    How to Respond to Racist Patients: Recommendations from a Literature Review

    Introduction: Dealing with racist patients is not uncommon, and these interactions can sever the therapeutic alliance, as well as leave providers feeling isolated, dehumanized, and ashamed. Investigation of published recommendations for handling these situations can give victims, peers, and institutions the tools necessary to prepare, protect, and support providers through these challenging encounters. Methods: This paper is a literature review. For inclusion in this review, studies must have met the criteria of providing recommendations for healthcare providers or institutions on how to deal with racist patients. Excluded articles did not include recommendations on how to handle such situations or did not otherwise meet inclusion criteria. PubMed Medline was searched in January 2022 using a combination of the following keywords and associated MeSH terms: “Racism” AND “Physician-Patient relations”. The resulted articles then underwent forward and backward citation searching. A total of 44 articles were included after evaluation of 272 articles via this process. Results: For the affected individual, recommended responses include addressing the comment firmly and directly in real time, setting boundaries and behavior expectation, reporting the incident to supervisors and to the hospital, and seeking support from peers and/or professionals. At the peer level, core practices are supporting the victim, addressing the patient if necessary, debriefing with the victim and team, and checking-in with the victim in the following days. At the institution level, core practices are enacting reporting system and tracking incidents, developing specific policies and procedures about biased patients, and training staff with focused antibias and antidiscrimination sessions. Discussion: There are steps to be taken at every level to create a supportive and inclusive practice that protects providers against racist patients. One limitation of this study was that it was not a systematic review so there may be other recommendations published that are not reflected here

    Laboratory Evaluation of Warm Mix Additives at Lower than Traditional Compaction Temperatures

    The study aims to evaluate the impact of warm mix additives (WMA) on the rheological and performance properties of asphalt binders and mixtures. For this purpose, PG 76-28 and PG 58-28 were obtained with one organic (Sasobit Redux) and three chemical WMA (Rediset, Evotherm, and Zycotherm). All WMA-modified asphalt binders were aged at three different aging levels. Laboratory tests such as performance grade (PG), critical temperature differential (ΔTc), asphalt binder cracking device (ABCD), linear amplitude sweep test (LAS), and Fourier transform infrared spectroscopy (FTIR) were performed. In addition to the binder testing plan, this study also evaluated the impact of WMA type, binder grade, and compaction temperature on the volumetrics and performance of the mixtures when compacted at lower than traditional temperatures (260℉, 220℉, and 180℉). The cracking, rutting, and moisture damage resistance of all mixtures were evaluated using the Indirect Tensile Cracking Test (IDEAL-CT), Asphalt Pavement Analyzer (APA), and tensile strength ratio (TSR), respectively. Laboratory testing showed WMA\u27s feasibility to improve the binder and mixture properties performance compared to the control mixture and binder. Finally, All mixtures were evaluated using the multi-factor analysis of variance (ANOVA), and ranking was developed based on the mean difference of each mixture

    The ability of glia to tightly regulate neuronal health and homeostasis in the CNS is conserved across species. Yet, despite the ability to degrade protein aggregates, glia are vulnerable to the accumulation of neurotoxic amyloid aggregates during neurodegenerative disease progress, and even exacerbate their spread. A developing narrative highlights glia as a double-edged sword in neurodegenerative diseases: initially capable of dynamically responding to amyloid aggregate-ladened dying neurons but also capable of inducing chronic inflammation and creating seeding-competent amyloid oligomers. Thus, uncovering the mechanisms that allow glia to control aggregate deposition while preventing the neurotoxic effects and seed generation is vital for the development of disease-modifying treatments. Here, we report that mutant huntingtin (mHTT) impairs glial clearance of axonal debris in a Drosophila model of Huntington’s disease (HD). Neuronal mHTT buildup impaired engulfment and clearance of injured axons while also accumulating in late phagosomes and lysosomes in glia. We identified Rab10 as a novel modifier of neuronal mHTT aggregate seeding in the glial cytoplasm. Finally, the prion-like propagation of mHTT between neurons and glia is dependent on the phagocytic receptor MEGF10 in a mammalian model of HD. Our findings suggest that dysfunctional processing by the glial phagolysosomal system is a key contributor to the spread of amyloid aggregates during neurodegenerative disease progression

    Utilizing primary human airway mucociliary tissue cultures to model ramifications of chronic E-cigarette usage.

    Electronic cigarettes are battery powered devices that use a vape-liquid to produce a vapor that is inhaled. A consequence of the rise in e-cigarette usage was the 2019 emergence of a vaping-induced respiratory disease denoted as \u27e-cigarette or vaping use-associated lung injury\u27 (EVALI). One of the suspected causes of EVALI is Vitamin E Acetate (VEA), which was found to be a diluent in certain illicit vape-pens, whereas nicotine is commonly diluted in equal parts propylene glycol and vegetable glycerin (PG:VG). The prevalent use of e-cigarettes and the emergence of a novel illness has made understanding how e-cigarette vapors affect our respiratory tissues a public health concern. We have designed and produced a simple device that can operate e-cigarettes and deliver the vapor to a chamber containing a standard cell culture multi-well plate. Here we utilize our device to model the response of human airway mucociliary tissue after chronic exposure to vapors produced from either PG:VG or VEA. We note several differences between how PG:VG and VEA vapors interact with and alter airway tissue cultures and suggest potential mechanisms for how VEA-vapors can exacerbate EVALI symptoms. Our device combined with primary human airway tissue cultures make an economical and compact model system that allows for animal-free investigations into the acute and chronic consequences of e-cigarette vapors on primary respiratory cells

