    Effect of magnesium on arrhythmia incidence in patients undergoing coronary artery bypass grafting

    Background: Cardiac arrhythmia after coronary artery bypass grafting (CABG) surgery is a common complication of cardiac surgery. The effect of serum magnesium, hypomagnesaemia treatment and prophylactic administration of magnesium in the development and prevention of arrhythmias is controversial and there are many different ideas. This study evaluates the therapeutic effects of magnesium in cardiac arrhythmia after CABG surgery. Methods: The clinical trial enrolled 250 patients who underwent CABG. Based on the initial serum levels of magnesium, patients were divided into two groups: hypomagnesium and normomagnesium. Based on bioethics committee requirements, patients in the hypomagnesium group received magnesium treatments until they attained normal magnesium blood levels. Both groups underwent CABG with normal blood levels of magnesium. After surgery, each group was randomly divided into two subgroups: one subgroup received a bolus dose of magnesium sulphate (30 mg/kg in 5 min) and the other subgroup received a placebo. Subgroups were under observation in the intensive care unit for 3 days and arrhythmias were recorded. Data from all four subgroups were analysed statistically and interpreted. Results: The results of this study showed that the occurrence of arrhythmia was not significantly different among subgroups (P > 0.05). There was no significant relationship between blood levels of magnesium and arrhythmia during the 3 days post-surgery (P > 0.05). Conclusion: The results of this study showed that magnesium sulphate administration did not significantly improve the incidence of arrhythmias in hypo- and normo-magnesium patients after CABG. There was no significant correlation between post-operative serum levels of magnesium and arrhythmia during 3 days

    Evaluation of Total and Free Testosterone Levels and Clinical Signs of Hypogonadism in male patients with Type 2 Diabetes

    Background and Objective: Diabetes mellitus is a common metabolic disease. Its association with low level of testosterone has already been shown in many studies. Considering the role of testosterone hormone in impotency, fatigue, and bone mass deficiency this study aimed to evaluate the total and free Testosterone levels and clinical signs of Hypogonadism in male patients with type 2 diabetes. Methods: Sixty-five diabetic male patients aged 35-70 years were randomly selected and obtained personal consent, enrolled to the study. The relevant questionnaire, such as patient demographics, BMI, past medical history, drug history, history of smoking, education level, employment, and etc were completed. Then, the patients were evaluated for total and Free testosterone levels and symptoms related to decreased testosterone, hypogonadism symptoms by ADAM questionnaire. Finally, the data were obtained and statistical analysis was performed. Results: The mean age of patients was 57.12 ± 4.7 years with age range of 38-69 years. According to the ADAM questionnaire, 51 (78.4%) of the patients were positive and 14 (21.6%) were negative. 16 (24.6%) of the 65 patients had hypogonadism. Among ADAM patients, 46 (90.1%) had erectile dysfunction, 33 (64.7%) had decreased libido, and 39 (76.4%) had three positive responses to the questions. Most people (75%) had BMI Hypogonadism of more than 30%. There was a significant relationship between BMI of patients and hypogonadism. Conclusion: Testosterone levels are commonly found low in diabetic men, most of whom have symptoms of hypogonadism. Body mass index is known as an independent risk factor for hypogonadism in T2D men. There was also a significant correlation between free testosterone levels and hypogonadism

    A comparison of three strategies for biopanning of phage-scFv library against diphtheria toxin

    The biopanning process is a critical step in phage display for isolating peptides or proteins with specific binding properties. Conventional panning methods are sometimes not so effective and may result in nonspecific or low-yield positive results. In this study, three different strategies including soluble antibody-capturing, pH-stepwise elution, and conventional panning were used for enrichment of specific clones against diphtheria toxoid. The reactivity of the selected clones was evaluated using an indirect enzyme-linked immunosorbent assay. The positive clones were screened using Vero cell viability assay. The neutralizing clones were expressed in HB2151 strain of Escherichia coli and soluble single-chain fragment variable (scFv) fragments were purified by nickel-nitrilotriacetic acid affinity chromatography. Finally, the ability of scFv fragments for neutralizing diphtheria toxin (DT) were evaluated again using Vero cell viability assay. After four rounds of panning, the soluble antibody-capturing method yielded 15 positive phage-scFv clones against diphtheria toxoid. Conventional panning and pH-stepwise elution model resulted from nine and five positive phage-scFv clones, respectively. Among all positive clones, three clones were able to neutralize DT in Vero cell viability assay. Two of these clones belonged to a soluble antibody-capturing method and one of them came from conventional panning. Three neutralizing clones were used for soluble expression and purification of scFvs fragments. It was found that these soluble scFv fragments possessed neutralizing activity ranging from 0.15 to 0.6 µg against two-fold cytotoxic dose 99% of DT. In conclusion, the results of our study indicate that soluble antibody-capturing method is an efficient method for isolation of specific scFv fragments

    Chemical Compounds Isolated from Aerial Part of Primula auriculata L.

