    Comparison of the inhibitory effect of different doses of subconjunctival bevacizumab application in an experimental model of corneal neovascularization

    AIM: To evaluate the inhibitory effect of subconjunctival bevacizumab as single- and multiple-dose application, and compare their effects on corneal neovascularization in a rat model. METHODS: Thirty adult Sprague-Dawley rats were used in this experimental study. The central cornea of the rats was cauterized chemically. The rats were randomly enrolled into three groups. All groups received subconjunctival injections. In Group 1 (control group, n=10), 0.05 mL 0.9% NaCl solution was injected on the first day. In Group 2 (single-dose group, n=10), 0.05 mL bevacizumab (1.25 mg) was injected on the first day. In Group 3 (multiple-dose group, n=10), four doses of 0.05 mL bevacizumab (1.25 mg) were injected on the first, third, fifth and seventh day. Slit-lamp examination of all rats was performed at the third and ninth day. Digital images of the corneas were taken and analyzed using image analysis software to calculate corneal neovascularization area. All rats were sacrificed on the tenth day. In corneal sections, the number of blood vessels, state of inflammation and collagen formation was evaluated histopathologically. RESULTS: In Group 3, corneal edema grades were significantly lower than Group 1 and Group 2 (P=0.02, and P=0.035, respectively). The mean percentage of neovascularized corneal area in Group 3 was significantly lower than Group 2 (P=0.005). On histopathological examination, Group 2 and Group 3 showed significantly less number of blood vessels than Group 1 (P=0.005, and P=0.001, respectively). Additionally, Group 3 showed significantly less number of blood vessels compared to Group 2 (P=0.019). Inflammation and edema grades were significantly lower in Group 3 compared to Group 1 (P=0.001). CONCLUSION: Subconjunctival bevacizumab injection is effective in inhibition of newly formed corneal neovascularization. The multiple-dose bevacizumab treatment seems to be more effective compared to single-dose treatment

    Hydrogen Economy Model for Nearly Net-Zero Cities with Exergy Rationale and Energy-Water Nexus

    The energy base of urban settlements requires greater integration of renewable energy sources. This study presents a "hydrogen city" model with two cycles at the district and building levels. The main cycle comprises of hydrogen gas production, hydrogen storage, and a hydrogen distribution network. The electrolysis of water is based on surplus power from wind turbines and third-generation solar photovoltaic thermal panels. Hydrogen is then used in central fuel cells to meet the power demand of urban infrastructure. Hydrogen-enriched biogas that is generated from city wastes supplements this approach. The second cycle is the hydrogen flow in each low-exergy building that is connected to the hydrogen distribution network to supply domestic fuel cells. Make-up water for fuel cells includes treated wastewater to complete an energy-water nexus. The analyses are supported by exergy-based evaluation metrics. The Rational Exergy Management Efficiency of the hydrogen city model can reach 0.80, which is above the value of conventional district energy systems, and represents related advantages for CO2 emission reductions. The option of incorporating low-enthalpy geothermal energy resources at about 80 degrees C to support the model is evaluated. The hydrogen city model is applied to a new settlement area with an expected 200,000 inhabitants to find that the proposed model can enable a nearly net-zero exergy district status. The results have implications for settlements using hydrogen energy towards meeting net-zero targets

    Cardiovascular disease is one of the most frequent causes of mortality and morbidity worldwide. Several variables have been identified as risk factors for cardiovascular disease. Recently, the role of receptor activator of nuclear factor kappa B, receptor activator of nuclear factor kappa B ligand, and the osteoprotegerin system has been recognized as more important in the pathogenesis of cardiovascular disease. Besides their roles in the regulation of bone resorption, these molecules have been reported to be associated with the pathophysiology of cardiovascular disease. There are conflicting data regarding the impact of osteoprotegerin, a glycoprotein with a regulatory role in the cardiovascular system.     Purpose: The aim of this study was to determine the frequency of anatomical parameters and their measurement as seen on normal posteroanterior (PA) chest radiographs in a Turkish population. Materials and Methods: We evaluated fifty normal PA chest radiographs of Turkish population adults. The frequency of anatomical parameters and their measurement with respect to the diaphragm was evaluated. Results: The frequency of anatomical parameters and their measurement with respect to the diaphragm is as follows: level (right side higher in 98%, left and right sides same level 2%), lobulation (88% absent, 12% present), eventration (98% absent, 2% present) and contour (90% smooth, 10% not smooth); level of hilum (right and left sides same level 52%, left side higher 36%, right side higher 12%); number of pairs of ribs (twelve 96%, unable to determine 4%); number of ribs superposing the lung parenchyma (seven 2%, eight 4%, nine 24%, ten 70%); distance from the lateral margin of the vertebral body to the aorta (16.44 +/- 4.35 mm); angle between the vertebral body and the aorticopulmonary line (16.04 +/- 3.110); the carina angle (58.46 +/- 11.130) and the cardiothoracic ratio (38.75 +/- 4.27). Conclusion: The data presented in this study may be useful in understanding normal thoracic structures. A knowledge of the normal anatomy as well as variations are significant for physicians in the assessment of chest radiographs

