Stewart-Treves Syndrome: A Case Report and Review of Literature
Stewart-Treves syndrome (STS) is defined as angiosarcoma arising in the setting of chronic lymphedema. It is typically presented in breast cancer patients who underwent axillary dissection. A 79-year-old woman presented to our clinic with a wound that developed secondary to lymphedema. Pathologic examinations diagnosed the lesion as angiosarcoma. STS is a rare and deadly entity. It is hard to diagnose and has 10% mean survival rate for 5 years. We emphasize the importance of early diagnosis that can be a lifesaver
The Impact of the Flipped Classroom Model on Students' Academic Achievement
The aim of this study is twofold. First, it aims to investigate the impact of the Flipped Classroom (FC) Model on students' academic achievement. Second, it reveals the students' opinions about the model itself. For four weeks, the students in the experimental group were taught in a blended learning context where the FC Model was applied, while the lessons in the control group were carried out through traditional blended learning. Both groups were administered a test before and after the Flipped Classroom sessions. To analyze the data, a two-way ANOVA for Mixed Measures was conducted to compare the means of test scores of each group. The results showed that there were no statistically significant differences between the scores of the two groups. Coming to classes prepared and completing the assignments in class, so that students did not need to do assignments at home, were among the positive aspects of the FC Model. The problems encountered in this model, however, are categorized under three main titles: Motivation, Content, and Learning. At the end of the study, the advantages and disadvantages of the FC Model are identified in accordance with the participants' opinions, and necessary suggestions made
Gastrointestinal Stromal Tumors: A Clinicopathologica and Immunohistochemical Study of 65 Cases
Objective: Hie clinical behavior of gastrointestinal stromal tumors is divergent. The aim of the present study was to define the clinicopathological features that determine the patient's outcome.
Material and Method: Sixty-five gastrointestinal stromal tumors were reviewed with their histological, immunohistochemical and clinical features and compared with their clinical outcome statistically.
Results: Tumors were located in the stomach (n=39, 60%), small intestine (n=22, 33.8%) and large intestine (n=4, 6.2%). Immunohistochemically, CD 117 positivity was found in 90.8%, whereas CD34, Smooth muscle actin, Desmin and S100 positivity was found in 73.3%, 61.7%, 11.7% and 28.3% of tumors respectively. All six "CD 117-negative" cases expressed DOG-1. The mean Ki-67 proliferation index was 8.69%+/- 12.76. Liver metastasis was detected in seven cases. A significant association was detected between decreased mean survival time and increased tumor size (p<0.001), large bowel localization (p-0.047), mitosis (p<0.001), the presence of necrosis (p=0.001), metastasis (p=0.033), Ki-67 proliferation index (p-0.002) and risk category (p<0.001). CD 34 positivity was mostly seen in the stomach (p-0.001), and CD 34 positive tumors had longer overall survival (92.85.+/- 5.77 months versus 67.21 +/- 13.68 months) (p=0.046). Higher Ki-67 proliferation index (6%) was also correlated with the presence of metastases (p=0.015).
Conclusion: Our study indicates that in addition to well-known risk factors such as increased tumor size, high mitotic activity and metastasis; higher Ki-67 proliferation index, the presence of necrosis, and CD34 negativity also correlate with shorter survival time
The need for technology management education for undergraduate programs: A conceptual framework
As the National Research Council [17] described the technology management as “a process, which includes planning, directing, control and coordination of the development and implementation of technological capabilities to shape and accomplish the strategic and operational objectives of an organization’’, Technology Management education is becoming more important for solving problems within organizations. Therefore, institutions of higher education need to lead the way in developing programs, curriculums, departments, and institutes addressing and meeting the needs of industries and the organizations. In this paper we evaluate the need of technology management undergraduate programs and provide some insights about this need. We realize the importance of technology management in both industrial and academic contexts and how important it is to have an interaction and collaboration between the industry organizations and universities. However, there is a lack of technology education on undergraduate level at today’s universities curriculum. While there are many graduate programs on technology management in universities around the world, scholars and practitioners are paying less attention to undergraduate programs on technology management education. Therefore, we focus more on technology management education in undergraduate programs by outlining several assumptions, proposing a conceptual framework based on current debates in literature, and articulating its importance in the academic. © 2018 by authors, all rights reserved
Evaluation of Different Restoration Combinations Used in the Reattachment of Fractured Teeth: A Finite Element Analysis
Objective. The purpose of this study was to test different restoration combinations used for constructing fractured endodontically treated incisors by reattaching their fractured fragments. Methods. Seven types of 3-D FEM mathematical root canal-filled models were generated, simulating cases of (OB) reattaching fractured fragments; (CrPL) reattaching fractured fragments + ceramic palatinal laminate; (CmPL) reattaching fractured fragments + composite palatinal laminate; (CM) reattaching fractured fragments + coronal 1/3 of the root was filled using core material; (BP) reattaching fractured fragments + glass fiber post; (CP) composite resin restoration + glass fiber post; and (OC) composite resin restoration. A 100-N static oblique force was applied to the simulated teeth with 135 degrees on the node at 2 mm above the cingulum to analyze the stress distribution at the tooth. Results. For enamel tissue, the highest stress values were observed in model BP, and the lowest stress values were observed in model CmPL. For dentine tissue, the highest stress concentrations were observed around the fracture line for all models. Conclusions. Reattachment of fractured fragments by bonding may be preferred as a restoration option for endodontically treated incisors; also, palatinal laminate decreases the stress values at tooth tissues, especially at the enamel and the fracture line