    TOWARDS A PRACTICAL METHOD FOR MONITORING KINETIC PROCESSES IN POLYMERS WITH LOW-FREQUENCY RAMAN SPECTROSCOPY

    Unlike liquids and crystalline solids, glassy materials exist in a constant state of structural nonequilibrium. Therefore, a comprehensive understanding of material kinetics is critical for understanding the structure-property-processing relationships of polymeric materials. Amorphous materials universally display low-frequency Raman features related to the phonon density of states resulting in a broad disorder band for Raman shifts below 100 cm-1, which is related to the conformational entropy and the modulus. This disorder band is dominated by the Boson peak, a feature due to phonon scattering because of disorder and can be related to the transverse sound velocity of the material, and a well-defined shoulder due to a van Hove singularity. Quasi-elastic Rayleigh scattering also contributes to the signal, particularly in liquid-phase materials. We have demonstrated that we can measure both glass transition and polymerization kinetics by normalizing the disorder band to its shoulder and monitoring its evolution as a function of time and temperature. We also demonstrate a relationship between the chemical and structural kinetics, which appears to relate to the softness of the material, which we verify via rheological analysis. Low-frequency Raman spectroscopy is, therefore, a promising technique for thermo-structural analysis of polymers. Not only is it chemically agnostic and contactless, but it requires neither intensity calibration nor multivariate analysis

    The Positive Impact of Gender Affirming Care on Mental Health Outcomes Among Transgender & Nonbinary LGBTQ+ Adolescents and Young Adults, A Review of Global Literature

    LGBTQ+ adolescents and young adults (AYA) continue to face an increased burden of adverse mental health outcomes, including major depressive disorder, generalized anxiety disorder, and suicidal ideation. Among this group, transgender and gender nonbinary (TNB) individuals experience a disproportionate burden of these outcomes. Citing the positive impact of gender-affirming care in TNB adults, this literature review aims to examine the role of gender-affirming care in the context of mental health outcomes, specifically in the TNB AYA population. A review of global literature was conducted by searching four databases (Pubmed, CINAHL, Embase, PsychInfo). Eligibility criteria include studies that reported mental health outcomes among persons receiving gender-affirming therapies, studies conducted among TNB AYA of ages 18 and/or younger, and data collected no earlier than 2013. All of the studies were independently screened and synthesized following PRISMA guidelines. Of the 229 initial search items, 18 met the inclusion criteria for this literature review. Of the 18 included items, 17 cited a statistically significant decrease in the prevalence of depressive symptoms and suicidality associated with gender-affirming hormone replacement (GAH) and gender-affirming surgery (GAS) therapies, of which depression symptoms were quantified using the PHQ-9 scale. Of important note, depressive symptoms, and suicidality varied between the different types of GAS, such as double mastectomy, vaginoplasty, and phalloplasty. The results of this literature review highlight the therapeutic advantage of gender-affirming care, GAH or GAS, in the context of alleviating adverse mental health outcomes in the TNB AYA population. The findings of this review were limited by the lack of disaggregated data by race, socioeconomic status, and access to gender-affirming therapies. Further research avenues include qualitative research aimed at capturing the lived experiences of TNB AYA receiving gender-affirming care and specific quantitative analysis of the individual gender-affirming surgeries on their respective roles in mediating adverse mental health outcomes

    Use of Point-of-Care Ultrasound in the Diagnosis of Postpartum Cardiomyopathy

    Postpartum cardiomyopathy (PPCM), also known as peripartum cardiomyopathy is a rare form of heart failure (HF) that occurs in the late stages of pregnancy or in the early postpartum period. For it to be classified as PPCM, it must occur in the absence of another identifiable cause for HF and have left ventricular (LV) systolic dysfunction with an LV ejection fraction (LVEF) less than 45%. Here we present the case of a 46-year-old female G3P2 presented 5 days postpartum cesarean section delivery in acute respiratory distress where point of care ultrasound was used for assessment of the lungs to visualize B-lines and significant reduction in ejection fracture of the heart, allowed us to make the decision to pursue management of heart failure and have successful outcomes for this patient

    What is the Efficacy of the Newly Developed HPV Vaccine, Cecolin, in Providing Protection Against HPV-Induced Cervical Cancers in Females Aged 9-14 Years Old

    In low to middle income countries, there\u27s a rise in invasive cervical squamous cell carcinoma. Affordability emerges as a crucial factor influencing this trend. This research is based on the development of a new HPV vaccine made in China called Ceolin priced at $47 and its efficacy to be incorporated into the WHO vaccination program. A peer review was conducted based on an ongoing randomized phase 3 clinical trial of a 2 dose regimen bivalent Cecolin vaccine (types 16,18) . The study includes 1025 females aged 9-14 years old split amongst 5 groups. The control group received Gardasil at the following intervals: 0 and 6 months. Experimental groups received Cecolin at the following intervals: 6-, 12-, 24-month or one dose of Gardasil followed by one dose of Cecolin at month 24. Outcome measures assessed Anti-HPV16 and 18 antibodies. Data for the 6 month dosing group showed that 96% of participants had antibody positive development for both HPV strains following Cecolin administration and produced a comparable immune response to Gardasil. IgG levels for HPV 16 of Cecolin and Gardasil were 1,567 and 1,444 IU/mL, respectively, and 424 and 336 IU/mL, respectively, for HPV-18. The potential incorporation of Cecolin into the WHO vaccination program can expand vaccine options, leading to improvements in accessibility when offering cost effective options to consumers as well as decreasing the incidence of cervical cancer in underdeveloped countries. HPV vaccinations align with osteopathic medicine principles of preventative care, preventing health disparities and patient education
