of October 2018. 21 patients were on cisplatin, capecitabine and 32 were on DCF. Demographic findings, response rate, mortality rate, overall survival (OS) and progression-free survival (PFS), drug toxicity, and length of stay were compared.
Results: In cisplatin, capecitabine and DCF groups, complete response rate (CR) was 0% and 3.1%, relative response (PR) were 42.8% and 31.2%, and stable disease were 33.3% and 46.9%, and progression disease were 19.1% and 15.6%, respectively. The overall response rate (ORR) was 42.8% and 34.3%, respectively. Also, among the drug side effects, only cisplatin-induced neuropathy was prominent, and other side effects and hospitalization associated with complications and costs were fewer.
Conclusion: The results of the present study showed that in patients with advanced gastric cancer, ORR, OS and PFS in cisplatin, capecitabine regimen are relatively equal in comparison with DCF regimen. Also toxicity and duration of hospitalization due to complications and cost of treatment with cisplatin and capecitabine were less. Therefore, seems more appropriate regimes
Assess the result of peritoneal dialysis (PD) in patients with stubile and instuble hemodynamic in Ardabil Emam Komeini Hospital in recent five years
Background and Objective: Peritoneal dialysis is a therapeutic approach in the treatment of patients with irreversible renal failure that has been developed in many countries and in our country this treatment has recently been used in a larger scale.The aim of this study was to evaluate the results of peritoneal dialysis in patients with stable and unstable hemodynamics in Imam Khomeini Hospital in Ardabil during the last five years.
Materials and Methods: In this case series study, we retrospectively reviewed the records of patients undergoing peritoneal dialysis in two groups with and without unstable hemodynamics who were treated at Imam Khomeini Hospital in Ardabil between 2011 and 2016. And etiology of primary disease (diabetic nephropathy, chronic glomerulonephritis, hypertensive nephroclerosis, amyloidosis, reflux nephropathy, etc.) and reasons for choosing patients for this type of treatment (vascular problems and hemodialysis or kidney transplant failure, high age, history) Ischemic heart disease and myocardial infarction, etc. were evaluated and the outcome Results were recorded including completion of treatment, failure to respond to treatment, complications, and death of patients. Serum urea and creatinine levels as well as systolic and diastolic blood pressure and urine volume of patients at the beginning and end of admission were recorded in the checklists and the results were statistically evaluated.
Results: 77.8% of patients in this study had unstable hemodynamics. The majority of patients were male. Toxicity was the most frequent in both leave and death. There was no significant relationship between pre-dialysis urine volume and stable hemodynamics, but there was a significant relationship between hemodynamics and urine volume after dialysis. There was a significant relationship between creatinine, blood pressure, urea before and after dialysis with hemodynamics. But only the relationship between blood pressure and disease outcome was significant.
Conclusion: In this study, the majority of patients undergoing peritoneal hemodialysis were patients with unstable hemodynamics and poisoning. Therefore, peritoneal dialysis can be used in emergency cases
Assessment of occupational health and safety climate in workers small workshops in Ardabil province
Background & Objectives: Workers in small workshops in terms of Facilities and sanitation, occupational accidents and work-related risk factors are adverse conditions and at risk. Assess and identify areas that are most difficult to help plan interventions and training more focused on that part.
Methods: this study kind of cross-sectional study and population of Ardabil province is 368 workshops from small workshops. Going a random the workshops and face to face interviews to gather information. and after completing all the questionnaires using the software SPSS data were analyzed.
Results: In the Obligations of the employer, Environment, health and safety, Understanding risk, Job Satisfaction and awareness was acceptable and in the communications and accountability employer was unacceptable. Age, work experience, education Climate occupational health and safety not correlated and the incidence of work-related disease
and a history of occupational accidents was significant relationship.
Conclusions: Overall climate Occupational Health and Safety is acceptable. With the information available, Create conditions for dialogue between workers, Transfer experiences people who have past caused by work accidents and disease, and adopt strategy for participation employer the design and implementation of occupational health and safety programs Can be applied to improve the situation
The relationship between healthy behaviors and BMI with dental caries in primary schools in Ardabil city in 2017
Background and objective: School plays a significant role in establishing and adhering to healthy behaviors in children. Dental caries is one of the crucial problems associated with lack of adherence to the desired healthy behaviors in students. The present study aims to determine the relationship between healthy behaviors and dental caries in elementary school students in Ardabil city, Iran, in 2018.
Methods: The present study is a correlation study which was conducted on 581 elementary school students selected through cluster random sampling method in Ardabil city in 2018. Data were collected using questionnaires, including demographic information questionnaire, global school-based student health survey questionnaire containing questions related to dietary and hygiene behaviors, and physical activity questionnaire PAQ-C. Anthropometric (height, weight, BMI) indices were measured using the weighing scale and stadiometer, and dental caries was evaluated using the decayed, missing or filled teeth index (DMFT for permanent and dmft for primary teeth).
Results: The mean age of participants was 9.61±1.76 years, and %50.8 (295) of them were female. %20.7 (120) were obese, and %19.1 (111) were overweight. The overall prevalence of caries was %74.9 (n=435) in primary teeth, %72.8 (n=423) in permanent teeth, and %96.6 (n=561) in all teeth. The average dmft, DMFT, and dmft+DMFT (all teeth) scores were 4.16±3.72, 1.96±1.76 and 6.1±3.5, respectively. There was a significant inverse relationship between BMI and caries index of primary teeth (r=-0.188 and P<0.0001), permanent teeth (r=-0.177 and P=0.004), and all teeth (r=-0.212 and P<0.0001). There was no relationship observed between dental caries and healthy behaviors (including dietary, hygiene and physical behaviors).
Conclusion: The findings showed that dental caries prevalence among elementary school children in Ardabil city is higher than global standards, obese and over-weight children experience fewer caries compared to normal-weight children, and no relationship exist between healthy behaviors and dental caries
Evaluation of bone density and its relationship with radiographic stage in knee osteoarthritis in years 2017-18
Backgrounad and objective: Osteoarthritis is the most comon arthritis and its incidence is directly related to age. According to the related research in the last 30 years , a relationship is reported between severity of osteoarthritis and bone density. Bone density is measured by the means of dual energy x-ray absorptiometry (DEXA scan) in practice and the severity of osteoarthritis is evaluated based on radiographic images and Kellgren Lawrence criteria . In this study we measured these two parameters in our patients to find an association between them.
methods: 123 patients with knee osteoarthritis diagnosis who referred to rheumatology clinic at Imam Khomeini hospital in Ardabil, were selected to participate in our study . Age, sex, weight , height and other variables were collected and analyzed with SPSS program.
Results: A significant correlation is found between Tscore and age of the patients. The Pearson correlation coefficient was -0.38. A significant correlation was also found between Tscore and age and also between Tscore and BMI. For evaluating correlation of Tscore and radiographic stage of patients with osteoarthritis, we used Pearson correlation and the result was positive (coefficient= -0.22). we also conducted multiple regression models to assess each factor's significance. Only age and BMI had a significant effect on Tscore.
Conclusion:A relationship was found between radiographic stage and Tscore in correlation statistics . In regression models , Only BMI and age had a significant effect on Tscore. These results are in accordance to previous studie