    Arial parts of Primula genus (Primulaceae) are well known to treat various ailments in Iranian Traditional Medicine. Our preliminary studies demonstrated that the medium polar to polar extracts of Primula auriculata had a significant cytotoxic activity and Induction of apoptosis effects on some cancerous cell lines. The current study focused on phytochemical investigation and characterization the compounds from P. auriculata active extracts. Dried and powdered aerial part of plant was macerated with petroleum ether, dichloromethane, and methanol successively, for 24 hours, three times. Dichloromethane and methanolic extracts were separately examined by different chromatographic techniques such as VLC, CC, and TLC to Fractionation and isolation compounds present in the plant extracts. The structures of purified compounds were elucidated on the basis of spectral data, particularly H-NMR and C-NMR experiments. 2-phenylchromone and 3, 4, 5-trihydroxy benzoic acid was identified for the first time from P. auriculata. In this research we have isolated compounds from pharmacological active extracts which is most likely responsible for cytotoxic action of P. auriculata. Further studies are required to determine activity and mechanism of action of isolated compounds

    هدف از پژوهش حاضر رابطه عدالت سازمانی و معنویت در محیط کار با تعهد سازمانی در بین معلمین آموزش و پرورش شهر اردبیل بوده، که با روش تحقیق توصیفی از نوع همبستگی انجام شده است. جامعه آماری شامل تمامی معلمین آموزش و پرورش شهر اردبیل بوده که جهت بدست آوردن حجم نمونه از فرمول کوکران استفاده شد و 160 نفر به صورت تصادفی ساده از بین آنها انتخاب گردید. برای گردآوری داده ها از پرسشنامه های عدالت سازمانی نیهوف و مورمن، معنویت سازمانی کینجرسکی و اسکریپینگ و تعهد سازمانی آلن و می یر استفاده شد. یافته ها نشان داد که در فرضیه اصلی بین عدالت سازمانی و معنویت در محیط کار با تعهد سازمانی رابطه مستقیم و معناداری وجود دارد و در فرضیه های فرعی بین ابعاد عدالت سازمانی، عدالت رویه ای و مراوده ای و ابعاد معنویت با تعهد سازمانی رابطه مستقیم و معنا داری وجود دارد و همچنین از یافته های بدست آمده می توان چنین نتیجه گرفت که در بین ابعاد عدالت سازمانی، متغیر عدالت مراوده ای و در بین ابعاد معنویت در محیط کار، متغیر جذابیت کاری نسبت به دیگر متغیرها، پیش بینی کننده قوی تری برای تعهد سازمانی می باشند

    The Role of Quality of Work Life and Organizational Commitment on Teachers' Job Burnout

    هدف پژوهش حاضر بررسی رابطه كيفيت زندگي كاري و تعهد سازماني با فرسودگي شغلي معلمان مقطع متوسطه شهرستان بروجن بود. اين پژوهش در چهارچوب پژوهش هاي توصيفی قرار دارد و از نوع همبستگی است. جامعه آماري اين پژوهش شامل کلیه معلمان مقطع متوسطه شهرستان بروجن بود که داری 5 سال سابقه خدمت به بالا بودند که تعداد 150 نفر می¬باشند که از میان آنها به روش نمونه گیری تصادفی طبقه¬ای و بر اساس جدول کرجسی و مورگان 108 نفر به عنوان نمونه آماری انتخاب شدند. از پرسشنامه¬های کیفیت زندگی کاری والتون، پرسشنامه تعهد سازمانی مادي استيريز و پورتر و پرسشنامه فرسودگی شغلی ماسلش و جکسون جهت گردآوري اطلاعات استفاده شد. به منظور توصیف داده‌های این پژوهش از شاخص‌های آمار توصیفی (مانند ميانگين و واريانس) و نیز از شاخص‌های آمار استنباطي مانند ضریب همبستگی پیرسون و رگرسیون استفاده شد. داده‌ها به کمک نرم‌افزار spss تحلیل شد. نتایج ضریب همبستگی پیرسون نشان داد که بین كيفيت زندگي كاري با فرسودگي شغلي معلمان رابطه معناداری وجود دارد، بین تعهد سازماني با فرسودگي شغلي معلمان نیز رابطه معناداری برقرار است. همچنین بین كيفيت زندگي كاري با تعهد سازماني معلمان رابطه معناداری وجود دارد. از طرفی نتایج رگرسیون نشان داد که كيفيت زندگي كاري و تعهد سازماني مي توانند فرسودگي شغلي معلمان را پيش بيني كنند

