    Esketamine has emerged as a promising treatment option, particularly noted for its effectiveness in reducing postpartum depression, alleviating pain, and promoting faster recovery. Despite its therapeutic benefits, recent research has raised concerns regarding potential adverse effects associated with high doses of esketamine in patients. Specifically, there is growing interest in understanding the potential risks linked to the administration of esketamine during cesarean delivery, given its increasing use in obstetric anesthesia. This comprehensive review aims to investigate the potential associations between the use of esketamine during cesarean delivery and the subsequent risk of related diseases or physiological disorders. The goal is to provide clinicians and researchers with valuable insights into the safety profile of esketamine when administered during cesarean delivery, thereby informing clinical practice and guiding future research directions

    Efficacy of Current Sanitation Recommendations at Reducing Rates of Toxic Shock Syndrome in Menstruating Women Using a Menstrual Cup

    Introduction: Menstrual toxic shock syndrome (mTSS) is a rare but serious complication associated with the prolonged use of intra-vaginal menstrual products. Significant research has been aimed at reducing incidence of mTSS that has resulted in proven sanitary practices. Reusable menstrual products have become increasingly appealing to consumers due to cost savings as well as the feel-good notion that comes with reducing waste. It is unclear if hygiene recommendations for menstrual cups are as efficacious at preventing the incidence of menstrual toxic shock syndrome when compared to that of tampons. Methods: A database search was conducted using PubMed, Medline, and Embase to review literature published after 1978. Specifications outlined in existing publications relating to menstrual cups and mTSS that have been found to reduce incidence of mTSS were chosen and analyzed for consistency in instructions for use of the top 5 most popular menstrual cup brands. Results: Of the five brands analyzed according to recommendations aimed at reducing mTSS in women using menstrual cups, only one brand met 3/5 recommendations. Discussion: MC hygiene protocols should be updated based on the suggestions presented in the reviewed literature in order to further prevent incidence of mTSS. Popular MC brands fall short of these recommendations most frequently in sizing, insertional instruction, and cleansing. Conclusion: The CDC guidelines lack clear guidance on hygiene measures for menstrual cups and need to be updated with specific information gleaned from research. Further studies should continue to examine the relationship between MCs, tampons, and mTSS

    Evaluating C-Reactive Protein Associated Risk of Cardiovascular Disease in Patients with Autism Spectrum Disorder

    This study examines the relationship between C-reactive protein (CRP) levels and cardiovascular disease (CVD) risk in 293 patients with Autism Spectrum Disorder (ASD) from the Rowan-Virtua Regional Integrated Special Needs Center (RISN Center). Baseline CRP values were gathered from EMR and evaluated for CVD risk. ASD patients have increased prevalence of CVD risk factors, and the literature demonstrates a strong relationship between CRP and CVD risk in this population. Patients were stratified by severity of ASD to investigate potential differences. Severe limitations in EMR data and overall sample size limited the generalizability and significance of this study. Larger scale studies with ample serum CRP values are needed to adequately assess CVD risk in the RISN Center’s ASD patient population. Regular monitoring and evaluation of CRP levels in ASD patients could lead to better management of cardiovascular health in patients with ASD

    Designing Dual-Responsive Drug Delivery Systems: The Role of Phase Change Materials and Metal-Organic Frameworks.

    Stimuli-responsive drug delivery systems (DDSs) offer precise control over drug release, enhancing therapeutic efficacy and minimizing side effects. This review focuses on DDSs that leverage the unique capabilities of phase change materials (PCMs) and metal-organic frameworks (MOFs) to achieve controlled drug release in response to pH and temperature changes. Specifically, this review highlights the use of a combination of lauric and stearic acids as PCMs that melt slightly above body temperature, providing a thermally responsive mechanism for drug release. Additionally, this review delves into the properties of zeolitic imidazolate framework-8 (ZIF-8), a stable MOF under physiological conditions that decomposes in acidic environments, thus offering pH-sensitive drug release capabilities. The integration of these materials enables the fabrication of complex structures that encapsulate drugs within ZIF-8 or are enveloped by PCM layers, ensuring that drug release is tightly controlled by either temperature or pH levels, or both. This review provides comprehensive insights into the core design principles, material selections, and potential biomedical applications of dual-stimuli responsive DDSs, highlighting the future directions and challenges in this innovative field

    Identifying the Relationship Between Post-Concussion Mental Health and Athletic Status: Is It the Same for All Demographics?

    Purpose: The purpose of this study is to investigate whether athletic status was linked to fewer symptoms of anxiety and depression post-concussion, as well as to explore whether the impact of athletic status on mental health varied by demographics (i.e. age, sex, race, insurance status). The results of this study may assist in identifying at-risk individuals early to help prioritize those who may benefit from early referral to mental health resources. Introduction: Mental health concerns post-concussion are relatively common, with roughly 1 in 3 individuals experiencing these symptoms. It has been previously shown that youth who participate in sports, boys, younger children, white individuals and those with private insurance tend to report fewer mental health symptoms. Methods: A multivariate analysis was run utilizing RCADS scores from patients (and their caregivers) who were seen in the Minds Matter Concussion Program for specialized behavioral health care. This analysis utilized athletic status and demographics (i.e. age, sex, race, insurance status) as independent variables predicting RCADS Anxiety and Depression scores. Results: Data was analyzed and presented in both table and figure form to present the relationships studied. Discussion: In most instances, athletic status was linked to lower mental health symptoms and did not vary by demographic variables. A notable exception was the impact on patient-reported anxiety, which suggests that playing a sport is linked to lower anxiety only for White patients. This relationship was not seen in patients of color. Conclusion: Based off previous research, lower mental health symptoms across demographic variables in athletes was an anticipated finding. However, the finding that playing a sport being linked to only lower anxiety for White patients begs the question about what about sports may be protective for White youth but not youth of color. Future research should investigate the mechanism through which sports may be protective, as well as the unique needs of patients of color to best support them

