Untersuchung plasmabasierter Kompositbiomarker in der Differentialdiagnostik von Alzheimer-Demenz und Vaskulärer Demenz
Aufgrund des demografischen Wandels stehen demenzielle Erkrankungen nicht nur im Zentrum der medizinischen Forschung, sondern erhalten auch einen immer größer werdenden Stellenwert in der gesundheitsökonomischen und gesellschaftlichen Diskussion. Vor diesem Hintergrund hat sich die Demenz- Diagnostik über die Jahre kontinuierlich weiterentwickelt. Dabei stellt vor allem die Vaskuläre Demenz neben der Alzheimer-Erkrankung eine wichtige und häufig unterdiagnostizierte Differentialdiagnose dar.
Im Rahmen dieser Forschungsarbeit wurde unter Zuhilfenahme zweier prospektiver Beobachtungsstudien der Demenzforschungsgruppe der Universitätsmedizin Göttingen, die biomarkerbasierte Differentialdiagnostik zwischen Patienten mit Alzheimer-Erkrankung, vaskulär bedingter kognitiver Beeinträchtigung und Mischdemenz beleuchtet.
Dazu wurde ein breites Spektrum an etablierten Alzheimer-assoziierten Biomarkern (Aß42, Aß40, Aß42/40 ratio, p-Tau), Biomarkern des neuronalen Schadens (Tau, NFL), neueren Biomarkern (GFAP, LCN2) und Kompositbiomarkern (p-Tau/Aß42, p-Tau/Aß40, GFAP*p-Tau/Aß42, GFAP*p-Tau/Aß40 und Tau/p-Tau) in Gruppen und Subgruppenvergleichen gemessen und analysiert.
Hierbei konnte gezeigt werden, dass die Kompositbiomarker im Allgemeinen eine höhere diagnostische Genauigkeit als die anderen Biomarker aufwiesen. Wobei p-Tau unter letzteren die höchste diagnostische Trennschärfe in der Differenzierung von Patienten mit Alzheimer-Erkrankung oder Mischdemenz von solchen mit vaskulär bedingter kognitiver Beeinträchtigung erzielte.
Aufgrund des Mangels an kausalen Antidementiva ist gerade eine frühe Diagnosestellung für den rechtzeitigen Beginn einer symptomatischen Therapie essenziell. Vor diesem Hintergrund können blutbasierte Biomarker als kostengünstige, wenig invasive und ubiquitär verfügbare Screening-Instrumente in Risikobevölkerungsgruppen eingesetzt werden und das Therapieansprechen in Arzneimittelstudien überwachen.Due to demographic change, dementia is not only at the center of medical research but is also becoming increasingly important in the health economic and social debate. Taking that into consideration, dementia diagnostics has developed continuously over the years. Vascular dementia in particular is an important and often underdiagnosed differential diagnosis alongside Alzheimer's disease.
In the context of this research work, the biomarker-based differential diagnosis between patients with Alzheimer's disease, vascular cognitive impairment and mixed dementia was examined with the help of two prospective observational studies by the Dementia Research Group of the University Medical Centre Göttingen.
A broad spectrum of established Alzheimer's-associated biomarkers (Aß42, Aß40, Aß42/40 ratio, p-Tau), biomarkers of neuronal damage (Tau, NFL), newer biomarkers (GFAP, LCN2) and composite biomarkers (p-Tau/Aß42, p-Tau/Aß40, GFAP*p-Tau/Aß42, GFAP*p-Tau/Aß40 and Tau/p-Tau) were measured and analyzed in groups and subgroup comparisons.
It was shown that the composite biomarkers generally had a higher diagnostic accuracy than the other biomarkers. Of these, p-Tau achieved the highest diagnostic accuracy in differentiating patients with Alzheimer's disease or mixed dementia from those with vascular-induced cognitive impairment.
Due to the lack of causal anti-dementia medication, early diagnosis is essential for the timely initiation of symptomatic therapy. Taking that into consideration, blood-based biomarkers can be used as inexpensive, less invasive and ubiquitously available screening instruments in at-risk population groups and monitor treatment response in drug studies.2025-02-2
Late parenthood as a topic in medicine - An ethical analysis of age and gender aspects in the discourse of German print media
Die vorliegende Dissertation hatte das Ziel zu untersuchen, wie späte Elternschaft in deutschen Printmedien dargestellt wird.
Zunächst wurde anhand einer deskriptiven Literaturanalyse untersucht, welche Faktoren die Festlegung von Altersgrenzen begründen könnten. Hierbei ging es im ersten Schritt um soziale Argumente, die auf die Rolle der Personen und ihres Kindes/ihrer Kinder innerhalb der Gesellschaft abzielen. Dabei wurden die Medikalisierung der Reproduktion, die Gestaltung von Lebensentwürfen sowie die individuelle Wahrnehmung von Infertilität und Kinderwunsch in den Blick genommen. Desweiteren wurden medizinethische Überlegungen zur reproduktiven Autonomie, Gerechtigkeit, Nicht-Schaden und Fürsorge ausgeführt. Auch die Funktion von Eltern und die Auswirkungen von assistierten reproduktionsmedizinischen Techniken und später Elternschaft auf das Kindeswohl wurde beleuchtet.
Im zweiten Schritt wurden biologische Argumente ausgeführt, die erhöhte Risiken für Mütter und Kinder aufzeigen, die durch späte Mutter- oder Vaterschaft entstehen können. Eine der zentralen Erkenntnisse war, dass nicht nur das Alter der Mutter, sondern auch das Alter des Vaters ein höheres Risiko für negative gesundheitliche Auswirkungen nach sich zieht. Die aufgeführten Aspekte wurden im Ergebnisteil in Pro- und Kontra-Argumente für oder gegen späte Elternschaft und/oder assistierte reproduktionsmedizinische Techniken eingeteilt.