    Evaluation of right ventricular function in operated tetralogy of Fallot patients with tissue Doppler imaging before and after pulmonary valve replacement

    Background: This study aims to investigate the efficacy of tissue Doppler echocardiography for the diagnosis of right ventricular dysfunction and for the evaluation of the response to pulmonary valve replacement. Methods: Between December 2008 and December 2010, a total of 15 patients (8 males, 7 females; mean age: 14.5 +/- 4.5 years; range, 7 to 22 years) with pulmonary valve insufficiency in the follow-up of total correction of tetralogy of Fallot who underwent pulmonary valve replacement were included in the study. Echocardiographic imaging was performed before and one, three, and six months after replacement. Using pulse wave Doppler, early diastolic E and late diastolic A waves were measured in the flow velocity curve of tricuspid valve. Tissue Doppler measurements were made from the basal segment of right ventricular free wall and interventricular septum. S, e, and a myocardial velocities, isometric contraction time, isovolumetric relaxation time and ejection time were measured. Myocardial performance index was calculated for each patient. Results: In pre-pulmonary valve replacement phase, s, e, a velocities and ejection time were lower in the interventricular septum compared to the basal segment of right ventricular free wall, while isometric contraction time, isovolumetric relaxation time and myocardial performance index values were higher. In the first post-replacement month, there was a significant decrease in the s value of interventricular septum, compared to pre-pulmonary valve replacement period; isometric contraction time decreased and myocardial performance index increased in the first post-pulmonary valve replacement month in the free wall of right ventricle, while isovolumetric relaxation time decreased in the postoperative third month, and s velocity, isometric contraction time and myocardial performance index increased in the postoperative sixth month. Pre-pulmonary valve replacement tissue Doppler values at the septum showed that myocardial systolic and diastolic dysfunction developed earlier than the basal segment of right ventricular free wall. This result showed that right ventricular recovery was not complete in the sixth post-pulmonary valve replacement month. Conclusion: Tissue Doppler can show right ventricular dysfunction before replacement, but it may be insufficient to visualize right ventricular recovery until six months after treatment

