Computertomographie bei COVID-19 Pneumonien und pulmonalen Non-COVID Erkrankungen: Diagnostische Genauigkeit der radiologischen Beurteilung und maschineller Lernverfahren
Since early 2020, the novel Covid-19 pneumonia has been spreading worldwide and poses a diagnostic challenge. Medical imaging techniques such as computed tomography (CT) play an important role in diagnosing Covid-19. CT-based diagnosis, generates a large number of CT images, whose interpretation by radiologists is both time-consuming and subject to subjective assessment.
In the present study, the diagnostic performance of radiologists in distinguishing between Covid and non-Covid patients using the CO-RADS and COV-RADS classification systems is compared with that of machine learning (ML) algorithms, in order to evaluate whether the diagnostic efficiency of radiologists in clinical routine can be improved by ML algorithms. Included were patients who tested PCR-positive for SARS-CoV-2 between March 1, 2020, and January 31, 2021 (Covid cohort), or who, between 2001 and 2020, had a differential diagnosis relevant to Covid-19 (non-Covid cohort) and therefore received a CT scan. The non-Covid cohort covers a wide range of differential diagnoses, such as pneumonias of other etiologies, interstitial lung diseases (ILD), vasculitides, malignancies, COPD, cystic fibrosis, tuberculosis, and cardiovascular diseases.
In this study, the CT scans were systematically assessed by radiologists in a blinded manner and evaluated using CO-RADS and COV-RADS. The resulting data were extracted as semantic annotations for training the ML algorithms. After data preprocessing, addressing class imbalances, and performing feature selection, ML algorithms were implemented to predict Covid-19 pneumonia using 10-fold cross-validation. Both the diagnostic performance of the radiologists (based on CO-RADS and COV-RADS) and the ML algorithms were evaluated as area under the curve (AUC).
A total of n=237 individuals (75% male, mean age 60 ± 18 years) with n=74 Covid-19 and n=163 non-Covid cases from the patient collective of the University Medical Center Göttingen were included retrospectively. First, the entire cohort was analyzed; subsequently, a smaller subgroup (n = 139) was examined in more detail, which, in addition to Covid-19 pneumonias, also included pneumonias of other etiologies, ILDs, and vasculitides.
In the overall cohort analysis, the radiologists achieved diagnostic accuracies of AUC=0.74 and AUC=0.73 using CO-RADS and COV-RADS, respectively.
The best-performing ML algorithm, the Gradient Boosting Classifier (GBC), achieved an AUC=0.75 under feature selection. In the subgroup analysis, the radiologists achieved AUC=0.61 and AUC=0.62 with CO-RADS and COV-RADS, respectively, while the best-performing GBC achieved an AUC=0.67 under feature selection. The diagnostic difference between radiologists and ML algorithms was not significant in either the overall cohort analysis or the subgroup analysis.
In summary, both the diagnostic performance of radiologists based on CO-RADS and COV-RADS, as well as the ML algorithms, for detecting Covid-19 pneumonia is moderate. Differentiating Covid-19 pneumonia within the subgroup analysis appears particularly complex. The results indicate that there is no statistically significant difference in the diagnostic accuracy of the ML algorithms compared to the radiologists. Thus, the information processing of the available CT image features in Covid-19 pneumonia by radiologists seems to be adequate.2025-03-1
The impact of NOTCH receptors on the radiochemotherapy sensitivity of colorectal cancer cells
The treatment of colorectal cancer remains a challenge to this day. Despite numerous advances in therapy, the prognosis, especially in advanced or metastatic stages, remains poor. A current standard therapy for locally advanced rectal carcinomas is combined radiochemotherapy followed by surgical resection. The response to this neoadjuvant therapy is highly heterogeneous, ranging from complete remission to complete resistance. One promising approach is to resensitize tumor cells to radiochemotherapy by manipulating cell signaling pathways. In previous work, our research group was able to identify the NOTCH signaling pathway as a potential target. RNAi-based silencing of the NOTCH transcription factor RBPJ led to increased rectal cancer cell lethality under radio(chemo)therapy. In this thesis, the role of NOTCH receptors 1, 2, and 3 in the response to radio(chemo)therapy was evaluated. To this end, the respective receptors were silenced using siRNA in colorectal cancer cells. Subsequently, the sensitivity of the cells to radio(chemo)therapy was determined. The results showed that in cells with high NOTCH receptor expression, receptor inhibition led to significant resensitization to radio(chemo)therapy in all but one case. In the exception, increased resistance to radiotherapy was observed. These findings highlight the importance of NOTCH receptors in mediating radio(chemo)resistance in colorectal cancer, as well as the need for a differentiated approach regarding the role of the individual receptors.2026-01-2
Neuronal-exosomes biomarkers from plasma of neurodegenerative disease patients
The dementia group of diseases share some features such as the degeneration of neurons, which leads to progressive and irreversible loss of cognitive functions. Since almost 10 years, dementia syndromes were declared a common public health problem because over 131 million patients will be diagnosed with dementia by 2050. The aetiology of these pathologies is based on protein misfolding/aggregates, hence, protein misfolding has recently emerged as a new field, in which the prion protein, Tau, α-synuclein, and Aβ are studied. Furthermore, some researchers are considering “prion-like” pathologies, i.e. Alzheimer's disease, Parkinson’s disease, both as the most common ones, even other disorders like Huntington’s Disease (HD) and Amyotrophic Lateral Sclerosis (ALS) as prion-like pathologies. The illness duration of Alzheimer’s disease and/or Parkinson’s disease is approximately 5-10 years.
There is another group of neurodegenerative disorders, in which the disease duration is just weeks or a few months, these are known as rapidly progressive dementias. One of the first differential diagnostics is made on prion diseases, which Sporadic Creutzfeldt-Jakob disease is the most prevalent form affecting approximately 90% of the cases. A difficult challenge for the physicians is to distinguish between the different rapidly progressive dementias. Even though, the implementation of new diagnostic tools and technologies in the recent years have allowed physicians to perform a more accurate differential diagnosis, the lack of diagnostic tools is still an issue.
Extracellular vesicles are defined by the International Society of Extracellular Vesicles as the vesicles released from the cell. There are three types of them: Apoptotic bodies, Microvesicles/Ectosomes and exosomes. Exosomes are the smallest ones among them, with a size of ∼40–160 nm. The fact that exosomes are released from many different types of cells and found in almost all body fluids, make them approachable for diagnostic purposes. In neurodegenerative disease, neuronal-derived exosomes can be collected from blood, CSF, saliva and urine and several potential biomarkers were discovered. However, there is a lack of an effective, rapid method to isolate neuronal-derived exosomes.