Medientheoretisch wurden dann die Unterschiede zwischen Boulevard- und Qualitätsmedien ausgeführt und die Bedeutung von Informationen hervorgehoben. Mittels einer Stichprobe aus 221 Artikeln mit dem inhaltlichen Schwerpunkt auf Problemfeldern menschlicher Reproduktion in Deutschland, die laut des Artikels entweder im Geschlecht oder im Alter eines oder beider Elternteile gründen, wurde mit ATLAS.ti eine qualitative Inhaltanalyse nach Mayring durchgeführt. Die Artikel stammten aus sieben Printmedien (Süddeutsche Zeitung, Frankfurter Allgemeine Zeitung, Der Spiegel, Die Zeit, Brigitte, Men’s Health, Bild), wovon Süddeutsche Zeitung, Frankfurter Allgemeine Zeitung, Der Spiegel und Die Zeit den Qualitätsmedien zugeordnet wurden. Brigitte, Bild und Men’s Health wurden als Boulevardmedien klassifiziert.
Anschließend wurde ausgewertet, wie sich die Darstellung des Alters von Frauen und Männern unterscheidet und welche Argumente Boulevard- und Qualitätsmedien zur Begründung von Altersgrenzen verwenden und ob diese sich mit den in der Literaturrecherche gefundenen Argumenten decken. Hierbei zeigte sich, dass das Alter von Frauen häufiger thematisiert wurde. Es gab eine Häufung der Nennung von Alter zwischen 30 und 40 Jahren, besonders häufig wurden 35 Jahre genannt. Das Alter von Männern wurde seltener beschrieben und 46-50 Jahre wurde bei Männern am häufigsten genannt. Hinsichtlich der verwendeten Argumente gab es keine großen Unterschiede zwischen den Qualitäts- und Boulevardmedien: Beide Medienarten informierten insgesamt ausgeglichen und nannten sowohl Pro- als auch Kontra-Argumente – mit einer leichten Überlegenheit der Qualitätsmedien gegenüber den Boulevardmedien bei der Bandbreite der berichteten Kontra-Argumente. Eine Stereotypisierung fand anhand von nicht numerischen Altersangaben („Oma-Mama“, „Opa-Dads“) statt. Anders als es im Sinne des eingangs besprochenen „double standard of aging“ zu erwarten gewesen wäre, betraf diese Stereotypisierung jedoch beide Geschlechter und auch beide Medienarten gleichermaßen.
Für die Zukunft wäre die Erweiterung der fundierten und ausgewogenen Berichterstattung über die Auswirkungen des Alters auf die Fertilität von Frauen und vor allem auch von Männern wünschenswert. Auch die Grenzen der Möglichkeiten und die rechtlichen und finanziellen Herausforderungen der assistierten reproduktiven Techniken sollten thematisiert werden. Nur mit ausreichend Wissen kann reproduktive Autonomie gefördert werden. Dabei können die Medien mitwirken und sowohl das individuelle als auch das gesellschaftliche Bewusstsein unterstützen.The dissertation’s goal was to examine how parenthood later in life is presented in the media.
As a first step, a descriptive literature analysis examined which factors could be used to justify the definition of age limits. For this purpose, social factors which aim at the role of the people or the child/the children within society were analyzed. The medicalization of reproduction, the design of life plans as well as the individual conception of infertility and the desire to have children were explored. Furthermore, medical ethical considerations about reproductive autonomy, justice, non-maleficence and beneficence were elaborated on. The function of parents and the effects of assisted reproductive technologies and late parenthood on the welfare of children were also examined. In a second step, biological arguments showing increased risks for mothers and children which could be due to late mother- or fatherhood were studied. One of the central results was that not only the mother’s age but also the father’s age leads to a higher risk for negative health effects.
In the results section, all the aspects mentioned were categorized as pro or contra arguments for or against late parenthood and/or assisted reproductive technologies. In terms of media theory, the differences between boulevard and quality media were presented and the importance of information was highlighted. Based on a sample of 221 articles with the focus on problem areas of human reproduction in Germany which, according to the articles, were based on the gender or the age of one or both parents, were studied by means of a qualitative content analysis as described by Mayring. The articles were from seven print media (Süddeutsche Zeitung, Frankfurter Allgemeine Zeitung, Der Spiegel, Die Zeit, Brigitte, Men’s Health, Bild), of which Süddeutsche Zeitung, Frankfurter Allgemeine Zeitung, Der Spiegel and Die Zeit were classified as quality media. Brigitte, Bild and Men’s Health were classified as boulevard media.
It was subsequently evaluated how the presentation of the age of women and men differed and which arguments boulevard and quality media used to justify age limits and if these were coherent with the arguments that were found during the literature research. The results showed that the age of women was mentioned more often. There was an accumulation of the age between 30 and 40 years and the age 35 was mentioned the most. The age of men was described less often and the age span 46-50 years was mentioned the most.
In respect to the arguments used there were no discrepancies between quality and boulevard media: both media genres provided balanced information and named both pro and contra arguments – with a slight dominance of the quality media over the boulevard media in regard to the range of the contra arguments mentioned. Stereotyping happened based on non-numerical age labels (“Grandma-Mum”, “Grandpa-Dads”). This stereotyping affected both genders and both media genres, unlike it was initially presumed based on the described pre-known “double standard of aging“.
