Three-component synthesis of chromeno β-lactam hybrids for inflammation and cancer screening
Highly diastereoselective synthesis of chromeno β-lactam hybrids was achieved by an efficient one-pot three-component reaction. With this procedure, the desired β-lactam products were obtained in good yields and with exclusive cis stereoselection, by combining a variety of benzaldehydes, malononitrile, and either 5,5-dimethylcyclohexane-1,3-dione or 4-hydroxycoumarin in the presence of 1,4-diazabicyclo [2.2.2]octane under reflux conditions. These adducts were structurally characterized on the basis of IR, 1D and 2D NMR spectra, X-ray analysis, H–H COSY and H–C HSQC two-dimensional NMR experiments, and elemental analysis. Each of the synthesized compounds was screened for anti-inflammatory and anticancer activities. β-Lactams 5b and 8b showed a 53.4 and 19.8 anti-inflammatory ratio, respectively, and 5b appeared more active than the well-known dexamethasone corticosteroid used for the treatment of rheumatoid and skin inflammation. β-Lactams 5a, 5b, 5e, 5f, 5g, 8c, 8j and 8p also showed good antitumor activity against the SW1116 (colon cancer) cell line without notable cytotoxicity towards the HepG2 control cell line
The evaluation of pharmacy space design & its effect on pharmacists’ and patients’ attitude on the quality of pharmaceutical services
Background: In spite of the great efforts to stabilize pharmacist’s position in health care system, most of the people consider the pharmacists as a seller and distributor of drugs rather than professional medicine counselor.Improper design of Pharmacy such as “unsuitable counter design, glass barrier, lack of acceptable waiting area and counselling area “are categorized as important barriers in patient-pharmacist interaction. The aim of the present study was evaluation of pharmacy space design & its effect on pharmacists’ and patients’ attitudes on the quality of pharmaceutical services.
Methods: This descriptive and analytical study was carried out in two phases. In first phase 30 pharmacies were evaluated according to four perspective which include: counter height, glass barrier, waiting area and counselling area. In second phase of study, the effect of these parameters on pharmacist and patient’s attitudes toward the quality of pharmaceutical services were assessed through two type of questionnaire for patients and pharmacists (30 pharmacists & 90 patients).data were obtained and analyzed through appropriate statistic tests. SPSS version 23 was used for data analysis.
Results: There were significant correlation between glass barriers, waiting area, counseling area with quality of counseling in both groups. In the absence of glass barrier, presence of appropriate counselling and waiting area, the quality of counselling was higher in both groups. There was significant correlation between height of counter and quality of counselling in patient group and at the height of 75-95 cm, the quality of counselling was the highest. No significant difference was observed in pharmacist group for height of counter.
Conclusions: This study concluded that improper design of pharmacy is one of the most important barriers in patient-pharmacist relationship. This problem has negative effects on the quality of counseling and eventually patient’s satisfactio
The Pattern of Imipenem/Cilastatin Administration in Imam Khomeini Hospital (Ardabil) in 2018
Background & objectives: Nowadays, bacterial resistance and the increase in the therapeutic costs are considered as the most important global concerns of medical care system regarding complicated infections. Imipenem is a member of the carbapenem class of beta-lactam antibiotics prescribed mostly in our hospitals because of its broad activity against bacterial infections. Drug Utilization Evaluation (DUE) process is an official, ongoing and systemic program that collects information in order to identify and improve the adverse effect of drugs and the cost of medicalization. The objective of this study was to evaluate the administration and use of imipenem in the Imam Khomeini Teaching hospital in Ardabil in 2018.
Methods: In this prospective, descriptive, cross- sectional study, 110 hospitalized patients, who received imipenem from September to December of 2018, were included in this study. Patient's demographic data, dosing, dosage adjustment in renal failure and other co-prescribed antimicrobial drugs were extracted from current medical file of hospitalized patients and evaluated with medical guidelines.
Results: Sixty four percent of patients received imipenem in the first day of hospitalization and 75.5% of patients were empirically received imipenem while antibiogram test was ordered for only 24.5% of patients. Serum creatinine were ordered for most of the patients, but correct dose regimens for patients who get non-empiric antibiotic therapy were only 25.5%.
