Tree-water relations in pure and mixed forests: Drivers of root water uptake depth, stem water and growth dynamics
Angesichts des zunehmenden Drucks, dem Wälder durch den Klimawandel und steigende Wasserlimitierung ausgesetzt sind, ist es für die Vorhersage der Widerstandsfähigkeit von Ökosystemen und für eine nachhaltige Waldbewirtschaftung von entscheidender Bedeutung zu verstehen, wie Baumarten an Wasser gelangen und es nutzen. In dieser Arbeit werden die Mechanismen untersucht, die den Beziehungen zwischen Bäumen und Wasser – dem Baumwasserhaushalt – zugrunde liegen. Der Schwerpunkt liegt dabei auf den artspezifischen funktionalen Merkmalen, ihren Interaktionen in Mischbeständen und der Rolle der Standortbedingungen - und das auf verschiedenen räumlichen und zeitlichen Skalen.
Mischwälder gelten als eine Schlüsselstrategie zur Förderung widerstandsfähiger und multifunktionaler Waldökosysteme. In Mitteleuropa hat die Rotbuche (Fagus sylvatica L.) - die dominierende Laubbaumart, die üblicherweise in natürlichen Monokulturen wächst - in den letzten Jahren Anzeichen eines trockenheitsbedingten Vitalitätsrückgangs gezeigt. Bei der Fichte (Picea abies (L.) H. Karst) - der bisher ökonomisch wichtigsten Baumart in Europa - haben Trockenheit, Borkenkäferbefall und Stürme zu massiven Verlusten geführt. Infolgedessen hat die Douglasie (Pseudotsuga menziesii (Mirb.) Franco), ein schnellwüchsiger und vermutlich trockenheitsresistenterer Nadelbaum aus dem westlichen Nordamerika, in der europäischen Forstwirtschaft an Beliebtheit gewonnen. Die Einführung von Nadelbäumen in Mischwäldern mit Rotbuchen könnte eine vielversprechende Strategie sein, um ökologische und ökonomische Funktionen in Einklang zu bringen und gleichzeitig den Übergang zu widerstandsfähigeren Waldsystemen zu schaffen. Die Auswirkungen solcher Mischungen auf den Baumwasserhaushalt - eine wesentliche Komponente der Waldfunktionen - sind jedoch kaum verstanden.
In dieser Arbeit werden vier Schlüsselmerkmale und -prozesse untersucht: die Wasseraufnahmetiefe der Wurzeln (Kapitel 1), die Aufnahme und der Transport von Wasser und Stickstoff aus dem Unterboden (Kapitel 2), die tägliche Rehydrierung des Stamms (Kapitel 3) sowie die Wasser- und Wachstumsdynamiken im Stamm bei fortschreitender Trockenheit (Kapitel 4), wobei Buchen-Nadelholz-Mischungen und Reinbestände verglichen werden.
In Kapitel 1 zeigte die Analyse stabiler Isotope, dass Buche und Douglasie tieferes Wasser aufnehmen als Fichte – eine Eigenschaft, die bei Trockenheit als vorteilhaft gilt. Die Buche nahm in Mischungen mehr Wasser aus tieferen Bodenschichten auf, während die Fichte in Mischung mit der Buche Wasser aus flacheren Bodenschichten aufnahm, was ihre Trockenheitsanfälligkeit vergrößerte. Im Gegensatz dazu zeigte die Douglasie keinen Unterschied zwischen Reinbestand und Mischung. Die Wasseraufnahmetiefe der Wurzeln variierte auch mit der Bodenart, wobei die tiefste Aufnahme auf gut durchlässigen, sandigen Böden und die geringste auf im Unterboden leicht staunassen Böden zu verzeichnen war.
Während die Wasseraufnahme aus tieferen Bodenschichten als wichtiges Merkmal bei Trockenheit wahrgenommen wird, blieb tiefe Nährstoffaufnahme bisher weitgehend unerforscht. In Kapitel 2 wurden die Wechselwirkungen zwischen Wasser- und Nährstoffaufnahme untersucht, indem gleichzeitig markiertes Wasser und Stickstoff bei ihrer Aufnahme durch die Wurzeln im Unterboden verfolgt wurden. Die Douglasie nahm auf sandigen Böden mehr Unterbodenwasser auf als auf lehmigen Böden, und sie nahm mehr Unterbodenwasser auf als die Buche. Bei allen Bäumen wurde Wasser schneller transportiert als Stickstoff, ein quantitativ wichtiges Ergebnis unter Geländebedingungen. Die Ergebnisse unterstreichen die wichtige Rolle unterirdischer Prozesse bei der Gestaltung des Wasser- und Nährstoffkreislaufs der Bäume.
In Kapitel 3 analysiere ich mit hochauflösenden Dendrometern die tägliche Stammrehydrierung als Indikator für Wasserstress. Die Nachbarschaft (vom Reinbestand bis zur Mischung) beeinflusste die Rehydrierung der Stämme bei Trockenheit erheblich, nicht jedoch bei hoher Bodenfeuchtigkeit. Buche rehydrierte effektiver, wenn sie mit Douglasie gemischt war, aber nicht mit Fichte. Die Douglasie profitierte von der Buche, solange sie die Nachbarschaft nicht dominierte, während sich die Beimischung von Buche negativ auf die Rehydrierung der Fichte auswirkte. Die Buche wies durchweg eine höhere Rehydrierungskapazität auf als die Nadelbäume, selbst bei steigendem atmosphärischen Verdunstungsanspruch.
In Kapitel 4 wurden täglicher Saftfluss, Wassergehalt und die Rehydrierung des Stamms, und die Wachstumsreaktionen während der Dürre im Jahr 2022 verfolgt. Alle Variablen wurden sukzessive verringert, wobei die kritischen Reduzierungen (70-90 %) bei beiden Arten innerhalb eines ähnlichen Bodenfeuchtebereichs auftraten. Bei der Buche blieben Wachstum und Saftfluss jedoch länger erhalten, während die Douglasie sie früher einschränkte. Der reine Douglasienbestand erschöpfte die Bodenfeuchtigkeitsreserven schneller als die reine Buche und die Buchen-Douglasien-Mischung, was dazu führte, dass die Bäume dort kritische physiologische Schwellenwerte früher erreichten. Interessanterweise wiesen die Unterschiede zwischen dem Wassergehalt der Stämme und der Rehydrierung des gesamten Stammes auf eine Verschiebung der Prioritäten bei der Rehydrierung des Gewebes bei Trockenheit hin.
