Charakterisierung von Endokrinopathien und Knochenstatus bei behandelten Beta-Thalassaemie-Patienten
Ziel dieser Arbeit war die Analyse der endokrinen Funktion sowie des Wachstums- und Knochenstatus bei Patientinnen und Patienten mit Beta-Thalassaemia major der Kohorte der KUNO-Kinderklinik des Universitätsklinikums Regensburg nach kurativer Therapie mittels allogener Stammzelltransplantation oder CRISPR/Cas9-Gentherapie. Untersucht wurden Patientinnen und Patienten im Alter von 5 bis 25 Jahren über einen Zeitraum von 2015 bis Ende 2023. Analysiert wurden unter anderem die kortikotrope, thyreotrope und gonadotrope Achse sowie Parameter des Elektrolythaushalts, der Knochendichte (DXA), des Wachstums und des Vitamin-D- und Kalzium-Stoffwechsels.
Die vorliegende Studie zeigt, dass eine kurative Therapie (allogene Stammzelltransplantation oder CRISPR/Cas9-Gentherapie) bei Patientinnen und Patienten mit β-Thalassaemia major unterschiedliche Effekte auf den endokrinen Haushalt und den Stoffwechsel entfalten kann. Während einzelne Achsen eine funktionelle Erholung aufwiesen, blieben andere dauerhaft beeinträchtigt.
Besonders hervorzuheben ist die beobachtete Reversibilität der partiellen corticotropen Insuffizienz bei einer Patientin, was auf eine funktionelle Regenerationsfähigkeit der adrenocorticotropen Achse nach metabolischer Stabilisierung hindeuten könnte. Auch ein präinterventioneller Hypoparathyroidismus erwies sich postinterventionell als nicht persistent. Im Gegensatz dazu zeigte sich bei zwei Patientinnen und Patienten eine fortbestehende gonadotrope Insuffizienz, welche durch eine Hormonsubstitution behandelt werden musste und somit die Vulnerabilität der Gonadenfunktion gegenüber eisenbedingter oder chemotherapieinduzierter Schädigung unterstreicht. Zudem entwickelte ein Patient nach der kurativen Therapie eine Autoimmunthyreopathie vom Typ Hashimoto, was die Notwendigkeit einer endokrinologischen Langzeitüberwachung insbesondere im Hinblick auf Autoimmunphänomene nach Stammzell-Transplantation unterstreicht.
Die Analyse der somatotropen Achse ergab bei einem relevanten Teil der Kohorte eine signifikante Verbesserung von IGF-1-Werten und Körpergröße, wobei vor allem ältere Patientinnen und Patienten mit pubertärem Wachstumspotenzial profitierten. Der BMI blieb insgesamt stabil.
Hinsichtlich der Knochengesundheit zeigte sich eine stabile Knochendichte trotz bekannter Risikofaktoren wie einer periinterventionellen Glukokortikoidexposition. Die DXA-Z-Werte blieben unterhalb des Normbereichs, was angesichts des jungen Alters und des noch ausstehenden „peak bone accrual“ altersphysiologisch erklärbar ist. Die beobachtete Stabilität kann als positives Signal gewertet werden, möglicherweise unterstützt durch verbesserte Kalziumspiegel und Vitamin-D-Zufuhr.
Der Glukosestoffwechsel zeigte sich bei zwei Patientinnen und Patienten mit Thalassaemie-assoziiertem Typ-3c-Diabetes weiterhin beeinträchtigt. Trotz antihyperglykämischer Therapie mit Metformin und GLP-1-Rezeptoragonisten kam es zu keiner Abnahme des Insulinbedarfs. Dies stimmt mit der Literatur überein, welche auf die fortbestehende Pankreasdysfunktion durch persistierende Eisenablagerung hinweist.
Insgesamt bestätigt die Analyse die Multisystemnatur der β-Thalassaemie und hebt die Notwendigkeit einer langfristigen, interdisziplinären Betreuung hervor – insbesondere zur Überwachung und Therapie endokriner Folgeerkrankungen, des Wachstums, der Knochendichte und des Glukosestoffwechsels.
