    Effects of Adding Pentoxifylline to Captopril on Primary Hypertension:A Pilot Randomized Clinical Trial

    Because of the key role blood viscosity plays in the regulation of blood pressure (BP) and the hemorheological effects of pentoxifylline (PTX), this study was conducted to evaluate whether PTX can reduce BP when added to captopril in patients with stage 1 hypertension. In this randomized clinical trial 62 patients with stage 1 hypertension were entered. The intervention group (n = 30) received 1200 mg PTX in 3 divided doses plus 25 mg captopril 3 times a day, whereas the control group (n = 32) received only 75 mg captopril in 3 divided doses. Measurements of BP were done at baseline and in the first and second months after entering the study. Major adverse cardiac events during this period were recorded. When the systolic BP levels in the intervention and the control groups were compared, no significant differences at baseline (150.4 ± 6.03 versus 150.4 ± 6.2, P = .98) or first (138.4 ± 9.4 versus 142.3 ± 5.6, P = .08) or second (134.6 ± 8.9 versus 137.4 ± 6.0, P = .20) month of the study were noted. Similarly no significant difference was observed in the diastolic BP at baseline (91.7 ± 3.9 versus 92.0 ± 3.7, P = .84) or first (85.5 ± 5.1 versus 86.9 ± 3.8, P = .27) or second (82.6 ± 5.7 versus 84.0 ± 3.5, P = .31) month. Based on the results of present study, adding PTX as a hemorheological agent to captopril could not significantly reduce blood pressure in the patients with stage 1 hypertension

    Echinococcus contamination ratio and its related risk factors in Moghan plain, northwest of Iran

    Introduction: Alveolar and cystic echinococcosis (AE and CE) are caused by the larval stages of Echinococcus multilocularis and Echinococcus granulosus, respectively. This study aimed to determine the prevalence of human AE and CE diseases among the tribes, livestock breeders, and farmers in Moghan plain, northwest of Iran. Methods: Screening of hydatid cystic fluid antigen (HCF‑Ag) was done by enzyme‑linked immunosorbent (ELISA) kit. Briefly, HCF‑Ag was collected, and the prevalence of the disease was evaluated using specific antigens for AE and CE (Ag‑5, Ag‑B, and Em2+) and questionnaires. A total of 2453 serum samples were randomly collected from normal populations of five different areas of Moghan plain. The serology was evaluated using ELISA kit and specific antigens for AE and CE Result(S): Of the 2453 serum samples, 36 were positive for AE (1.46%). The infection rate was higher in males than females (1.94% vs. 0.94%). The age group of 4–19 years had the lowest and 40–59 years had the highest infection rate (0.2% vs. 2.5%). Among CE‑positive serum samples, 178 samples were positive to Ag‑5 (7.3%) and 167 samples were positive to Ag‑B (6.8%). Discussion & Conclusion: Comprehensive studies on AE and CE diseases, especially their risk factors, are very rare in Iran.[10] However, CE is more prevalent and has been reported from all countries in the Middle East and North Africa. Albeit E. granulosus is highly prevalent in Iran, Turkey, Iraq, Morocco, Tunisia, and Libya[1,2]. Our findings showed that the highest infection rates were in the Borran and Eivazlou regions, and the lowest level of infection was for Parsabad area of Moghan plain. In areas with higher contamination, the risk factors associated with CE and AE were the water quality, the method of washing vegetables, and occupational and hydatid disease knowledge of the inhabitants

    REPORT OF A SUCCESSFUL TREATMENT OF THORACIC PENETRATING TRAUMA TO THE IVC WALL IN A 13 YEAR OLD BOY BY AN AIR GUN BULLET