Zusammenfassend zeigt diese Arbeit, dass der Wasserhaushalt von Bäumen stark von der Baumart, der Artidentität benachbarter Bäume, den Standortbedingungen und der Wasserverfügbarkeit abhängen. Ein tieferes Verständnis des Zusammenspiels dieser Faktoren kann Waldbewirtschaftungsstrategien unterstützen, die darauf abzielen, widerstandsfähige, multifunktionale Wälder zu fördern. Hinsichtlich des Wasserhaushalts erweisen sich Mischungen aus Rotbuche und Douglasie als vielversprechende Option.As forests face increasing pressure from climate change and growing water limitation, understanding how tree species access and use water has become crucial for predicting ecosystem resilience and guiding sustainable forest management. This thesis investigates the mechanisms underlying tree-water relations – focusing on species-specific functional traits, their interactions in mixed stands, and the role of site conditions – across varying spatial and temporal scales.
Mixed-species forests are considered a key strategy to foster resilient and multifunctional forest ecosystems. In Central Europe, European beech (Fagus sylvatica L., “beech”) – the dominant broadleaf species, commonly growing in natural monocultures – has shown signs of drought-induced vitality decline in recent years. More severely, Norway spruce (Picea abies (L.) H. Karst, “spruce”) – so far the most important timber species in Europe – has suffered large-scale diebacks due to drought, bark beetle outbreaks, and storms. As a result, Douglas fir (Pseudotsuga menziesii (Mirb.) Franco), a fast-growing and presumably more drought-resistant conifer native to western North America, has gained popularity in European forestry. Introducing conifers into mixed forests with European beech may offer a promising strategy to balance ecological and economic functions, while transitioning toward more resilient forest systems. However, the impact of such mixtures on tree-water relations – an essential component of forest functioning – remain poorly understood.
This thesis investigates four key traits and processes: root water uptake depth (Chapter 1), subsoil uptake and translocation of water and nitrogen (Chapter 2), daily stem rehydration (Chapter 3), and stem water and growth dynamics under progressive drought (Chapter 4), comparing beech-conifer mixtures and pure stands.
In Chapter 1, stable isotope analysis revealed that beech and Douglas fir accessed deeper water than Norway spruce, a trait considered beneficial under drought. Beech increased its water uptake depth in mixtures, whereas spruce shifted to shallower uptake in mixture with beech, aggravating its drought exposure. In contrast, Douglas fir showed no difference in mixture. Root water uptake depth also varied with soil type, with the deepest uptake on well-drained, sandy soils and the shallowest on stagnic soils.
While deep water uptake is recognized as an important trait, deep nutrient uptake remains largely unexplored. Chapter 2 focused on the interactions of water and nutrient uptake by simultaneously tracing labeled water and nitrogen from the subsoil. Douglas fir showed higher subsoil uptake on sandy soils than on loamy soils, and greater subsoil uptake than beech. Across all trees, water was translocated faster than nitrogen, an important finding in field conditions. The findings underscore the powerful role of belowground processes in shaping tree water and nutrient cycling.
In Chapter 3, I used high-resolution dendrometers to analyze daily stem rehydration as a water stress indicator. Neighborhood identity significantly influenced stem rehydration under drought, but not at high soil moisture. Beech rehydrated more effectively when mixed with Douglas fir, but not with spruce. Douglas fir benefited when beech was not dominating the neighborhood, whereas beech admixture had a negative effect on spruce rehydration. Beech consistently showed higher rehydration capacity than conifers, even under increasing atmospheric demand.
Chapter 4 tracked daily sap flow, stem water content, stem rehydration, and growth responses during the 2022 drought. All variables were successively downregulated, with critical reductions (70-90%) occurring within a similar soil moisture range for both species. However, beech maintained growth and sap flow longer, while Douglas fir showed earlier declines. The pure Douglas fir stand depleted soil moisture reserves faster than pure beech and the beech-Douglas fir mixture, leading the trees to reach physiological thresholds sooner. Interestingly, differences between stem water content and whole-stem rehydration pointed to shifting tissue rehydration priorities under drought.
In summary, this work shows that tree-water relations are strongly shaped by species identity, tree neighborhood, site conditions, and drought. A deeper understanding of the interplay among these drivers can support forest management strategies aimed at fostering resilient, multifunctional forests. Impact of transfusion strategy in the administration of red blood cell concentrates in upper gastrointestinal bleeding on clinical outcome: A restrospective data analysis
Die Transfusion von Erythrozyten-Konzentraten ist wesentlicher Bestandteil der Therapie bei akuten oberen gastrointestinalen Blutungen, welche zu den häufigsten gastrointestinalen Notfällen zählen. Ziel dieser retrospektiven und monozentrischen Arbeit war es, herauszufinden, ob eher ein liberales oder eher ein restriktives Transfusionsmanagement das bessere Outcome für die Patienten erbringt. Dies sollte auch für verschiedene Vorerkrankungsgruppen untersucht werden.
Grundlage dieser Arbeit waren die klinischen Daten von 265 volljährigen Patienten mit akuter oberer gastrointestinaler Blutung, welche in den Jahren 2008 - 2018 im Rahmen der Behandlung in der Universitätsmedizin Göttingen sowohl eine Bildgebung als auch Erythrozyten-Konzentrate erhielten. Die Einteilung in die Therapiegruppen erfolgte anhand des Hämoglobin-Werts zum Zeitpunkt der ersten Transfusion in diesem Blutungsereignis (restriktive Gruppe < 7,0 g/dl, n = 68 und liberale Gruppe 7,0g/dl, n = 197).
Bei den Patienten wurden sechs große Gruppen von Vorerkrankungen erhoben, nämlich kardiale, hepatische, pneumologische, nephrotische, maligne und neurologische Vorerkrankungen sowie die Feststellung einer Erkrankung an Diabetes Mellitus. Die meisten Patienten wiesen mehrere Vorerkrankungen aus verschiedenen Vorerkrankungsgruppen auf. So hatten 215 Patienten (81,1%) Vorerkrankungen aus mindestens drei der verschiedenen Hauptgruppen. Unterschiede zwischen den beiden Therapiegruppen hinsichtlich der Häufigkeit in den einzelnen Vorerkrankungsgruppen waren in keinem Fall signifikant und es gab auch keine eindeutige Tendenz über alle Vorerkrankungen hinweg betrachtet. Bei zwei genauer untersuchten Vorerkrankungen zeigte sich nur bei der arteriellen Hypertonie mit 11,1%-Punkten ein deutlicher Unterschied zwischen den beiden Gruppen (restriktive Gruppe 41,2% versus liberale Gruppe 52,3%; p = 0,11), nicht hingegen bei der Leberzirrhose mit nur 1%-Punkt Unterschied.