For the future an increase in well-founded and balanced media reporting about the effect of age on the fertility of women and of men as well would be desirable. The issue of the limitations of the possibilities and the legal and financial challenges of assisted reproductive technologies should also be broached. Autonomy can only be furthered with sufficient knowledge. The media can help with this and can promote both the individual as well as the society’s awareness.2025-02-2
Analyse der Phagozytoseaktivität in humanem Vollblut gesunder Probanden unterschiedlicher Altersgruppen und an Clostridioides difficile-Enteritis erkrankter Patienten
This dissertation deals with the analysis of phagocytosis activity in human whole blood from healthy volunteers of different age groups and patients suffering from Clostridioides difficile enteritis. For this purpose, two assays were established to quantify phagocytosis activity by simulating bacteremia.
It was shown that older test subjects had a limited ability to eliminate phagocytosed bacteria. In addition, we were able to show that people with Clostridioides difficile enteritis had significantly reduced phagocytosis activity compared to subjects adjusted for age and gender.
In a further experiment, we used flow cytometry to demonstrate the dependence of phagocytosis activity - including that of leukocyte subpopulations - on the concentration of the bacteria to be phagocytosed. Clear differences between young and old test subjects could not be demonstrated here.
With advancing age and the associated increased susceptibility to infectious diseases, the limitations of the innate immune defense increase. An analysis of the limited abilities may provide options for treatment in the future.2025-03-1
Finding Path Towards AI-based Services in the Finance Industry
The ongoing revolution toward AI-based services in corporate financial services presents immense opportunities for automation, efficiency gains, and improved accuracy in areas such as credit scoring, financial statement analysis, and risk assessment. However, this transformation brings numerous structural, technical, organizational, and regulatory challenges. Financial institutions must navigate the complexities of AI implementation while balancing innovation with stringent regulatory requirements, such as those outlined in the European AI Act and the Digital Operational Resilience Act (DORA). These regulations emphasize transparency, monitoring, resilience, and accountability, requiring organizations to ensure secure, compliant, and fair AI deployments. Key challenges include mitigating algorithmic biases, addressing data imbalances, and developing robust frameworks for AI model monitoring and maintenance. Corporate financial applications of AI must not only outperform traditional methods in cost-effectiveness and predictive accuracy but also comply with evolving regulations that distinguish between the needs of private and corporate customers. Additionally, imbalanced datasets, particularly in applications like corporate credit scoring, can lead to biased outcomes. This necessitates advanced preprocessing techniques and continuous monitoring to ensure fairness, reliability, and accuracy. Given the dynamic and heavily regulated nature of the financial sector, AI adoption requires structured methodologies, strategic documentation, and proactive collaboration among financial institutions, regulators, and technology providers. This dissertation critically examines the 'problem triangle,' encompassing AI research, regulatory demands, and industry-specific requirements. It aims to provide scientific and practical recommendations for addressing the technical, structural, and regulatory barriers to AI adoption. By fostering transparency, resilience, continuous monitoring, and operational efficiency, the research seeks to help financial institutions unlock AI’s transformative potential while maintaining regulatory compliance and customer trust.2025-02-1
Culinary Medicine – A new teaching approach to improve medical students’ counselling skills for nutrition-associated diseases. Review and exploratory pilot study at Göttingen medical school
Global health is increasingly impaired by the burden of Non-Communicable Diseases. NCDs like cardiovascular and metabolic diseases or cancer are often associated to peoples’ nutrition. Either the disease’s pathology is based on an unhealthy diet and/or the therapy contains nutrition related changes besides other behavior adjustments. To counter this burden of diseases, health workers and especially physicians should be more attentive to intervene and counsel patients for a healthy diet. Physicians are predestined to act as role model for their patients and to initiate the nutrition therapy process.
To sensitize doctors as early as possible, the new teaching approach Culinary Medicine was implemented in many of the US medical Schools over the last years. This evidence-based field combines nutrition science and nutrition therapy with hands-on cooking classes, where theoretical knowledge can be practical implemented. Several studies from different medical schools with different curricula showed that Culinary Medicine classes seem feasible for improving medical students’ counselling skills in nutrition topics, their personal cooking and dietary behavior and their attitudes towards nutrition in medicine.
Due to the huge success of various CM curricula in the US, the German medical school of the Universitätsmedizin Göttingen decided to implement their own CM curriculum on the theoretical basis of the LEKuP, a German guideline for nutrition therapy in in-and outpatient care. First running of the elective course was conducted in the winter semester 2020/2021. To identify the impact of participation in the CM elective, a pre- and post-survey was filled out by the students. Evaluation included personal cooking and dietary behavior, attitudes towards nutrition in medicine, counselling competencies, theoretical knowledge about the LEKuP and the WHO-5 Well-Being Index. First evaluation results showed significant improvement of medical students’ counselling competencies and knowledge about the LEKuP.
Further course implementation with more participants and in more medical institutions is needed to confirm those prior findings in German medical schools. A multisite evaluation in cooperation with the medical schools of Gießen and Brandenburg is already in progress.