    Introduction: Penetrating traumas to the thorax could be potentially serious. Although rare, vena caval penetrating injuries are the results of gunshot wounds. Whether caused by blunt or penetrating mechanisms, caval wounds are highly lethal so that half of the patients die before reaching the hospital and another 50% may die perioperatively. However, it is technically difficult vena caval repair with direct suturing technique can lead to narrowing and deep vein thrombosis. Materials & Methods: In the case we are reporting, a 13-year-old boy shot by an air gun through his right hemi thorax had referred to the trauma center. In his initial chest radiography the position of the bullet seemed to be in the middle mediastinum. He was then undergone the right sided thoracotomy with the preoperative diagnosis of right lower lobe laceration of the right lung and mediastinal injury. Having removed about 200 cc blood after thoracotomy and pericardiotomy, an air gun bullet appeared right in the wall of thoracic inferior vena cava that penetrated the right lung through and through on its way to IVC. There were not any ongoing bleeding because of its tightly package. Knowing that ligature of the IVC is not practicable and the cardiopulmonary bypass pump was not accessible, the only way to repair perforation was the release of the abdominal IVC, making an incision at the diaphragm then gripping it and immediate suturing the defect which was done successfully. Immediately following completing the vital assignment of potentially lethal bleeding source, tractotomy and ligature of small bronchi and vessels then repairing the diaphragmatic cut, finally a right chest tube and a pericardial tube were prepared. Results: During staying at hospital no complications like rebleeding or air leakage occurred and the patient successfully improved. Conclusion: Considering that, a general surgeon at a trauma center may encounter a mediastinal penetrating trauma, it is necessary to him or her to know the basis of handling with the vital surgeries of mediastinal great vessels in the absence of cardiopulmonary pump

    Comparison of the Effect of Probiotic with Caraway Mixture Syrup on Infant Colic

    Background and Objective: Infant colic is a relatively common behavioral syndrome in this period. The main mechanism of colic is still unknown and no effective treatment is indicated for it. Recently a relation between colic-infected children and the microbial balance changes in their intestines has been reported. Accordingly, we decided to compare the effect of probiotic with the caraway mixture syrup on the fertility colic by designing a study. Methods: In this clinical trial study, 100 infants with chicken implantation were introduced. Data extraction was done through a questionnaire during the course of the treatment through the follow-up of the infants (duration of crying, severity of crying, crying frequency and complications of treatment, etc.). Infants were randomly divided into two groups, based on random block design: probiotics (group one; 50) and caraway mixture syrup (group 2; 50). All the information was recorded and analyzed for each infant in the data collection checklists. Results: The mean age of the patients was 30.58±14.18 days. Two groups were similar according to age, sex, birth weight, parental education level, history of atopy in family, history of colic in previous childen and crying at presentation (p>0.05). There was no difference between groups based on the crying frequency, severity of crying and duration of crying on the 7th day. Also crying frequency, severity of crying and duration of crying (p>0.05) on the 15th day were similar between groups. However, the side effect of seventh (p=0.17) and fifteenth (p=0.11) days of treatment were similar between groups. Conclusion: Both probiotic and caraway mixture syrup are effective in the treatment of colic in infancy and had same side effects

    The Effectiveness of low dose Ketamine and Propofol in prevention of Shivering in patients during Cesarean section under Spinal Anesthesia

    Background and Objective: Postoperative shivering is one of the common complications after anesthesia and it is usually followed by many problems such as increase in Oxygen consumption, production of Carbon-dioxide, intra optical pressure and the pain of surgical site. The present study was designed to evaluate the effectiveness of low dose Ketamine and Propofol in prevention of Shivering in patients during Cesarean section under Spinal Anesthesia. Methods: In this randomized triple blind study, 147 patients who were candidate for elective cesarean section under spinal anesthesia, with ASA class I or II and aged 15-45 years, were randomly divided into three groups. After withdrawal of embryo, in one group 10 mg of Ketamin, in other one 10 mg of Propofol and in last group 2 cc Normal-salin was injected. During surgery and half an hour after operation in recovery room, the degree of shivering and nausea and vomiting in patients was evaluated and recorded in questionnaires. Results: There were no significant differences among three mentioned groups regarding of age, weight, gestational age, past surgical history and ASA (p>0.05). There was a significant difference between placebo group with Ketamine (p=0.004) and Propofol (0.032) group in during surgery shivering. Half an hour after surgery in recovery room, there was also a significant difference between placebo group with Ketamine (p=0.041)and Propofol(p<0.0001) group in shivering. In incidence of nausea-vomiting, there was no significant difference between groups during of surgery(p=0.318), but half an hour after surgery in recovery room there was a significant difference between placebo group with Ketamine and Propofol group(p=0.006). Conclusion: The present study showed that both of Ketamine and Propofol are effective in prevention of postoperative shivering and nausea-vomiting, as a result, these drugs can be used for control of shivering and nausea- vomiting in prohibition of other drugs