Die beiden Therapiegruppen wurden hinsichtlich verschiedener Parameter und in Bezug auf die vier definierten Outcome-Punkte (Studienendpunkte) Mortalität, Reblutungsrate, Krankenhaus-Aufenthaltsdauer und Anzahl der benötigten Erythrozyten-Konzentrate durch verschiedene statistische Auswertungen miteinander verglichen.
Beim primären Outcome-Punkt zeigte sich eine signifikant geringere Mortalität der Patienten in der restriktiven gegenüber der liberalen Therapiegruppe (7,35% versus 18,27%; p = 0,036). Die Tendenz der längeren Überlebenszeiten in der restriktiven Therapiegruppe zeigte sich auch in den Einzelauswertungen für die Vorerkrankungsgruppen. Die Signifikanzen schwankten zwar je nach Vorerkrankung leicht, lagen jedoch immer deutlich unter dem kritischen Wert von p = 0,05. Es ist allerdings schwierig zu beurteilen, welche Vorerkrankungen den größten Einfluss auf die höhere Mortalität bei einer liberalen Therapiestrategie haben, da die meisten Patienten mehrere Vorerkrankungen hatten. Eine Adjustierung für die entsprechenden Confounder war aufgrund der Stichprobengröße nicht möglich.
Insgesamt lag die Reblutungsrate dieser Arbeit mit 3,4% unter den erwarteten Werten aus der Literatur. Es bestand hier kein signifikanter Unterschied zwischen den beiden Therapiegruppen, wenngleich mehr Patienten in der liberalen Therapiegruppe eine Reblutung bekamen. Diese Tendenz passt zum aktuellen Forschungsstand.
Bezüglich der Krankenhaus-Aufenthaltsdauer gab es einen nicht signifikanten Unterschied von einem halben Tag zwischen den beiden Therapiegruppen. Insgesamt war die Krankenhaus-Aufenthaltsdauer länger als in anderen Studien. Das liegt vermutlich daran, dass ausschließlich Patienten mit stattgehabter Transfusion betrachtet wurden und exsanguinierende Blutungen oder bestimmte Vorerkrankungen nicht ausgeschlossen waren.
Über alle Patienten betrachtet gab es einen signifikanten Unterschied in der Anzahl der benötigen Erythrozyten-Konzentrate in den beiden Therapiegruppen. In der restriktiven Gruppe wurden pro Patient mehr Erythrozyten-Konzentrate benötigt als in der liberalen Therapiegruppe (5,89 versus 4,79; p < 0,001). Nach Adjustierung für die restriktive Gruppe (Hämoglobin-Wert von 5,5 g/dl und < 7 g/dl) gab es keinen nennenswerten Unterschied mehr zwischen beiden Therapiegruppen (4,32 versus 4,29).
Insgesamt gab es in dieser Studie nur zwei Verdachtsfälle auf Transfusionsreaktionen, je ein Fall in der restriktiven und der liberalen Gruppe. Aufgrund der geringen Fallzahl konnte keine weitere statistisch sinnvolle Auswertung erfolgen.
Hinsichtlich der endoskopischen und operativen Interventionen gab es lediglich geringe Unterschiede zwischen beiden Gruppen. In beiden Gruppen benötigten prozentual etwa gleich viele Patienten keine endoskopische oder operative Intervention.
Die vorliegende Arbeit hat gezeigt, dass eine monozentrische und retrospektive Studie mit einem relativ kleinen und vorgegebenen Patientenkollektiv, in dem die weit überwiegende Anzahl an Patienten mehrere Vorerkrankungen aus verschiedenen Hauptgruppen hat, schnell an die Grenzen der statistisch sinnvollen Auswertbarkeit stößt. Sinnvoll wäre es, Patienten mit einem vergleichbaren Vorerkrankungsprofil hinsichtlich Art und Ausprägung in den Therapiegruppen „restriktiv“ versus „liberal“ zu vergleichen. Dabei müsste eine weitere Stratifizierung hinsichtlich des Allgemeinzustands und bestimmter möglicher Confounder wie Laborwerte, Medikation sowie hinsichtlich übriger Therapiebestandteile erfolgen. Hier müsste insbesondere das gesamte Volumenmanagement vergleichbar sein.
So konnte die vorliegende Arbeit nur die sicher interessante Tendenz zeigen, dass eine restriktivere Transfusionsstrategie bei einer oberen gastrointestinalen Blutung und stattgehabter Erythrozyten-Transfusion für alle Vorerkrankungsgruppen eine signifikant geringere Mortalität ergab. Dies entspricht in der Tendenz den Ergebnissen der vorhergehenden Studien. Eine Quantifizierung der unterschiedlichen Transfusionsstrategien hinsichtlich der Behandlungseffekte konnte nicht erfolgen.The transfusion of red blood cell concentrates is an essential component in the management of acute upper gastrointestinal bleeding, which is among the most common gastrointestinal emergencies. The aim of this retrospective, single-center study was to determine whether a liberal or restrictive transfusion strategy results in better patient outcomes. Furthermore, the impact of various comorbidities was assessed.
This analysis was based on clinical data from 265 adult patients with acute upper gastrointestinal bleeding who received endoscopic treatment as well as red blood cell transfusion between 2008 and 2018 at the Universitätsmedizin Göttingen. Patients were allocated to treatment groups according to their hemoglobin levels at the time of the first transfusion during the bleeding episode: the restrictive group (<7.0 g/dl, n = 68) and the liberal group (≥7.0 g/dl, n = 197).
Comorbidities were classified into major categories, including cardiac, hepatic, musculoskeletal, nephrological, pulmonary, and neurological conditions, as well as the presence of diabetes mellitus. The majority of patients exhibited multiple comorbidities, consequently resulting in their representation across multiple categories. A total of 215 patients (81.1%) had at least one comorbidity, with the majority belonging to two or more categories. Differences between the treatment groups with respect to the distribution of comorbidity categories were not statistically significant, nor did any clear trends emerge when considering all comorbidities collectively. A significant difference was observed only in the prevalence of arterial hypertension, which was more common in the liberal group (52.3% vs. 41.2%, p = 0.11), whereas the prevalence of liver cirrhosis differed by less than 1%.
The two transfusion strategies were compared across several parameters and four predefined outcomes: mortality, rebleeding rate, length of hospital stay, and the number of red blood cell units required. Various statistical analyses were applied to evaluate these outcomes.