In the present study, we proposed a sensitive, reliable, and reproducible method for isolation of neuronal exosomes from blood samples. This method can also be a proficient and appropriate method for diagnostic laboratories working on neurodegenerative diseases. Our aim was to optimize a sensitive and reliable method to isolate neuronal exosomes from plasma sample, in special from healthy controls, Alzheimer's disease and Creutzfeldt-Jakob disease patients. At the end, we were able to evaluate suitability of this new protocol for diagnostic purposes in terms of time and cost.
Our cohort included 98 healthy control samples for the protocol optimization and 40 patients for the validation of neurodegenerative biomarkers.
To optimize the final nanowire-based protocol, several attempts were done to obtain the best yield. Techniques such as immunoprecipitation and commercial kit SmartSECTM were also performed to collect neuronal exosomes. Since we needed to validate this nanowire-based protocol according to the International Society of Extracellular Vesicles, the exosomes were measured by nanoparticle tracking analysis and characterized by electron transmission microscopy. Additionally, we proved our eluates against different exosome markers; 4 internal exosome markers (Alix, Annexin V, Flotillin 1 & TSG101) and 3 surface markers (CD9, CD63 & CD81) were found by immunoblotting.
After we collected the neuronal derived exosomes from plasma, we performed ELISA of degenerative CSF biomarkers. We measured the levels of P-Tau (181P), Total Tau, Aβ 1-40, Aβ 1-42, SFPQ and also performed a preliminary mass-spectrometry analysis with these neuronal exosomes. Total-Tau, P-Tau (181P-S199), SFPQ, PrP, Aβ 1-40, were successfully measured by this method. The preliminary mass-spectrometry analysis showed a proteomic profile with 1250 identified proteins, in which 319 proteins showed statistical significance. The proteins with most statistical significance were Ras-related protein Rab-10 (RAB10), Heterogeneous nuclear ribonucleoprotein R (HNRNPR), and Cell division control protein 42 homolog (CDC42).
To conclude this new nanowire-based protocol targeting neuronal exosomes is an accurate, rapid, effective, standardized, and reproducible method. Nanowires must be coated with two neuronal markers (L1CAM and NCAM) and one exosomal surface marker (CD81).
Our next steps are: To increase the number of samples and measure them by ELISA, optimize downstream analysis such as RT-QuIC to detect aggregated-prone proteins and to confirm new possible biomarkers discovered from the mass spectrometry analysis by ELISA.2026-02-1
Phenotypic and functional characterization of the cutaneous immune response upon contact with sodium lauryl sulfate
Natriumlaurylsulfat (NLS) findet als sogenannte Irritanzkontrolle regelhaft Anwendung in der Epikutantestung bei der allergologischen Diagnostik von Kontaktallergien und begegnet uns im Alltag bspw. in Seifen, Reinigungsmitteln und Zahnpasta. Hierbei reagieren 22,1% aller Testpatienten im Pflastertest positiv auf NLS 0,25% aq. und zeigt dann auch signifikant häufigere Reaktionen auf andere Kontaktallergene, die Ursache hierfür bleibt jedoch unklar (Heetfeld et al. 2020; Schwitulla et al. 2014).
Da bisherige Untersuchungen meist davon ausgingen, dass es sich bei NLS um ein Irritanz handelt, obwohl der Pathomechanismus der NLS-spezifischen Reaktion noch nicht klar entschlüsselt wurde, untersuchte ich in meinem Promotionsprojekt diesen genauer. Es zeigte sich jedoch, dass die ermittelten Daten die Einordnung der im Mausmodell gewonnenen Ergebnisse in die Reaktionsausprägung beim Menschen schwierig gestaltete. Die Ergebnisse der NLS-spezifischen Kontakdermatitis waren wenig spezifisch und teilweise widersprüchlich zu bereits bekanntem Wissen. Während für die Krotonöl- und Oxazolon-Behandlung meist die zu erwartenden Ergebnisse ermittelt werden konnten.
Zur Analyse der Kontaktdermatitiden wurde zunächst eine Charakterisierung der Maximalreaktion des klassischen Irritanz Krotonöl, des klassischen Kontaktallergens Oxazolon und von NLS selbst ermittelt. Dies geschah im Mausmodell mittels topischer Behandlung mit Krotonöl, Oxazolon/DNFB und NLS. Bezüglich der einzelnen Reaktionen wurde die zeitliche Dynamik mit Ohrdickenmessung erfasst.
Hinzu kam eine Entnahme und Aufarbeitung der Ohren zum jeweiligen Schwellungsmaximum der Substanzen zur weiteren Analyse mittels Immunhistochemie, Durchflusszytometrie und quantitativer Echtzeit-PCR zur Charakterisierung der Immunantwort.
Bei den Versuchstieren wurde eine Fokussierung auf Wildtyp-Mäuse (B6) gesetzt, um eine Vergleichbarkeit mit Folgeexperimenten in verschiedenen Knockout-Stämmen zu gewährleisten. Es wurde gezeigt, dass die Hautreaktion in immundefizienten Mäusen im Vergleich zu gesunden Wildtyp-Mäusen deutlich reduziert war, während ein Hautbarrieredefekt keinen Einfluss auf die Reaktionsausprägung hatte.
Weder ein Sensibilisierungspotenzial gegenüber NLS noch eine systemische Wirkung von NLS auf die kutane Immunantwort konnte im Mausmodell nachgewiesen werden.
Die NLS-spezifische kutane Immunantwort ähnelt interessanterweise am ehesten der Vehikel-Kontrolle und unter den Testsubstanzen nach den gewonnene Erkenntnissen noch am ehesten dem Irritanz Krotonöl, wohingegen es zum Allergen Oxazolon die signifikantesten Differenzen zeigt.
Eine beispielhafte Hautstanze eines positiv auf NLS getesteten Patch-Test-Patienten rundet dieses Promotionprojekt ab, lieferte jedoch teilweise ebenfalls differente Ergebnisse zu den histologischen Ergebnissen aus dem Mausmodell.
Es bleibt also weitestgehend unklar, warum 22,1% aller im Patch-Test positiv auf NLS getesteten Patienten diese Reaktionsausprägung zeigen. Möglicherweise ist ein Mausmodell nicht dazu geeignet, um diese offenbar für den Menschen spezifischen NLS-Reaktionsausprägungen abzubilden und genauer untersuchen zu können.
