The effect of perioperative hemadsorption in patients operated for acute infective endocarditis
Patients operated for infective endocarditis (IE) are at high risk of developing an
excessive systemic hyperinflammatory state, resulting in systemic inflammatory re-
sponse syndrome and septic shock. Hemoadsorption (HA) by cytokine adsorbers
has been successfully applied to remove inflammatory mediators. This randomized
controlled trial investigates the effect of perioperative HA therapy on inflammatory
parameters and hemodynamic status in patients operated for IE. A total of 20 patients
were randomly assigned to either HA therapy or the control group. HA therapy was
initiated intraoperatively and continued for 24 hours postoperatively. Cytokine levels
(IL- 6, IL- 1b, TNF- α), leukocytes, C- reactive protein (CRP), and Procalcitonin (PCT)
as well as catecholamine support, and volume requirement were compared between
both groups. Operative procedures included aortic (n = 7), mitral (n = 6), and mul-
tiple valve surgery (n = 7). All patients survived to discharge. No significant differ-
ences concerning median cytokine levels (IL- 6 and TNF- α) were observed between
both groups. CRP and PCT baseline levels were significantly higher in the HA group
(59.5 vs. 26.3 mg/dL, P = .029 and 0.17 vs. 0.05 µg/L, P = .015) equalizing after
surgery. Patients in the HA group required significantly higher doses of vasopres-
sors (0.093 vs. 0.025 µg/kg/min norepinephrine, P = .029) at 12 hours postopera-
tively as well as significantly more overall volume replacement (7217 vs. 4185 mL
at 12 hours, P = .015; 12 021 vs. 4850 mL at 48 hours, P = .015). HA therapy did
neither result in a reduction of inflammatory parameters nor result in an improve-
ment of hemodynamic parameters in patients operated for IE. For a more targeted use
of HA therapy, appropriate selection criteria are required.2025-10-0
Proenkephalin as a guide to cessation of renal replacement therapy in the ICU - a prospective observational study
Acute Kidney Injury (AKI) represents a significant and frequent health concern within intensive care units (ICU), with an incidence rate ranging from 20% to 50% (Hoste et al. 2015; Susantitaphong et al. 2013). AKI often correlates with electrolyte imbalances, hypervolemia, and other complications, contributing to increased morbidity and mortality rates (Kidney Disease: Improving Global et al. 2012). Whilst the definition of AKI, primarily based on serum creatinine (SCr) and diuresis, and the criteria to indicate the need for dialysis are stated in several quidelines, criteria for discontinuation of the dialysis remain uncertain. Thus, there is a dire need for more effective diagnostic and prognostic tools in the management of AKI to enable early intervention and enhance patient outcomes.
Proenkephalin (PENK), a biomarker currently under investigation, has shown promising results in its ability to predict AKI, particularly in ICU patients undergoing dialysis (Kim et al. 2017; Kim et al. 2020).
This dissertation investigated whether PENK is associated with the established kidney parameters, the severity of AKI and the development of kidney function within patients undergoing dialysis. The results show an unhomogenous correlation of PENK and the established kidney parameters at the start and throughout the renal replacement therapy. Also, the dynamics of PENK don't correlate with the dynamics of the established kidney parameters significantly. In this cohort, PENK was unable to predict neither the success of a discontinuation from dialyses nor the survival rate. PENK however was able to imply subgroups of AKI patients at the start of dialysis. Furthermore, PENK did indicate a dynamic under dialysis that the established kidney parameters were not able to track.2025-09-1
A journey toward independence in young mandrills ("Mandrillus sphinx"): the intertwined effects of early life adversity and maternal investment
Widrige Umstände in der frühen Kindheit (Early Life Adversities, ELAs) können sich nachhaltig auf die Individualentwicklung auswirken und sowohl das Überleben als auch das körperliche Wachstum in Wildtierpopulationen beeinflussen. Dabei spielt die mütterliche Fürsorge eine entscheidende Rolle bei der Gestaltung dieser Entwicklungen und kann die Auswirkungen verschiedener Widrigkeiten abfedern. Jüngste Erkenntnisse deuten zwar darauf hin, dass ELAs auch generationenübergreifende Auswirkungen haben können - die frühen Erfahrungen einer Mutter wirken sich auf die Fitness ihrer eigenen Nachkommen aus -, aber die Mechanismen, die diesen Auswirkungen zugrunde liegen, sind nach wie vor kaum bekannt. In dieser Arbeit habe ich anhand von mehr als 10 Jahren demografischer und verhaltensbezogener Daten von wild lebenden Mandrills analysiert, wie ELAs das frühe Überleben und die körperliche Entwicklung beeinflusst und die Mutter-Kind-Beziehung prägen, mit besonderem Augenmerk darauf, wie der Verlust der Mutter während der frühen Entwicklung von Weibchens das eigene mütterliche Verhalten im späteren Leben bestimmt. Die Ergebnisse zeigten, dass, erstens, die Folgen der kumulativen ELAs zwar nicht während des