    Comparison of human acellular amniotic membranes with acellular amniotic membranes pretreated with MPLA for repair of fascia in rats

    A decellularized amniotic membrane matrix is an ideal scaffolding system. This study compared the effectiveness of amniotic membrane and amniotic membrane pretreated with monophosphoryl lipid A (MPLA) for repairing the fascia of the abdomen with tension sutures. A total of 24 rats were randomly divided into three groups of eight each. In the control group, the fascia was repaired only with non-absorbable monofilament yarn under tension. In the second and third groups, in addition to the control treatment, acellular amniotic membrane was used and MPLA pretreated amniotic membrane was used, respectively. At 3, 7 and 14 days after treatment, the fascia repair was examined macroscopically and microscopically in all groups. Macroscopic examination showed that the use of MPLA pretreated significantly different from the other groups only in the fibrin exudate. Changes in the fascia rupture pressure showed significant differences between groups. Group three, which was sutured with MPLA pretreated acellular membrane, showed greater amounts of collagen, monocytes and neovascularization, especially at days 7 and 14. The results show that MPLA pretreated acellular amniotic membrane helped to repair abdominal fascia to some extent

    Osteogenic effect of electromagnetic fields on stem cells derived from rat bone marrow cultured in osteogenic medium versus conditioned medium in vitro

    Objectives This study assessed possible osteogenic differentiation caused by electromagnetic fields (EMF) on rat bone-marrow-derived stem cells (rBMSCs) cultured in osteogenic medium (OM) or in human adipose-stem cell-conditioned medium (hADSC-CM). Materials and Methods The rBMSCs were divided into negative and positive control groups, cultured in alpha-MEM plus 10% FBS or OM respectively. CM and CM + EMF groups, cultured cells in hADSCs-CM or exposed to EMF (50 Hz, 1 mT) for 30 min/day plus hADSCs-CM, respectively. Cells from the OM + EMF were simultaneously cultured in OM and exposed to EMF. Osteogenesis was investigated through alkaline phosphatase activity, alizarin red staining and real-time PCR. Results A meaningfully higher level of ALP activity was observed in the OM + EMF group compared to the other groups. There was a considerable increase in Runx2 expression in the CM + EMF group compared to the positive control and CM groups and a significant increase in Runx2 expression in the OM + EMF in comparison with all other groups after 21 days. Runx2 expression increased significantly in the CM, CM + EMF and positive control groups on day 21 compared to the same groups on day 14. From days 14-21, Ocn expression increased in the CM and CM + EMF groups, but both groups showed a significant decrease compared to the positive controls. CM and EMF had no effect on Ocn expression. On day 21, Ocn expression was significantly higher in the OM + EMF group than in the positive control group. Conclusion The synergistic effect of EMF and OM increased the expression of Runx2 and Ocn in rBMSC

    Sleep characteristics of middle-aged adults with non-alcoholic fatty liver disease: findings from the Shahrekord PERSIAN cohort study

    Background: Several studies have reported short sleep duration in people with non-alcoholic fatty liver disease (NAFLD) but other sleep characteristics have been less studied. We aimed to assess the cross-sectional association of NAFLD with sleep duration and quality in an Iranian population sample. Methods: We used data from 9,151 participants in the Shahrekord Prospective Epidemiological Research Studies in Iran (PERSIAN) Cohort Study, including 1,320 that were diagnosed with NAFLD. Log-binomial regression models sequentially adjusted for sociodemographic, lifestyle, clinical and biological variables were used to estimate relative risks (RR) and 95% confidence intervals (95% CI) for the association between NAFLD and sleep characteristics. Results: Participants with NAFLD had shorter sleep duration, later wake-up time and bedtime, worse sleep efficiency, and more frequent daytime napping and use of sleeping pills, in age- and sex-adjusted models. After controlling for sociodemographic, lifestyle, clinical, and biological variables the associations remained strong for sleep efficiency (per 10%, RR = 0.92, 95%CI: 0.88–0.96) and use of sleeping pills (RR = 1.48, 95%CI: 1.17–1.88). The association between NAFLD and sleep efficiency was stronger in participants aged > 60 years (RR = 0.81, 0.70–0.93) and 40–60 years (RR = 0.87, 0.82–0.94), compared with those aged < 40 years (P-heterogeneity < 0.001). More frequent daytime napping in participants with NAFLD, compared with non-NAFLD, was observed in males but not females (P-heterogeneity = 0.007), and in those with body mass index (BMI) < 30 but not in obese participants (P-heterogeneity < 0.001). Conclusions: Diagnosis of NAFLD is associated with several poor sleep characteristics in middle-aged Iranians. Although longitudinal studies would help to clarify the direction of causality, our study shows that poor sleep is an important aspect of NAFLD