The long-term objective of this new teaching concept is to improve medical students’ competences in consulting behavior and nutrition topics and therefore, sharpen the view for the need of interventions concerning nutrition related diseases. In this way, aspiring physicians can transfer their theoretical nutrition knowledge into lifestyle interventions that are relevant to patients’ everyday life. In addition, the initiation of a multidisciplinary therapy process for personal nutrition counselling including other health care professionals like dieticians or nutrition specialist will take place more often. As a consequence, interprofessional cooperation and collaboration is intensified for the benefit of positive disease outcome. This contributes to the optimization of holistic patient care and conquers the burden of nutrition-associated diseases.2025-04-2
The influence of the stem cell factor KLF4 on TGFβ activity in CHD1-depleted prostate cancer
Im Verlauf des Prostatakarzinoms entwickelt sich häufig eine Kastrationsresistenz, die mit einer schlechteren Prognose und erhöhten Mortalitätsraten einhergeht. Eine Mutation des chromatin-remodelers CHD1 spielt eine entscheidende Rolle in diesem Prozess, da diese möglicherweise zu einem verminderten Ansprechen auf TGFβ und einer reduzierten Apoptose führt. Es wurde festgestellt, dass die Expression des TGFβ-Inhibitors SMAD7 abhängig vom CHD1-Status unterschiedlich reguliert wird. Während in Zellen mit CHD1-Wildtyp eine TGFβ-Stimulation zu einem Anstieg der TGFβ-Targets führt, geschieht dies in Zellen mit CHD1-Depletion überwiegend nicht. Ein Knockdown des Stammzellfaktors KLF4 in Kombination mit TGFβ-Stimulation konnte die Expression der TGFβ-Targets wiederherstellen. Im vorliegenden Kontext wird dem TGFβ-System eine tumorhemmende und KLF4 eine tumorfördernde Rolle zugesprochen. Diese Ergebnisse legen nahe, dass die gezielte Beeinflussung von KLF4 oder SMAD7 potenzielle therapeutische Ansätze zur Unterdrückung des Tumorprogresses im fortgeschrittenen Prostatakarzinom darstellen könnten. Weitere Forschung ist erforderlich, um die Auswirkungen von KLF4 auf Migration, Proliferation und Metastasenbildung zu untersuchen.In the course of prostate cancer, castration resistance often develops, which is associated with a poorer prognosis and increased mortality rates. A mutation of the chromatin remodeler CHD1 plays a crucial role in this process, as it may lead to a reduced response to TGFβ and reduced apoptosis. It was found that the expression of the TGFβ inhibitor SMAD7 is regulated differently depending on CHD1 status. While TGFβ stimulation leads to an increase in TGFβ targets in cells with CHD1 wild type, this predominantly does not occur in cells with CHD1 depletion. Knockdown of the stem cell factor KLF4 in combination with TGFβ stimulation was able to restore the expression of TGFβ targets. In the present context, the TGFβ system is thought to play a tumor- suppressive role and KLF4 a tumor-promoting role. These results suggest that targeting KLF4 or SMAD7 may represent potential therapeutic approaches to suppress tumor progression in advanced prostate cancer. Further research is needed to investigate the effects of KLF4 on migration, proliferation and metastasis.2025-04-0
Adhäsionsverhalten von Multispezies-Biofilmen aus Aggregatibacter actinomycetemcomitans, Staphylococcus epidermidis und Streptococcus sanguinis auf Titanoberflächen in vitro
Objective: Biofilm-associated peri-implant diseases are a major cause of the loss of osseointegrated dental implants. The accumulation of oral biofilms is primarily influenced by the surface topography of biomaterials particularly their roughness. The microbial spectrum of peri-implantitis is variable, with certain bacterial species being strongly associated with peri-implant inflammations. While the adhesion of mono-species biofilms in relation to surface characteristics is widely described in the literature, studies involving multi-species biofilms are rare. The changes in the species composition of multi-species biofilms depending on the roughness of titanium surfaces have not yet been investigated. The present study aims to investigate the adhesion behavior of multi-species biofilms composed of the non-pathogenic early colonizer Streptococcus sanguinis and the peri-implant disease-associated bacteria Aggregatibacter actinomycetemcomitans and Staphylococcus epidermidis.
Materials/Method: Four titanium surfaces were compared: two clinically used titanium implant surfaces (Hager & Meisinger GmbH, Neuss, Germany and Semados titanium implants, BEGO GmbH & Co KG, Bremen, Germany) and two laboratory-prepared titanium surfaces (super-polished and sandblasted). The titanium surfaces were characterized using confocal and scanning electron microscopy. A. actinomycetemcomitans and S. epidermidis were cultured together with S. sanguinis in a multi-species biofilm on the four titanium materials. The cultivated mono- and multi-species biofilms were analyzed using fluorescence and scanning electron microscopy. Biofilm formation was determined based on the amount of total DNA extracted from the bacteria. The relative composition of the multi-species biofilms was analyzed using real-time PCR.
Results: The average roughness (Ra) value for implant surface I (Hager & Meisinger) was 1.325 µm and for implant surface II (BEGO) was 1.405 µm. The Ra values for the super-polished (0.062 µm) and sandblasted (0.505 µm) surfaces were lower. The free surface energy (average values: super-polished: 55.430 mN/m, sandblasted: 63.934 mN/m, implant surface I: 65.214 mN/m, implant surface II: 59.006 mN/m) was measured using contact angle measurement. Fluorescence and scanning electron microscopy images showed successful microbial colonization by single- and multi-species cultures on all test surfaces. Statistical analysis of DNA concentrations and Ct values revealed no significant differences between the test materials.
Conclusion: The present in vitro study found no significant impact of the surface topography of the investigated implant surfaces on the quantity of biofilm accumulation and the composition of bacterial multi-species biofilms. The clinical applicability of these findings must be verified through future in vivo studies.2025-04-0
NFATc1 and CRAC channel signaling in acute pancreatitis
Abstract
Acute pancreatitis (AP) is an inflammatory disorder of the pancreas which is associated with an overall mortality of up to 5%. As its prevalence rises, the burden on the healthcare system and patients becomes increasingly more substantial, especially considering that no curative treatments are currently available.