gesamten frühen Lebenszeitraums (<4 Jahre) sichtbar waren, dass sie aber deutlich wurden, wenn man sich speziell auf das erste Lebensjahr konzentriert (Kapitel 2). Bemerkenswert ist, dass diese Ergebnisse einen potenziellen Zusammenhang zwischen ELA und dem Überleben im Erwachsenenalter aufzeigten, da ELAs sich stärker auf die körperliche Entwicklung als auf das frühe Überleben auswirkte. Zweitens zeigte die Untersuchung der Unterschiede in den Beziehungen zwischen Mutter und Kind, dass sich diese Beziehungen während des ersten Lebensjahres verändern und dass mütterliche Eigenschaften, insbesondere Rang und Alter, das Betreuungsverhalten stark beeinflussen (Kapitel 3). Darüber hinaus zeigten die Ergebnisse, dass Mütter, die während ihrer eigenen Entwicklung den Tod der Mutter erlebten, ein anderes Betreuungsverhalten gegenüber ihren Nachkommen an den Tag legten (Kapitel 4), was auf einen potenziellen Mechanismus für die Umsetzung der intergenerationalen Auswirkungen von ELAs hindeutet. Schließlich haben die Ergebnisse dieser Arbeit wiederholt signifikante Unterschiede zwischen den Geschlechtern aufgezeigt, was die unterschiedlichen Lebensverläufe der beiden Geschlechter unterstreicht, die sich bereits im ersten Lebensjahr herauszubilden beginnen. Diese Arbeit erweitert unser Verständnis darüber, wie ungünstige Bedingungen während des frühen Lebens die individuelle Entwicklung beeinflussen können. Darüber hinaus unterstreicht sie die Bedeutung der Integration umfassender Verhaltensdaten über Mutter-Nachwuchs-Paare, um diese komplexen Beziehungen weiter zu entschlüsseln und besser zu verstehen, wie soziales Verhalten diese Dynamik beeinflussen kann.Early life adversity (‘ELA’) can have lasting effects on development influencing both survival and physical growth in wild animal populations, with maternal care playing a crucial role in shaping these trajectories and potentially buffering the impact of various adversities. While recent evidence suggests that ELA can also have intergenerational effects – where a mother’s early experiences affect the fitness of her own offspring – the mechanisms driving these effects remain poorly understood. In this thesis, using more than 10 years of demographic and behavioral data from wild mandrills, I analyzed how ELA influences early survival and physical development, and shapes mother-infant relationships, with a particular focus on how the loss of a mother during female’s early development determines her own maternal behavior later in life. The results showed that, first, while the consequences of cumulative ELA were not apparent during the entire early life period (<4 years), they became evident when specifically focusing on the first year of life (Chapter 2). Remarkably, these findings identified a potential link between ELA and survival during adulthood, as ELA had a more pronounced impact on physical development than on early survival. Second, the study of variation in mother-infant relationships showed that these relationships change during the first year of life and that maternal traits, particularly rank and age, strongly shape caregiving behaviors (Chapter 3). Moreover, the results showed that mothers who experienced maternal death during their own development exhibited different caregiving behaviors toward their offspring (Chapter 4), suggesting a potential pathway for the implementation of intergenerational effects of ELA. Finally, the results of this thesis repeatedly demonstrated significant differences between the sexes, underscoring separate life history trajectories of each sex, which start to develop as early as during the first year of life. This work extends our understanding on how adverse conditions during early life can influence individual development. Furthermore, it emphasizes the relevance of integrating comprehensive behavioral data on mother-offspring pairs to further unravel these complex relationships and better understand how social behaviors may influence these dynamics.2025-12-2
Rezidivmuster und -wahrscheinlichkeit unter Berücksichtigung von Prädiktoren sowie Prognostikatoren beim lokal fortgeschrittenen Mammakarzinom nach lokal ablativer Therapie
Invasive breast carcinoma is the most common malignant disease in women, and one in eight women will develop it during their lifetime. It is a malignant condition in which all pillars of oncological therapy are applied. A decisive therapeutic option in this context is postoperative radiotherapy. Accordingly, all patients receive irradiation of the affected breast after breast-conserving surgery. After mastectomy, radiotherapy is indicated under certain conditions. For postmastectomy radiotherapy (PMRT), many radiation oncology departments worldwide use a bolus. This refers to a tissue-equivalent gel sheet intended to shift the maximum photon dose toward the upper skin layers, since the former tumor bed after ablation often lies only a few millimeters below the skin surface. At UMG as well, the use of at least one 5 mm bolus during PMRT is routine practice. However, whether bolus application is meaningful in all cases remains controversial and was examined in this study.
