The Evaluation of the Relationship between the Metabolic Syndrome and Periodontal Disease in the Patients Referring to Imam Khomeini Hospital in Ardabil in 2018 and 2019.
BACKGROUND&OBJECTIVE: Metabolic syndrome (MetS) is a common chronic condition that increases the cardiovascular disease risk and is also linked to periodontitis. The study aim was to determine if an association exists between periodontal disease severity, parameters and metabolic syndrome (MetS) in a group of northwestern Iranian patients.
METHODS: A total of 100 participants (50 males and 50 females, age range 25- 78 years) including 50 patients with MetS and 50 participants without MetS were selected for this study. Information on sociodemographic characteristics, dental behaviours and smoking status was collected from a self- administrated questionnaire. Periodontal status including pocket depth (PD), clinical attachment level (CAL) and bleeding on probing (BOP) as well as number of missing teeth was recorded. The Independent sample t test were used to evaluate the differences between periodontal disease parameters and MetS between the 2 groups.
RESULTS: CAL, BOP and missing teeth were significantly higher in the MetS group than in the control group and was significantly increased by a number of MetS components. PD did not change between two group. The severity of periodontal disease, as measured by average PD and average CAL was significantly greater among patients with MetS compared to those without MetS. Patients with dyslipidemia, hypertention and central obesity without dysglycemia have CAL, BOP and missing teeth higher than control group (P < .05).
CONCLUSION: Much severe periodontal disease was found in MetS participants and increased with number of MetS components. Participants with MetS had higher periodontal parameters (except PD). Also with eliminating of diabetic patients, results did not change
Identification of a potential SARS-CoV2 inhibitor via molecular dynamics simulations and amino acid decomposition analysis
Considering lack of validated therapeutic drugs or vaccines against contagious SARS-CoV2, various
efforts have been focused on repurposing of existing drugs or identifying new agents. In an attempt
to identify new and potential SARS-CoV2 inhibitors targeting specific enzyme of the pathogen, a few
induced fit models of SARS-CoV2 main protease (Mpro) including N-aryl amide and aryl sulfonamide
based fragments were subjected to a multi-step in silico strategy. Sub-structure query of co-crystallographic
fragments provided numerous ZINC15 driven commercially available compounds that entered
molecular docking stage to find binding interactions/modes inside Mpro active site. Docking results
were reevaluated through time dependent stability of top-ranked ligand-protease complexes by
molecular dynamics (MD) simulations within 50 ns. Relative contribution of interacted residues in binding
to the most probable binding pose was estimated through amino acid decomposition analysis in
B3LYP level of theory with Def2-TZVPP split basis set. In confirmation of docking results, MD simulations
revealed less perceptible torsional distortions (more stable binding mode) in binding of
ZINC_252512772 (DGb �9.18 kcal/mol) into Mpro active site. H-bond interactions and hydrophobic
contacts were determinant forces in binding interactions of in silico hit. Quantum chemical calculations
confirmed MD results and proved the pivotal role of a conserved residue (Glu166) in making permanent
hydrogen bond (98% of MD simulations time) with ZINC_252512772. Drug-like physicochemical
properties as well as desirable target binding interactions nominated ZINC_252512772 as a desirable
in silico hit for further development toward SARS-CoV2 inhibitors
Prevalence and causes of thrombocytopenia in neonates of the neonatal intensive care unit in Booali hospital in Ardabil in 2016-2019
Background: Thrombocytopenia is the most common hematologic disorder in neonates in neonatal intensive care unit(NICU). Intrauterine infections and maternal disorders like hypertension and preeclampsia wich are associated with acute or chronic placental insufficiency, are primary causes of early-onset thrombocytopenia. Late-onset thrombocytopenia is usually indicator of severe disease late-onset thrombocytopenia is an indicator of severe disease especially in case of a rapid decline in platelets.
Aim: The aim of this study was to determine the prevalence and causes of thrombocytopenia in NICU of Booali hospital in Ardabil.