    Ethnicity based differences in statin use and hypercholesterolemia control among patients with premature coronary artery disease-results of I-PAD study

    Background: Statins use is the most important treatment for high LDL cholesterol in patients with premature coronary artery disease (CAD). Previous reports have shown racial and gender differences in statin use in the general population, but this wasn't studied in premature CAD based on different ethnicities. Methods and results: Our study includes 1917 men and women with confirmed diagnosis of premature CAD. Logistic regression model was used to evaluate the high LDL cholesterol control in the groups and the OR with 95% confidence interval (CI) was reported as the effect size. After adjustment for confounders, the odds of controlling LDL in women taking Lovastatin, Rosuvastatin, and Simvastatin were 0.27 (0.03, 0.45) lower in comparison with men. Also, in participant who took 3 types of statins, the odds of controlling LDL were significantly different between Lor and Arab compared with Fars ethnicity. After adjustment to all confounders (full model), the odds of controlling LDL were lower for Gilak in Lovastatin, Rosuvastatin, and Simvastatin by 0.64 (0.47, 0.75); 0.61 (0.43, 0.73); 0.63 (0.46, 0.74) respectively and higher for Arab in Lovastatin, Rosuvastatin, and Simvastatin by 4.63 (18.28, 0.73); 4.67 (17.47, 0.74); 4.55 (17.03, 0.71) respectively compared to Fars. Conclusions: Major differences in different gender and ethnicities may have had led to disparities in statin use and LDL control. Awareness of the statins impact on high LDL cholesterol based on different ethnicities can help health decision-makers to close the observed gaps in statin use and control LDL to prevent CAD problems

    Association between ultra-processed foods consumption and micronutrient intake and diet quality in Iranian adults: a multicentric study

    Objective: To identify ultra-processed foods (UPF) contribution to daily energy and nutrient intake in Iranians and examine whether UPF intake is associated with nutrient profile and diet quality. Design: In this cross-sectional study, a validated FFQ was used to evaluate usual dietary intake over the preceding year. NOVA system was applied to categorise foods based on their levels of processing. Diet quality was evaluated using the nutrient adequacy ratio (NAR), Nutrient Rich Food Index (NRF) and hybrid nutrient density. Setting: The LIPOKAP study conducted in five cities of Iran (Isfahan, Birjand, Bandar Abbas, Kermanshah and Shahrekord). Participants: A total of 1994 adults aged ≥18 years were recruited using stratified multistage random cluster sampling method. Results: UPF were responsible for 8·5 % of daily energy intake. In the adjusted model, UPF consumption was inversely associated with carbohydrate, protein, refined and whole grains, fibre, fruit and meat, but was positively linked to energy, total fat, saturated and trans fatty acids and cholesterol. Compared with those in the lowest tertile, individuals in the highest tertile of UPF had smaller NAR for Ca, Mg, Zn, Fe, phosphorus, thiamin, niacin, folate and vitamin C. Both NRF and hybrid nutrient density decreased when the share of daily energy intake from UPF increased. Conclusion: The higher consumption of UPF is associated with poorer diet quality and lower nutrient intake. It is recommended that UPF be replaced with minimally processed foods to improve diet quality and nutrient profile

