    The study of patient care behaviors and its effective factors from the perspective of nurses working in Ardabil educational hospitals 2017

    Background and Aim: Understanding nurses perceptions of care behaviors and determining the factors affecting it is the first step in correcting inappropriate behaviors and improving the quality of nursing care. Therefore, this study was conducted with the aim of "determining care behaviors and its determinants from nurses' point of view in Ardabil educationalhospitals ". Methods: This was a descriptive cross-sectional study with 450 nurses working in Ardabil educational-therapeutic hospitals in 2017. Sampling method was stratified random. Data were collected using a standard three-part questionnaire on demographic characteristics, Wolf caring behaviors and the determinants of nursing care behaviors. Data were analyzed using descriptive statistics (mean and standard deviation) and analytical (Mann-Whitney, Kruskal-Wallis, Anova and Multivariate Regression Analysis) statistics in SPSS v21 software. Results: Nurses' caring behaviors with a mean and standard deviation of (5.27±0.24) were at the optimum level. From nurses' point of view, physical dimension of care (5.43±0.7) was more important than psychosocial dimension (5.17 ± 0.6) (p = 0.001). The most important caring behaviors for nurses were medication (5.58±0.44) and the least important caring behaviors were the patient's calling with her favorite name (4.02±0.86). Mean scores of caring behaviors in female nurses (5.26 ± 0.7) were more than males (4.97 ± 0.54) (p = 0.001). The most important factor influencing nurses' caring behaviors was the responsibility (4.49 ± 1.01) and the least important factor related to the patient's characteristics (3.50±1.1). Conclusion: The results showed that nurses' care behaviors were in the desired and focused on the physical aspect of care. Therefore, in order to provide patient satisfaction, nurses should pay attention to both dimensions of caring behaviors (physical and psychosocial). It is suggested that psychosocial care should be emphasized in nursing in-service training

    Effect of pentoxifylline and captopril combination therapy versus captopril monotherapy on blood pressure control in patients with hypertension at Tabriz Shahid Madani Hospital: A pilot randomized clinical trial

    Purpose Hypertension has received too much attention because of its high prevalence, its association with concomitant risk factors and mortality. Considering the pathophysiology of hypertension and the key role of blood viscosity (BV) in the regulation of blood pressure (BP), and also hemorheological effects and cardiovascular benefits of pentoxifylline (PTX), this study was conducted to evaluate whether PTX can reduce BP when added to captopril in patients with stage 1 hypertension. Methods In this pilot randomized clinical trial 62 patients at the age of 18 to 80, with diagnosis of stage 1 hypertension were entered. The intervention group (n=30) received 1200 mg PTX in three divided doses plus 25 mg captopril three times a day whereas, the control group (n=32) received only 75 mg captopril in three divided doses. Measurements of BP were done according to American Heart Association (AHA) guidelines at baseline, first and second month of the study period. Major adverse cardiac events (MACEs) during this period were recorded as secondary end points of the study. Results Comparing the systolic blood pressure (SBP) levels in the groups of intervention and control no significant differences were noted at baseline (150.4±6.03 vs.150.4±6.2, p=0.98), first (138.4 ±9.4 vs. 142.3 ±5.6, p=0.08) and second (134.6 ±8.9 vs. 137.4±6.0, P=0.20) months of the study respectively. Similarly, no significant differences were observed in the diastolic blood pressure (DBP) at baseline (91.7±3.9 vs. 92.0 ±3.7, p=0.84), first (85.5±5.1 vs. 86.9 ±3.8, p=0.27) and second (82.6 ±5.7 vs. 84.0 ±3.5, p=0.31) months. About baseline demographic data of patients, no significant differences were observed between the two groups. Discussion and Conclusions Based on the results of present study, adding PTX as a hemorheological agent to captopril could not significantly reduce blood pressure in the patients with stage 1 hypertension. Our findings are in accordance with previous studies however, they were not well designed for measuring BP. Further larger-scale studies are needed to confirm these findings

    Phytochemical Analysis and Various Biological Activities of the Aerial Parts of Scrophularia Atropatana Growing in Iran