Material and Methods: To determine the prevalence of thrombocytopenia in neonates of NICU in Booali hospital of Ardabil, all of the neonates admitted to the NICU from years 1395 through 1397 were evaluated. Gender, gestational age, birth weight, major bleeding , maternal-placental issues, sepsis, blood transplantation and ... were assessed in these patients. Quantitative and qualitative tests were conducted in SPSS software 25. Patients’ information was kept private and results published without patient names; in accordance to ethical guidelines.
Results: Overall prevalence of thrombocytopenia in this study was 28.19 percent. The mean age and weight of neonates with thrombocytopenia were 3.60 days and 2123.43 grams respectively. In neonates with thrombocytopenia the mean gestational age was 33.05 weeks. In terms of neonatal factors, 111 neonates had respiratory distress syndrome, 80 neonates had jaundice and 83 neonates had sepsis. Among neonates, 76 patients (50.30 percent) survived their disease and 70 patients expired.
Conclusion: Prevalence of thrombocytopenia in neonates (28.19 percent) is in complete agreement with prior studies. The highest rate of thrombocytopenia has been reported in NICU where 12 to 35 percent of patients have thrombocytopenia. In this study 45 patients (29.80 percent) with thrombocytopenia had a major bleeding. In similar studies only 5-15 percent of severe thrombocytopenic patients had a major bleeding event in NICU
Patterns of high-risk behaviors associated with HIV among male prisoners: A latent class analysis
Background: Acquired Immunodeficiency Syndrome (AIDS) is one of the most important health challenges worldwide. Moreover, the prevalence of the Human Immunodeficiency Virus (HIV) infection differs among Iranian prisoners and the general population. The present study aimed to identify the patterns of HIV-related high-risk behaviors in male prisoners.
Methods: In this cross-sectional study, 2832 inmates were examined for HIV-related high-risk behaviors. The required data were collected using a questionnaire on high-risk behaviors, including a history of heterosexual and homosexual intercourse, and a history of drug use disorders. A Latent Class Analysis (LCA) was performed to analyze the obtained data in WinLTA software.
Results: Five latent classes were defined for the male prisoners, as follows: low-risk (20%), moderate-risk (23%), Injection Drug Use (IDU) (8%), heterosexual intercourse/methamphetamine use (38%), and high-risk (11%). The LCA results revealed that high-risk sexual behaviors, IDU, and sharing injection equipment in prisons significantly influence the classification. The obtained data suggested that a history of imprisonment was not frequent in class 2 (OR=1.0033, %95CI:0.9936–1.01) and class 4 members (OR=1.0053, %95CI:0.9929–1.0179). However, it was more prevalent in class 3 (OR=1.0164, %95CI:1.0068–1.0262), and 5 (OR=1.0211, %95CI:1.0129–1.0293). Heterosexual contact had the highest prevalence (75%) in this regard. The results showed that the lowest prevalence of high-risk behaviors was associated with morphine use (0.3%). Among illicit drugs, methamphetamine was the most prevalent drug (42%) in the studied subjects.