    Phospholipase A2 (PLA2) catalyzes the hydrolysis of phospholipids into arachidonic acid and lysophospholipids. Arachidonic acid is modified by cyclooxygenases into active compounds called eicosanoids that act as signaling molecules in a number of physiological processes. Excessive production of eicosanoids leads to several pathological conditions such as inflammation. In order to block the inflammatory effect of these compounds, upstream enzymes such as PLA2 are valid targets. In the present contribution, molecular dynamic analysis was performed to evaluate the binding of diclofenac, 9-hydroxy aristolochic acid (9-HAA) and indomethacin to PLA2. Obtained results revealed that 9-HAA could form a more stable complex with PLA2 when compared to diclofenac and indomethacin. Furthermore, analysis of intermolecular binding energy components indicated that hydrophobic interactions were dominant in binding process. On the basis of obtained data, inhibitors bearing fused rings with hydrogen acceptor/donor substituent(s) interacted with His48 and Asp49 residues of the active site. More affinity toward PLA2 might be envisaged through negatively charged moieties via interaction with Trp31, Lys34 and Lys69

    The comparative study of dexamethasone and ondansetron with dexamethasone and metoclopramide on PONV and shivering in patients undergoing laparoscopic cholecystectomy

    Aims and background: Postoperative nausea and vomiting is a distressing situation with a prevalence of 10 - 30%. As dexamethasone and metoclopramide are inexpensive drugs and cause little complications, and ondasetron in turn is a good and high quality drug for relieving patients ftom nausea and vomiting, we compared the effect of dexamethasone and metoclopramide versus dexamethasone and ondasetron on nausea - vomiting and shivering in patients undergoing laparoscopic cholecystectomy, in the study. Materials and methods: The present study is a randomized, double - blind clinical trial. After obtaining consent from pations, study was performed on 88 patients allocated with sampling and blocking method. The first group received the combination of dexamethasone and ondasetron, and the second group received the combination of dexamethasone and metoclopramide. The anesthetist injecting drugs,was unaware of the type of injectable medicine. The anesthetic method and the drugs used for anesthesia were the same in both groups. After the operation, the questionair asked questions about nausea - vomiting and shivering for up to 24 hours (at intervals of 1 - 6 -12 and 24 hours), and all the answers were recorded for each participant. Finally all findings were analyzed statistically using spss version 20. Findings: In this study 88 patients were divided into 2 groups. the first group received the combination of dexamethasone and ondasetron, and the second group received combination of dexamethasone and metoclopramide. the mean average age in the D+O group was 41.07 years and in the D+M group was 42.16 years. The result of this study showed that there was insignificant decrease in nausea and vomiting after laparoscopic cholecystectomy;also none of the patients in the study groups showed any shivering. Conclusion: The result of this study showed that the combination of ondasetron and dexamethasone could not significantly reduce the incidance of post operative nausea and vomiting compared to combination of dexamethasone and metoclopramide. No patients in the study groups had shivering

    High-level expression of tetanus toxin fragment C in Escherichia coli

    Fragment C is the C-terminal domain of the heavy chain of tetanus toxin that can promote the immune response against the lethal dose of this toxin. Therefore, this portion can be considered as a candidate vaccine against tetanus infection, which occurs by Clostridium tetani. The present study aimed to compare the expression of tetanus toxin fragment C in Escherichia coli BL21 (DE3) pLysS cells having a high tolerance to toxins between two different expression vectors, namely pET22b and pET28a, using the sodium dodecyl sulfate polyacrylamide gel electrophoresis and western blot analyses. After DNA extraction from Harvard CN49205 strain of C. tetani, the gene of interest was amplified using polymerase chain reaction, and then sequenced and cloned into the expression vectors of pET22b and pET28a, transformed into competent BL21 (DE3) pLysS cells, and finally expressed using an optimized protocol. The cells were induced with isopropyl β-D-1-thiogalactopyranoside (IPTG) at four different incubation temperatures (i.e., 37, 33, 30, and 25 °C) and three different incubation times (i.e., 1, 2, and 3 h). Although the SDS-PAGE and western blot analyses confirmed the expression of the recombinant fragment C (r-fragment C) ligated into both of the expression vectors, pET28a showed a higher rfragment C expression level than the other vector (38.66 mg/L versus 32.33 mg/L, P<0.05). An optimal expression condition was acquired 3 h after 1 mM IPTG induction at 25 °C. The results demonstrated that E. coli BL21 (DE3) pLysS as an expression host in combination with pET-28a as an expression vector was a more compatible expression system to express the fragment C of tetanus toxin, compared to E. coli BL21 (DE3) pLysS/pET-22b expression system. Overall, these results may represent an opportunity to improve the expression system for the production of tetanus toxin vaccine using recombinant protein strategy