Conclusion: High rate of empiric prescription without considering the result of antibiogram test and immediate initiation of antimicrobial therapy at the time of admission were the most important aspects of irrational use of imipenem observed in this study. Paying more attention to sampling, culturing and sensitivity test and prescription of imipenem based on specific guidelines are recommended
Investigating Hospital Preparedness in Ardabil Province Against Unexpected Accidents
Background: The most crucial issue in the disaster and accidents is the health management of the affected community because accidents and disasters always have a significant impact on hygiene, general health, and the wellbeing of the stricken community. Considering the hospitals’ function and their front line position in managing the victims of accidents and having to continue and even increase their activity at the time of accidents and disasters, they should have a program of coping with accidents. In this regard, the current study investigated the preparedness of hospitals in Ardabil Province, Iran, in response to unexpected events.
Materials and Methods: This was a descriptive, analytical, and cross-sectional study conducted in 15 hospitals in Ardabil Province in 2017. To collect study data, a two-part questionnaire was used. Its first part investigated the demographic data of the hospitals. The second part was the national tool to assess hospital preparedness in accidents and disasters with 91 questions in 9 areas of preparedness (command and control, communication, safety and security, triage, capacity increasing, continuity of essential services, workforce, resource management, and post-disaster recovery). The collected data were analyzed using descriptive statistics.
Results: The findings of this study revealed that out of the 15 studied hospitals, only 4 were well prepared, 3 were poorly prepared, and 8 hospitals were moderately prepared for crisis management. On average, out of the 9 investigated aspects, triage, human resource management, and recovery after disasters had the lowest status.
Conclusion: Considering the obtained results and the importance of preparing for unexpected events, it can be useful to plan and have an explicit instruction, and then implement and evaluate it to increase the preparedness to confront unexpected accidents in all hospitals
Prevalence of malocclusions in anterior-posterior aspect of 11-13 years old female students in Ardabil schools in 2018-2019
Background: Malocclusion is a dentistry term for incorrect relation between the teeth. Since many factors such as inheritance, race, environmental factors such as bad habits and early loss of primary teeth play an important role in these abnormalities and the age range of maximum growth of girls is 13 to 11 years, the present study was conducted with the aim of determining prevalence of malocclusions in anterior-posterior aspect of 11-13 years old female students in Ardabil schools in 2018-2019.
Materials and Methods: A total of 400 female students aged 11-13 years old in Ardabil schools were randomly selected after obtaining a license from Ardabil University of Medical Sciences and Health Services. The extent of horizontal (anterior) overlap of the maxillary central incisors over the mandibular central incisors was measured using periodontal probe. The occlusal relationship of the left and right molar and canine teeth was recorded according to the Angel classification.
Results: 400 female students of whom 122 (30.5%) were 11-year-old, 150 (37.5%) were 12-years-old, and 128 (32%) were 13-year-olds were evaluated. The prevalence of bad oral habits was 12.3%. Anterior crossbyte was observed in 34 (8.5%) patients. The prevalence of molar occlusion of class I, II, and III was 71.5% (286 cases), 14.5% (58 patients) and 14% (56 patients), respectively. The prevalence of canine occlusion of class I, II, and III was 55.3% (221 cases), 17% (68 patients) and 6.6% (26) respectively and permanent canine teeth were not erupted in 85 (21.3%) patients. The prevalence of normal overjet was 71% (284) and the prevalence of increased, decreased, and reversed overjet was 23.8% (95 people), 4.3% (17 people) and 1% (4 people), respectively. Facial profile was straight in 267 people (66.8%), convex in 108 people (27%), and concave in 25 people (6.3%) Conclusion: The present study among female students aged 11 to 13 years old in Ardebil showed that like most similar studies, class I occlusion has the highest prevalence and Class III has the lowest prevalence; Also, the most common overjet is normal and the most common facial profile was straight
Prevalence of Musculoskeletal Disorders in Steel Industry Workers and its Association with RULA’s Method Results
Aim:Musculoskeletal Disorders (MSDs) are the main cause of occupational disorders and disabilities in the developing countries. The goal of this study was to assess the prevalence of the MSDs in steel Industryworkers using Nordic Musculoskeletal Questionnaire (NMQ), and its relationship with the Rapid Upper Limb Assessment (RULA) results. Instruments & Methods: The present research was a descriptive-analytic study conducted on the Iranian steel industry in 2018. A total of 17 workstations were randomly selected and NMQ was used to explore the prevalence of the symptoms of Work-related Musculoskeletal Disorders (WMSDs). Afterwards, the workers’ postures were assessed via RULA. Finally, the results were analyzed in SPSS 22 through the chi-square test, independent t-test, and analysis of variance (ANOVA) method.Findings:This study showed the significant relationship between the results from the Nordic assessment of the back, knee, and neck within the past 12 months and profession type (p0.05).Conclusion: The findings of this study revealed that in order to considerably reduce the musculoskeletal disorders in the steel industry workers, immediate measures must be taken to correct the back and neck postures especially in the scrap shear operators, welders, ingot shear operators, electrical technician, tower operator, and guillotine operators
Immunophenotype study by flow cytometry in patients with acute myeloblastic and lymphoblastic leukemia in Buali Educational and Medical Center of Ardabil province from September 2010 to October 2018
Background and Aim: Leukemia is one of the most common types of malignancy in children, with an approximate prevalence of 40 people per million people, accounting for about 30% of all cancers in children under 15 years.