The triggers of AP are very diverse, including bile acid reflux, abuse of alcohol, and cigarette smoking. Diverging disease causes complicate the development of uniform therapy approaches and research into the molecular pathology. However, disruption of the calcium (Ca2+) homeostasis in the exocrine pancreatic acinar cells (PACs) has been identified as a central and mostly cause-independent pathological driver. Ca2+ hereby accumulates in the cytoplasm of PACs and drives acinar cell damage by disrupting mitochondrial function, blocking ATP production, and ultimately resulting in cell death. As such, targeting Ca2+ signaling has emerged as an attractive therapeutic target, with the Ca2+ channel inhibitor CM4620 currently enrolled in phase II clinical trials. Similarly, investigating the role of Ca2+ downstream effector proteins is becoming increasingly more interesting in hopes of identifying targetable proteins in the transcriptional regulation of Ca2+-originated signaling pathways. Hereby, we investigated the role of the Ca2+ activated transcription factor nuclear factor of activated T-Cells c1 (NFATc1). NFATc1 regulates diseases severity by driving cytokine production with subsequent recruitment of immune cells, as well as acinar cell damage by regulation of Ca2+ influx and mitochondrial fitness. Notably, we demonstrate a novel regulatory relationship between the entry of extracellular Ca2+ and NFATc1 activity. Activation of NFATc1 requires an influx of Ca2+ and can be blocked by inhibiting Ca2+ influx. We demonstrate that in addition, NFATc1 regulates the expression of STIM1, a Ca2+ channel component, thereby facilitating Ca2+ entry and self-activation. This regulatory relationship promotes a pathological positive feedback loop in the context of AP by driving acinar cell damage and death through increased intracellular Ca2+ signaling activity. We explore the genetic deletion of NFATc1, as well as the use of CM4620 in order to inhibit AP progression driven by the aforementioned feedback loop and establish NFATc1 facilitated Ca2+ signaling and acinar cell damage as a novel central driver of AP pathogenesis.2025-07-1
Mechanistische Einblicke in die zelluläre Elektrophysiologie des postoperativen Vorhofflimmerns
Tritt Vorhofflimmern erstmals im Kontext einer vorangegangenen Operation auf, so wird dies
als postoperatives Vorhofflimmern (poAF) bezeichnet. Es stellt eine häufige perioperative
Komplikation dar und erhöht nicht nur unmittelbar die Morbidität und Mortalität, sondern auch
massiv das Risiko, ein Vorhofflimmerrezidiv zu entwickeln.
Die Pathophysiologie des poAFs ist nur unvollständig bekannt. Es wird angenommen, dass
postoperative Trigger wie autonome Dysfunktion oder Inflammation auf ein vulnerables atriales
Substrat treffen müssten, um poAF zu initiieren. Dann kann es innerhalb des Atriums zur Entwicklung von kreisenden Erregungen (englisch: re-entry) und ektopen Erregungen (frühen oder
späten Nachdepolarisationen) kommen. Unter Alternans wird der Wechsel der Form von Aktionspotential und Ca2+-Transient von Schlag zu Schlag zwischen zwei kontrastierenden
Zuständen verstanden. Alternans innerhalb des Atriums könnte zur elektrischen Heterogenität
und damit zur Erleichterung von kreisenden Erregungen beitragen – seine Beteiligung bei poAF
ist hingegen bis dato nicht bekannt. Es existierten bislang keine Studien, die sich auf die arrhythmogenen Mechanismen wie frühe Nachdepolarisationen (EAD), frühe Nachdepolarisationen
der späten Phase 3 (LP3EAD), späte Nachdepolarisationen (DAD) und Alternans bei Gewebeproben von poAF-Patienten fokussierten. Aus diesem Grund wurden diese an isolierten atrialen
Myozyten von Patienten, die sich einer kardialen Operation unterzogen, überprüft. Die Myozyten wurden isoliert und anschließend mit dem Ca2+-Farbstoff Fluo-3 beladen. Gleichzeitige
current-clamp sowie Epifluoreszenz-Messungen ermöglichten die zeitgleiche Aufzeichnung von
Aktionspotentialen und Ca2+-Transienten unter ansteigenden Frequenzen bis 8 Hz. Als Kontrollgruppe fanden Myozyten von Patienten im Sinusrhythmus (SR) Verwendung.
Bei poAF ließ sich im Gegensatz zu chronischem Vorhofflimmern kein elektrisches remodelling
erfassen. Verändert waren demgegenüber die Zeitkonstante τ des Ca2+-Transienten-Abfalls sowie über alle Frequenzen betrachtet die Amplitude der Ca2+-Transienten. Unter
Zusammenschau dieser Ergebnisse und der meiner Arbeitsgruppe kann auf eine verringerte
Ca2+-ATPase des sarkoplasmatischen Retikulums (SERCA2a)-Funktion bei poAF geschlossen
werden. Diese erscheint ebenfalls ursächlich für die erhöhte Vulnerabilität zu Aktionspotential-
und Ca2+-Transienten-Alternans bei poAF zu sein. Bestärkt wurde dieser Zusammenhang durch
eine Simulationsstudie, die unter verringerter SERCA2a-Funktion ebenso Alternans an atrialen
Myozyten prognostizierte. Im Gegensatz zum chronischen Vorhofflimmern stellte sich bei
poAF keine erhöhte Neigung zu DAD dar.