    Çalışmanın amacı; Türkçe konuşan geriatrik bireylerde, HKNÖ’nin Türkçe uyarlaması ve kültürel adaptasyonunu sağlayarak, geçerlik ve güvenilirliğini ölçmektir. Çalışmamıza, 65-80 yaş arası, kooperasyon ve anlamaya engel olabilecek herhangi bir mental problemi olmayan ve araştırmaya gönüllü olarak katılan 103 sağlıklı geriatrik birey (yaş ortalaması:71.09±4.45 yıl, vücut kütle indeksi:27.08 ±4.4 kg/m2) dahil edildi. Bireylere Türkçe çevirisi yapılan HKNÖ ile kriter geçerliğini belirlemek amacıyla Tampa Kinezyofobi Ölçeği (TKÖ) uygulandı. Ölçeğin güvenilirliğinin göstergesi olan zamana göre değişmezliği test etmek için 48 saat sonra, HKNÖ ikinci kez uygulandı. Ankette yer alan tüm sorular arası iç tutarlılık değeri 0.863 olarak bulundu. Biyolojik alt boyutta yer alan sorular arası Cronbach’s α katsayısı 0.913, Psikolojik alt boyutta 0.797 olarak bulundu. Biyolojik parametre alt boyutlarının (morfolojik, harekete geçmek için bireysel ihtiyaç, enerji kaynakları, biyolojik dürtülerin gücü) Cronbach’s α 0.800 ve üstü iken, psikolojik parametre alt boyutlarının (kendini kabul etme, motor yatkınlığın öz değerlendirmesi, vücut bakımı) iç tutarlılık değeri sırasıyla 0.834, 0.790, 0.642 bulundu. TKÖ ve HKNÖ alt boyut puanları arasındaki korelasyonlar incelendiğinde TKÖ toplam puanı ile HKNÖ toplam puanı, biyolojik ve psikolojik alt boyutları arasında istatistiksel olarak anlamlı, pozitif yönde korelasyon belirlendi (p<0.05). Sonuç olarak, yetişkinde motor inaktivitenin ve hareket korkusunun sebeplerinin belirlenmesi amacıyla geliştirilmiş ve şimdiye kadar başka herhangi bir dilde adaptasyonu yapılmamış olan HKNÖ’nün, Türk geriatrik bireylerde geçerli ve güvenilir bir anket olduğunu ayrıca farklı hastalıklarda, daha büyük örneklemlerde ve yaş gruplarında incelenmesi gerektiğini düşünmekteyiz. The aim of this study is to provide the Turkish adaptation and cultural adaptation of KCS in Turkish-speaking geriatric individuals and to measure their reliability and validity. Our study included 103 healthy geriatric individuals (the average age: 71.09±4.45 years, body mass index: 27.08±4.4 kg/m2) the aged of 65-80 years, who had no mental problems that could interfere with cooperation and understanding and who volunteered to participate in the study. Tampa Scale of Kinesiophobia (TSK) was applied in order to determine the validity of the criterion with the KCS translated into Turkish. After 48 hours of testing for variability, which is an indicator of the reliability of the scale, KCS was applied for the second time. The internal consistency value of the questionnaire was 0.863. The Cronbach’s α coefficient was 0.913 among the questions in the biological sub-dimension, and 0.797 in the psychological sub-dimension. While the biological parameters sub-dimensions (morphologic, individual need for stimulation, energetic substrates, power of biological drives) have Cronbach's α coefficient of 0.800 and above, the internal consistency of psychological parameter sub-dimensions (self-acceptance, self-assessment of motor susceptibility, body care) is respectively 0.834, 0.790, 0.642. When the correlations between TSK and KCS subscale scores were examined, there was a statistically significant positive correlation between total score of TSK and total score of KCS biological and psychological subscales (p<0.05). As a result, we think that the KCS questionnaire, which has been developed in order to determine the causes of motor inactivity and fear of movement in adults, is a valid and reliable survey in Turkish geriatric individuals and should be examined in different diseases and age groups

    Bacterial detection using bacteriophages and gold nanorods by following time-dependent changes in Raman spectral signals

    This study attemps to develop bacterial detection strategies using bacteriophages and gold nanorods (GNRs) by Raman spectral analysis. Escherichia coli was selected as the target and its specific phage was used as the bioprobe. Target bacteria and phages were propagated/purified by traditional techniques. GNRs were synthesized by using hexadecyltrimethyl ammonium bromide (CTAB) as stabilizer. A two-step detection strategy was applied: Firstly, the target bacteria were interacted with GNRs in suspensions, and then they were dropped onto silica substrates for detection. It was possible to obtain clear surface-enchanced Raman spectroscopy (SERS) peaks of the target bacteria, even without using phages. In the second step, the phage nanoemulsions were droped onto the bacterial–GNRs complexes on those surfaces and time-dependent changes in the Raman spectra were monitored at different time intervals upto 40 min. These results demonstrated that how one can apply phages with plasmonic nanoparticles for detection of pathogenic bacteria very effectively in a quite simple test. © 2018, © 2018 Informa UK Limited, trading as Taylor & Francis Group

    Recurrence of Primary Vascular Leiomyosarcoma Five Years after Initial Diagnosis in the Lower Extremity

    Primary leiomyosarcomas of vascular origin are rare tumors. They frequently arise within the inferior vena cava; however, the peripheral vein was also affected. To date, only a few hundred cases have been reported in the world literature. Although it is an extremely aggressive tumor, the symptoms may be unspecific, especially in the lower extremities. In this report, we present a case of primary vascular leiomyosarcoma, arising from the short saphenous vein, with symptoms mimicking thrombus in the initial diagnosis. The diagnosis of leiomyosarcomas was confirmed by standard H&E staining and immunohistochemical staining. Recurrence of the tumor has been observed five years after surgical treatment. Due to its rarity, experience in the management of this type of tumor is limited. The mainstay of treatment for these tumors is complete surgical resection. The purpose of the presented case is to discuss the clinicopathological features and management options of this tumor, under the light of the most recent literatures