Conclusion: The present study indicated a high prevalence of high-risk sexual behaviors and methamphetamine use in the investigated participants. We observed the co-occurrence of HIV-related high-risk behaviors in male prisoners. Thus, providing safe sex education for prisoners is highly recommended. It is also necessary to pursue care programs about IDU as a key risk factor for HIV transmission in prisons
Evaluation of Gelatinolytic and Collagenolytic Activity of Fasciola hepatica Recombinant Cathepsin-L1
Background: Cysteine proteases of the liver fluke, Fasciola hepatica, participate in catabolism of proteins, migration of the fluke through host tissues and combat host immune system. Objectives: In this study, we evaluated proteolytic activity of F. hepatica recombinant cathepsin L1 (rCL1) against gelatin and collagen as common substrates. Material and Methods: The coding sequences of F. hepatica CL1 were cloned and expressed in E. coli, in our previous study. The rCL1 was purified by nickel affinity chromatography with a HisTrap Column. The protein concentrations of the purified fractions were determined by Bradford assay. Rat collagen type-1 was treated with distinct amounts of rCL1 at 37 °C, overnight, and the byproduct was analyzed by SDS-PAGE. Furthermore, we used bovine skin gelatin as zymography substrate to evaluate the gelatinolytic activity of the purified rCL1. Results: Recombinant CL1 was capable to digest intact type-1 collagen within 24 h and the gelatinlytic activity of rCL1 was visible at approximately 37 kDa region, with optimal activity at acidified conditions (pH 4). Conclusion: Findings provide a possible mechanism by which a major secretory molecule of F. hepatica could be involved in parasite survival as well as its pathogenesis
Calcitriol and erythropoietin protect against cardiac injury induced by renal ischemia reperfusion
Cardiac abnormalities and dysfunction are the most important complications after renal ischemia-reperfusion (IR). Thus, investigation and development of effective treatment to decrease cardiac damage induced by renal ischemia are necessary. This study examined the effects of treatment with calcitriol and erythropoietin (EPO) on cardiac injury induced by renal ischemia. Wistar albino rats were unilaterally nephrectomized and subjected to 45 min of renal pedicle occlusion, followed by 24 h reperfusion. Calcitriol and EPO were administered before ischemia. After 24 h reperfusion, blood samples were collected for the determination of biochemical parameters, and kidney and cardiac samples were taken for histological studies. Renal IR increased BUN-Cr levels, lipid profiles, and myocardial injury markers (CK-MB and LDH). Histopathological findings of the IR group confirmed that there were glomerular atrophy and acute tubular necrosis in the renal tissues and lymphocyte infiltration and intercellular edema in the cardiac samples. Treatment with calcitriol and EPO boosted cardiac and renal functions and improved the morphological changes. It seems that calcitriol or EPO administration could protect against the kidney and cardiac damage induced by IR. Also, the combination of calcitriol and EPO may exert more beneficial effects than either agent used alone
A study of the lifestyle of patients referred to Imam Khomeini Hospital in Ardabil with st-elevation MI in 2020
Background: The access rate of angina pectoris, acute myocardial infarction and sudden death has been investigated epidemiologically, that they are different according to the risk factors, age, sex and origin at the individual level and they’re also different based on countries, geographical regions and caste at the demographic level during the time. The purpose of this study is to evaluation patient's lifestyle that have referred to Imam Khomeini Hospital, Ardabil, in 2020.
Aim: To determine patient’s lifestyle with st-elevation MI that have referred to Imam Khomeini Hospital, Ardabil.
Matherials and Methods: In this cross-sectional study the statistical population was patients with st-elevation MI that have referred to Imam Khomeini Hospital from Summer 2019 to the end of spring 2020. the sample size was 245 people.
Results: Among 245 patients in this study 170 (67.3%) of them were men and 95 (32.7%) were women. the mean age of men was 63/1 by 21.7 standard deviation and women mean age was 64.3 by 20.3 standard deviation. as a matter of physical activity 151 patients (61.6%) had low degree of physical activity, in other words statistical analysis by chi-square techniques demonstrated that the majority of the patients in our society have a low-grade of physical activity. regarding the association between BMI and mean sex BMI was 29.3 in men and 34.2 in women. Statistical analysis has demonstrated that there is no significant difference in salt consumption between the sexes.concerning cigarette smoking among 245 patients indicates that 125 people has consumed cigarette, 66.6% of men (22.1p/y) and 18.7% of women (15.2p/y). statistical analysis by chi-square techniques shown that in cigarette consumption there is a significant difference between the sexes.The average daily sleeping in men is 6.25 hours a day and it is 7.1 hours in women. statistically there is no affiliation between sex and the sleep. About the type of the consumable oil 71 (47%) of urban patients used saturated oil and 80 (53%) of them used unsaturated oil. and about rural areas dominant of the consumable oil was filled by saturated one, thus 80 (85.1%) of them used saturated oil. 29 men (17.5%) and 7 women (8.7%) consumed drugs (opium). 13 (7.8%) of men and 4 (5%) of women has consumed methadone. and about tramadol consumption, generally 3.6% of patients consumed tramadol. On average, 41 men consumed 0.07 noggin (shot of alcohol) on a daily basis and it was 0.03 in women. statistical analysis has shown that there is a significant difference between sex and alcohol consumption. Each noggin is considered equivalent to 50cc .