A total of 377 mastectomized patients were retrospectively analyzed for influencing factors on radiotherapy, such as the use of a bolus. The treatment period for the included patients ranged from 2008 to 2018. The defined endpoints were therapy-associated side effects—particularly radiation-induced skin erythema—as well as oncological treatment outcome. In addition, lifestyle factors and tumor characteristics were evaluated for their impact on the respective endpoints.
The study demonstrated a significantly increased incidence of skin erythema with bolus use. Skin erythema also occurred more frequently in patients who smoked. With regard to the potential benefit of bolus therapy, no improved oncological treatment outcome could be demonstrated in the subgroup treated with a bolus. However, the data were limited by the short follow-up period and the small number of patients receiving bolus therapy, and the significance is therefore restricted.
Recent studies show a consensus on the use of a bolus in patients with high-risk features such as initial skin involvement. However, the use of a bolus in tumors without skin infiltration remains controversial. No survival benefit could be demonstrated in numerous studies, whereas increased skin-related side effects were reported in nearly all of them. The optimal management of the bolus during PMRT therefore remains unresolved and requires further studies.2025-12-0
Retrospektive Analyse der intravitrealen operativen Medikamentenapplikation (IVOM). Entwicklung der Sehschärfe mit geringem Ausgangsvisus.
Patients with severely reduced best-corrected visual acuity (BCVA) are frequently excluded from intravitreal injection (IVI) therapy due to lower threshold values established in early clinical trials.
Since 2006, a BCVA limit—set at 1.3 logMAR —has been widely applied in clinical practice as a criterion for initiating or continuing IVI treatment.
However, with the advent of more effective anti-VEGF agents and advances in diagnostic imaging, these historically defined cut-off values may no longer reflect current therapeutic potential.
This study aims to reassess the validity of this lower BCVA threshold and evaluate whether patients with very low visual acuity of ≥0.1 logMAR can still benefit functionally and morphologically from IVI therapy.
This retrospective, single-center study analyzed 329 treatment-naive patients diagnosed with neovascular age-related macular degeneration (nAMD), diabetic macular edema (DME), or retinal vein occlusion (RVO), all presenting with a baseline BCVA ≥0.1 logMAR. Visual acuity changes were assessed over 6–24 months. Morphological changes were evaluated using SD-OCT images in a defined subgroup with BCVA ≥1.3 logMAR, focusing on structural features based on ORCA criteria. Across all three diagnoses, IVI treatment led to a significant improvement in visual acuity. Mean baseline BCVA of all diseases was 1,3 ± 0.4 logMAR, decreasing to 0.94 ± 0.5 logMAR after 6 months. This gain remained stable throughout the 12-, 18-, and 24-month follow-up periods. Over 30% of all patients reached a BCVA of ≤ 0.2 logMAR within 6 months. Furthermore, 5% of AMD and DME patients and 10% of RVO patients achieved a BCVA of ≤ 0.5 logMAR. The analysis revealed that a high proportion of patients with BCVA ≥1.3 logMAR showed marked morphological improvement—for example, a reduction in intra- and subretinal fluid. Notably, in several cases, structural improvements on OCT were observed without a corresponding gain in visual acuity.
Conversely, functional improvements without morphological changes were rare.
This study shows that even patients with low visual acuity may benefit from IVI therapy. Functional and especially morphological outcomes suggest that baseline BCVA should not serve as the sole criterion for therapy decisions. A more individualized approach—considering OCT biomarkers and patient needs—is warranted in clinical practice.2025-10-1
Psychological Effects of Age and Hormonal Contraceptive Use in the Context of Female Reproductive Decision Making
Fortpflanzungsentscheidungen sind ein zentraler Aspekt des menschlichen Lebens, der sowohl von biologischen als auch von psychologischen Faktoren geprägt ist. Während sich frühere Forschungen häufig auf fertilitätsbezogene Resultate konzentriert haben, ist weniger darüber bekannt, ob und wie das Alter und die Verwendung hormoneller Verhütungsmethoden psychologische Dimensionen in der reproduktiven Entscheidungsfindung, wie Partnerpräferenzen, Sexualität und Wohlbefinden, verändern. In meiner Dissertation behebe ich diese Lücke durch drei empirische Studien, in denen ich untersuche, wie diese Faktoren die Alltagserfahrungen von Frauen beeinflussen. Insbesondere untersuche ich, wie das Alter von Frauen mit ihren Partnerpräferenzen zusammenhängt und wie sich die Verwendung hormoneller Verhütungsmethoden auf die Sexualität und das Wohlbefinden von Frauen auswirkt. In der ersten Studie analysierten wir die Rolle des Alters für die weiblichen Partnerpräferenzen in Bezug auf einen idealen romantischen Partner anhand von Querschnittsdaten von 17 254 Single, heterosexuellen Frauen aus 147 