The primary endpoint revealed a significantly lower mortality rate in the restrictive group compared with the liberal group (7.35% vs. 18.27%, p = 0.030). A trend toward longer survival in the restrictive group was also evident in subgroup analyses by comorbidity, although significance levels varied and remained above the critical threshold of p = 0.05. Given that most patients suffered from multiple comorbidities, it is difficult to determine which conditions contributed most strongly to the observed mortality differences. Adjustments for relevant confounders were not feasible due to the limited sample size.
The overall rebleeding rate of 3.4% observed in this study was lower than the expected range reported in the literature. No significant difference was found between the two transfusion groups, although more patients in the liberal group experienced rebleeding, consistent with current evidence.
The length of hospital stay did not differ significantly between groups, with only about half a day separating the two strategies. Compared to prior studies, however, hospitalization was somewhat longer, possibly reflecting the inclusion of patients with inpatient transfusion as well as cases with exsanguinating bleeding or specific comorbidities.
A significant difference was observed in the number of red blood cell units required per patient: those in the restrictive group received more transfusions than those in the liberal group (5.89 vs. 4.79 units, p < 0.001). After adjusting for baseline hemoglobin levels (≤5.5 g/dl and 7 g/dl), however the difference narrowed (4.32 vs. 4.29 units).
There were only two suspected transfusion reaction cases in the entire cohort, one in each group, which precluded further meaningful statistical evaluation.
Regarding endoscopic and surgical interventions, only minor differences were observed, with similar proportions of patients in both groups requiring additional procedures.
This study demonstrates that a single-center, retrospective analysis with a relatively small and heterogeneous patient population—most of whom presented with multiple comorbidities across different categories—rapidly encounters limits in statistical power. A more robust approach would involve comparing restrictive and liberal transfusion strategies within patient cohorts stratified by comorbidity profiles and overall clinical status, ideally controlling for confounders such as laboratory values, medications, and other therapeutic measures, particularly fluid management.
Despite these limitations, this study supports the trend toward restrictive transfusion strategies in upper gastrointestinal bleeding. Among patients receiving inpatient red blood cell transfusion, the restrictive approach was associated with significantly lower mortality across all comorbidity categories. This finding is consistent with previous reports in the literature. However, precise quantification of the differential treatment effects between restrictive and liberal strategies was not possible within this study.2025-11-1
Observation of Floquet states in graphene
Recent advances in the field of condensed-matter physics have unlocked the potential to realize and control emergent material phases that are not accessible in thermal equilibrium. One of the most promising concepts in this regard is Floquet engineering, the coherent dressing of matter via time-periodic perturbations. However, the broad applicability of Floquet engineering to quantum materials is still in question. Crucially, the theoretically predicted Floquet-induced effects in monolayer graphene, the most sought-after case, could not be verified or only be explored in indirect experimental approaches. More specifically, a report of the light-induced anomalous Hall effect in graphene is critically debated. Furthermore, a time-resolved photoemission experiment even indicates that Floquet effects might not be realizable in graphene and other (semi-)metals that exhibit relatively short decoherence times. Here, we resolve this long-standing debate by using electronic structure measurements to provide direct experimental evidence of Floquet engineering in graphene. We report light-matter-dressed Dirac bands by measuring the contribution of Floquet sidebands, Volkov sidebands, and their quantum path interference to graphene’s photoemission spectral function. Fully supported by experiment and theory, we demonstrate that Floquet engineering in graphene is possible, even though ultrafast decoherence processes occur on few tens of femtosecond timescales. Our results pave the way for the experimental realization of Floquet engineering strategies in (semi-)metallic systems, promising the coherent stabilization of light-induced states with potentially non-trivial topological properties.2026-01-2
The characterization of macrophages and microglia, as well as T cell inflammation and blood-brain barrier dysfunction in different subtypes of inflammatory demyelination
Die Multiple Sklerose (MS) ist durch entzündliche Entmarkung gekennzeichnet, wobei histopathologische Studien unterschiedliche Läsionssubtypen aufgezeigt haben. Diese Dissertation befasste sich mit der Charakterisierung von Makrophagen, Mikroglia, T-Zell-Infiltration und Störungen der Blut-Hirn-Schranke (BHS) in frühaktiven Läsionen der MS-Subtypen II und III. Mittels Immunhistochemie, Immunfluoreszenz und MRT-Analysen wurden zelluläre Immunprofile und strukturelle Veränderungen untersucht. Subtyp-II-Läsionen waren mit einer Einwanderung peripherer Makrophagen, Antikörper- und Komplementablagerungen sowie einer Tendenz zu höheren CD4-positiven Effektor-T-Zellen assoziiert. Im Gegensatz dazu zeigten Subtyp-III-Läsionen eine vorherrschende Aktivierung ortsständiger Mikroglia, eine Zunahme CD8-positiver zytotoxischer T-Zellen und eine distale Oligodendrogliopathie. BHS-Störungen waren in beiden Subtypen nachweisbar, jedoch in Subtyp II deutlicher ausgeprägt. Zudem wiesen Subtyp-II-Läsionen scharf begrenzte Läsionsränder auf, während Subtyp III diffus definierte, teilweise Baló-ähnliche konzentrische Entmarkungsmuster zeigte. Diese Ergebnisse unterstützen das Konzept grundsätzlich unterschiedlicher immunpathologischer Mechanismen bei der Läsionsentstehung in der MS. Während Subtyp II vor allem durch die Infiltration extrinsischer Immunzellen und antikörpervermittelte Schädigung bestimmt ist, zeichnet sich Subtyp III durch intrinsische Mikrogliaaktivierung und neurodegenerative Prozesse aus. Ein vertieftes Verständnis dieser Mechanismen könnte zur Entwicklung individuellerer und gezielterer Therapieansätze bei MS beitragen.Multiple sclerosis (MS) is characterized by inflammatory demyelination, with histopathological studies revealing distinct lesion subtypes. This dissertation focused on the characterization of macrophages, microglia, T-cell infiltration, and blood–brain barrier (BBB) disturbances in early active lesions of MS subtypes II and III. Using immunohistochemistry, immunofluorescence, and MRI analyses, cellular immune profiles and structural alterations were investigated.
Subtype II lesions were associated with the infiltration of peripheral macrophages, deposition of antibodies and complement, and a tendency toward higher numbers of CD4-positive effector T cells. In contrast, subtype III lesions showed predominant activation of resident microglia, an increase in CD8-positive cytotoxic T cells, and distal oligodendrogliopathy. BBB disturbances were present in both subtypes but were more pronounced in subtype II. Moreover, subtype II lesions displayed sharply demarcated borders, while subtype III lesions exhibited diffusely defined, partly Baló-like concentric demyelination patterns.