    Scrophularia atropatana (S. atropatana), an Iranian plant belonging to the family of Scrophulariaceae, was assigned for its chemical compositions and biological activities of essential oil (EO) and extracts of the aerial parts of the plant during the flowering stage. Combination of GC-MS and GC-FID was assessed for analyzing the chemical compositions of the EO from the aerial parts of S. atropatana. Furthermore, Brine shrimp lethality test and DPPH assay were performed to evaluate general toxicity and free-radical-scavenging properties, respectively. Furthermore, anti-proliferative and antimicrobial activities were assessed by MTT assay and disc diffusion methods correspondingly. Additionally, all the potent samples (extracts) and its fractions in the MTT assay were further studied for the presence of various compounds by GC-MS apparatus. MeOH extract and 40% sep-pak fraction indicated high amounts of total phenolic (TPC), total flavonoid content (TFC), and antioxidant properties. In the case of general toxicity, among the extracts, dichloromethane (DCM) extract showed noticeable effect. Furthermore, DCM extract was indicated potent ability to eliminate breast tumor cells and minimum efficacy on normal cells. Anti-microbial activity of all samples was ignorable. The potent extracts and fractions which had more anti-proliferative activity were further elucidated by GC-MS and showed high amounts of Alkanes and fatty acids. In the case of EO constituents, non-terpenoids were the major compounds. To sum up, it seems BSLT could be a good preliminary approach for evaluating the cytotoxicity in MCF-7 cell line. Additionally, antioxidant activity, TPC, and TFC contents of all samples were in consistent with each other

    Internet addiction evaluation and its relationship with sleep quality and anxiety in students of Ardabil University of Medical Sciences in 2018-2019

    Background: With the development of technology and increasing access to the Internet and cyberspace, we are witnessing a new type of addiction and its impact on health. Internet addiction is an emerging phenomenon that results from easy access to computers and online information and over-dependence that can have multiple effects on people. The purpose of this study was to determine the relationship between internet addiction and sleep quality and anxiety among students of Ardabil University of Medical Sciences. Materials and Methods: This descriptive-correlational study was performed on 376 students of Ardabil University of Medical Sciences in 1397. Samples were selected by stratified random sampling from each faculty. Data were collected using demographic questionnaires, Young's Internet Addiction Questionnaire, Petersburg Sleep Quality, and Beck Anxiety Inventory. Data were analyzed by SPSS software (version 24) and descriptive statistics (mean frequency and standard deviation) and analytical statistics (chi-square, univariate regression and Pearson correlation coefficient). Results: The results showed that 38% of students had internet addiction, 77.9% had poor sleep quality and 46.3% had abnormal anxiety. Pearson's correlation coefficient showed that Internet addiction had a negative and inverse relationship with sleep quality and a significant positive relationship with anxiety (p <0.001). When examining the relationship between anxiety and sleep quality with internet addiction, addiction predicts sleep quality (R2 = 0.06). A separate study of addiction predicts both. The results of Chi-square test showed that the average anxiety level of female students was higher than boys and there was a significant positive relationship between internet addiction and educational level (p <0.001). Conclusion: The findings of this study showed the prevalence of internet addiction and its effect on the quality of sleep and anxiety in students. Given the increasing prevalence of Internet use, it is suggested that measures be taken to promote awareness of the impact of internet addiction on health, as well as self-care and prevention of complications among students

    Educational Needs Assessment of Diabetic Pregnant Mothers in Meshgin Health Centers in 2018