Ländern und zeigten auf, dass die meisten Partnerpräferenzen sich nicht zwischen Frauen unterschiedlichen Alters unterscheiden. Allerdings gab es einen signifikanten altersbedingten Unterschied bei Partnerpräferenzen für Eigenschaften, die mit der Elternschaft zusammenhängen, was auf eine Verschiebung der reproduktiven Prioritäten im Laufe des Lebens von Frauen hindeutet. Aufgrund des Querschnittscharakters dieser Analyse ist es jedoch unmöglich, zwischen Alters-, Kohorten- und Zeitperiodeneffekten zu unterscheiden. In der zweiten und dritten Studie sind wir tiefer in den Bereich der weiblichen Reproduktionsentscheidungen vorgedrungen. Wir untersuchten die kausalen Auswirkungen der Verwendung hormoneller Verhütungsmethoden auf Sexualität und Wohlbefinden. Für diese Untersuchung verwendeten wir robuste Längsschnittdesigns mit einer deutschen Stichprobe von bis zu 5 232 Frauen und 24 306 jährlichen Beobachtungen. Die Ergebnisse zeigten, dass die Verwendung hormoneller Verhütungsmethoden im Durchschnitt positive Behandlungseffekte auf die sexuelle Häufigkeit und die sexuelle Zufriedenheit hat, aber keinen (oder nur vernachlässigbar kleinen) Einfluss auf das Wohlbefinden (d. h. Depressivität, Lebenszufriedenheit und Selbstwertgefühl). Entscheidend für die Interpretation der Ergebnisse ist jedoch, dass es eine relativ große Heterogenität darin gab, wie stark sich hormonelle Verhütungsmethoden auf die Sexualität auswirkten: Während einige Frauen erhebliche positive Effekte auf ihre Sexualität erlebten, gab es auch einige Frauen, die nur geringe oder gar keine Veränderungen erfuhren. Im Gegensatz dazu waren die Auswirkungen auf das Wohlbefinden durch eine relativ geringe Heterogenität gekennzeichnet, was auf eine einheitlichere Auswirkung über Frauen hinweg hindeutet. Trotz der analytischen Stärken dieser Studien bleibt die Schätzung der Behandlungsheterogenität methodisch herausfordernd. Weitere Forschung wird benötigt, um die Variabilität der individuellen Behandlungsreaktionen zu verstehen und zu erklären. Zusammen tragen diese drei empirischen Studien zu einem tieferen Verständnis der Frage bei, wie Alter und die Verwendung hormoneller Verhütungsmethoden die psychologischen Schlüsselfaktoren beeinflussen, die bei der Entscheidungsfindung von Frauen im Bereich der Fortpflanzung eine Rolle spielen. Die Studien unterstreichen wie relevant es bei Verhütungsberatung ist, individuelle Unterschiede (z. B. Neurotizismus) zu berücksichtigen. Außerdem ebnen die Ergebnisse den Weg für eine stärker personalisierte, evidenzbasierte Beratung in der klinischen Praxis. Angesichts des Potenzials für Missverständnisse in der Öffentlichkeit und der erhöhten Wahrscheinlichkeit einer Verstärkung durch die Medien, insbesondere angesichts der jüngsten polarisierten Diskussion über hormonelle Verhütungsmethoden, trägt die Forschung in diesem Bereich auch die Verantwortung, die psychologischen Auswirkungen sorgfältig zu kontextualisieren, um unnötige Angst zu vermeiden und einen informierten, gerechten Zugang zu Verhütungsmethoden zu ermöglichen.Reproductive decision making is a central aspect of human life, shaped by both biological and psychological factors. While prior research has often focused on fertility-related outcomes, little is known about if and how age and hormonal contraceptive use influence psychological dimensions in the context of reproductive decision making, such as partner preferences, sexuality, and well-being. In my dissertation, I address this gap through three empirical studies examining how these factors affect women’s everyday experiences. Specifically, I investigate how women’s age is linked to their partner preferences and how hormonal contraceptive use impacts sexuality and well-being in women. In the first study, we analyzed the role of age for female partner preferences in an ideal romantic partner using cross-sectional data of 17,254 single, heterosexual women from 147 countries, indicating that most partner preferences do not differ between women of different ages. However, there was a significant age-related difference in partner preferences for traits linked to parenting, suggesting a shift in reproductive priorities over the life course of women. Yet, due to the cross-sectional nature of this analysis, it is impossible to distinguish between age, cohort, and time period effects. In the second and third study, we delved deeper into the realm of female reproductive decision making. We investigated the causal effects of hormonal contraceptive use on sexuality and well-being. For this investigation, we used robust longitudinal designs with a German sample of up to 5,232 women and 24,306 yearly observations. Findings showed that hormonal contraceptive use has positive average treatment effects on sexual frequency and sexual satisfaction, but no (or only a negligibly small) impact on well-being (i.e., depressiveness, life satisfaction, and self-esteem). Notably, the analysis revealed a relatively high heterogeneity in the effects of hormonal contraceptive use on sexuality, with some women experiencing significant positive effects and others little to no change. In