These findings support the concept of fundamentally distinct immunopathological mechanisms in lesion development in MS. While subtype II is primarily characterized by infiltration of extrinsic immune cells and antibody-mediated damage, subtype III is driven by intrinsic microglial activation and neurodegenerative processes. A deeper understanding of these mechanisms may contribute to the development of more individualized and targeted therapeutic approaches in MS.2025-11-1
In vitro testing of histone deacetylase inhibitors (HDACi) in combination with temozolomide radio-chemotherapy as a treatment strategy for pediatric high-grade gliomas (pedHGG)
Pediatric high-grade gliomas (pedHGG) are highly malignant pediatric brain tumors associated with a fatal prognosis and include, among others, diffuse intrinsic pontine gliomas (DIPG), which in 65 % carry an H3.3K27M mutation, leading to an epigenetic imbalance resulting in global hypotrimethylation and hyperacetylation at H3K27. Due to the lack of effective conventional temozolomide radio-chemotherapy, new combined treatment options using epigenetic drugs might be promising. Therefore, the present in vitro drug study aimed to analyze the use of five HDAC inhibitors (HDACi) – vorinostat, domatinostat, mocetinostat, resminostat, and panobinostat – as therapeutic adjuvants together with temozolomide (TMZ) and irradiation (IRR) in pedHGG/DIPG.
For this purpose, tumor cell lines derived from primary pediatric glioblastoma (H3WTpedHGG) and DIPG cells (H3.3K27M-DIPG) differing in their H3 mutation status were employed. Western blot analyses were conducted to investigate the epigenetic effect of HDACi on histone modifications. As a focus of this study cell viability assays and synergy analyses have been performed to investigate if HDACi combined with TMZ and IRR led to an increased therapeutic efficacy in terms of sensitivity and synergy.
The study confirmed that all five HDAC inhibitors led to histone hyperacetylation in H3WTpedHGG and H3.3K27M-DIPG cells, regardless of H3 mutation status. Notably, domatinostat (no longer available on the market) and mocetinostat, both benzamide class inhibitors, demonstrated the highest growth-inhibitory capacities and exhibited additive and sensitizing effects mainly in combination with TMZ across all H3WTpedHGG/H3.3K27M-DIPG cell lines, while radio-sensitization effects with these two inhibitiors were mainly observed with H3WT-pedHGG cells. These findings suggest that benzamides may be promising drug candidates for combination therapies in these tumors. Vorinostat, resminostat, and panobinostat, all hydroxamic acid inhibitors, revealed therapeutic efficacy and synergistic cytotoxicity together with TMZ and IRR in H3WTpedHGG cells, while H3.3K27M-DIPG cells became resistant to resminostat and panobinostat. In general, H3.3K27M-DIPG cells were less susceptible to any treatment than H3WT-pedHGG cells. This findings may hint to an overall more resistant cellular phenotype, impeding the success of any or most single and combinatory treatment regimens.
The crucial limiting factors of this in vitro study are the presence of various cell line specific effects, potential off-target toxicities, and the development of resistance upon HDACi treatment making a definite prediction on H3 mutation dependence difficult. Overcoming these limiting properties, by using isogenic DIPG cell lines and performing experiments on functional characteristics with and without temozolomide radio-chemotherapy, would be an essential approach to further investigate HDAC enzymes as probable epigenetic targets in pedHGG/DIPG.
Future preclinical trials on pedHGG should aim to combine standard radio-chemotherapy with different novel treatment approaches, including other tumor-specific epigenetic targeted therapies and immunotherapeutic strategies, to achieve synergistic drug combinations thatideally can cross the blood brain barrier. To this end, additional in vivo drug experiments should be considered in the future. Most importantly, oncological research on pedHGG should be conducted independently of adult glioblastoma research, as the mission statement of every pediatric clinician already states: “Children are not little adults.”2026-01-1
The potential of virtual fencing for cattle and goats with regard to learning behaviour and animal welfare
Die moderne Zäunungstechnologie „Virtual Fencing“ (virtuelles Zäunen) erfreut sich weltweit zunehmender Beliebtheit, weil sie verspricht, die Arbeit von Landwirten zu erleichtern. Anders als bei physischen Zäunen, sind die virtuellen Zaunlinien für die Tiere unsichtbar und werden von akustischen Signalen angekündigt, die von Halsbändern abgegeben werden, die die Tiere tragen müssen. Die Zaunlinien können von den Landwirten jederzeit aus der Ferne in einer App
geändert werden, was das System flexibel macht. Voraussetzung dafür sind eine stabile Mobilfunkverbindung und ein GPS-Signal, mit dem die Landwirte ihre Tiere auch orten und überwachen können.
Ähnlich wie bei elektrischen Zäunen gibt es einen elektrischen Impuls als Strafe, wenn sich das Tier auf das akustische Signal hin nicht abwendet. Um sicherzustellen, dass das Tier innerhalb des virtuellen Zauns bleibt und keine wiederholten Stromimpulse erhält, muss es lernen, die Signale zu assoziieren und angemessen auf das akustische Signal zu reagieren. Obwohl viele Studien bewiesen haben, dass Rinder, Schafe und Ziegen im Allgemeinen zu diesem Lernprozess in der Lage sind, bestehen nach wie vor Bedenken hinsichtlich des Wohlergehens von virtuell eingezäunten Tieren.
Die vorliegende Arbeit befasst sich mit diesen Fragen aus verschiedenen Blickwinkeln.
In Kapitel eins wurde eine systematische Übersichtsarbeit durchgeführt, in der die Ergebnisse von Studien über Virtual Fencing mit Rindern im Hinblick auf den Lernprozess der Tiere und ihr Wohlergehen analysiert wurden. Es wurde nachgewiesen, dass Rinder lernen, elektrische Impulse zu vermeiden, da das berechnete Verhältnis zwischen der Anzahl der elektrischen Impulse und der akustischen Signale im Laufe der Zeit deutlich abnahm. Auch bei den
untersuchten Tierschutzparametern zeigten sich zwischen den virtuell und den physisch eingezäunten Rindern keine signifikanten Unterschiede, die zuverlässig auf das virtuelle Zaunsystem zurückgeführt werden könnten. Aufgrund der geringen Anzahl von Studien zu diesem Zeitpunkt wurden jedoch weitere Untersuchungen empfohlen.