    Evaluation of Haptoglobin Genotype and Some Risk Factors of Cancer in Patients with Early Stage Esophageal Cancer

    Introduction: Esophageal cancer is one of the most lethal gastrointestinal cancers that has a complex and diverse etiology, with several genetic and nutritional factors involved in its etiology. The purpose of this study was to investigate the type of haptoglobin genotype and its relationship with some nutritional and biochemical risk factors affecting the prevalence of esophageal cancer in patients with early stage esophageal cancer. Materials and methods: In this study, 44 patients (20 males and 24 females) with early stage esophageal cancer and 44 healthy subjects, classified as control group, (19 males and 25 females) were selected. Haptoglobin (HP) genotype was determined employing PCR technique. Nutritional data were analyzed using standard food frequency questionnaire (FFQ) method. Serum levels of malondialdehyde (MDA), nitrate and nitrite were measured employing the colorimetric method. Serum levels of p53 protein were measured using the enzyme-linked immunosorbent assay (ELISA) technique. Results: The results of our study showed for the first time that HP1-1 genotype was the most prevalent genotype in esophageal cancer patients in Golestan province, Iran. HP2-2 genotype was the most frequent in the control group. Serum levels of MDA were significantly higher in the patients’ group compared to the control group (P=0.001). Weight and body mass index (BMI) were significantly lower in the patients’ group than the control group (P<0.01). Food frequency analysis revealed that the consumption of fruits and vegetables in the patients’ group was lower than that of the control group (P<0.05). Conclusion: The results of our study showed for the first time that HP1-1 genotype is the dominant genotype in patients with esophageal cancer in Golestan province. As well, modification of nutritional pattern and consumption of high level of antioxidant compounds can be effective in reducing the prevalence of esophageal cancer in this region

    Quinolinic acid (QA) known as a neuro-active metabolite associated with the kynurenine pathway. At high concentrations, QA is often involved in the initiation and development of several human neurologic diseases, like Alzheimer's disease. Because of the QA action as the NMDA receptor, it is considered as a potent excitotoxin in vivo. Since it is probable that different mechanisms are employed by QA, activation of NMDA receptors cannot fully explain the revealed toxicity and it is even believed that there are multiple unknown mechanisms/targets leading to QA cytotoxicity. Herein we report accelerated amyloid oligomerization of 1N4R Tau under the effect of QA, in vitro, then the molecular structure, morphology and toxicity of the protein aggregate were documented by using various theoretical/experimental approaches. The possible mechanism of action of QA-induced Tau oligomerization has also been explored

    Comparison Cytotoxic Effects of Mangifera Indica L. and Juglans Regia Aqueous Extract on Human Chronic Lymphocytic Leukemia

    Natural products isolated from plant sources are well known for their pharmacological potential in diversity of disease treatments such as inflammatory or cancer conditions. Mango (Mangifera indica L.) and Juglans regia are thought to be rich of functional phytochemicals. To elucidate the anticancer activity of Juglans regia (JR) and Mangifera indica L (MI) aqueous extract were investigated on chronic lymphocytic leukemia (CLL) B lymphocytes and their mitochondria and the results were compared with those of normal B lymphocytes. Cellular parameters such as viability and caspase 3 activity, and mitochondrial parameters such as reactive oxygen species (ROS), mitochondria membrane potential (MMP), mitochondrial swelling and cytochrome c release were evaluated. Our results showed the extract of Mangifera indica L increased cytotoxicity and caspase 3 activation through mitochondria pathway only in CLL B lymphocytes and also the extract of Juglans regia not showed cytotoxicity and caspase 3 activation on CLL and healthy B lymphocytes. Our in vitro findings on isolated mitochondria showed that mitochondrial ROS formation, MMP collapse, and mitochondrial swelling and cytochrome c release were significantly (P < 0.05) increased after addition of Mangifera indica only in cancerous mitochondria. These results showed that Mangifera indica can act as a promising source for anti-cancer drug candidates by directly and selectively targeting mitochondria and inducing selective mitochondria mediated apoptosis on CLL B lymphocytes