Autonome Dysfunktion wurde durch eine Superfusion mit dem β-adrenergen Pharmakon Isoprenalin simuliert. Infolgedessen ließ sich eine Anhebung des Ruhemembranpotentiales, eine Zunahme der Ca2+
-Transienten-Amplitude und ein schnellerer Abfall des Ca2+-Transienten gleichermaßen bei SR und bei poAF registrieren. Unverändert blieb dagegen die Aktionspotentialdauer bei 90% Repolarisation unter β-adrenerger Stimulation. Für die adrenerge Stimulation wurden in der Literatur sowohl Alternans fördernde als auch protektive Effekte beschrieben, abhängig davon, ob die adrenerge Stimulation tendenziell eher die Ca2+-Freisetzung aus dem sarkoplasmatischen Retikulum oder die Ca2+
-Sequestration fördere. Das simulierte Überwiegen des Sympathikus hatte bei poAF im Rahmen meiner Versuche keine signifikante Zunahme der Alternansneigung zur Folge. Eine erhöhte Vulnerabilität offenbarte sich jedoch bei den SR- Myozyten. Ein potentieller Erklärungsansatz besteht darin, dass die sympathische Stimulation zu einer gesteigerten Ca2+-Sequestration vor dem Hintergrund einer bei poAF erniedrigten SERCA2a führe und es somit nicht zu einer weiteren Zunahme des Auftretens von Alternans käme. Bei den SR-Myozyten scheinen hingegen die Alternans fördernden Effekte zu dominieren. Die Verwendung von Isoprenalin war verbunden mit einem signifikant häufigeren
Auftreten von EAD bei poAF. Es zeigten sich zudem gehäuft DAD, wenn auch nicht mehr als
bei SR. Ein gemeinsamer Pathomechanismus kann in diesem Zusammenhang angenommen
werden. Die proarrhythmische Wirkung der sympathischen Stimulation erscheint bei poAF primär getragen zu sein von Nachdepolarisationen und weniger von Alternans. LP3EAD wurden
als Mechanismus zur Reinduktion von Vorhofflimmern nach Kardioversion bekannt. Eine
gleichzeitige parasympathische und sympathische Stimulation sei zu ihrer Auslösung vonnöten.
Die Gabe von Isoprenalin und Applikation des Parasympathomimetikums Carbachol nach Stimulation bis 8 Hz war indessen in keinem Fall in der Lage, LP3EAD bei SR oder poAF auszulösen.
Die Inkubation atrialer Myozyten von SR-Patienten mit Tumornekrosefaktor-α diente der Simulation einer postoperativen Inflammation. Diese förderte eine erniedrigte Ca2+-Transienten-Amplitude zutage genauso wie eine größere Zeitkonstante τ des Ca2+-Transienten-Abfalls. Erste Hinweise konnten aus diesen Versuchen gewonnen werden, die eine erhöhte Vulnerabilität zu Alternans demaskierten und über die Effekte auf poAF-Myozyten spekulieren lassen.
Die Versuche an Myozyten von Patienten mit paroxysmalem Vorhofflimmern ergaben im Vergleich zur Literatur inkonsistente Ergebnisse. Deutlich wurde allerdings die starke Tendenz zu
vermehrten DAD. Vermehrtes Auftreten von Alternans konnte entgegen der Literatur bei Ver-
suchen in vivo nicht nachgewiesen werden. Hingegen präsentierten die Myozyten von Patienten
mit chronischem Vorhofflimmern ein klassisches elektrisches remodelling mit verkürzten Aktionspotentialdauern bei 90% Repolarisation. Darüber hinaus ließ sich ebenso wie für poAF eine
Vulnerabilität zu Alternans des Aktionspotentials und der Ca2+-Transienten ermitteln, sodass
100% der Myozyten Aktionspotential-Alternans aufwiesen. Alternans auf zellulärer Ebene
wurde erstmalig von mir an solchen Myozyten festgestellt. Wie vielfach in der Literatur beschrieben, wiesen diese Myozyten eine signifikant erhöhte Anzahl an DAD auf.
Zusammenfassend kann festgehalten werden, dass dem Alternans bei allen Formen des Vorhofflimmerns eine starke pathophysiologische Bedeutung zugeschrieben werden kann.
Alternans bei poAF kann auf eine verminderte SERCA2a-Funktion zurückgeführt werden. Unterschiedliche Berichte existieren dahingegen über die Funktionen dieser bei paroxysmalem und
chronischem Vorhofflimmern. Die zellulären Mechanismen des Alternans bei diesen Formen
bedürfen weiterer Forschung. Nachdepolarisationen, wie sie klassischerweise bei paroxysmalem
und chronischem Vorhofflimmern auftreten, finden sich bei poAF nur nennenswert unter β- adrenerger Stimulation. Diese waren primär EAD und konnten erstmals unter Isoprenalin bei humanen atrialen Myozyten nachgewiesen werden. Ihr Pathomechanismus ist derzeit noch unbekannt. LP3EAD sind in Anbetracht meiner Ergebnisse eher ein unwahrscheinlicher Mechanismus in der poAF-Genese.