contrast, the effects on well-being were characterized by relatively low heterogeneity, suggesting a more uniform response across women. Despite the strengths of these analyses, challenges remain in estimating and explaining treatment heterogeneity, with variability in individual treatment responses that require further exploration. Together, these three empirical studies contribute to a deeper understanding of how age and hormonal contraceptive use shape key psychological factors involved in women’s reproductive decision making, highlighting both stable patterns and individual variability in partner preferences, sexuality, and well-being. The findings underscore the importance of considering individual differences (e.g., neuroticism) in contraceptive counseling, paving the way for more personalized, evidence-based guidance in clinical practice. Given the potential for public misunderstanding and the increased likelihood of media amplification, especially in light of recent polarized discourse around hormonal contraceptive use, research in this domain also carries the responsibility of contextualizing psychological effects carefully, in order to avoid unnecessary alarm and to support informed, equitable access to contraception.2025-09-1
Die Rolle der Glutathion S-Transferase Omega 1 in einem nativen und Cisplatin-resistenten Keimzelltumor
Testicular germ cell tumors represent the most common malignant disease in young men between the ages of 20 and 40. Histologically, they are classified according to the WHO classification into different subtypes, with a clinically relevant distinction made between seminomas and non-seminomas for therapeutic purposes. Since the introduction of cisplatin-based high-dose chemotherapy, excellent cure rates have been achieved. Nevertheless, a clinically relevant proportion of patients does not respond adequately to treatment due to cisplatin resistance. Currently, reliable biomarkers for the early detection of such resistance as well as for the development of novel therapeutic approaches are lacking.
Glutathione S-transferase omega 1 (GSTO1), an isoform of the glutathione S-transferases, is involved in various biochemical processes of biotransformation. Increased expression of GSTO1 has been associated with several pathological processes, including tumorigenesis and drug resistance. In the present study, GSTO1 was analyzed using mass spectrometry-based proteomics in both cisplatin-sensitive and cisplatin-resistant cell lines. GSTO1 was found to be upregulated in the resistant cell line, suggesting a possible functional role in the development of cisplatin resistance.
To further explore this, a GSTO1 knockdown was performed in order to investigate its effects on cellular activity and relevant signaling pathways. Following cisplatin exposure, a dose-dependent reduction in metabolic activity was observed, with the resistant cell line showing a diminished response. Control experiments with non-specific RNA confirmed the specificity of these effects. In addition, GSTO1 knockdown led to alterations in key signaling pathways such as JNK, AKT, and ERK, which play crucial roles in the regulation of proliferation and apoptosis.
In summary, the results of our experimental model suggest that GSTO1 contributes to the establishment of cellular survival mechanisms. Enhanced activation of survival signaling as well as stabilization of resistance support a potential role of GSTO1 as a biomarker. This may open up novel therapeutic avenues for overcoming cisplatin resistance in testicular germ cell tumors.2025-11-2
Impact of physical fitness on physical strain and quality of external chest compressions
Hintergrund:
Qualitativ hochwertige, externe Thoraxkompressionen (ECC) sind entscheidend für eine erfolgreiche kardiopulmonale Reanimation (CPR). Eine ausreichende Kompressionstiefe und -frequenz aufrechtzuerhalten, ist körperlich anstrengend und wird durch Ermüdung der Helfer negativ beeinflusst. Die körperliche Fitness kann sowohl die Qualität der Kompressionen als auch die körperliche Belastung beeinflussen, insbesondere wenn die Thoraxkompressionen unter korrigierendem Feedback durchgeführt werden. Über die körperliche Belastung während qualitativ hochwertiger ECC ist jedoch wenig bekannt.
Methoden:
Daten von vierzig ausgebildeten medizinischen Fachkräften (30 Männer, 10 Frauen) wurden analysiert. Die körperliche Fitness wurde mittels Fahrrad- und Ruderergometrie mit kontinuierlicher Spirometrie und Herzfrequenzmessung bestimmt. Anhand der Herzfrequenz bei 75 Watt (HR75) im Rudertest wurden die Teilnehmenden in die Gruppe „fit“ oder „unfit“ eingeteilt. Jeder führte zwei neunminütige ECC-Sequenzen an einer Reanimationspuppe ohne und mit Feedback (Kompressions-Ventilations-Verhältnis 30:2) durch. Kompressionsleistung (Tiefe, Frequenz) und Belastungsparameter (Atemfrequenz [AF], Herzfrequenz [HF], respiratorischer Quotient [RQ]) wurden unter beiden Bedingungen analysiert. Zur Abschätzung der körperlichen Belastung wurde der Verlauf der Herzfrequenz während Feedback-korrigierter ECC mit der individuellen maximalen Herzfrequenz (HFmax) aus der Ruderergometrie verglichen.