Es wurden einige Hypothesen zu möglichen Gründen für eine positive kutane NLS-Immunreaktion im Pflastertest geliefert und diskutiert, welche zukünftige Forschungsprojekte inspirieren sollen.Sodium lauryl sulfate (SLS) is regularly used as a so-called irritancy control in patch testing in the allergological diagnosis of contact allergies and we encounter it in everyday life, for example in soaps, cleaning products and toothpaste. 22.1% of all test patients react positively to SLS 0.25% aq. in the patch test and then also show significantly more frequent reactions to other contact allergens, but the cause of this remains unclear (Heetfeld et al. 2020; Schwitulla et al. 2014).
Since previous studies mostly assumed that SLS is an irritant, although the pathomechanism of the SLS-specific reaction has not yet been clearly deciphered, I investigated this in more detail in my doctoral project. However, it turned out that the data obtained made it difficult to classify the results obtained in the mouse model in terms of the reaction in humans. The results of SLS-specific contact dermatitis were not very specific and partly contradicted existing knowledge. For croton oil and oxazolone treatment, the expected results were mostly determined.
To analyze the contact dermatitis, the maximum reaction of the classic irritant croton oil, the classic contact allergen oxazolone and SLS itself was first characterized. This was done in the mouse model using topical treatment with croton oil, oxazolone/DNFB and SLS.
With regard to the individual reactions, the temporal dynamics were recorded by measuring the thickness of the ears.
In addition, the ears were removed and processed at the respective swelling maximum of the substances for further analysis using immunohistochemistry, flow cytometry and quantitative real-time PCR to characterize the immune response. The test animals were focused on wild-type mice (B6) in order to ensure comparability with follow-up experiments in various knockout strains. It was shown that the skin reaction in immunodeficient mice was significantly reduced compared to healthy wild-type mice, while a skin barrier defect had no influence on the severity of the reaction.
Neither a sensitization potential to SLS nor a systemic effect of SLS on the cutaneous immune response could be demonstrated in the mouse model. Interestingly, the SLS-specific cutaneous immune response is most similar to the vehicle control and, according to the findings obtained, among the test substances, most similar to the irritant croton oil, whereas it shows the most significant differences to the allergen oxazolone.
An example skin punch from a patch test patient who tested positive for SLS rounds off this doctoral project, but also provides some different results to the histological results from the mouse model.
It therefore remains largely unclear why 22.1% of all patients who tested positive for SLS in the patch test show this degree of reaction. A mouse model may not be suitable for imaging and investigating in more detail these SLS responses that are apparently specific to humans.
Some hypotheses about possible reasons for a positive cutaneous SLS immune response in the patch test were provided and discussed, which should inspire future research projects.2025-04-0
Genetic drivers of metastatic colorectal cancer liver colonization
Metastasis is the leading cause of death from colorectal cancer (CRC), yet the genes and pathways driving CRC metastatic progression are poorly defined. Among the steps in the metastatic cascade, liver colonization is a key bottleneck and a promising target for clinical intervention. Using xenograft and syngeneic mouse models, we validated three hits from a previous genome-wide liver colonization screen as causal drivers of CRC metastasis in vivo: ITPR3, an intracellular calcium channel; RELB, a transcription factor in the non-canonical NFkb pathway; and UBR4, an E3 ubiquitin-protein ligase.
We subsequently focused on ITPR3 for further characterization and mechanistic follow-up. Analysis of publicly available transcriptomic data revealed increased ITPR3 expression in human liver metastases and modulation of upstream pathways toward enhanced ITPR3 signaling. Furthermore, genes within these pathways differentially regulated liver metastatic progression in vivo. Functional experiments showed that ITPR3 deficiency reduces early metastatic cancer cell survival in vivo and hinders CRC cell proliferation and colony formation under conditions of cellular detachment in vitro. Collectively, these findings suggest that ITPR3 promotes colorectal cancer metastasis by enabling CRC cells to survive transient substratum detachment during early liver colonization.2025-04-1
Die Wirkung von Alizarin, Calcein und dem Oberflächenmarker PKH2 auf das Proliferationsverhalten von Rothirsch-Geweihzellen
In einer experimentellen Studie sollte anhand von Zellkulturen die Wirkung von
Fluoreszenzfarbstoffen auf das Proliferationsverhalten von Rothirsch-Geweihzel
len überprüft werden. Die Untersuchung wurde in zwei Versuchsanordnungen
durchgeführt. In der ersten Anordnung wurden über 27 Tage vier Gruppen von
Zellen in 6-Well-Platten kultiviert. Die drei Verumgruppen wurden jeweils mit den
Fluoreszenzfarbstoffen Calcein, Alizarin-Komplexone und PKH2 als Zusatz zu
DME-Medium inkubiert. Eine Kontrollgruppe lief parallel zu jeder Verumgruppe
mit. Innerhalb der 27 Tage des Versuch wurden an 12 Tagen die Zellzahl be
stimmt, so dass für jede Gruppe 12 Serien als Tripletts kultiviert wurden, so dass
insgesamt 144 Wells im ersten Versuch mit Zellkulturen beimpft wurden. Die Zell
zahl wurde mit der MTT-Methode bestimmt, wobei die Zellzahl einer genau fest
gelegten Exstinktion des MTT-Tests zugeordnet wurde. Dazu kann eine Neu
bauer-Zählkammer zum Einsatz. Von jedem Well wurden 3 Extinktionen gemes
sen. Aus den 9 Messwerten pro Versuchstag wurden dann der Mittelwert berech
net und für die weitere Darstellung in den Grafiken benutzt. Zur Differenzierung
der Proliferation wurden auch die Berechnung der mittleren Generationszeit, die
Anzahl der Generationen und die kumulative Zellzahl herangezogen.
In der zweiten Versuchsanordnung wurde neben der Zellzahlbestimmung die
Calceinintensität in den fluoreszierenden Zellen gemessen, um einen Zusam
menhang zwischen der Proliferation und der Farbstoffbeladung ableiten zu kön
nen.
Als wesentliche Ergebnisse konnte festgestellt werden, dass die Farbstoffe einen
geringen Einfluss auf die Proliferation der Zellen haben, der mit zunehmender
Kultivierungszeit aber immer größer wird. Alle Farbstoffe haben in den ersten 8
bis 10 Tagen keinen Einfluss auf die Proliferation. Gegenüber der Kontrollgruppe
zeigt Alizarin die stärkste Hemmung der Proliferation, gefolgt von Calcein und
PKH2. Am deutlichsten wird dies durch die kumulativen Zellzahl, der mittleren
Generationszeit und der Anzahl der Generationen alle in Abhängigkeit von der
Kulturzeit vermittelt. Auch die Normalisierung der zeitabhängigen Zellzahl und
ihre Differenzenbildung mit der Kontrollgruppe vermittelt brauchbare Ergebnisse,
während der einfache Auftrag der Zellzahl gegen die Kulturzeit sich als unüber
sichtlich darstellt.
