    Investigation of the relationship between hTERT serum level and pathologic data with the risk factors of breast cancer in Ardabil province during 1396-97

    Background and objective: Breast cancer is the second most common cancer in the world that consists 1∕3 of all women’s cancers. Because of the incremental setting of telomerase in cancer cells, the goal of this case-control study is to investigate the relationship between hTERT serum level, as the most important part of telomerase, with pathologic data and risk factors of breast cancer. Methods: 30 patients of women with breast cancer (new cases) and 30 cancer free controls were chosen randomly. After completing the questionnaires, venous blood samples(4 cc) were taken and after separating the serum, mRNA was extracted and after synthesis of cDNA by Real-time PCR, hTERT serum level measured and data analyzed by SPSS 24 version software. Results: The Results of the study showed that the mean of hTERT serum level in control’s group was 0.74 and in patient’s group was 9.88 that there was significant correlation between them. There was significant correlation between hTERT serum level and menarche age and HER-2 expression level but, significant correlation was not found with other pathologic data and risk factors. Also the study showed that by weight gain and using of sausage, burger, potato, pizza, soft drinks and sweet the hTERT’s level increases and by increasing regular physical activity time and using of bread, yogurt, vegetables, fruits, milk, fish, chicken, beans and pickle it’s level decreases. Conclusion: The results of study showed that menarche at the age of younger than 13 , high levels of HER-2 expression, low physical activity and using of some foodstuffs can be accompanied with poor prognosis in breast cancer

    Carbapenem-resistant Klebsiella pneumoniae in Iran: a systematic review and meta-analysis

    The global emergence and dissemination of carbapenem-resistant (CR)-Klebsiella pneumoniae is an important healthcare concern owing to the limited therapeutic options that are available. The aim of the present study was to evaluate the prevalence of CR K. pneumoniae in different parts of Iran. A comprehensive literature search was performed in national and international databases. After applying predefined inclusion and exclusion criteria, 36 articles reporting prevalence of CR K. pneumoniae were collected. The pooled prevalence of CR K. pneumoniae was estimated to be 11.3% (95% CI: 0.084–0.15). The highest and lowest prevalence rates of CR K. pneumoniae were in Isfahan 58% (95% CI: 0.48-0.67) and Tehran 0.004 (95% CI: 0.00-0.05), respectively. The highest and lowest resistance rates were seen against aztreonam (55%, 95% CI: 0.48–0.62) and amikacin (23%, (95% CI: 0.17–0.29), respectively. The findings of the present study revealed that the prevalence of CR K. pneumoniae is alarmingly high in the majority of Iranian hospitals

    Five to ten percent of FMF patients have unfavorable response to the colchicine as a standard therapy. Biologic treatments have been shown to be highly effective, but there are often unavailable, because the price is unaffordably high. This study shows the striking effect of combined dapsone and colchicine therapy in such patients and recommends it as an alternative therapy in colchicine-resistant (CR) patients

    Sensitivity and Specificity of Phalen’s Test and Tinel’s Test in Patients with Carpal Tunnel Syndrome

    Abstract: Carpal tunnel syndrome ( CTS ) is the most common cause of paraesthesia in the hands and it is the most common entrapment neuropathy in the upper and lower limbs.The symptoms are variable, but episodes of burning pain or tingling in the hand are common. Diagnosis of CTS is mainly clinical based on suggestive history and Physical examination. Several physical signs are associated with carpal tunnel syndrome, including tinal,s test and phalen,s test. The objectives of this study are to determine the sensitivity and specificity of the phalen’s and tinnel’s tests in the diagnosis of carpal tunnel syndrome and to analyze how such factors as patients age and duration of symptoms influence these parameters. Methods : this is a prospective study comparing tow groups ( patient & control ). Fifty cases ( 40 females and 10 males ) who fulfilled the national institute of occupational safety and health ( NIOSH ) criteria for diagnosis of CTS attending physiotherapy unit in loghman hakim hospital in Tehran-iran the control group included ( 40 females and 10 males ) who were healthy asymptomatic volunteer. The duration of the study was eight months from 1 st may 2017 to 1 st February 2017. Both groups were assessed by phalen and tinel tests. Results: in the patient group 39(78%) cases were having positive phalen and 33(66%) cases had positive tinel tinel test while in the control group 3 (6%) were having positive phalen and 12(24%) controls had positive tinel test. The resulting specificity and sensitivity of phalen test for the patient and control groups were 94% and 78% respectively while specificity and sensitivity of tinel test were 77% and 66% respectively

    Formulation and optimization of a new cationic lipid-modified PLGA nanoparticle as delivery system for Mycobacterium tuberculosis HspX/EsxS fusion protein: An experimental design

    Polymeric particles and liposomes are efficient tools to overcome the low immunogenicity of subunit vaccines. The aim of the present study was formulation and optimization of a new cationic lipid-modified PLGA nanoparticles (NPs) as a delivery system for Mycobacterium tuberculosis HspX/EsxS fusion protein. The cationic lipid-modified PLGA NPs containing HspX/EsxS fusion protein were prepared using a modified double emulsion solvent evaporation method. Scanning electron microscopy and dynamic light scattering (DLS) tools were used to determine physical properties of hybrid NPs. A multi-level full factorial design was used to evaluate the influence of two factors of PLGA:DDA ratio (w/w) and PVA concentration (%) on size, surface charge, polydispersity index, encapsulation efficiency and yield. Finally, the optimal formulation was achieved based on desired responses. Mathematical models were obtained to indicate the relation between the studied factors and responses. The DDA concentration showed an increasing effect on surface charge and also a decreasing effect on particle size, encapsulation efficiency and yield. Higher amounts of DDA, increased surface charge of NPs, however, the size, encapsulation efficiency and yield were decreased. The influence of various concentrations of PVA on different physical characteristics of PLGA:DDA hybrid NPs was variable. The optimal formulation was consisted of 0.91 (55:5, w/w) ratio of PLGA:DDA and 0.5% PVA. The hybrid NPs showed acceptable particle size distribution, strong positive surface charge, prolonged antigen release and good encapsulation efficiency in comparison to PLGA alone. However, further preclinical and clinical studies are needed