Ergebnisse:
Ohne Feedback erreichten nur 45 % die korrekte Kompressionstiefe und 60 % die korrekte Frequenz; die Kompressionstiefe nahm unabhängig von der Fitness ab (r = .282). Unfitte Helfer komprimierten zu flach und zu schnell, während fitte Helfer im Zielbereich lagen. Der Anstieg der körperlichen Belastung während der Herzdruckmassage ohne Feedback war unabhängig von der Fitness (AF: r = .075; HF: r = .065; RQ: r = −.041). Feedback verbesserte die Kompressionsqualität, die Belastung stieg jedoch bei unfitten Helfern stärker an als bei fitten (AF: r = .204; HF: r = .335; RQ: r = .252). Die körperliche Belastung entsprach einer Ruderleistung von 75–100 Watt.
Schlussfolgerung:
Die körperliche Fitness hat einen wesentlichen Einfluss auf die Qualität der ECC und die individuelle körperliche Belastung des Helfers während der Herzdruckmassage. Feedback verbessert die Kompressionsqualität, kann jedoch zu einer Überlastung und potenziellen Gefährdung unfitter Helfer führen. Die Thoraxkompressionen sollten von dem fittesten verfügbaren Helfer durchgeführt werden. Regelmäßiges Ausdauertraining und definierte Fitnessanforderungen (75–100 Watt) werden für Fachkräfte empfohlen.Background:
High-quality external chest compressions (ECC) are essential for successful cardiopulmonary resuscitation (CPR). Maintaining adequate compression depth and rate is physically demanding and affected by rescuer fatigue. Physical fitness may influence both compression quality and physical strain, particularly when corrective feedback is provided. However, little is known about the physical strain on rescuers during high-quality ECC.
Methods:
Data from forty trained medical professionals (30 male, 10 female) were analyzed. Fitness was assessed using bicycle and rowing ergometry with continuous breath-by-breath ergospirometry and heart rate (HR) monitoring. Based on heart rate at 75 watts (HR75) during rowing, participants were classified as “fit” or ”unfit.” Each performed two nine-minute ECC sequences on a manikin (compression/ventilation ratio 30:2), one without and one with feedback. Compression performance (depth, rate) and physiological parameters (respiratory rate [RR], HR, respiratory quotient (RQ]) were recorded under both conditions. To estimate workload, heart rate progression during feedback-controlled ECC was compared with the individual maximal heart rate (HRmax) during rowing.
Results:
Without feedback, only 45% achieved correct compression depth and 60% the correct rate; depth declined over time independently of fitness (r = .282). Unfit rescuers performed shallower and faster compressions, whereas fit rescuers remained within target ranges. The increase in physical strain during ECC without feedback was independent of fitness (RR: r = .075; HR: r = .065; RQ: r = -.041). Feedback improved compression quality, but caused a stronger increase in strain among unfit rescuers (RR: r = .204; HR: r = .335; RQ: r = .252). Workload during high-quality ECC corresponded to rowing at 75–100 watts.
Conclusion:
Physical fitness substantially affects ECC quality and individual physical strain. Feedback enhances compression quality but may overstrain unfit rescuers, posing potential health risks. ECC should be performed by the fittest available rescuer. Regular endurance training and defined fitness requirements (75–100 Watt workload) are recommended for professionals.2025-12-2
Multicentric evaluation of the online elective Culinary Medicine in the winter semester 2020/2021 and summer semester 2021
Non-communicable diseases (NCDs) are one of the greatest challenges in modern medicine. NCDs include cardiovascular diseases, cancer, chronic pulmonary diseases, and diabetes. All of these diseases are associated with four common risk factors: tobacco use, excessive alcohol consumption, poor diet and insufficient physical activity.
Not only does nutrition play a major role in the development of many diseases, but nutrition can also be a therapeutic approach.
Despite this important role of nutrition in medicine, it is clearly underrepresented in teaching.
In order to strengthen the role of nutrition in medical teaching, some Culinary Medicine curricula are already being taught with great success, particularly in the USA.
For this reason, a new CM curriculum based on the LEKuP was developed at the Universitätsmedizin Göttingen and taught for the first time in Göttingen, Brandenburg and Giessen in the summer semester 2020/2021.
The aim of this study was to conduct a multicentre evaluation of the new Culinary Medicine teaching concept.
The curriculum comprises seven modules, each with 4 teaching hours. The modules include a theoretical introduction and a subsequent practical cooking unit on the respective indications. Due to the pandemic, the course was conducted virtually. Five questionnaire dimensions were used to collect standardized data on consultation skills, attitudes towards nutritional medicine in everyday medical practice, nutritional knowledge, personal eating and cooking habits and the subjective well-being of participants (WHO-5) before and after the course.