In Kapitel zwei wird ein Virtual Fencing-Experiment mit Schwerpunkt auf dem Lernverhalten und dem Wohlergehen von Ziegen beschrieben. In einem Cross-over-Design wurden zwanzig „Blobe“-Ziegen mit Nachkommen 12 Tage lang entweder virtuell oder physisch eingezäunt und durchliefen ein Virtual Fencing-Trainingsprotokoll von Hamidi et al. (2022). Zur Bewertung des Wohlbefindens wurden Beobachtungen der grundlegenden Verhaltensweisen der Ziegen und fäkale Kortisolmetabolitenmessungen als Stressindikator durchgeführt. Abgesehen von den höheren Werten für aufmerksames Stehen in den virtuell eingezäunten Gruppen wurden keine Unterschiede zwischen den Zaunsystemen festgestellt. Darüber hinaus deutete die Entwicklung der Anzahl akustischer Signale und elektrischer Impulse auf ein erfolgreiches Lernen auf Herdenebene hin. Insgesamt waren die Ergebnisse vielversprechend, aber es sind weitere
Studien erforderlich, um herauszufinden, ob virtuelle Zäune das Wohlergehen der Ziegen negativ beeinflussen kann.
Angesichts der Bedenken hinsichtlich des Wohlergehens virtuell eingezäunter Tiere und der rechtlichen Hürden für virtuelle Zäune in vielen europäischen Ländern befasst sich Kapitel drei mit dem Potenzial akustischer Signale von Virtual Fencing-Halsbändern. In einem ersten Experiment wurden verschiedene akustische Signale und Vibrationen getestet, um einen möglichen Ersatz für den elektrischen Impuls zu finden. Keines der getesteten Signale war
jedoch effizient genug, um das Verhalten von Rindern ausreichend und langfristig zu verändern.
Das zweite Experiment basierte auf der Idee, dass das Bewegungsverhalten von Rindern auf der Weide beeinflusst werden könnte, um das Weidemanagement zu verbessern. Obwohl verschiedene Trainingsansätze ausprobiert wurden, konnte den Rindern nicht beigebracht werden, ihre Bewegungsrichtung auf dem Weg zu einer Futterquelle an akustische Signale anzupassen. Schließlich wurde empfohlen, für den ersten und zweiten Versuch jeweils andere Signale und ein anderes Trainingsprotokoll zu testen.
Insgesamt zeigen die Ergebnisse der drei Kapitel in Kombination mit den Erkenntnissen aus der Allgemeinen Diskussion, dass zumindest Rinder virtuell eingezäunt werden können, da sie in der Regel zu dem notwendigen Lernprozess fähig sind und in den bisher durchgeführten Studien kein eindeutiger Nachteil für ihr Wohlbefinden nachgewiesen werden konnte. Für diese Tierart sollten gesetzliche Änderungen in Betracht gezogen werden, wenn Virtual Fencing oder der Einsatz von Elektrizität am Tier verboten sind, und es sollten offizielle Regelungen in den
Ländern eingeführt werden, in denen virtuelle Zäune kommerziell verfügbar sind, um den Tierschutz zu gewährleisten. Gleichzeitig sollte die Erforschung eines möglichen Ersatzes für den elektrischen Impuls fortgesetzt werden, um in bestimmten Fällen eine stromfreie Alternative zu haben.
Da die derzeitige Forschung für Ziegen noch begrenzt ist, sind weitere Studien erforderlich, insbesondere zur Bewertung des Wohlbefindens von virtuell eingezäunten Ziegen.
Abschließend lässt sich sagen, dass Virtual Fencing viele Vorteile für Landwirte, Tiere und die Umwelt mit sich bringt und das Potenzial hat, physische Zäune zukünftig unter bestimmten Bedingungen zu ersetzen.The modern fencing technology ‘virtual fencing’ is becoming increasingly popular worldwide because it promises to make farmers’ work easier. Instead of actual physical fences, the virtual fence lines are invisible to the animals and are indicated by acoustic signals emitted by collars that the animals must wear. The fence lines can be modified remotely by the farmers at any time via an app, making the system flexible. This requires a stable mobile phone connection and a GPS signal, which also allows the farmers to locate and monitor their animals.
Similar to electric fences, an electric pulse acts as a punishment if the animal does not turn away in response to the acoustic signal. To ensure that the animal remains within the virtual fence and does not receive permanent electric pulses, it must learn to associate the signals and react appropriately to the acoustic signal. Although many studies have proven that cattle, sheep, and goats are generally capable of this learning process, concerns still exist regarding the welfare of virtually fenced animals.
The present thesis addresses these issues from various perspectives.
In Chapter one, a systematic review was conducted, analysing the results of virtual fencing studies with cattle regarding the animals’ learning process and welfare. It was demonstrated that cattle learn to avoid receiving electric pulses as the calculated ratio of the number of electric pulses to acoustic signals decreased significantly over time. Also, the welfare parameters examined revealed no significant differences between the virtually and physically fenced cattle that could be reliably attributed to the virtual fencing system. Still, due to the small number of studies at that time, further investigations were recommended.
Chapter two describes a virtual fencing experiment with a focus on the learning behaviour and welfare of goats. In a cross-over design, twenty ‘Blobe’ goats with offspring were either virtually or physically fenced for 12 days and underwent a virtual fencing training protocol by Hamidi et al. (2022). Observations of the goats’ basic behaviours and faecal cortisol metabolite
measures as stress indicator were conducted to evaluate welfare. Except for higher values of standing vigilant in virtually fenced groups, no differences were found between the fencing systems. In addition, the development of the numbers of acoustic signals and electric pulses indicated successful learning at herd level. Overall, the results were promising, but further
studies are needed to find out whether virtual fencing can negatively affect the welfare of goats.
Based on the concerns on the welfare of virtually fenced animals and the legal hurdles of virtual fencing in many European countries, Chapter three deals with the potential of acoustic signals emitted by virtual fencing collars. In a first experiment, different acoustic signals and vibrations were tested to find a potential replacement for the electric pulse. However, none of the signals tried were efficient enough to change the behaviour of grazing cattle sufficiently and in the long-term. The second experiment was based on the idea that the movement behaviour of cattle on pasture could be influenced to improve grazing management. Although various training approaches were tried, cattle could not be taught to adjust their direction of travel in response to an acoustic signal while walking to a feed source. Finally, it was recommended to test different signals and a different training protocol for the first and second experiment, respectively.
Overall, the results of the three chapters in combination with the insights from the General Discussion demonstrate that at least cattle can be virtually fenced, as they are generally capable of the necessary learning process and no clear disadvantage for their welfare has been proven in the studies conducted so far. For this animal species, legal changes should be considered
where virtual fencing or the use of electricity on animals is prohibited, and official regulations are introduced in countries where virtual fencing is commercially available in order to ensure animal welfare. At the same time, research into a potential replacement for the electric pulse should be continued to have a current-free alternative for individual cases.