Conclusion: 29 men (17.5%) and 7 women (8.7%) consumed opium . In other words according to the study by Amani and the other co-workers opium consumption rate is about 1% nationwide, that indicates it’s high statistic in STEMI patients. motionless and overweight have a high prevalence in heart disease patients. The form of dominant consumable oil in these patients is saturated oil that is more conventional in rural areas
Study on the serum cholinesterase activity in women with polycystic ovarian syndrome
Background: Polycystic ovarian syndrome (PCOS) is a chronic complex disorder that affects about 1 in 10 women worldwide. There is a complex relationship with genetic, metabolic, endocrine, environmental, and lifestyle factors in PCOS and the etiology that remains poorly understood. Insulin resistance has been associated highly with PCOS. It is believed insulin resistance is the main pathogenic factor related to the increased rate of metabolic disturbances among women with PCOS. Butyrylcholinesterase (BuChE) is a non-specific type of cholinesterase enzyme that hydrolyzes different types of choline esters. Butyrylcholinesterase may be indirectly involved in the pathogenesis of insulin resistance. Studies have shown that BuChE may be indirectly contributing to the pathogenesis of type 2 diabetes mellitus by causing insulin resistance. This study was conducted to address the question of whether women with polycystic ovarian syndrome have higher butyrylcholinesterase activity than controls and also to determine the association between the activity of this enzyme and insulin resistance.
Methods: With a case-control study, 56 women diagnosed with PCOS according to the Rotterdam 2003 criteria as case group and 62 non-PCOS women as control group were enrolled in the study. Menstrual status along with metabolic parameters such as fasting serum insulin, fasting plasma glucose (FPG), homeostatic model assessment of insulin resistance (HOMA-IR), body mass index (BMI) were measured. BuChE activity was measured by using Ellman assay.
Results: women with PCOS had significantly higher menstrual irregularity (P=0/001), FIns (P=0/008), insulin resistance (P=0/014) and butyrylcholinesterase activities (P=0/004) than non-PCOS women. In women with PCOS, BuChE activity was significantly correlated with BMI. In Insulin resistant women, BuChE activity was significantly higher in case group (P=0/012).
Conclusion: These results indicate that higher activity of BuChE is associated with pathogenesis of PCOS and insulin resistance could be a link for this connection.

A Qualitative and Quantitative Assessment of the Risk of Human Errors in Midwifery Tasks in Child Delivery Ward Using Engineering Approach (EA) and Predictive Human Errors Analysis (PHEA)
Introduction: Maternity wards are of the highly sensitive wards in hospital. Errors in midwifery tasks can lead to life threatening risks for the mother and infant and higher medical costs. The present study is an attempt to qualitatively and quantitatively assess human errors in midwife staff using preventive human error analysis and engineering approach. Methods: The study was carried out as a case study in the maternity ward of Shoushtar Women Hospital. The participants were selected through convenient sampling and seven midwifery experts took part in the study. The work process in maternity ward was categorized into four categories of admittance, pre-labor, labor, and post-labor and the tasks and sub-tasks were determined based on hierarchical task analysis (HTA). Afterwards, human errors were quantified using EA technique and then, using PHEA technique a description of error in each tasks and error control solutions were provided. Results: The results clarified that the highest risks of human errors were in the tasks like cervix check, serum therapy, infant’s body check, preparing delivery equipment, and wearing personal protective equipment. Conclusion: Since, midwifery tasks are rule base and regulations and they are performed at a higher level of awareness and cognition, preventing errors entails continuous presence of a midwife next to patient’s bed along with an assistant midwife and codification of an infant examination checklist. Programming empowerment education including safety education to midwives is also recommended