Aim:Due to the high rate of this disease, a reliable method is necessary for the correct diagnosis of the disease. In this dissertation, we have studied immunophenotype by flow cytometry in patients with acute myeloblastic and lymphoblastic leukemia in Bu Ali center of Ardabil.
Materials and Methods: In this study, which was performed at Bu Ali Ardabil Medical Center, 131 patients who were admitted with suspected leukemia and bone marrow biopsy were performed, one sample was sent for pathology and one sample was sent for flow cytometry. And the results were included in the study.
Background : The highest frequency of the disease was in all pre-B cell age groups and the highest age of onset was the same as AML in 1 to 10 year olds. Based on the frequency distribution of acute leukemia based on flow cytometry, in our study 107 patients (81.7%), ALL and 19 patients (14.5%) AML and 1 case of Hodgkin's lymphoma (0.8%), 1 case of CML (0.8%) and 3 cases (2.3 %) Was reported normal.
Results: A reliable method for the correct diagnosis of leukemia as one of the most common types of malignancy in children is essential. In our study, 52.7% of patients are pre-cell ALL in terms of flow cytometry, of which 90.08% are consistent with pathology and 9.92% are inconsistent. Due to the high rate of 9.92%, inconsistencies between diagnostic methods of flow cytometry and pathology should be considered Perform a more accurate diagnostic procedure. Also, due to the weakness of flow cytometry in the diagnosis of AML M4 and AML M6, other methods such as cytology should be considered to diagnose these types

The study of the level of depression and its associated educational factors in Boys' high Schools students of Gifted and Ordinary schools in Ardabil during the 2018-2019
Background and objective: An adolescent who goes through the childhood and adolescents' ages ranging from 11 to 20 years is subject to significant changes. One of the most common and debilitating problems in adolescents is depression which its consequences include; poor mental and educational performance, role impairment, increased conflict with parents, and suicidal thoughts or suicide attempts. The aim of the present study is to compare the level of depression and its related educational factors among high school students in Gifted high schools and ordinary schools in the city of Ardabil in the school 2018-2019.
Methods:In this descriptive-analytical study, the sample was randomly selected from all male students of Gifted high school and the second year of high school from ordinary schools and data were collected through a questionnaire. The data collection tools were children's depression inventory (CDI), Beck Depression Inventory and demographic inventory.
Results: In this study 160 high school students including 120 Gifted students in first, second and third grades of high school and 40 ordinary school students in second grade were analyzed and compared.
Studying the level of parents' education in students showed that in 60% of students at least one of the parents had university education and in ordinary students, 42.3% at least one of the parents had university education.
The findings showed that there was no relationship between the level of depression and the level of education of the parents, the amount of parental assistance in the courses, the study hours, the study schedule, the parents' expectations of academic performance, the amount of anxiety caused by the exams, the thinking of the exam, punishment in the school, the parent's divorce and taking part in the refresher courses. There are relationships between variables of the level of punishment by the parents due to the study issues, the degree of teacher satisfaction, the feeling of school safety, the competition, the satisfaction of the class hours, the satisfaction of the examinations, the death of one of the parents, the satisfaction of the principal, and the economic and the variables of the level of depression.
Conclusion:In general, seemingly, depression in Gifted high school students is more than ordinary schools and this depression can be due to family and study pressures in these individuals, but the family's economic situation continues to have the greatest impact on the depression level