Since the current research for goats is still limited, further studies are needed, especially to assess the welfare of virtually fenced goats.
Finally, virtual fencing brings many benefits for farmers, animals, and the environment, and has the potential to replace physical fences in the future under certain conditions.2025-10-2
Evaluation of Contradictions as a Data Quality Measure for Real-world and Clinical Research Data
In health research discourse, there is the assumption that a controlled, planned clinical research naturally produces cleaner datasets, while real-world data suffers from quality deficiencies due to its spontaneous collection without anticipation of secondary research use. However, this binary view simplifies a complex reality. Both clinical research and real-world data exhibit quality issues, challenging the convention that data origin determines data integrity. What proves to be more vital is the way data quality (DQ) is being assessed—particularly when examining the exhibited relationships between data items that reflect the true complexity of healthcare information. Health data is inherently interconnected e.g., height relates to weight and age; medication dosages connect to patient characteristics and medical conditions. Yet most data quality assessment (DQA) studies limit their logical evaluations to only a handful of interdependent items, missing the broader web of relationships that could reveal critical quality issues. Contradictions within datasets serve as an important DQ indicator (measurable attribute of data) that extend far beyond simple data entry errors. When values contradict each other, they reveal misunderstandings of domain knowledge, failures in data capture infrastructure, and problems in data transformation processes. Given the complexity of the contradiction patterns, a comprehensive assessment becomes essential.
Comprehensive DQA in health research requires evaluating data on multiple dimensions, such as completeness, conformance, and plausibility, to determine the suitability of data for intended use. These dimensions are used to group related measurable attributes of data referred to as indicators. While some DQ indicators are implemented as generic rules (e.g., completeness), contradiction, which denotes pair(s) of measurements or facts that cannot logically coexist, are multi-dimensional. Some contradictions require common knowledge for their evaluation—e.g., the disparity in age information at baseline and during follow-up visits. However, contradictions in the pre-analytic states of blood samples require knowledge of experts. Although various tools have been developed to assess contradictions in health data, the evolving nature of contradiction rules presents the following challenges for existing tools: 1) limited reusability across diverse health datasets due to incompatible predefined rules, 2) insufficient contextual information required for comprehensive contradiction assessment, and 3) lack of consistent representation of contradiction patterns to support transparent rule implementations and computational efficiency.
To address these gaps in a systematic manner, the first step of this thesis was to adapt an existing tool developed based on an existing framework to assess broader contradiction rules introduced in the German Corona Consensus (GECCO) dataset. Building on the experience from the modification of Schmidt et al.'s tool, this thesis developed a new framework that generalizes the DQA tool to accept custom contradiction rules compatible with diverse health datasets. This is intended to empower domain experts in defining effective contradiction rules on an ad-hoc basis while maintaining alignment with established DQ indicators.
Although the primary dependencies between health data items form the basis for contradiction rule definition, the required context for a comprehensive analysis is equally relevant. For example, diabetes mellitus (DM) and insulin medication (INS) are two interdependent items, however, it is the timestamps that indicate if the INS was administered before the DM diagnosis, which is a prelude for a conclusive contradiction finding. Through a qualitative grading scheme for analyzing metadata defined in a study database, this work created a mechanism to measure the availability of the contextual information required to support a context-aware contradiction analysis. However, having access to rich contextual information is only part of the solution—the way contradiction rules are structured and implemented is equally critical for effective DQA.
While researchers have worked extensively on harmonizing DQ indicators, the challenge extends beyond categorization to implementation efficiency. Though the taxonomies for contradictions, such as logical and empirical contradictions, reflect the domain description, the rules implemented in the DQA systems do not rely on semantics. Extracting the structure of the varied contradiction patterns ensures transparency and consistency in rule implementation. This work evaluated the performance of different rule implementations and offered an optimization method that integrates the fastest and traceable units of the rules in one unified implementation. This is relevant because broadly defined DQA rules can cause performance degradation in large database infrastructures.
With these several perspectives on contradictions as a DQ indicator, the contributions in this thesis enhance contradiction assessment by improving reusability across diverse health datasets, enabling context-aware assessments, and optimizing rule implementation for transparency and computational efficiency. Effect heterogeneity in gender inequality and environmental regulation: From linear models to machine learning
Equality Innovation Fund of the University of GöttingenUm gezielte Politikmaßnahmen zur Förderung gesellschaftlicher Ziele wie Gleichberechtigung der Geschlechter und ökologische Nachhaltigkeit entwickeln zu können, ist ein umfassendes Verständnis von Effektheterogenität unabdingbar. Während Durchschnittseffekte allgemeine Orientierung bieten, ermöglicht die Analyse heterogener Effekte einen Fokus auf spezifische Subgruppen. Diese Dissertation erweitert die Forschung zu heterogenen Effekten durch vier Artikel mit unterschiedlichen Datensätzen und statistischen Methoden: von der linearen Regression hin zu maschinellen Lernmodellen. Die Fallstudien beschäftigen sich mit geschlechter- und umweltökonomischen Fragestellungen in Japan, Deutschland und China.
Der erste Artikel analysiert mittels einer moderierten Regressionsanalyse, wie Geschlechterrollen den Zusammenhang zwischen Arbeits-Familien-Konflikt und Beschäftigungsfluktuation bei über 700 Müttern in Japan prägen. Die Ergebnissen zeigen, dass egalitär eingestellte Mütter seltener mit freiwilligem Jobwechsel auf Arbeits-Familien-Konflikte reagieren. Die Studie unterstreicht die Bedeutung von Heterogenität innerhalb eines Geschlechts auf das Arbeitsmarktverhalten.
Der zweite Artikel nutzt einen Paneldatensatz von über 300 Professor*innen (2013--2021) an einer repräsentativen deutschen Universität, um das akademische geschlechtsspezifische Lohngefälle zu untersuchen. Basierend auf Mincer-Lohngleichungen dokumentieren wir ein konditionales Lohngefälle von 5{,}3\,\%. Die Heterogenitätsanalyse zeigt, dass die Gehaltsdifferenzen auf geringere Erträge aus Bleibeverhandlungen zurückzuführen sind, mit einem stärkerem Effekt in frühen Karrierephasen.
Der dritte Artikel untersucht den Zusammenhang zwischen Umweltregulierung und Innovation auf Unternehmensebene. Anhand von über 400.000 chinesischen Industrieunternehmen (1995--2007) stellen wir fest, dass Unternehmen in regulierten Regionen ihre Patentanmeldungen nach Einführung der Two-Control-Zones-Politik im Vergleich zu unregulierten Regionen erhöhten. Der Effekt ist am stärksten bei Unternehmen mit höherer Innovationskapazität sowie in emissionsärmeren Sektoren und Städten mit strengerer lokaler Durchsetzung der Maßnahme.