Im zweiten Versuchsansatz wurde lediglich Calcein über 44 Tage weiter unter
sucht, da Alizarin-Komplexone nur eine geringe Fluoreszenz und sich PKH2 für
den längeren Einsatz unter den vorliegen Versuchsbedingen nicht stabil gegen
das Ausbleichen zeigte. Calcein hingegen ließ sich sehr gut detektieren. Das Ma
ximum seiner Zellkonzentration konnte zwischen dem 18. und 28. Tag in der Kul
tur festgestellt werden. Es wurde jedoch nur von wenigen Zellen aufgenommen,
die ab dem 28 Tage zum Ende des Versuchs hin stark in ihrer Zahl abnahmen.
Aufgrund der vorliegenden Ergebnisse kann vermutet werden, dass insbeson
dere Alizarin und Calcein, die häufigsten Fluorochrome in Tierversuchen zu Kno
chenexploration, keinen wesentlichen Einfluss auf die Zellproliferation haben, da
sie zumeist im beschränkten Umfang und nur kurzfristig appliziert werden.In an experimental study, the effect of fluorescent dyes on the proliferation behavior of red deer antler cells was examined using cell cultures. The investigation was conducted in two experimental setups.
In the first setup, four groups of cells were cultured in 6-well plates over a period of 27 days. The three test groups were incubated with the fluorescent dyes Calcein, Alizarin Complexone, and PKH2 as an additive to the DME medium. A control group was run in parallel to each test group. Over the 27-day experiment, the cell count was determined on 12 different days, with 12 series cultivated as triplicates for each group. This resulted in a total of 144 wells inoculated with cell cultures in the first experiment. The cell count was determined using the MTT assay, where the cell number was correlated with a precisely defined extinction value of the MTT test. A Neubauer counting chamber was used for this purpose. Three extinction values were measured for each well, and the mean of the 9 measurements per test day was calculated and used for graphical representation. To further differentiate proliferation, the average generation time, the number of generations, and the cumulative cell count were also analyzed.
In the second experimental setup, in addition to cell count determination, the Calcein intensity in fluorescent cells was measured to identify a potential correlation between proliferation and dye uptake.
The key findings indicate that the dyes have a minimal effect on cell proliferation, though this influence increases with longer cultivation times. During the first 8 to 10 days, none of the dyes affected proliferation. Compared to the control group, Alizarin exhibited the strongest inhibition of proliferation, followed by Calcein and PKH2. This effect was most evident in the cumulative cell count, average generation time, and the number of generations, all dependent on the culture duration. Normalization of time-dependent cell counts and their differences from the control group provided useful results, whereas simply plotting cell count against culture time appeared unclear.
In the second experimental approach, only Calcein was further investigated over 44 days, as Alizarin Complexone exhibited low fluorescence and PKH2 was not stable against fading under the given experimental conditions for long-term use. Calcein, however, was well-detectable. Its maximum cell concentration was observed between days 18 and 28 in culture. However, it was only taken up by a few cells, which significantly declined in number from day 28 until the end of the experiment.
Based on these results, it can be assumed that Alizarin and Calcein, which are among the most commonly used fluorochromes in animal experiments for bone exploration, do not have a significant impact on cell proliferation, as they are typically applied in limited amounts and only for short durations.2025-02-2
Application of imputed sequence-level genotypes to genomic analyses in German Warmblood horses

Wilhelm Schaumann Stiftung.Der Einsatz von Gesamtgenom-Sequenz (Whole Genome Sequence, WGS)-Daten in genomischen Anwendungen ist den üblicherweise verwendeten Genotypdaten von Einzelnukleotid-Polymorphismus (Single Nucleotide Polymorphism, SNP)-Chips potenziell überlegen, doch ihre Erzeugung in großem Maßstab ist kostspielig. Eine kostengünstige Alternative zur Gewinnung von Daten auf Sequenzebene, insbesondere für eine große Anzahl von Individuen, ist die Anwendung der Genotyp-Imputation, d.h. die Vorhersage von Genotypen, die nicht direkt in einer Studienkohorte bestimmt wurden. Durch die Erhöhung der Markerdichte kann dieses Verfahren die Leistungsfähigkeit von genomischen Analysen verbessern, sofern die Genauigkeit der Imputation ausreichend hoch ist. Für Pferde sind nur wenige Informationen zur Genotyp-Imputation vorhanden und die Verfügbarkeit von genomischen Daten im Allgemeinen und WGS-Daten im Besonderen ist begrenzt. Das Ziel dieser Dissertation war es daher, die Genotyp-Imputation in deutschen Warmblutpferden, einer großen Pferdepopulation von weltweiter Bedeutung, zu implementieren und die resultierenden Sequenzdaten für verschiedene genomische Anwendungen zu nutzen. Dabei sollte ihre Eignung für diese Analysen untersucht und Varianten identifiziert werden, die mit ausgewählten Phänotypen von Interesse assoziiert oder für diese kausal sind. Zunächst wurden anhand öffentlich verfügbarer WGS-Daten von 317 Pferden verschiedener Rassen ein Mapping und Variant Calling durchgeführt, um ein Referenzpanel für die Genotyp-Imputation in Pferden zu erstellen. Dieses wurde anschließend dazu verwendet, den Effekt diverser Einflussfaktoren auf die Genauigkeit der Imputation zu untersuchen, um auf dieser Grundlage eine optimale Strategie für die Genotyp-Imputation in Warmblutpferden zu entwickeln. Dabei konnte nachgewiesen werden, dass die Imputationsgenauigkeit von der Größe und Zusammensetzung des Referenzpanels, der Markerdichte des zur Genotypisierung verwendeten SNP-Chips, der Allelfrequenz der imputierten Marker und der verwendeten Imputationssoftware beeinflusst wird. Die auf der Grundlage dieser Ergebnisse entwickelte angepasste Strategie, die zu einer genomweiten Imputationsgenauigkeit von 0,66 führte, wurde zur Imputation einer Kohorte von 4972 deutschen Warmblutpferden von mittlerer SNP-Dichte auf Sequenzebene herangezogen. Der resultierende Datensatz wurde anschließend zur Identifizierung von Genomregionen verwendet, die mit dem Exterieur der Pferde assoziiert waren, welches ein wichtiges Selektionskriterium in der Pferdezucht darstellt und ein Beispiel für komplexe Merkmale bei Pferden ist. Mittels genomweiter Assoziationsstudien (GWAS) konnten für verschiedene Exterieurmerkmale neue quantitative Merkmals-Loci (Quantitative Trait Loci, QTL) identifiziert werden. Darüber hinaus wurden für alle untersuchten Merkmale Heritabilitäten und genetische Korrelationen geschätzt. Eine GWAS für die Widerristhöhe, welche als Referenzmerkmal zur Validierung des imputierten Datensatzes und der Methodik diente, bestätigte einen bereits zuvor beschriebenen QTL auf Chromosom 3 nahe der Gene LCORL und NCAPG. Die anschließende Feinkartierung der Region ermöglichte die Identifizierung möglicher Kausalvarianten, darunter eine Nonsense-Mutation innerhalb der kodierenden Sequenz von LCORL. Derselbe QTL war ebenfalls mit einer Reihe anderer Exterieurmerkmale assoziiert, die untereinander zudem hohe genetische Korrelationen innerhalb der QTL-Region zeigten, was insgesamt auf eine hohe Relevanz dieses QTL für die Ausprägung