Students' counselling competences improved significantly (n= 70, p≤ 0.001). An increase in specialist knowledge and an improvement in well-being were also significant.
Even when delivered virtually, the new Culinary Medicine teaching concept is an effective method of promoting medical students' specialist knowledge and practical advisory skills in nutrition-associated diseases. Optimization of the diagnostic process for post-traumatic stress disorder and complex post-traumatic stress disorder according to ICD-11
Die Etablierung der Diagnose der komplexen Posttraumatischen Belastungsstörung (kPTBS) in das neue Klassifikationssystem (ICD-11) hat neue diagnostische Verfahren diesbezüglich mit sich gebracht. Jedoch erfassen diese Verfahren nicht differenziert Symptomhäufigkeit und Leidensdruck der (k)PTBS-spezifischen Symptomatik getrennt voneinander. Zudem wurde weder eines dieser Verfahren explizit zur Behandlungsplanung und zum Symptommonitoring entwickelt noch ist ein entsprechendes Tool vorhanden. Ziel der vorliegenden Arbeit war es, diese Lücke zu schließen, indem das Trauma-Inventar-Göttingen zur Erfassung von (komplexer) Posttraumatischer Belastungsstörung nach ICD-11 (TIGER) zur Behandlungsplanung und Verlaufsmessung entwickelt und untersucht wurde. Der TIGER erfasst die Häufigkeit und den Leidensdruck der (k)PTBS-Symptomatik sowie von Funktionsbeeinträchtigungen getrennt voneinander. Die in der Dissertation enthaltenen Studien untersuchten folgende, im Hinblick auf die Behandlungsplanung und Verlaufsmessung relevante Aspekte: Die Nutzungsintention von Behandler*innen für den TIGER, die interne Struktur des TIGER sowie therapierelevante Unterschiede zwischen der Posttraumatischen Belastungsstörung (PTBS) und der kPTBS. Die erste Studie untersuchte die Nutzungsintention des TIGER von Behandler*innen von Patient*innen mit Traumafolgestörungen. Die Ergebnisse zeigten, dass die Behandler*innen den TIGER als nützlich und praktikabel wahrnehmen und für die Behandlungsplanung sowie für die Verlaufsmessung einsetzen würden. Die zweite Studie untersuchte die Retest-Reliabilität des TIGER an Patient*innen mit PTBS oder kPTBS bei einer Latenz von zwei Wochen. Die Ergebnisse zeigten moderate bis gute ICC-Werte der Skalen von .66 (Wiedererleben) bis .86 (Problematische Beziehungsgestaltung) hinsichtlich der Symptomhäufigkeit und von .56 (Traumaassoziierte Vermeidung) bis .90 (Problematische Beziehungen) hinsichtlich des Leidensdrucks. Die dritte Studie konnte aufgrund der unerwartet langsam verlaufenden Rekrutierung und hohen Dropouts noch nicht abgeschlossen werden, weshalb in der Arbeit Zwischenergebnisse präsentiert werden. Vorab wurde die interne Struktur des TIGER anhand einer Stichprobe mit Patient*innen mit Verdachtsdiagnose oder gesicherter Diagnose einer PTBS, Borderline-Persönlichkeitsstörung (BPS) oder beidem, überprüft. Dabei konnten interne Konsistenzen (Cronbachs Alpha) von .75 (Hyperarousal) bis .91 (Negatives Selbstbild) für die Symptomhäufigkeit und von .82 (Hyperarousal) bis .89 (Affektive Dysregulation) für den Leidensdruck ermittelt werden. Hinsichtlich der konvergenten Validität reichten die Korrelationen zwischen den TIGER-Skalen und den Skalen des International Trauma Questionnaire (ITQ) für die PTBS-Symptomatik von r = .50 (Traumaassoziierte Vermeidung) bis r = .76 (Wiedererleben) und für die DSO-Symptomatik von r = .59 (Negatives Selbstbild) bis r = .68 (Problematische Beziehungen). Hinsichtlich der diskriminanten Validität fielen die Korrelationen mit entsprechenden Skalen des Borderline Personality Questionnaire (BPQ) deutlich geringer aus. Die vorläufigen Ergebnisse der Untersuchung zu den Unterschieden in den klinischen Merkmalen zwischen der PTBS und kPTBS ergaben eine große Variabilität innerhalb beider Gruppen sowie in der kPTBS-Gruppe einen signifikant höheren Leidensdruck und eine signifikant höhere Symptomhäufigkeit in der Skala Traumaassoziierte Vermeidung. Weiterhin konnten keine signifikanten Unterschiede zwischen der PTBS und kPTBS hinsichtlich des Alters zum Traumatisierungszeitpunkt, der Anzahl interpersoneller Traumata, der Depressivität und der Ängstlichkeit gefunden werden.