Humidification therapy; long-term effects in COPD and OSAS patients
Humidification therapy is widely used in patients during invasive ventilation and the importance of heating and humidifying the inhaled air is well defined in both the acute and long term setting. On the other hand the continuous usage of humidification in noninvasive ventilation is controversial and the long term effects are still not well defined. The usage of noninvasive ventilation is especially important in hypercapnic Chronic obstructive pulmonary disease (COPD) and also the obstructive sleep apnea syndrome (OSAS) patients and compliance to this treatment is essential in long term. Limited number of studies have shown that heated humidification is preferred by COPD patients and might help in mucus clearance, reduce mucus viscosity and help in expectoration, but there was shown no priority as far as concerns the lung function or blood gas parameters. Humidification might also be important in long term oxygen treatment since the inhalation of dry air can lead to ciliary dysfunction, alterations in mucus properties and mucociliary clearance impairment. Studies concerning OSAS patients showed that humidification helps to reduce the nasal symptoms but plays no role in increasing the compliance of patients in long term. Multi-center studies with large number of patients are needed to identify patients' groups who are likely to benefit from the addition of humidification to noninvasive therapy
Monitoring nodule progression in chest X-ray images
Lung nodules are frequently observed in cases of cancer. Nodules can be monitored with technologies such as computed tomography (CT) or magnetic resonance imaging (MRI). However. x-ray imaging is a low-cost method as well as its widespread usage. In this context, monitoring the nodules in short intervals by x-ray imaging gives benefits in many aspects. In this study, a three-stage novel approach is proposed to trace the nodule progressions from the lung x-ray images, automatically. In the first stage, x-ray images of a patient taken at different times must be registered to evaluate the nodule progression. To perform the registration, feature extraction and matching methods are employed, and then the homography matrix is calculated. In the second stage, according to previously known nodule positions, matched nodules are detected on registered images. Mismatched nodules in the first image are considered as lost, while the nodules only found in the second image are evaluated as newly appeared. In the last stage, nodules are considered as closed contours consisting of pixel set where closed contour area is calculated after nodule matching process. In this way, growth and shrink states are determined numerically. To test the proposed approach, a patient data set provided by Baskent University, Department of Radiology is used. The validation of the test results is performed by an expert radiologist According to the results obtained, the presented nodule progression trace system is found promising
Comparable survival using a CMV-matched or a mismatched donor for CMV+ patients undergoing T-replete haplo-HSCT with PT-Cy for acute leukemia: A study of behalf of the infectious diseases and acute leukemia working parties of the EBMT
The role of donor CMV serostatus in the setting of non T-cell depleted haplo-HSCT with post-transplant cyclophosphamide (PT-Cy) has not been specifically addressed so far. Here we analyzed the impact of the donor CMV serological status on the outcome of 983 CMV seropositive (CMV+), acute leukemia patients receiving a first, non T-cell depleted haplo-HSCT registered in the EBMT database. The 1-year NRM was 21.3% (95% CI: 18.4-24.8) and 18.8% (95% CI: 13.8-25.5) in the CMV D+/R+ and D-/R+ pairs, respectively (p = 0.40). Similarly, 1-year OS was 55.1% (95% CI: 50.1-58.0) and 55.7% (95% CI: 48.0-62.8) in the same groups (p = 0.50). The other main outcomes were comparable. No difference in NRM nor OS was observed after stratification for the intensity of conditioning and multivariate anaysis confirmed the lack of significant association with NRM or OS. In conclusion, the choice of a CMV-seronegative donor did not impair early survival of CMV-seropositive patients with acute leukemia after a first, non T-cell depleted haploidentical HSCT and PT-Cy among this series of 983 consecutive patients. Future research may focus on the assessment of the hierarchy of all the donor variables. © 2017 Macmillan Publishers Ltd., part of Springer Nature
Broncholithiasis presenting with lithoptysis
Broncholithiasis is defined as calcified or ossified material in the bronchial lumen. The most frequently seen symptoms are a non-productive cough and hemoptysis. Occasionally, broncholytic expectoration can be seen, which is known as lithoptysis. As this is not a common finding, a case diagnosed with lithoptysis is presented here
Serum glial cell line-derived neurotrophic factor levels and impulsivity in heroin addiction: a cross-sectional, case-control study of 129 heroin addicts
OBJECTIVE: Glial cell line-derived neurotrophic factor (GDNF), being a protective of dopaminergic neurons, is reported to modulate addictive behaviours and have a role as a negative regulator for biochemical and behavioural adaptations to drug abuse. We aimed to reveal impulsivity and serum GDNF levels in patients with heroin addiction and investigate their relationships in order to contribute to the understanding of behavioural aspects and biological mechanisms in heroin addiction via this study.
METHODS: This study was performed at the Department of Psychiatry of Ankara Numune Training and Research Hospital, Turkey. We recruited 129 heroin-dependent patients and 90 age, sex, and smoking-matched healthy controls with no major psychopathology. Barratt Impulsivity Scale-11, Hospital Anxiety and Depression Scale (HADS) and sociodemographic data form were applied to all participants. Laboratory analysis for serum GDNF levels was performed for each participant's blood sample.
RESULTS: Total impulsivity scores and scores of Attentional Impulsivity, Motor Impulsivity, and Unplanned Impulsivity subscales were all higher in heroin addicts compared to the controls. Heroin addicts had also lower serum GDNF levels and lower GDNF levels were associated with high impulsivity and high HADS scores in heroin addicts.
CONCLUSION: Decrement in GDNF levels in heroin addiction seems as to be an important data which could be associated with impulsivity, anxiety, and depressive symptoms. GDNF could find a prominent place among the target molecules in the treatment of heroin addiction