des Exterieurs von Pferden hindeutet. Der imputierte Datensatz wurde außerdem zur Identifizierung embryonal letaler Mutation verwendet, welche durch das Screening genomischer Daten auf Haplotypen oder Varianten ohne Homozygote ermittelt werden können. Während die geringe Anzahl sequenzierter Pferde im Referenzpanel die Entdeckung von Varianten mit vorhergesagtem hohem oder moderatem Effekt und gleichzeitig signifikantem Mangel an Homozygoten nicht zuließ, ermöglichte die deutlich größere Stichprobengröße der imputierten Warmblutpferde die Identifizierung von 72 solcher Mutationen. Ihre weitere Charakterisierung ließ jedoch Zweifel an ihrer potenziellen Letalität aufkommen und machte deutlich, dass bei der Verwendung imputierter Daten für diese Anwendung größte Vorsicht geboten ist. Letztendlich stellten sich die vielversprechendsten Kandidatenmutationen als Artefakte der Variant-Calling-Pipeline heraus, die aufgrund der Ergebnisse der Genotyp-Imputation als vermeintlich signifikant im Sinne eines Mangels an Homozygoten eingestuft wurden. Zusammenfassend lässt sich festhalten, dass die Verwendung imputierter Sequenzdaten für GWAS sowie die Feinkartierung von Exterieurmerkmalen und insbesondere der Widerristhöhe als Beispiele für komplexe Merkmale bei Pferden erfolgreich war, während sie sich für die Identifizierung embryonal letaler Varianten anhand eines Mangels an Homozygoten als weniger effizient erwies. Der Effekt der Genotyp-Imputation und das damit verbundene Risiko von Imputationsfehlern lassen sich jedoch nicht ohne weiteres von anderen Faktoren wie der Stichprobengröße trennen, die bekanntermaßen ebenfalls die Leistungsfähigkeit von genomischen Analysen beeinflussen. Nichtsdestotrotz stellt die Genotyp-Imputation eine kosteneffiziente Methode dar, um die Anzahl der Individuen mit Daten auf Sequenzebene zu erhöhen, die in verschiedenen genomischen Anwendungen eingesetzt werden können. Dies ist insbesondere bei einer Spezies wie dem Pferd relevant, bei der die Verfügbarkeit echter WGS-Daten vergleichsweise begrenzt ist.The use of whole-genome sequence (WGS) data in genomic applications is potentially superior to the commonly used genotype data from single nucleotide polymorphism (SNP) arrays, but their generation on a large scale is expensive. An affordable alternative to obtain sequence-level data for large numbers of individuals is to apply genotype imputation, which is the prediction of genotypes not directly assayed in a study sample. By increasing the marker density, imputation can improve the performance of genomic analyses, provided that its accuracy is sufficiently high. In horses, information on genotype imputation is rather scarce and the availability of genomic data in general and WGS data in particular is limited. Therefore, the aim of this thesis was to implement genotype imputation in German Warmblood horses, a large horse population of global importance, and to apply the resulting imputed sequence-level data to various genomic applications in order to investigate their suitability for such analyses and to identify variants associated with or causal for selected phenotypes of interest. First of all, mapping and variant calling were performed on publicly available WGS data from 317 horses of diverse breeds to establish a reference panel for genotype imputation in horses, which was then used to investigate the effect of several factors on the accuracy of imputation in order to develop an optimal strategy for genotype imputation in warmblood horses. Imputation accuracy was found to be influenced by the size and composition of the reference panel, the marker density of the genotyping array, the minor allele frequency of the imputed markers and the software used for imputation. Based on these findings, an adapted strategy, which resulted in a genome-wide imputation accuracy of 0.66, was developed and used to impute a cohort of 4972 German Warmblood horses from medium SNP density to sequence level. The resulting dataset was used to discover genomic regions associated with equine conformation, which is an important selection criterion in horse breeding and an example of complex traits in horses. Applying genome-wide association studies (GWAS), novel quantitative trait loci (QTL) were detected for various conformation traits. Furthermore, heritabilities and genetic correlations were estimated for all investigated traits. A GWAS for withers height, which served as a reference trait to validate the imputed dataset and methodology, confirmed a previously reported QTL on chromosome 3 near the LCORL and NCAPG genes. Subsequent fine-mapping of the region enabled the identification of candidate causal variants, including a nonsense mutation within the coding sequence of LCORL. The same QTL was also associated with several other conformation traits, and high genetic correlations were observed between these traits for the QTL region, indicating its high relevance for the manifestation of equine conformation in general. The imputed dataset was further used to detect embryonic lethal mutations, which can be identified by screening genomic data for haplotypes or variants without homozygotes. While the small number of sequenced horses in the reference panel did not allow for the discovery of high- or moderate-impact variants with a significant absence or reduction of homozygotes, the increased sample size of the imputed warmblood horses facilitated the identification of 72 such mutations. However, their further characterisation raised doubts about their potential lethality and highlighted that great caution should be taken when using imputed data for this application. Eventually, the most promising candidate mutations turned out to be artefacts from the variant calling pipeline, which were classified as putatively significant in terms of the absence of homozygotes based on the results of genotype imputation. In conclusion, the application of imputed sequence-level data was successful for GWAS and fine-mapping of equine conformation traits and in particular withers height as examples of complex traits in horses, but was less efficient for the identification of embryonic lethal variants based on a deficiency of homozygous individuals. However, the effect of genotype imputation and the associated risk of imputation errors are not easily separated from other factors that affect the performance of genomic analyses, such as sample size. Nevertheless, if handled with care, genotype imputation is a cost-effective means to increase the number of individuals with sequence-level data to be used in genomic applications, especially in a species such as the horse where the availability of real WGS data is comparatively limited.2025-02-2
In-vitro-Charakterisierung der kardialen Elektrophysiologie beim Noonan- Syndrom und Noonan-Syndrom mit multiplen Lentiginosen anhand eines stammzell-basierten 3D-Modells
Noonan syndrome and Noonan syndrome with multiple lentigines are classified as RASopathies, resulting from mutations in the RAS/MAPK signaling pathway. Patients typically exhibit a range of symptoms, with cardiac abnormalities posing the highest health risks. Currently, no definitive causal therapy exists, although early treatments targeting the signaling pathway have shown promising results. Despite identifying the genetic mutations involved, the correlation between genotype and phenotype remains unclear.
