    Sodium valproate versus levotiracetam in migraine prophylaxis: a double-blind randomized clinical trial

    Background and Objective: Sodium valproate is an anti-seizure and stabilizing agent used for prophylactic use of migraine headaches. Despite the efficacy of this drug due to complications that cause some patients to not tolerate the drug. The purpose of this study was to compare the efficacy of levothyramine as a less complicated drug and sodium valproate in migraine prophylaxis. Methods: This study was performed in a clinical trials study. 100 migraineurs who referred to the Alavi neurology clinic and indicated the onset of prophylaxis of migraine were studied in two groups of 50, one of which was a group of levetiracetam and another group of sodium valproate Received. Patients received daily 500 mg of drug for 3 months and the effect of the drug on the number of attacks, severity of attacks, and MIDAS score was measured. In all studies, P value was less than 0.05. Results:. In both cases, significant improvement was observed in the number of attacks, severity and MIDAS score. In all of the criteria, levetiracetam and sodium valproate were almost identical, except for reducing the number of headache attacks that had a significantly better effect on the levothyramine acetate group (P = 0.0001) Conclusion: Our study showed that levothyramine acetamin can be used as a good alternative in those who do not tolerate sodium valproat

    Application of ATRA as a complementary drug in chemotherapy

    The side effects and drug resistance are two limits in the application of chemotherapeutic drugs. All-trans retinoic acid, ATRA as a natural compound may help to overcome these limitations. To assess this hypothesis, we investigated the efficacy of combined treatment by chemotherapeutic drugs and ATRA on different cancer cell lines and stem cells using different methods including cell viability, cell apoptosis, cell cycle analysis, RT-PCR, and ELISA. Results showed that the combination therapy increases the effectiveness of chemotherapeutic drugs in terms of increasing the expression of the proteins leading to apoptosis and arresting the cells in G1. In conclusion, ATRA has synergistic effects with chemotherapy leading to usage of low dosages of drugs, especially in stem cells. Low doses of drugs in combination with ATRA promise new opportunities in cancer treatment without worrying much about chemotherapy side effects and drug resistance

    Apoptosis is fundamental in several morphogenetic processes and ultimately determines the mass, shape, and function of the various tissues and organs that form the animal body. This process is a gene-regulated process that plays fundamental roles in several normal and pathological conditions. Apoptosis is most often detected during embryonic development. Although the nervous tissue is traditionally regarded as being fundamentally constituted by postmitotic nonproliferating cells, analysis of cell proliferation and apoptosis in vivo has recently gained an increasing importance mainly during embryonic development because there is large evidence that drug-induced apoptosis is the most likely candidate for the behavioral deficits. These effects occur at therapeutically relevant blood levels, and require only a relatively brief exposure.     Mammography perceptions among women in an Iranian setting: A qualitative study

    Introduction & Aim: Breast cancer is the most common cause of death due to cancer in Iranian women. Mortality from the disease could be reduced by screening and early diagnosis of the disease and its treatment in the early stages. Mammography is one of the most effective methods for diagnosis of breast cancer in the early stages. The purpose of this study was to explore the perception and experience of mammography among women in Ardabil. Methods: A qualitative study was conducted based on the conventional content analysis approach on 14 women who attended in Fatemi Hospital in Ardebil, Iran from April to August 2018. Data were collected through in-depth semi-structured interviews. Purposeful sampling was continued until data saturation. Results: During the data analysis, three main categories emerged, including: 1- Low awareness 2- Shame and embarrassment 3- Fear of mastectomy 4- Concerns about costs Conclusion: The experiences of women indicate that the need for education and psychosocial and economic support from them in the field of care and prevention of breast cancer is necessary

    Comparative study of Hemodynamic changes in patients undergoing diagnostic bronchoscopy using Remifentanil – Propofol Versus Remifentanil – Ketamine

    Background and objectives: Fiberoptic bronchoscopy is an essential common diagnostic method for lung diseases. The procedure is an invasive procedure and enough sedation is required. Patients who are candidate for bronchoscopy are involved with respiratory problems. Airway manipulation in such patients may result in exacerbation of respiratory problems. To prevent respiratory complication we have to induce light sedation or partial unconsciousness. in this study the effect of two groups of drugs: Remifentanil-Propofol (RP) & Remifentanil-Ketamine (RK) for sedation of patients undergoing diagnostic bronchoscopy was assessed. Methods: This double blind clinical trial was performed on 76 pulmonary patients with class 1, 2&3 ASA, scheduled for diagnostic bronchoscopy under intravenously sedation. The patients accidentally divided to 2 groups. One of the groups received single dose of propofol & infusion of remifentanil intravenously, and another groups received single dose of ketamine & infusion of remifentanil intravenously. Hemodynamic parameters of patients during the procedure were recorded. Consciousness state of patients, satisfaction scores of bronchoscopist & patients were noted at the end of the procedure. Taken results were statistically analysed by spss software, chi square, independent t-test & repeated measures. Results: Though, the mean of heart rate & blood pressure in the patients of RK group was greater than RP group clinically, but there was no significant difference between two groups statistically (Pvalue>0.05). consciousness state, satisfaction scores of bronchoscopist & patients between groups were not significant(pvalue>0.05) too. Conclusion: Blood pressure stability in patients group RP was considerable clinically but was not significant statistically. Use of the least dose of drugs can be one of the causes. Then further similar researches with different dose of drugs & more samples recommended