    Background & objectives: Gestational diabetes is one of the most common medical problems during pregnancy, which has many negative consequences for mother and fetus .Knowledge of women about gestational diabetes and educational planning based on needs assessment can be effective for early diagnosis and reduction of possible complications of gestational diabetes .The aim of this study was to determine the educational needs of diabetic pregnant mothers in Meshgin health centers in 2018. Methods: This descriptive, cross-sectional study was conducted by- census on 689 pregnant women referring to Meshgin health centers in the spring and summer of 2018. Data were collected using a valid and reliable questionnaire of Hashemi-Beni and colleagues (2015) including three parts: demographic information, knowledge and educational needs assessment for mothers with gestational diabetes. Data were analyzed using SPSS 16 software. Results: The mean age of pregnant women participating in the study was 26.94±6.22 years, with an average gestational age of 24.36±9.18 weeks. 22 persons of 689 pregnant women were diagnosed with gestational diabetes, and the prevalence of gestational diabetes was 3.2%. The mean score of knowledge of diabetic pregnant mothers about gestational diabetes questions was 9.95±3.79 out of 13. In all items of gestational diabetes, at least one third of diabetic mothers reported need for education. Conclusions: Diabetic pregnant mothers, despite of high knowledge about gestational diabetes, still need to be trained in this regard. Therefore, it is necessary to hold leveled training courses to complete the information of mothers for better care of themselves

    Background and Objective:. Half of the world's population is made up of women, and it is natural that certain diseases of this group affect the entire human population. In the first trimester of pregnancy, 15 to 20 percent of pregnancies result in miscarriage. According to the National Center for Abortion Health, termination of pregnancy is less than 20 weeks, and this condition accounts for about 15% of pregnancies. Misoprostol is widely used for induction of labor in the second trimester; Therapies, early termination of pregnancy, duration of term delivery are used. The purpose of this study was to compare the effects of vaginal misoprostol and isosorbide menonitrate and control group on cervical spine preparation in patients under 20 weeks at Alavi Hospital in Ardabil. Methods. 120 pregnant women with miscarriage were enrolled in the study. Rats were randomly divided into 3 groups: misoprostol, and isosorbide mononitrate and control group. And the effect of the drug on cervical dilatation, time needed for abortion, and drug side effects Results: The mean age of patients in the misoprostol group was 29 ± 6.8 years and in the isosorbide group was 6 ± 26 years and in the control group was 32.5 ± 6.8 years. The interval of abortion was not significantly related to dilatation rate. Nausea, diarrhea, and fever were higher in the mesoprostol group. Headache in the isosorbide group was significantly higher than the other groups. The bleeding rate of misoprostol group was significantly higher than the other two group

    Incidence of ventilator-associated pneumonia and its related risk factors in patients ‎admitted to neonatal intensive care unit (NICU) ‎

    Background and objective: Ventilator-associated pneumonia is defined as inflammation of the lung parenchyma due to infectious agent activity 48 hours after the start of mechanical ventilation in patients who have tracheal intubation and did not have pneumonia at the time of intervention. Due to the lack of local information available, the high mortality rate and the results of this study can help improve child care standards under mechanical ventilation in NICU. The aim of this study was to investigate the incidence of ventilator-associated pneumonia and its related risk factors in patients admitted to neonatal intensive care unit (NICU) in Ardabil city hospital. Methods: This descriptive cross- sectional study was performed on 100 neonates admitted to NICU of Bu-Ali hospital in Ardabil city in 2018. For each neonates, necessary information were collected by a checklist including incidence of ventilator-associated pneumonia and related risk factors. Collected data were analyzed by statistical methods in SPSS version 19. Results:In this study, 100 neonates admitted to NICU of Ardabil city hospital, 61% of them were boys and the rest were girls. Out of all neonates, 48% had ventilator-associated pneumonia. There was a significant relationship between duration of mechanical ventilation time and the incidence of ventilator

    Background : Acute lymphoblastic leukemia (ALL) represents a clonal expansion and arrest of normal lymphoid hematopoiesis in bone marrow. ALL remains the most common malignancy in children. The survival rate of the patients is significantly increased since the 1960s. Aim: This study was under taken to evaluate the 3-year and 5- year overall survival (OS) rates of patients with ALL in a single center in IRAN. Methods: A total of 80 children with ALL up to 15 years old who had been diagnosed at the Oncology Department in boali Hospital from March 1997 to October 2004 were evaluated for their age, gender, as well as FAB types, presenting features.and 55 patients who treated by High risk pre B cell (COG) evaluated for outcomes of therapy and relapse. Results: The 85% of the patients were in 1 to 9 years old range. In this series, 43 patients (53.8%) were male. There were a complete remission induction rate of 92.7% during first induction course of therapy. Three-year overall survival was 85.5% and Five-year overall survival was 81% and. Relapse rate after first remission was 20% and death after relapse was 50% and 30% of total deaths was at induction period. Overall infections was major cause of deaths. Conclusion: Three-year overall survival(OS) in children with new ALL who were undergoing chemotherapy by high risk pre B cell(COG) protocol in buali hospital was 85.5% and Five-year overall survival was 81%.Bone marrow relapse reduce by this protocol. High mortality rate was mainly due to infections which decreased overall survival in this study