This research aimed to deepen the understanding of the cardiac phenotype in affected individuals by studying electrophysiology using miniaturized three-dimensional heart tissues. Induced pluripotent stem cells (iPSCs) derived from patients' skin or blood were differentiated into cardiomyocytes and combined with collagen and fibroblasts to create 3D bioartificial cardiac tissues that displayed spontaneous contractions, facilitating disease modeling.
Electrophysiological measurements involved using microelectrode arrays to assess extracellular cardiac field potentials, which provide insights into action potentials and excitation propagation. Additionally, genetically encoded calcium indicators were employed to visualize intracellular calcium oscillations within cardiomyocytes through fluorescence microscopy. The study focused on two specific mutations in the RAS/MAPK pathway: SHP2Q510E/+ and RAF1S257L/+. These were compared against isogenic controls and partially treated with a pathway inhibitor.
Findings revealed that SHP2-mutant tissues exhibited prolonged field potentials and calcium transients compared to corrected tissues, with the inhibitor partially restoring the normal phenotype. For RAF1-mutant tissues, those without calcium indicators showed shortened field potentials, while those with calcium indicators demonstrated prolonged potentials and altered calcium parameters. The presence of the calcium indicator appeared to have a more significant impact on the electrophysiological phenotype than the underlying mutation, raising concerns about its suitability.
Overall, this research advances the understanding of cardiac electrophysiology in RASopathies by elucidating the underlying pathomechanisms and establishing methods for synchronized measurements of field potential, force development, and calcium oscillations in bioartificial heart tissues. Impact of Regular In-Hospital Follow-ups on Ventilation Effects and Compliance in Patients with Non-Invasive Home Ventilation
Patienten mit einer ventilatorischen Insuffizienz entwickeln eine Hyperkapnie. Diese kann akut auftreten oder chronisch vorliegen. Ist die Hyperkapnie chronisch, besteht eine metabolische Kompensation mit erhöhtem Bikarbonat (HCO3-). Die Therapie der Hyperkapnie ist eine Beatmung des Patienten, die bei chronisch ventilatorischer Insuffizienz in aller Regel nicht-invasiv über eine Maske durchgeführt wird. Die beiden häufigsten Erkrankungen, bei denen deshalb eine Indikation zur nicht-invasiven Beatmung (non invasive ventilation, NIV) besteht, sind die chronisch obstruktive Lungenerkrankung (chronic obstructive pulmonary disease, COPD) und das Obesitas-Hypoventilations-Syndrom (OHS).
Es gibt mittlerweile viele Studien, die die Indikation zur NIV bei diesen Erkrankungen aufzeigen und positive Effekte beleuchten. Beispielsweise konnte u. a. gezeigt werden, dass die Mortalität unter NIV sinkt. Mit zunehmender Erfahrung wird NIV immer häufiger angewendet. Seit 2009 gibt es eine deutsche S2k-Leitlinie zu diesem Thema, die 2017 überarbeitet wurde.
Diese sieht auch die regelmäßige Überprüfung der NIV und der Beatmungseinstellungen vor. Studien hierzu gibt es jedoch wenig. Die vorliegende Studie soll die Einflüsse der routinemäßigen, stationären Beatmungskontrollen auf die Therapiecompliance der Patienten sowie die Auswirkungen auf Blutgasanalysen untersuchen. Ergänzend wurde deskriptiv erfasst, wie häufig und welche Änderungen der Beatmungstherapie erfolgt sind.
Insgesamt wurden Daten im Zeitraum von April 2015 bis Januar 2018 von 133 Patienten (83 Patienten/ 62,4% mit COPD, 50 Patienten/ 37,6% mit OHS) in stabilem klinischen Zustand ausgewertet, die stationär zur Beatmungskontrolle in der Lungenfachklinik Immenhausen waren. Es handelt sich um eine retrospektive Studie.
Bei 83 der Patienten (62,4%) wurde eine Änderung der Beatmungstherapie im Rahmen der Kontrolluntersuchung vorgenommen. Bei 76 Kontrollen (57,1%) wurden hierbei die Beatmungsparameter angepasst, während bei 26 Patienten (19,5%) bzw. 5 Patienten (3,6%) die Beatmungsmaske getauscht bzw. ein Atemgasanfeuchter ergänzt wurde. Nach durchschnittlich 258 Tagen erfolgte die nächste Beatmungskontrolle.
Die Therapiecompliance der Patienten wurde über die Nutzungszeit der NIV ausgewertet, welche mit 5,5 Stunden pro Tag im Mittel als ausreichend zu bewerten war. Der durchschnittliche Kohlenstoffdioxidpartialdruck (PCO2) lag mit 42,4 mmHg unter NIV im Normbereich. Hier fanden sich in der Kontrolluntersuchung keine Unterschiede abhängig davon, ob Änderungen der Beatmungstherapie vorgenommen wurden. Auch in der Subgruppenanalyse nach Erkrankung war kein signifikanter Unterschied erkennbar.
Unterschiede waren jedoch in Bezug auf das HCO3- nachweisbar. In der Gruppe der Patienten, bei denen Änderungen der Beatmungstherapie vorgenommen wurden, war das HCO3- mit 27,6 mmol/l über die Norm erhöht, während das HCO3- bei Patienten ohne Änderungen mit 25,5 mmol/l signifikant niedriger und im Normbereich lag. Diese Ergebnisse waren auch in der Subgruppenanalyse nach Erkrankung nachweisbar. Ferner konnte ein signifikanter Zusammenhang zwischen der Nutzungszeit der NIV und den Werten der Blutgasanalyse nachgewiesen werden. So nahmen mit einer Zunahme der Nutzung der NIV um eine Stunde durchschnittlich PCO2 um 0,383 mmHg und HCO3- um 0,296 mmol/l ab.
Die Subgruppenanalyse nach Erst- und Folgekontrollen konnte einen signifikanten Unterschied in der Therapiecompliance nachweisen. Diese war bei Erstkontrollen mit 6,1 Beatmungsstunden pro Tag höher als mit 5,0 Stunden pro Tag bei Folgekontrollen.