Der vierte Artikel untersucht mit einem Causal-Forest-Ansatz, wie Umweltregulierung auf Innovation zwischen Unternehmen, Branchen und Städten variiert. Anhand von über 128.000 chinesischen Industrieunternehmen identifizieren wir Unternehmensgröße als primären Antreiber heterogener Effekte, gefolgt vom Fachkräfteanteil der Industrie, Bevölkerungsdichte und Marktkonzentration. Die Ergebnisse implizieren, dass die Innovativität kleiner und mittlerer Unternehmen gezielt unterstützt werden sollte.Understanding effect heterogeneity is crucial for designing targeted policies that promote societal goals such as gender equality and environmental sustainability. Although average effects provide general guidance, analyzing heterogeneous effects allows for the efficient targeting of specific subgroups. This dissertation advances the study of heterogeneous effects through four articles leveraging distinct datasets and estimation techniques, from linear to machine learning models, spanning gender and environmental economics in Japan, Germany, and China.
The first article employs moderated regression analysis to investigate how perceived gender roles shape the relationship between work-family conflict and employment fluctuation among over 700 mothers in Japan, using a two-wave panel. Results indicate that mothers with more egalitarian gender role attitudes are significantly less likely to leave employment voluntarily in response to work-family conflict. The study underscores the importance of within-group heterogeneity in shaping labor market behavior.
The second article uses a unique panel dataset of over 300 faculty employed between 2013 and 2021 at a renowned German university to examine the magnitude and sources of the academic gender pay gap. Based on extended Mincer wage equations, we document a conditional gender pay gap of 5.3\% in our sample. Heterogeneity analysis reveals that the differentials can be attributed to lower returns from retention
negotiations, with a stronger impact during the earlier stages of academic careers.
The third article studies the link between environmental regulation and firm-level innovation. Using data from over 400,000 Chinese manufacturing firms between 1995 and 2007, we find that firms in regulated regions increased their patent applications following the introduction of China's Two Control Zones policy compared to those in unregulated regions. The effect is strongest among firms with higher innovation capacity, achieved through human capital, government support, and access to financial resources, as well as in low-emission sectors and cities with stricter policy enforcement.
The fourth article applies a data-driven causal forest approach to explore how environmental regulation effects on firm innovation vary across firms, industries, and cities. Using a partially balanced dataset of over 128,000 Chinese manufacturing firms, we identify firm size as the primary source of heterogeneity, followed by industry-level skill intensity, population density, and market concentration. While we find positive average effects, larger firms show stronger innovation responses, suggesting small and medium-sized firms require targeted support under stricter regulations.2025-10-3
Outcomes after circumcision in children - factors influencing parental satisfaction
In Germany, slightly more than 100,000 circumcisions are performed annually, with 10.9% of boys aged 0–17 years being circumcised. The decision to circumcise a child often presents a challenge for parents. Medically indicated circumcision is performed according to clinical guidelines for pathological phimosis after unsuccessful conservative treatment (primarily with corticosteroid-based ointments). However, depending on the underlying condition – such as suspected lichen sclerosus or severe scarring - primary circumcision may also be indicated. Reported complication rates vary widely in the literature (0.19–16%), largely depending on whether the procedure was performed by medical professionals or non-professionals, and by differing definitions and classifications of complications. Severe complications are generally rare.
This retrospective questionnaire-based study investigated significant factors influencing parental satisfaction after their child’s circumcision. A total of 379 patients circumcised at the University Medical Center Göttingen between 2005 and 2012 were invited; 66 parents (17.4%) responded. Data from questionnaires and medical records were analyzed (significance level p < 0.05).
Overall, 68.2% of parents were satisfied, while 31.8% were dissatisfied. Factors significantly associated with lower parental satisfaction included preoperative agitation and anxiety in children and parents (p = 0.040), postoperative pain (p = 0.003), and unsatisfactory cosmetic results (p ≤ 0.001). These findings are consistent with previous studies. The preoperative phase is often associated with considerable anxiety, particularly in younger children. Perioperative anxiety is also linked to increased postoperative pain and higher analgesic requirements. Cosmetic outcome remains the strongest predictor of parental satisfaction and the most common reason for planned revision surgeries.

The Role of Lymphadenectomy in Esophagectomy for Esophageal Carcinomas and Carcinomas of the Gastroesophageal Junction
Esophageal carcinoma, a malignant neoplasm of the upper gastrointestinal tract, is among the most aggressive cancers in terms of prognosis and mortality. While squamous cell carcinoma remains the most common histological subtype worldwide, an increasing incidence of adenocarcinoma has been observed in Western countries. Early lymphatic metastasis, bidirectional lymphatic drainage, and the influence of neoadjuvant therapy on lymphatic pathways represent major challenges when determining the appropriate extent of lymphadenectomy. Radical lymphadenectomy aims to remove potentially positive lymph nodes, allowing for more accurate histopathological staging and improved locoregional tumor control. However, the optimal extent of lymphadenectomy remains unclear. Therefore, this study aimed to assess the prognostic significance of the number of resected lymph nodes as a potential predictor of improved outcomes.
The present work is a retrospective cohort study including all patients who underwent subtotal esophagectomy with lymphadenectomy at our institution between 2015 and 2022 due to a malignant esophageal disease. All patients were included regardless of their pretreatment status. The cohort was divided into three groups according to the number of resected lymph nodes (29 nodes). Univariate and multivariate analyses were performed to assess the impact of lymphadenectomy on predefined endpoints, including complication rate, overall survival, and progression-free survival.
Overall, no increase in postoperative morbidity was observed with more radical lymphadenectomy. The results suggest that the surgeon has the greatest influence on the complication rate. With respect to overall and progression-free survival, lymph node status and depth of tumor invasion were identified as independent prognostic factors. The increasing number of resected lymph nodes could not be clearly established as a prognostic factor but showed a trend toward improved overall survival. The removal of infracarinal lymph nodes had no positive impact on overall or progression-free survival.
The findings of this study underscore good surgical practice and the generally favorable outcomes of surgical therapy for esophageal carcinoma. To adequately determine the appropriate extent of lymphadenectomy in this setting, further studies are required. In particular, future stratified analyses should consider the influence of neoadjuvant therapy, tumor (T) stage, and histological grading to reach a consensus on the optimal extent of lymphadenectomy.2025-12-2