    The Diabetes Mellitus-Related Problems among Diabetic Elderly

    Diabetes mellitus (DM) is one of the main cause of death and disability in the elderly. We aimed to evaluate the diabetes mellitus-related problems in elderly participants attended the diabetes clinic of Ardabil. In a cross-sectional analytical study, 91 elderly participants were randomly enrolled. Standard Problem Area in Diabetes scale was used. Data were obtained through interview and analyzed using SPSS version 22 by descriptive and analytical methods. The mean and standard deviation of the Total Problems Related to Diabetes Mellitus was 31.51 ± 15.37. There were significant relationships between Total Problems Related to DM and marital status (P=0.02), the score of depression-related problems and gender (P=0.04) and the score of treatment barriers with the family composition (P=0.009) and marital status (P=0.005). Elderly with further chronic illness have had additional treatment barriers (P=0.02). Most of the elderly are suffering from various DM-associated problems. They needed to be taken into account in promotion planning sex, family composition, and numbers of chronic diseases. Key words: Elderly, Diabetes Mellitus, Problem Areas in Diabete

    Synthesis of ZnO and nZVI Nanoparticles Using Pomegranate Peel Extract and Putting it on Activated Carbon Produced from Pomegranate Peel and Its Application for Removal of Cephalexin from Aqueous Media

    Background and Objectives: Concerns have been raised about the presence of high concentrations of antibiotics in the environment in recent years. Conventional water purification and treatment processes cannot remove these resistant pollutants. There are various ways to remove these compounds, but the adsorption method has been more effective. Therefore, this study was performed to evaluate the efficiency of activated carbon coated with ZnO and nZVI nanoparticles for removal of cefalexin from aqueous solutions. Materials and Methods: The present study was performed experimentally at laboratory scale in a closed system. In this study, ZnO and nZVI nanoparticles were synthesized using pomegranate peel extract. The effects of pH, adsorbent dose, initial antibiotic concentration and reaction time on the efficiency of the adsorption process were studied. Isotherms (Langmuir and Freundlich) and adsorption kinetics at different concentrations were evaluated for process evaluation. The structure of the adsorbents and their morphology were investigated by FTIR, FESEM, BET and EDX techniques. Cefalexin concentration was determined using a Perkin Elmer Lambda 25 UV/VIS Spectrometer spectrophotometer at 261 nm. Results: The results showed that the removal rate of cephalexin increased with increasing adsorbent dose and contact time and decreased with increasing initial cephalexin concentration. The results of the isotherm and adsorption kinetics studies showed that the adsorption process on both adsorbents was more consistent with the Langmuir isotherm model with 0.9987 and 0.9991 correlation coefficients for AC-nZVI and AC-ZnO, respectively, and follows pseudo-second order kinetics. The maximum adsorption capacity of cefalexin for AC-ZnO and AC-nZVI based on Langmuir model was 83.33 and 89.28 mg/kg, respectively. The specific surface area of AC-ZnO, AC-nZVI and AC were calculated as 745.65, 827.66 and 738.17 m2/g, respectively. The results indicate the effectiveness of sodium hydroxide recovery. These results indicate that AC-nZVI and AC-ZnO were recoverable for cephalexin uptake. Conclusion: The removal rate by both types of adsorbents was significantly high indicating the effective role of nanoparticle stabilization on activated carbon. As a result, using nanoparticle stabilization technology on activated carbon as an inexpensive and efficient adsorbent can be used to remove cefalexin from aqueous media