    The efficacy of Sublingual Buprenorphine in controlling pain After cesarean Surgery under Spinal Anesthesia

    Aims and background: One of the major challenges in women’s surgery, including cesarean section, is postoperative pain control. Postoperative pain as one of the most common problems in the postoperative phase and can lead to a significant reduction in the quality of surgical operations, along with other problems such as nausea and vomiting, hypotension and shivering. Therefore, the present study we aimed to study the efficacy of sublingual Buprenorphine in postoperative pain control following cesarean surgery and its effects among patients. Materials and Methods: The present study is a randomized, double-blind clinical trial. Patients aged 18_45years old and all candidates underwent cesarean section at Alavi Hospital in Ardabil in 2017. After obtaining consent from patients for inclusion in the study, 80 patients were selected using simple sampling method and divided into two groups of 40. In the first group,) control group) 40 patients received fentanyl and placebo tablets, and the pump was used to inject intravenous fentanyl and the drug lasted for up to 24 hours. the second group )40 patients) received normal saline pump and buprenorphine pills. In the second group (sublingual buprenorphine + placebo pump), it was repeated 6 and 12 hours after the first dose. Then, VRS pain scores, vomiting nausea, sedation )from Ramsay sedation scale), and the amount of analgesic demand for pain control at 2, 6, 12, and 24 hours, and postoperative analgesia (time to first need analgesia) were recorded. Finally, all patient information entered into a pre-designed checklist and all the data entered into the SPSS v20 statistical analysis program and we analyzed the data. Findings: In this study, 80 patients were evaluated in two groups The results of VRS pain score in patients showed that, except within 24 hours after surgery, in other hours of study, the pain score in the two groups did not show any significant difference. In the control group, the demand for analgesics was higher than the buprenorphine group. In the early hours after surgery (hours 2 and 6), the incidence of nausea and vomiting was significantly lower in the buprenorphine group than in the control group, but the rest of it, no difference was observed. The sedation score was also evaluated, but no significant difference was observed in the two groups. Also, the incidence of other side effects in the two groups did not differ significantly. Conclusion: The results of this study showed that buprenorphine is an effective drug in reducing postoperative pain in postoperative patients and, due to its very low side effects, can be routinely used in patients

    An investigation of the efficacy on hepatitis B vaccination in hemodialysis patients in Ardabil, Iran

    Introduction: Hemodialysis patients usually demonstrate lower immune response to hepatitis B vaccine compared to non-uremic population. Objectives: The present study aimed to determine the level of response to hepatitis B vaccination in patients under hemodialysis. Patients and Methods: This study was conducted on 172 live patients receiving hemodialysis in the dialysis department of Buali hospital in Ardabil, Iran (2015). To analyze their response to the vaccine, their vaccination titers were investigated. Before vaccination, the serological markers of hepatitis B and C were checked in all of the patients. Those for whom HBsAg and HBsAb results were negative and had not received the vaccinein the past entered the study. The patients received a double dose of hepatitis B vaccine at 0, 1, and 6 months after the beginning of dialysis. The response to the vaccine was investigated by measuring the level of patients’ hepatitis B antibody one month after receiving the last dose of the vaccine. Results: Seventy patients (40% of the total) displayed a proper immune response to the vaccine, 34 patients (19.8%) were without, and 68 patients (39.5%) were identified to have poor response. The results of Pearson’s correlation test indicated that there is a negative correlation between the patients’ age and their response to the vaccine. Conclusion: The results of this study indicated that higher age is one of the factors that reduce the effectiveness of hepatitis B vaccine in hemodialysis patients

    Cationic surfactant-modified clay as an adsorbent for the removal of synthetic dyes from aqueous solutions

    In this study, the potential of hexadecyl trimethyl ammonium bromide modified montmorillonite (HDTMA-Mt) to remove the synthetic dyes Alizarin Red S (ARS) and Bromocresol Green (BCG) from aqueous media was assessed. The effect of different factors including surfactant loading rate onto the clay, contact time, pH, adsorbent dosage and dye concentrations, on the removal of ARS and BCG in batch systems were investigated. The adsorbent was characterized by means of scanning electron microscopy (SEM), Fourier transform infrared spectroscopy (FTIR) and X-ray diffractometry (XRD). The equilibrium time for ARS and BCG was reached at 40 and 20 min, respectively, under optimized conditions (i.e. pH = 3, adsorbent dosage=1 g/L, surfactant loading rate onto the clay 70% of the cation exchange capacity (CEC) for ARS and 120% of the CEC for BCG, ARS concentration 50 mg/L and BCG concentration 500 mg/L). The adsorption rate of both dyes fitted the pseudo-second-order kinetic model and the equilibrium data was described by the Freundlich isotherm equation. The maximum monolayer adsorption capacities were equal to 666.6 and 1250 mg/g for ARS and BCG, respectively. Therefore, the HDTMA-Mt can be considered as an effective adsorbent for the removal of ARS and BCG from aqueous solution