Die häufigsten vorgenommenen Änderungen der Beatmungsparameter waren inspiratorischer Beatmungsdruck (52 Fälle/ 68,2%), Atemfrequenz (33 Fälle/ 43,2%) und Änderung der Sauerstoffgabe (31 Fälle/ 40,7%). Teilweise wurden mehrere Parameter gleichzeitig verändert. Es wurden Erhöhungen und Absenkungen vorgenommen. Muster waren hierbei retrospektiv nicht erkennbar.
Bei der Datenerhebung wurden im untersuchten Zeitraum insgesamt 829 Beatmungskontrollen betrachtet. Hierbei mussten in 91 der Fälle (11,0%) eine Behandlung wegen akuter klinischer Verschlechterung (COPD-/OHS-Exazerbation und/ oder kardialer Dekompensation) durchgeführt werden.
Die vorliegende Studie konnte keine signifikanten Unterschiede von Therapiecompliance, PCO2 und HCO3- abhängig von einer Änderung der Beatmungstherapie während einer NIV-Kontrolle nachweisen. Die Häufigkeit und Vielfältigkeit der vorgenommenen Änderungen, die in dieser Studie deskriptiv erfasst wurden, weisen trotzdem auf die Wichtigkeit dieser regelmäßigen Kontrollen hin. Gründe für diese Änderungen sind die mit der Zeit abnehmende Therapiecompliance zu verbessern und die Beatmung regelmäßig an die Bedürfnisse der Patienten anzupassen. Hierfür existiert bisher kein leitliniengerechtes Schema. Zudem benötigten eine nicht unerhebliche Zahl der Patienten eine akute medizinische Behandlung, die sich während der Beatmungskontrolle offenbarte. Vergleiche zu anderen Studien zeigen, dass ähnlich häufig Anpassungen der NIV bei Kontrolluntersuchungen vorgenommen wurden. In Langzeitstudien, die die positiven Effekte einer NIV belegen, wurden regelmäßige Beatmungskontrollen durchgeführt, um einen möglichst hohen Nutzen der Beatmung sicherzustellen.
Neuere Studien untersuchen die ambulante Durchführung von Beatmungskontrollen. Hierfür gibt es aber kein evidenzbasiertes Vorgehen. Bisher sind ambulante Versorgungsstrukturen in Deutschland nur unzureichend vorhanden. Die Datenübertragung zur Evaluation der NIV im häuslichen Umfeld ist fehleranfällig. Weiterhin fehlt es an Kriterien und Grenzwerten für die tagsüber durchgeführte Diagnostik zur Beurteilung der nächtlichen NIV. Bis diese Probleme behoben bzw. Fragen geklärt sind, sollten stationäre NIV-Kontrollen erfolgen, um eine ausreichende Patientenversorgung zu gewährleisten.Patients with ventilatory insufficiency develop hypercapnia. This can occur acutely or be chronic. If hypercapnia is chronic, there is a metabolic compensation with increased bicarbonate (HCO3-). In cases of chronic ventilatory insufficiency, the treatment for hypercapnia is ventilation of the patient, which is usually performed non-invasively via a mask. The two most common conditions for which there is an indication for non-invasive ventilation (NIV) are chronic obstructive pulmonary disease (COPD) and obesity hypoventilation syndrome (OHS).
There are now many studies that demonstrate the indication for NIV in these conditions and highlight positive effects. For example, it has been shown that mortality decreases under NIV. With increasing experience, NIV is being used more frequently. Since 2009, there has been a German S2k guideline on this topic, which was revised in 2017.
This guideline also provides for regular review of NIV and ventilation settings. However, there is few research on this. The present study aims to investigate the influences of routine inpatient ventilation follow-ups on therapy compliance as well as the effects on blood gas analyses. Additionally, it descriptively recorded how often and what changes in ventilation therapy occurred.
In total, data from 133 patients (83 patients/62.4% with COPD, 50 patients/37.6% with OHS) in stable clinical condition were evaluated during the period from April 2015 to January 2018, who were hospitalized for ventilation control at the Lungenfachklinik Immenhausen. This is a retrospective study.
In 83 of the patients (62.4%), a change in ventilation therapy was made during the follow-up. In 76 controls (57.1%), the ventilation parameters were adjusted, while in 26 patients (19.5%) and 5 patients (3.6%), the ventilation mask was changed or a breathing gas humidifier was added. The next ventilation control took place after an average of 258 days.
The therapy compliance of the patients was evaluated based on the usage time of NIV, which was assessed as sufficient at an average of 5.5 hours per day. The average carbon dioxide partial pressure (PCO2) was 42.4 mmHg under NIV, which is within the normal range. No differences were found in the control examination depending on whether changes in ventilation therapy were made. Also, in the subgroup analysis by disease, no significant difference was observed.
However, differences were detectable regarding HCO3-. In the group of patients where changes in ventilation therapy were made, HCO3- was elevated at 27.6 mmol/l, while HCO3- in patients without changes was significantly lower at 25.5 mmol/l and within the normal range. These results were also observable in the subgroup analysis by disease. Furthermore, a significant correlation between the usage time of NIV and the values of the blood gas analysis was demonstrated. With an increase in NIV usage by one hour, PCO2 decreased by an average of 0.383 mmHg and HCO3- by 0.296 mmol/l.
The subgroup analysis by first and follow-up controls showed a significant difference in therapy compliance. This was higher in first controls with 6.1 ventilation hours per day compared to 5.0 hours per day in later follow-up controls.
The most common changes made to the ventilation parameters were inspiratory ventilation pressure (52 cases/68.2%), respiratory rate (33 cases/43.2%), and changes in oxygen delivery (31 cases/40.7%). In some cases, multiple parameters were changed simultaneously. Increases and decreases were made, but no patterns were retrospectively recognizable.
During the data collection, a total of 829 ventilation controls were considered in the examined period. In 91 of these cases (11.0%), treatment was required due to acute clinical deterioration (COPD/OHS exacerbation and/or cardiac decompensation).
The present study found no significant differences in therapy compliance, PCO2, and HCO3- depending on changes in ventilation therapy during a NIV control. However, the frequency and variety of changes made, which were descriptively recorded in this study, indicate the importance of these regular follow-ups. Reasons for these changes include improving the decreasing therapy compliance over time and regularly adjusting ventilation to meet the patients' needs. So far, there is no guideline-compliant scheme for this. Additionally, a significant number of patients required acute medical treatment, which became apparent during the ventilation control. Comparisons with other studies show that adjustments to NIV were made with similar frequency during control examinations. In long-term studies demonstrating the positive effects of NIV, regular ventilation controls were conducted to ensure the highest possible benefit from ventilation.