    68 Malignant fibrous histiocytoma of paratesticular region in year-old man: presenting a rare case report

    Malignant fibrous histiocytoma (MFH) is a rare neoplasm of the scrotal region. We haven’t so much information about pathophysiology of this malignancy. There are four histological subtypes of MFH: myxoid, storiform-pleomorphic, inflammatory and giant cell. The storiform-pleomorphic type is the most common. We report a case of a 68 year-old man with a history of 7 months mass in his right inguinal region. Recently the mass became larger in size and took the entire scrotum. Physical examination revealed swelling and tenderness on the area and the patient also had ague. The ultrasound study showed an extreme hydrocele with a 20*13*9 cm mass in the right inguinal region that extended to scrotum but didn’t involve the orchid. Incisional biopsy of the mass was performed and microscopic examination showed paratessticular malignant fibrous histiocytoma (MFH) storiform-pleomorphic (SP) type. Microscopic view of the tumor consisted of highly pleomorphic spindle cells arranged in a storiform pattern along with areas of necrosis. According to the TNM and American Joint Committee on Cancer sarcoma staging system, the neoplasm was grade III sarcoma. The patient underwent radical right orchiectomy, dissection of ileo femoral lymph nodes and systemic chemotherapy. The management of MFH is explained with other sarcomas, but these tumors have a high rate of recurrence that may be accompanied by an increased malignant potential and it could be a risk marker for distant failure

    Effect of L-carnitine and conjugated linoleic acid supplements on haemoglobin levels and haptoglobin genotype in chronic kidney disease

    Objective: To investigate the efficacy of L-carnitine (LC) and conjugated linoleic acid (CLA) supplements on haemoglobin levels and inflammatory markers in chronic kidney disease (CKD) patients with different haptoglobin (HP) genotypes Methods: This clinical trial study was conducted at Imam Khomeini Hospital, Ardabil, and Labbafinejad Hospital, Tehran, Iran, from March 2014 to March 2015, and comprised male patients with CKD and anaemia. Anthropometric factors were recorded and demographic data was collected using general questionnaires. LC (1 g/day) and CLA (2.4 g/day) supplements were given to the patients for a month. Blood samples were taken to measure haematological and inflammatory markers at the beginning and end of the study. Haptoglobin genotypes were determined using polymerase chain reaction (PCR). SPSS 21 was used for data analysis. Results: Among the 40 patients in the study, HP2-2 genotype was the most prevalent genotype (62.5%). The level of haemoglobin was significantly increased in the patients at the end of the study (p< 0.05). No significant changes were found in the weight, body mass index and serum levels of Interleukin-6, high-sensitivity C-reactive protein, ferritin, total iron-binding capacity and iron (p>0.05 each). Conclusion:Regular diet supplementation with LC plus CLA can improve haemoglobin levels in CKD patients with anaemia

    Fatigue Severity and Some Related Factors in Type 2 Diabetic Patients Referred to Diabetes Clinic of Imam Khomeini Hospital in Ardabil, 2017

    Background and Aim: Fatigue is a common complaint in patients with type 2 diabetes, which can affect self-care behaviors and people's efforts to control diabetes. This study was conducted to determine the severity of fatigue and its related factors in patients with type 2 diabetes. Methods: In this descriptive-analytical study, 218 patients with type 2 diabetes mellitus who were referred to diabetes clinic of Imam Khomeini Hospital Ardabil in 2017 were selected by available sampling. Data were collected by an integrated fivepart questionnaire including: socio-demographic, fatigue questionnaire, Pittsburgh sleep quality, Beck depression, and visual analogue scale. Data were analyzed by SPSS software (version 22), descriptive (mean, standard deviation and frequency) and inferential statistics (Pearson correlation coefficient, simple and multiple regression model, and logistic regression). Results: The average fatigue score in the patients was 57.07±17.16, which indicated moderate fatigue. Pearson correlation test revealed a significant correlation between fatigue and pain (r=.440, p<0.01), depression (r=.590, P<0.01), Number of complications of the disease (r=0.38, P<0.01), Duration of diabetes (r= 0.21, P<0.01), duration of exercise (r= - 0.305, P<0.01), and sleep quality (r=.390, p<0.01). However, no significant correlation was found between fatigue and glycosylated hemoglobin and BMI. Data analysis using multivariate linear regression showed that four factors of pain, depression, duration of disease, and duration of exercise are predictors of fatigue in patients with type 2 diabetes. A logistic regression model was used to determine the effect of the type of drug used on fatigue. The results showed that there was a significant relationship between use Oral antidiabetic medicine (P = 0.047) and insulin (P = 0.014). Injection of insulin compared to the oral method was 2.99% more fatigue in the subjects. Conclusion: The present study showed that fatigue was a relatively common problem in patients with type 2 diabetes and factors such as pain, depression, and duration of disease and duration of exercise are factors that affect fatigue. Health care providers need to pay special attention to fatigue assessment in patients with diabetes and proper interventions be taken to reduce pain and diagnosis and control depression in these patients