PoAF stellt die häufigste perioperative Arrhythmie dar. Somit ist es notwendig, über effiziente
therapeutische sowie prophylaktische Strategien zu verfügen. Gezielte Therapieoptionen, die
ein günstiges Nebenwirkungsprofil aufweisen, bedürfen einer genauen Kenntnis der Pathophysiologie einer Erkrankung. Die vorliegende Arbeit kann hoffentlich dazu beitragen, dass künftig
passende Pharmaka für poAF vorhanden und ggf. klinische Screeningverfahren etabliert sind.Postoperative atrial fibrillation (poAF) refers to new-onset atrial fibrillation (AF) occurring in the setting of recent surgery. It is one of the most common perioperative complications, significantly increasing short-term morbidity and mortality while also greatly elevating the risk of future AF recurrence. Despite its clinical relevance, the pathophysiology of poAF remains incompletely understood. Current models suggest that postoperative triggers such as autonomic dysfunction or inflammation, act upon a vulnerable atrial substrate to initiate the arrhythmia. This can lead to the emergence of re-entrant circuits and ectopic activity, including early, late-phase 3 and delayed afterdepolarisations (EADs, LP3EADs and DADs). Additionally, alternans, which described the beat-to-beat alternation in action potential morphology and Ca²⁺ transient amplitude, is thought to promote electrical heterogeneity and facilitate re-entry. Although most of these mechanisms have been explored in in chronic AF, no prior studies have systematically investigated the potential contribution of these mechanisms i.e. EADs, LP3EADs, DADs and alternans to the arrhythmogenic substrate promoting poAF. To address this gap, we studied atrial myocytes isolated from atrial appendages obtained from patients undergoing cardiac surgery. Cells were enzymatically isolated, loaded with the Ca²⁺-sensitive dye Fluo-3, and examined using simultaneous current-clamp and epifluorescence measurements to record action potentials and Ca²⁺ transients at increasing stimulation frequencies up to 8 Hz. Myocytes from patients in sinus rhythm (SR) with no previous and postoperative documentation of AF served as controls.
PoAF myocytes showed no evidence of electrical remodelling, which is different from chronic AF where shortened action potential duration is the hallmark of electrical remodelling. In contrast, the susceptibility to action potential and calcium transient alternans was increased in poAF. Moreover, the onset of action potential alternans was detected at a significantly lower threshold frequency. Furthermore, we observed a reduction in the decay time constant (τ) and amplitude of the Ca²⁺ transient across all frequencies. Together with previous work from our group, these findings suggest reduced function of the sarcoplasmic reticulum Ca²⁺ ATPase (SERCA2a) in poAF. This impaired SERCA2a activity likely underlies the increased vulnerability to both action potential and Ca²⁺ transient alternans observed in poAF. Simulation studies further supported this link by predicting alternans under conditions of reduced SERCA2a function. We further probed the vulnerability of poAF myocytes to afterdepolarisations. Interestingly, unlike chronic AF, poAF myocytes did not display an increased propensity for DADs. To model autonomic dysfunction, we stimulated the atrial myocytes using the β-adrenergic agonist isoprenaline. Upon application, isoprenaline depolarised the resting membrane potential, increased Ca²⁺ transient amplitude, and accelerated transient decay in both SR and poAF atrial myocytes. Notably, action potential duration at 90% repolarisation remained unchanged under β-adrenergic stimulation. Previous studies have reported both pro-arrhythmogenic and protective effects of adrenergic stimulation, which is dependent on whether it primarily enhances sarcoplasmic reticulum Ca²⁺ release or sequestration. In our experiments, sympathetic stimulation did not increase the tendency for alternans in poAF myocytes. However, SR myocytes exhibited heightened susceptibility to alternans upon sympathetic stimulation. This difference may reflect the already compromised SERCA2a function in poAF, such that the enhanced sequestration through sympathetic stimulation might only partially compensate and prevent further increases in alternans. Furthermore, Isoprenaline exposure was associated with a significantly higher incidence of EADs in poAF. Although DADs also increased under these conditions, their frequency did not exceed that seen in SR controls. LP3EADs have also been proposed as a mechanism for AF reinduction after cardioversion, requiring combined parasympathetic and sympathetic stimulation. In our experiments, however, neither isoprenaline alone nor in combination with the parasympathomimetic carbachol (at up to 8 Hz pacing) was able to elicit LP3EADs in SR or poAF myocytes. Overall, the pro-arrhythmic effects of sympathetic stimulation in poAF appear to be primarily driven by afterdepolarisations rather than by alternans.
Lastly, we investigated the contribution of alternans to other forms of AF including paroxysmal and chronic AF. Experiments on myocytes from patients with paroxysmal AF produced results that were partly inconsistent with previous reports. While we observed a clear increase in DAD occurrence, we did not detect the elevated incidence of alternans often described in vivo. On the other hand, myocytes from patients with chronic AF displayed classic electrical remodelling, characterised by significantly shortened action potential duration. These cells also showed a pronounced vulnerability to alternans in both action potentials and Ca²⁺ transients, with 100% of cells demonstrating action potential alternans. Our findings for the first time demonstrate such cellular-level alternans in human atrial myocytes. Consistent with the literature, these myocytes also exhibited a significantly higher frequency of DADs.
