The Wide Open Windows of Cholera Street: On the Light and Sound Leaking Through/To the Private Space
Inspired by the subaltern studies the purpose of this article is to examine how the dichotomy of private/public in Metin Kacan's Agir Roman Novel is reproduced on the axis of the visual language used by Mustafa Altioklar's cinematic adaptation Cholera Street. The article is interested in the peculiar choice of slang usage and reads this as an invitation to blur the borders of private/public space that modern life demands to keep separate. In this sense, Cholera Street can also be regarded as a brilliant piece of social commentary, offering a vivid peek into the life of the "other" trapped in the peripheral neighborhood. This article unravels further how Cholera Street through visual film grammar and various metaphors sends strong critical messages about the silence of subalterns who often lack the means to speak for themselves and how the violation of privacy turns out to be a challenging act against the dominant order
Factors associated with social anxiety disorder in a group of obese Turkish female patients
Background/aim: Mental disorders may accompany obesity. This study aims to evaluate the association between social anxiety disorder (SAD) and obesity and the risk factors for SAD in obese female patients.
Materials and methods: A total of 114 obese patients and 110 healthy controls were included. The Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI), State and Trait Anxiety Inventory (STAI I-II), and Liebowitz Social Anxiety Scale (LSAS) were administered to assess anxiety, depression, and social anxiety levels. Scale scores were analyzed statistically.
Results: The rate of SAD in obese female patients was found to be 8.8%. Anxiety, depression, and social anxiety levels were significantly higher in the obesity group compared to the control group (P < 0.05). According to linear regression analyses, a significant association between LSAS anxiety level and age, prior surgery, social support, history of being teased, BDI, and BAI was found.
Conclusion: The present study shows that many factors are related to obesity and SAD in obese female patients. The clinical implications of these findings should be considered. Interventions for these factors may help prevent SAD in obese female patients
Effects of significant weight loss following bariatric surgery on red cell distribution width and mean platelet volume
Aim: Elevated red blood cell distribution width (RDW) and mean platelet volume (MPV) levels which are closely associated with chronic inflammation and platelet aggregation are suggested as independent predictors of obesity and cardiovascular diseases. However influence of significant weight loss following bariatric surgery on these parameters is unknown. Therefore we aimed to find out the effect of significant weight loss following laparoscopic sleeve gastrectomy (LSG) on RDW and MPV levels. Methods: The medical data of 98 morbid obese subjects (25 male, 73 female) who were operated between February 2015 and June 2017 according to indications of bariatric surgery in current guidelines including body mass index (BMI) >40 kg/m 2 or BMI= 35-40 kg/ m 2 with additional comorbidities were recorded. The difference between baseline and values at one year was expressed as a delta (Δ). Results: The mean age of our study population was 41.89±11.99 years and mean weight loss in one year after LSG was 45.41±13.13 kg (36.5%). BMI decreased from 46.60±7.11 kg/m 2 to 29.58±4.63 kg/m 2 . Compared to the baseline, significant decreases in RDW (14.61±1.69 % vs. 13.71±2.10 %; p<0.01) and MPV (8.63±1.45 vs. 7.92±1.24 fL, p<0.001) levels were found in the postoperative one-year values. In addition; ΔVKI was positively correlated with ΔRDW (r=0.343, p<0.01) and ΔMPV (r=0.322, p<0.01). Conclusion: We found that morbid obese subjects have significantly decreased RDW and MPV levels which are correlated to their weight loss in one-year follow-up after LSG. If we think that several factors may affect these parameters, conduction of further prospective large-scale studies are needed. © 2018, Logos Medical Publishing. All rights reserved
Hybrid approach: an alternative prior to corrective surgery in a patient with arch hypoplasia and complete atrioventricular septal defect
The hybrid approach is mostly preferred in patients with hypoplastic left heart syndrome or univentricular physiology. Here, the hybrid approach is applied as a palliative procedure prior to corrective surgery in a patient with complete atrioventricular septal defect associated with arcus hypoplasia and results are discussed according to the literature
Media-based clinical research on selfie-related injuries and deaths
BACKGROUND: The incidence of taking selfies and sharing them on social media as well as selfie-related behaviors is increasing, particularly among young people, possible leading to selfie-related trauma. Therefore, we performed this clinical study to draw attention to selfie-related injuries and deaths.
METHODS: We analyzed 159 selfie victims from 111 events or accidents, which were reported in the media sources. We evaluated vital results, demography, rhythmicity, preferences, event or accident types, selfie-related risk factors, affected body regions of victims with causes of injury, and death.
RESULTS: We found that the majority of selfie victims were students. Selfie-related injuries and deaths were reported most frequently in India, the US, and Russia. The most preferred site of taking selfies was the edge of the cliff. The most frequently reported event or accident type was falling from a height. Mostly multiple body parts were affected in selfie-related injuries and deaths. The most frequent causes of selfie-related deaths were multitrauma and drowning.