    Evaluation of Pb (II) Removal by Tea Pulp Modified with Magnetite Nanoparticle

    Tea waste was used to successfully synthesize magnetic nanoparticles (TWMNPs). In this investigation, Pb (II) was eliminated by tea waste modified with magnetite nanoparticles (TWMNPs) was investigated. To prepare the TWMNPs, FeCl3.6H2O was dissolved in double distilled water (DDW) and 20 g of pulp tea was added slowly and stirred, after 30 min TWMNPs adsorbent were separated through an external magnetic field and washed three times with double distilled water (DDW) and ethanol then dried at 60°C. The FESEM test of TWMNPs shows the particle size in the range of 15-20 nm and spherical/cuboid-shaped crystal structure of Fe3O4 (magnetite). X-ray analysis showed that the main XRD diffraction peaks of TWMNPs are related to Fe3O4, HighScore plus X'Pert software was used to identify the phase in this sample. The specific surface area of the prepared magnetite nanoparticles was 25.2 m2.g-1. The pore volume, maximum pore radius, and VSM of TWMNPs were 14.4 cm3.g-1, 2.3 nm, and 3.37-2.41, respectively. The effects of various parameters, such as contact time, pH, concentration, and adsorbent dosage, were studied. The experimental isotherm data were analyzed using the Langmuir and Freundlich models, and it was found that the removal process followed the Langmuir isotherm and the maximum adsorption capacity calculated by Langmuir fitting was 10.67 mg.g-1. In addition, the adsorption kinetics followed the first-order kinetic model and the value of rate constant was found to be 14.04 × 10-2 min-1. The results showed that increasing the pH level led to a rise in the response level and Pb (II) removal. Also, the trend in Pb (II) removal and response level had an increase with increasing the initial concentration of Pb (II). Increasing contact time from 5 to 20 minutes has a slight effect on Pb (II) removal. Considering the results, TWMNPs could lead to suitable results for the removal of Pb (II) from wastewater containing this metal. And the maximum adsorption capacity was found to be 10.67 mg.g-

    A case of multiple myeloma with unusual presentation

    Extramedullary plasmacytoma is a rare and unusual complication of multiple myeloma (MM). Some sites for the extramedullary involvement of MM are the liver, spleen, lymph node, lung, and skin. Renal failure is another complication of MM due to myeloma kidney, uric acid nephropathy, hemoconcentration, or light chain disease. This study reported a case of MM with unusual presentation. She was a 61-year-old woman who was admitted to the internal ward due to severe renal failure, anemia, mild proteinuria, hypertension, thrombocytopenia, and soft tissue mass in both lungs in the spiral computed tomography (CT) scan. In the skull X-ray, there was not any lytic lesion. Further, plasma cells greater than 30% were observed in serum protein electrophoresis pick of gamma region and in the bone marrow aspiration, so the final diagnosis of the patients was MM with extramedullary plasmacytoma.     Anuria in solitary kidney patient with gastroenteritis: A case report study

    For better management of acute kidney injury (AKI) and prevention of progressive renal injury, it is extremely important to manage infections such as gastroenteritis. In addition, anuria is considered a risk factor for AKI and even causes death in patients. An 18-month-old boy with vomiting and diarrhea for three days and anuria for 24 hours was referred to Farsan hospital. The patient had bulky and watery diarrhea about 7-8 times a day and vomited 3-4 times a day which was containing food particles. He had a fever on the first and second days. Ultrasound findings revealed that the right kidney did not exist, and the left kidney was affected by compensatory hypertrophy. Disorders in the patient’s blood biochemical factors were also observed. Acidosis and other biochemical disorders were treated with bicarbonate drip, allopurinol, Lasix drip, and dopamine drip.     Noise exposure and the risk of cancer: a comprehensive systematic review

    The association between noise exposure and increased risk of cancer has received little attention in the field of research. Therefore, the goal of this study was to conduct a systematic review on the relationship between noise exposure and the incidence of cancer in humans. In this study, four electronic bibliographic databases including Scopus, PubMed, Web of Science, and Embase were systematically searched up to 21 April 2022. All types of noise exposure were considered, including environmental noise, occupational noise, and leisure or recreational noise. Furthermore, all types of cancers were studied, regardless of the organs involved. In total, 1836 articles were excluded on the basis of containing exclusion criteria or lacking inclusion criteria, leaving 19 articles retained for this study. Five of nine case-control studies showed a significant relationship between occupational or leisure noise exposure and acoustic neuroma. Moreover, four of five case-control and cohort studies indicated statistically significant relationships between environmental noise exposure and breast cancer. Of other cancer types, two case-control studies highlighted the risk of Hodgkin and non-Hodgkin lymphoma and two cohort studies identified an increased risk of colon cancer associated with environmental noise exposure. No relationship between road traffic and railway noise and the risk of prostate cancer was observed. In total, results showed that noise exposure, particularly prolonged and continuous exposure to loud noise, can lead to the incidence of some cancers. However, confirmation of this requires further epidemiological studies and exploration of the exact biological mechanism and pathway for these effects