    Moving On Up: A Case Study of HIV and COVID-Induced Guillain-Barre Syndrome

    COVID and HIV are both common diseases, with COVID affecting an estimated 775 million and HIV affecting 39 million people worldwide. Guillain-Barre is a rare but feared complication of immune stimulation in which the body generates autoantibodies against gangliosides, found in the nodes of Ranvier, in the myelin, and in the ganglia; causing ascending paralysis. A mid-30s man with HIV maintained on Genvoya presented to the Jefferson Health New Jersey Early Intervention Program clinic with fatigue, fever, cough, and congestion and tested positive for SARS-CoV-2 on the following day. Two weeks later, he presented to the emergency department complaining of numbness, paresthesias, weakness, and abnormal gait of the left lower extremity. Imaging demonstrated vertebral artery stenosis and foraminal stenosis, but no acute cause of nerve impingement. Other meningitis and encephalitis testing was negative, but CSF contained red blood cells. Numbness continued to ascend to the torso and ganglioside testing was ordered while a 5-day course of intravenous immunoglobulin was started. His symptoms began improving one week after finishing treatment and EMG was normal 9 weeks later. At his last visit, he endorsed residual altered sensation in the perirectal area and left thigh, but motor function was intact. Close support from his husband and community was vital in early identification of key symptoms and entry to treatment. Guillain-Barre should be considered in patients with fulminant symptoms even in the setting of HIV- and HAART-induced peripheral neuropathy or spinal stenosis

    Physical Medicine & Rehabilitation Referrals in Cancer Patients

    Physical Medicine and Rehabilitation (PM&R) can address functional limitations in various conditions, including cancer. Cancer rehabilitation aims to address functional impairments resulting from both the disease itself and its treatments, striving to optimize physical, social, psychological, and vocational functioning. Existing literature indicates that PM&R services significantly benefit cancer patients by improving their overall quality of life. Studies have found that optimal utilization of PM&R services can increase the Functional Independence Measure (FIM) in cancer patients. Current National Comprehensive Cancer Network and American Cancer Society guidelines emphasize the importance of incorporating PM&R services into oncology care to enhance the function and quality of life of cancer survivors. A literature review revealed that more than 60% of cancer patients experience functional morbidity, but only 2.9% are referred to PM&R services. This underutilization of PM&R services is found in other surveys and presents as a multifactorial problem that needs to be addressed to improve patient outcomes and quality of life. Factors that contribute to underutilization of these services include lack of patient or physician awareness of the benefits of PM&R, limited resources, patient adherence, patient prognosis, and socioeconomic status. A survey was created to further understand referral patterns to PM&R services among oncologists. The results from the questionnaire will help identify effective interventions needed within the community to better address the issue

    INCORPORATING DIVERSE MULTIMODAL TEXTS WITHIN A SEVENTH GRADE LANGUAGE ARTS CLASSROOM TO INCREASE COMPREHENSION ACCESSIBILITY AND IMPROVE ENGAGEMENT

    This thesis examines if diverse, multimodal texts have an effect upon student engagement while increasing comprehension for students with varied learning styles. Applicable theories that gave direction to this research study were Multiliteracies Pedagogy, Culturally Responsive Teaching, Funds of Knowledge, and Constructivism. Throughout the unit’s duration, students were instructed with various types of multimodal texts that included song, video, graphic novels, and performance while also working with modern authentic, diverse traditional texts. Students were also given a choice of mode in how to respond and demonstrate understanding. The qualitative research study analyzed data sources such as student surveys; my teacher journal with detailed notes taken during lesson administration as well as quotes written from student discussions; and student artifacts such as videos of student skit performances, student journal entries, and student graphic novel spreads. Triangulation was employed to determine patterns and clear themes that were drawn from analyzed data. Patterns pointed to the importance of multimodal texts in addressing various learning styles, using diverse texts to spark conversations and student connections, and the important value of the graphic novel in the literacy classroom. Conclusions of the study address benefits perceived as well as limitations and suggestions for future research

    CHEMISTRY AND PROCESSING OF CARBON/CARBON COMPOSITES FROM FURAN-BASED POLYBENZOXAZINE RESINS

    Carbon/carbon (C/C) composites are valuable for applications requiring structural integrity at high temperatures. However, their widespread use is limited by high manufacturing costs. Monofunctional furan-based benzoxazine resins were identified as promising candidates for enhancing fabrication efficiency due to their low viscosity, high char yield, and high carbon density. A benzoxazine monomer synthesized from furfurylamine, formaldehyde, and benzyl alcohol (Bz-FA-H) exhibits a crosslinking reaction between the furan group and the hydroxymethyl para substituent on the phenol, not found with other monomers. An investigation using blends of monomers containing Bz-FA-H at various concentrations showed that increasing crosslink density resulted in increased bulk carbon density upon carbonization. In fiber-reinforced composites, volume shrinkage of the matrix during carbonization creates an interconnected crack structure that enhances densification efficiency in polymer infiltration and pyrolysis (PIP) processing. PIP studies were conducted using composites prepared by resin transfer molding (RTM). Characterization of the void content and its distribution within pyrolyzed samples in conjunction with modeling of the re-infiltration flow was used to understand the densification process better and for process optimization. Based on the findings, a layer-by-layer (LBL) approach was developed for the additive manufacturing of C/C composites, demonstrating the potential for the continuous fabrication of these systems and resulting cost savings