Recent studies are investigating the outpatient implementation of ventilation controls. However, there is currently no evidence-based approach for this. Outpatient care structures in Germany are still insufficiently available. Data transmission for the evaluation of NIV in the home environment is prone to errors. Furthermore, there is a lack of criteria and thresholds for daytime diagnostics to assess nighttime NIV. Until these issues are resolved or questions clarified, inpatient NIV controls should be conducted to ensure adequate patient care.2025-04-1
TransValid-A-Studie zur präoperativen, 5-FU-basierten Chemo-/Radiotherapie bei Rektumkarzinomen: Machbarkeit, Durchführbarkeit und erste Ergebnisse
Die multizentrische TransValid-A-Studie (ClinTrials.gov: NCT03034473; DFG-gefördert; GZ: LI 1727/3-1) prüfte unter real-world-Bedingungen die Durchführbarkeit und Effizienz einer 5-FU-basierten präoperativen Chemo-/Radiotherapie (CRT) unter gleichzeitiger Gewinnung und Asservation von Biomaterial bei Patienten mit lokal fortgeschrittenen Adenokarzinomen des unteren und mittleren Rektums (LARC; TU-Unterrand < 12 cm ab Anocutanlinie) der klinischen Stadien cUICC ≥ II. Nach einer peri-/postoperativ qualitäts-kontrollierten totalen mesorektalen Exzision (qTME) sollten alle Patienten eine adjuvante Chemotherapie (adCTx) mit 5-FU bzw. vorzugsweise mit FOLFOX erhalten. Der primäre Endpunkt war das krankheitsfreie Überleben (DFS). Zwischen 08/2011 und 01/2015 wurden von 19 aktiven Studienzentren (70,4% der registrierten Zentren) 194 auswertbare LARC-Patienten (F: 36,1%; M: 63,9%; medianes Alter: 64 J.; ECOG-0: 63,9%; -1: 28,4% u. -2: 7,7%) in die Studie aufgenommen. Die LARCs (cUICC-II: 14,4%; ≥ III: 85,1%) waren zu 41,8% und 57,7% im unteren (< 6 cm) und mittleren (6 - < 12 cm ab ACL) Rektum lo-kalisiert. 180 Patienten starteten die präoperative CRT protokollgemäß mit RT + 5-FU in 92,8%, mit RT + 5-FU + Oxaliplatin (OX) in 3,3% sowie mit RT + Capecitabin (CAP) in 3,9%. Anschließend wurde bei 174 (96,7%) Patienten eine qTME mit optimaler, moderater und schlechter Qualität in 84,5%, 7,5% und in 1,7% durchgeführt. Höhergradige Komplikationen (CTC-AE Grad 3 u. Grad 4) traten in 24,1% auf; die 30 Tage-Mortalität betrug 1,2%. Ein deutliches TU-Ansprechen auf die CRT zeigte sich anhand der gesteigerten Rate von Stadien ypUICC ≤ II mit 73,6% vs. der 14.4% cUICC-II-Stadien beim initialen Staging. Eine komplette Remission (CR) war in 20,1% erreicht worden; die Rate an ypUICC-Stadien 0 / I lag bei 19,5% und 26,4%. Ein multivariables Cox-Regressionsmodell unter Berücksichtigung der Risikofaktoren ypUICC-Status, LKM/LK-Ratio, Alter und CRM-Status ergab für den Prädiktor ypUICC ≤ I ein exzellentes DFS (HR: 2,755; 95%-CI [1,302; 5,8]; p=0,008) vs. ypUICC ≥ III. Über den Zeitraum der Multimodaltherapie erreichten 4 Zentren mit 60% - 70% die höchste Biomaterial-SOLL- / IST-Ratio; bei 6 weitere Zentren lag diese bei 20% - 30%. Die TransValid-A-Studie war sicher durchführbar und die aktiven Studienzentren lieferten eine Biomaterialquantität / -qualität, die für die Fortsetzung von translationalen Forschungsprojekten gut geeignet ist.The single-arm, multicenter TransValid-A study (ClinTrials. gov: NCT03034473; funded by the German Research Foundation, DFG; GZ: LI 1727/3-1) examined under real-world conditions the feasibility and efficacy of 5-fluorouracil (5-FU) based preoperative chemoradiotherapy (CRT) with simultaneous collection and preservation of biomaterial in patients with locally advanced adenocarcinomas of the lower and mid rectum (LARC; inferior tumor margin < 12 cm from the anal verge, av) of clinical (c) stages cUICC ≥ II. After a peri-/postoperative quality-controlled total mesorectal excision (qTME), all patients were to receive adjuvant chemotherapy (adCTx) with 5-FU or preferably with FOLFOX. The primary endpoint was disease-free survival (DFS). Between 08/2011 and 01/2015, 194 evaluable LARC patients (f: 36.1%; m: 63.9%; median age: 64 y; ECOG-0: 63.9%, -1: 28.4%, and -2: 7.7%) from 19 active study centers (70.4% of all registered centers) were enrolled in the study. The LARCs (cUICC-II: 14.4%; ≥ III: 85.1%) were localized in 41.8% and 57.7% in the lower (< 6 cm) and mid (6 to < 12 cm from av) rectum. 180 patients had started the preoperative CRT according to the protocol with RT + 5-FU (92.8%), with RT + 5-FU + oxaliplatin (OX; 3.3%) and with RT + capecitabine (CAP; 3.9%), respectively. Subsequently, 174 of the CRT-treated patients underwent qTME with optimal, moderate and poor quality in 84.5%, 7.5% and 1.7%, respectively. Major complications (CTC-AE grade 3 and grade 4) occurred in 24.1%; 30-day mortality was 1.2%. The 73.6% rate of stages ≤ II (vs. ≥ III in 26.4%) showed a clear tumor response to preoperative CRT. A complete remission (CR) was achieved in 20.1%; the rate of ypUICC stages 0 / I was 19.5% and 26.4%, respectively. The multivariable Cox regression model for DFS using the risk factors ypUICC status, LNM/LN ratio, age and CRM status, revealed only stages ≤ I to be significantly predictive for excellent DFS vs patients with stages ≥ III (HR: 2.755; 95%-CI [1.302; 5.8]; p=0.008). During MTT, 4 centers achieved the highest biomaterial ratio (planned/submitted) of 60% - 70%; in 6 other centers this rate was 20% - 30%. The TransValid-A study was safe to conduct and ensured biomaterial of good quantity and quality for ongoing translational research projects.2025-05-1