In conclusion, alternans appears to play a key pathophysiological role across all forms of AF. In poAF, alternans is likely driven by reduced SERCA2a function. In contrast, the role of SERCA2a in paroxysmal and chronic AF remains debated and warrants further investigation. While afterdepolarisations are a well-recognised feature of paroxysmal and chronic AF, they appear in poAF only under β-adrenergic stimulation. These were predominantly EADs and were demonstrated for the first time in human atrial myocytes under isoprenaline exposure. Their precise mechanisms remain unclear. Based on our findings, LP3EADs seem unlikely to play a major role in poAF development. Given that poAF is the most common perioperative arrhythmia, there is an urgent need for effective therapeutic and prophylactic strategies. Targeted treatments with favourable safety profiles require a detailed understanding of the underlying pathophysiology. It is hoped that this work will contribute to the development of appropriate pharmacological interventions and potentially inform future clinical screening approaches for poAF.2025-09-2
Investigation of the course of patient-related immediate laboratory analyses after electroconvulsive therapy

Einleitung/Hintergrund:
Die Elektrokonvulsionstherapie (EKT) ist ein etabliertes Verfahren zur Behandlung schwerer affektiver und psychotischer Störungen. Vor dem Hintergrund jahrzehntelanger klinischer Anwendung sowie großer epidemiologischer Datensätze steht die generelle Sicherheit der EKT nicht in Frage. Es existieren jedoch nur wenige Untersuchungen zu kurzfristigen Veränderungen z.B. des Säure-Basen-Haushalts und der Elektrolytkonzentrationen nach einer EKT. Im Rahmen der vorliegenden Arbeit wurden daher mittels serieller venöser Blutgasanalyse Veränderungen dieser Parameter systematisch erfasst und bezogen auf sicherheitsrelevante Aspekte evaluiert.
Methodik:
Patient*innen mit klinisch indizierter Akut- oder Erhaltung-EKT wurden in diese Studie eingeschlossen. Zu Erfassung kurzfristiger physiologischer Veränderungen wurden periphere venöse Blutgasanalysen unmittelbar vor, direkt nach sowie 5, 15, 30 und 60 Minuten nach EKT durchgeführt. Mittels eines Blutgasanalysegerätes wurden u.a. pH-Wert, Basenüberschuss, pvCO2, Bicarbonat- sowie Natrium-, Kalium-, Chlorid- und Kalzium-Konzentrationen in Blut analysiert. Die statistische Auswertung erfolgte mit Hilfe einer Varianzanalyse für Messwiederholungen (ANOVA).
Ergebnisse:
47 Patient*innen (33 weiblich, Altersdurchschnitt 56 ± 10 Jahre) nahmen an der Untersuchung teil. Es zeigten sich für fast alle untersuchten Parameter signifikante Veränderungen im Zeitverlauf nach der EKT. Besonders Parameter des Säure-Basen-Haushalts änderten sich tlws. deutlich. So sank der pH-Wert im Mittel von 7,39 auf 7,29 unmittelbar nach EKT und normalisierte sich innerhalb der darauffolgenden Stunde weitgehend ((F(3.4;156.4) =105.5; p<0.001; n=47). Es zeigten sich dazu korrespondierende signifikante Anstiege der Laktatkonzentration sowie des pvCO2- und Abnahmen der Bikarbonatkonzentration sowie des Basenüberschusses. Keine relevanten Änderungen wiesen die Natrium- und Chlorid-Konzentrationsverläufe auf. Während Hämatokrit und Hämoglobin-Werte über die einstündige Nachbeobachtung nach EKT leicht abfielen, zeigte sich nach EKT ein geringfügiger Anstieg der Blut-Glucose-Konzentration.
Schlussfolgerung:
Die EKT kann signifikante, jedoch vorübergehende und klinisch vermutlich nur wenig relevante respiratorische und/oder metabolische Azidosen verursachen. Veränderungen der Elektrolyt- und Glucose-Konzentrationen sowie des Hämoglobin- und Hämatokrit-Wertes waren marginal und mutmaßlich klinisch wenig relevant. Die Befunde unterstreichen jedoch die Bedeutung einer strukturierten klinischen Überwachung unmittelbar nach der EKT, insbesondere bei vorbestehenden somatischen Risiken, wie z.B. bei Vorliegen von Herz-Kreislauf- und/oder Lungen-Erkrankungen.Investigation of the course of patient-related immediate laboratory analyses after electroconvulsive therapy

Introduction/Background:
Electroconvulsive therapy (ECT) is a well-established intervention for the treatment of severe affective and psychotic disorders. Given its decades-long clinical use and extensive epidemiological data, the overall safety of ECT is well supported. However, there is a paucity of research examining immediate physiological changes, such as alterations in acid-base balance and electrolyte concentrations, following ECT. This study systematically investigated these parameters through serial venous blood gas analyses and evaluated them in terms of clinical safety.
Methods:
Patients receiving clinically indicated acute or maintenance ECT were included in the study. To assess short-term physiological changes, peripheral venous blood gas analyses were conducted immediately before, directly after, and at 5, 15, 30, and 60 minutes post-ECT. Using a blood gas analyzer, measurements included pH, base excess, pvCO₂, bicarbonate, as well as sodium, potassium, chloride, and calcium concentrations. Statistical analysis was performed using repeated measures analysis of variance (ANOVA).
Results:
A total of 47 patients (33 female, mean age 56 ± 10 years) participated in the study. Almost all measured parameters showed significant changes over time following ECT. In particular, acid-base parameters demonstrated marked alterations. The mean pH decreased from 7.39 to 7.29 immediately post-ECT and largely normalized within the subsequent hour (F(3.4;156.4) = 105.5; p < 0.001; n = 47). These changes were accompanied by significant increases in lactate and pvCO₂, and decreases in bicarbonate and base excess. Sodium and chloride levels remained stable. Hematocrit and hemoglobin values showed a slight decline over the 60-minute observation period, while blood glucose exhibited a modest increase following ECT.
Conclusion:
ECT can induce significant, yet transient and likely clinically insignificant respiratory and/or metabolic acidosis. Changes in electrolyte and glucose concentrations, as well as hemoglobin and hematocrit levels, were minor and presumably of limited clinical relevance. Nevertheless, the findings emphasize the importance of structured clinical monitoring immediately after ECT, especially in patients with pre-existing somatic risks, such as cardiovascular and/or pulmonary conditions.2025-10-2