Insgesamt zeigten die Studien, dass der TIGER ein nützliches und praktikables Tool zur Behandlungsplanung und Verlaufsmessung darstellt, welches Symptome differenziert erfasst und detailliert abbildet. Zur Optimierung der Diagnostik von PTBS und kPTBS nach ICD-11 wird folgendes Konzept vorgeschlagen: 1) vorläufige Diagnosestellung mittels ITQ und Clinical Checks (ITQ-CC), 2) Diagnosesicherung mittels International Trauma Interview (ITI), 3) Behandlungsplanung und Verlaufsmessung mittels TIGER, 4) Postmessung mittels ITQ und ITQ-CC. Im Hinblick auf weitere Forschung ist es primär wichtig, die Studie zu den Unterschieden in den klinischen Merkmalen final abzuschließen, um daraus therapierelevante Informationen für die Behandlungsplanung ableiten zu können. Das TIGER-Tool sollte zudem im Rahmen einer traumapsychotherapeutischen Behandlung untersucht werden, um sowohl die Akzeptanz von Patient*innen als auch die Erfassung von Veränderungen von Symptomen mittels des TIGER überprüfen zu können.The establishment of the diagnosis of complex post-traumatic stress disorder (cPTSD) in the new classification system (ICD-11) has brought with it new diagnostic procedures. However, these procedures do not differentiate between the frequency of symptoms and the level of suffering of the (c)PTSD-specific symptoms. Furthermore, none of these procedures has been explicitly developed for treatment planning and symptom monitoring, nor is a corresponding tool available. The aim of this thesis was to close this gap by developing and investigating the Trauma-Inventory Göttingen for the exploration of (c)PTSD according to ICD-11 (TIGER) for treatment planning and symptom monitoring across time. The TIGER assesses the level of suffering and frequency of (c)PTSD symptoms as well as functional impairments separately. In this thesis, the studies examined the following aspects relevant to treatment planning and symptom monitoring: The intention of therapists to use the TIGER, the internal structure of the TIGER, and differences in clinical characteristics between post-traumatic stress disorder (PTSD) or cPTSD. The First Study investigated the intention to use the TIGER among therapists of patients with trauma-related disorders. The results showed that the therapists perceived the TIGER as useful and practicable and would use it for treatment planning and symptom monitoring. The Second Study examined the retest reliability of the TIGER in patients with PTSD or cPTSD at a latency of two weeks. The results showed moderate to good ICC values for the scales ranging from .66 (Re-Experiencing) to .86 (Problematic Relationships) in terms of symptom frequency and from .56 (Trauma-Associated Avoidance) to .90 (Problematic Relationships) in terms of level of suffering. The Third Study could not yet be completed due to unexpectedly slow recruitment and high dropouts, which is why preliminary results are presented in this thesis. First, the internal structure of the TIGER was tested using a sample of patients with a suspected or confirmed diagnosis of PTSD, Borderline Personality Disorder (BPD) or both. Internal consistencies (Cronbach's Alpha) ranging from .75 (Hyperarousal) to .91 (Negative Self-Image) for symptom frequency and from .82 (Hyperarousal) to .89 (Affective Dysregulation) for level of suffering were determined. With regard to convergent validity, the correlations between the TIGER scales and the scales of the International Trauma Questionnaire (ITQ) for symptoms of PTSD ranged from r = .50 (Trauma-Associated Avoidance) to r = .76 (Re-Experiencing) and for symptoms of DSO from r = .59 (Negative Self-Image) to r = .68 (Problematic Relationships). With regard to discriminant validity, the correlations with scales from the Borderline Personality Questionnaire (BPQ) were significantly lower. The preliminary results of the study on the differences in the clinical characteristics between PTSD and cPTSD revealed a large variability within both groups as well as a significantly higher level of suffering and a significantly higher symptom frequency in the Trauma-Associated Avoidance scale in the cPTSD group. Furthermore, no significant differences were found between the PTSD and cPTSD regarding age at the time of traumatization, the number of interpersonal traumas, depressiveness and anxiety.
Overall, the studies showed that the TIGER is a useful and practicable tool for treatment planning and symptom monitoring, which assesses symptoms in a differentiated and detailed manner. To optimize the diagnostic process of (c)PTSD according to ICD-11, the following concept is proposed: 1) preliminary diagnosis using the ITQ and the Clinical Checks (ITQ-CC), 2) diagnosis confirmation using the International Trauma Interview (ITI), 3) treatment planning and symptom monitoring using the TIGER, 4) post-measurement using the ITQ and the ITQ-CC. With regard to further research, it is primarily important to finalize this study on the differences in clinical characteristics in order to derive treatment-relevant information for treatment planning. The TIGER tool should also be examined in the context of trauma-psychotherapeutic treatment in order to be able to check both the acceptance of patients and the assessment of changes in symptoms using the TIGER.2025-11-0