CONCLUSION: Selfie-related injuries and deaths have increased in the past years. Particularly, teenagers and young adults are at high risk for selfie-related traumas and deaths; therefore, drastic measures should be taken to reduce their incidence
Diagnostic value of acustic radiation force impulse imaging in the assessment of salivary gland involvement in primary Sjogren's sydrome
Aims: The aim of this study is to investigate the diagnostic value of Acoustic Radiation Force Impulse (ARFI) imaging in the assessment of salivary gland involvement in primary Sjogren's syndrome (pSS). Material and methods: Twenty five patients with pSS and 25 healthy volunteers were included. First, echostructures and the thickness of the submandibular and parotid glands were evaluated by B-mode ultrasonography. Then, ARFI imaging with Virtual Touch Quantification (R) was performed. Ten independent shear wave velocity measurements were taken from each gland. Finally, the mean shear wave velocity (SWV) values were calculated, and used for further analysis. Results: The mean SWV values of parotid and submandibular glands were significantly higher in the pSS patients than in the healthy control group (p 0.005). Conclusions: Our findings indicate that ARFI imaging may provide a non-invasive, simple and fast means of assessment of glandular impairment as an alternative test when other salivary gland tests are inconclusive or cannot be performed. ARFI may be a valuable adjunct for the clinical diagnosis of pSS
Assessment of Endotracheal Tube Position After Oral Intubation in Neonates
Aim: Endotracheal intubation is a common procedure in the delivery room and neonatal intensive care unit. We aimed to determine the accuracy of this method of endotracheal tube (ETT) placement in our neonatal cohort.
Materials and Methods: Data on infants requiring oral intubation were reviewed retrospectively. The initial ETT depth of insertion had been calculated using the Tochen 7-8-9 rule. The initial depth was compared to the mid-tracheal region. The differences between the initial and ideal depth was calculated and divided by the mid-tracheal distance. Infants were grouped according to their weights as = 3001g.
Results: We evaluated ETT placement in 160 neonates. The mean gestational age was 32.2 +/- 4.4 weeks (23 to 41 weeks) and the mean weight was 1989 +/- 829 g (560 to 3800 g). The mean range of the difference between the initial depth and ideal depth divided by mid-tracheal distance was 0.39 +/- 0.04, 0.35 +/- 0.04, 0.46 +/- 0.05, and 0.23 +/- 0.04 in infants weighing = 3001 g respectively (p=0.025). The differences between the 2001-3000 g group and the 1001-2000, also the 2001-3000 g group and the >= 3001 g group were statistically significant (p<0.05).
Conclusion: The 7-8-9 rule should be used to assess ETT length in neonates, especially in those weighing more than 3 kg. As this rule has low accuracy for extremely low birth weight neonates, its reliability may not be high for neonates weighing less than 3 kg in weight
A misleading cutaneous lesion for the surgeon: pyoderma gangrenosum
Pyoderma gangrenosum is a rare inflammatory skin disorder characterized by cutaneous papulo-pustules that evolve rapidly into large, painful, necrotic ulcers. These lesions are often initially diagnosed as wound infection and necrotizing fasciitis. Debridement, although the standard of care for wound infections, is paradoxically detrimental in pyoderma gangrenosum because of pathergy. Early diagnosis and treatment of pyoderma gangrenosum is very important because unnecessary debridement and associated morbidity can be prevented. HEADACHE CHARACTERISTICS IN GERIATRIC AGE GROUP ACCORDING TO ICHD-3 BETA VERSION
Introduction: The aim of this study was to screen patients aged >= 65 years admitted to the Baskent University Hospital Neurology Polyclinic between 2011 and 2013 and classify them according to the International Classification of Headache Disorders, 3rd edition.
Materials and Method: In total, 175 patients (126 females and 49 males) were screened. First, they were separated into primary and secondary headache groups, and the subgroup headache types were then defined within these groups according to the International Classification of Headache Disorders, 3rd edition; then, the concurrence of accompanying chronic diseases were examined.
Results: In the primary headache group, 21.1% of the patients had migraine, 39.4% had tension-type headache, and 51% had trigeminal neuralgia; in the secondary headache group, 24.2% of the patients had headache attributed to hypertension, 18.2% had headache attributed to temporal arteritis, 18.2% had headache attributed to cervical pathology, and 12.1% had headache attributed to intracranial mass.
Conclusion: This study was conducted to detect the most commonly seen headache types in geriatric population, and the results of this study are important in terms of increasing awareness of the evaluation of both primary and secondary headaches in geriatric patients. Because the study was conducted with a less patient population, it is important in terms of more accurately demonstrating the factors constituting the etiology of headache in this patient group
The Effect of Sumatriptan in lschemic Conditions in the Rat Heart
Objectives: The aim of this study was to investigate the effect of SUM on IR-induced injury in rat heart and its effect on IPC-induced protection.
Materials and Methods: The rats were randomly divided into four groups: IR, SUM-IR, IPC, and SUM-IPC. The mean arterial blood pressure and heart rate were recorded to calculate PRP. Standard limb lead 2 ECG were recorded to evaluate arrhythmia parameters.
Results: The PRP values in the SUM-IPC group were significantly lower than in the SUM-IR group at the beginning of reperfusion (p<0.05). The incidence of VT in the IPC, SUM-IR, and SUM-IPC groups was significantly lower than in the IR group (p<0.05). VF was only observed in the IR group.
Conclusion: SUM protects the heart against IR injury but is not as protective as IPC alone. Although SUM diminishes IPC-induced protection against VT, the preventive effect of SUM against VF may be predictive for cardioprotection in ischemic conditions