Die vorliegenden Ergebnisse verdeutlichen, dass kurative Therapieansätze wie die allogene Stammzelltransplantation und die CRISPR/Cas9-basierte Gentherapie das Potenzial haben, endokrine Dysfunktionen bei Patientinnen und Patienten mit β-Thalassämia major zu stabilisieren oder sogar rückgängig zu machen. Gleichzeitig bleiben bestimmte Organsysteme – insbesondere die gonadotrope Achse und der Glukosestoffwechsel – vulnerabel gegenüber irreversiblen Schädigungen. Dies unterstreicht die Bedeutung einer strukturierten endokrinologischen Nachsorge auch nach erfolgreicher kurativer Therapie. Künftige Studien sollten größere Kohorten über längere Zeiträume hinweg einschließen, um prognostische Marker für die endokrine Erholung zu identifizieren und individuelle Therapiestrategien weiter zu optimieren.
Diese Studie liefert wichtige Erkenntnisse zur endokrinen Regenerationsfähigkeit nach kurativen Therapieansätzen bei β-Thalassämia major und zeigt auf, welche hormonellen Achsen besonders anfällig für persistierende Funktionsstörungen bleiben. Die differenzierte Analyse endokriner Folgeerscheinungen nach Stammzelltransplantation und Gentherapie legt die Grundlage für personalisierte Nachsorgekonzepte und zielgerichtete Interventionen. Die beobachtete partielle Reversibilität einzelner Achsen eröffnet neue Perspektiven für eine frühzeitige diagnostische Stratifizierung und therapeutische Steuerung. Translational betrachtet betonen die Ergebnisse die Notwendigkeit, endokrinologische Parameter systematisch in zukünftige Studien zur Bewertung kurativer Therapien zu integrieren – sowohl als Surrogatmarker für Therapieerfolg als auch zur Identifikation möglicher Interventionsfenster
Thermal rectification in a qubit-resonator system
A qubit-oscillator junction connecting as a series two bosonic heat baths at different temperatures can display heat valve and diode effects. In particular, the rectification can change in magnitude and even in sign, implying an inversion of the preferential direction for the heat current with respect to the temperature bias. We perform a systematic study of these effects in a circuit QED model of qubit-oscillator system and find that the features of current and rectification crucially depend on the qubit-oscillator coupling. While at small coupling, transport occurs via a resonant mechanism between the subsystems, in the ultrastrong coupling regime the junction is a unique, highly hybridized system and current becomes largely insensitive to the detuning. Correspondingly, the rectification undergoes a change of sign. In the nonlinear transport regime, the coupling strength determines whether the current scales sub- or superlinearly with the temperature bias and whether the rectification, which increases in magnitude with the bias, is positive or negative. We also find that steady-state coherence largely suppresses the current and enhances rectification. Metabolisches Tumorvolumen in der FDG-PET/CT nach Induktionstherapie bei aggressiven Non-Hodgkin-Lymphomen
Diese Arbeit untersucht die PET-Positivität und das Metabolic Tumor Volume (MTV) in der End-of-Treatment-PET (EoT-PET) bei Patienten mit aggressiven Non-Hodgkin-Lymphomen anhand des Patientenkollektivs der OPTIMAL>60-Studie, einer deutschlandweiten, multizentrischen, prospektiven, randomisierten Studie, durchgeführt zwischen November 2011 und Januar 2024. Dazu wurden 1050 EoT-PETs der Studie mithilfe des Software-Paktes syngo.via von Siemens unter Anwendung des Deauville-Scores (DS) ausgewertet. DS 1, 2 und 3 wurden als PET-negativ, DS 4 und 5 wurden als PET-positiv gewertet.
Ziele der Forschung waren die Analyse der MTV-Messung mit verschiedenen Volumenschwellen, der Vergleich zweier PET-Befundungsverfahren (quantitativ SUV-basierter Deauville-Score versus visuelle Bewertung), die Überprüfung der Reproduzierbarkeit quantitativer PET-basierter Biomarker sowie die Analyse hinsichtlich eines Tumor-Sink-Effekts.
Es konnte gezeigt werden, dass die visuellen Kriterien der OPTIMAL>60-Studie zu einer signifikant höheren Positivitätsrate als die moderne Lugano-Klassifikation führen und dass in der EoT-PET die Messung des MTV von Herden, die nach Lugano-Kriterien PET-positiv sind, mit hoher Reproduzierbarkeit möglich ist
Are multiple limb injuries in severely injured patients negligible? Evaluation of progression and outcome using a new scoring system for extremity injury: the Extremity Severity Score (ESS)
Introduction
Extremity injuries appear to have less impact on the mortality of multiple trauma patients (ISS ≥ 16). The Primary Survey of the Advanced Trauma Life Support (ATLS) only lists pelvic and femur fractures among the extremity injuries. The aim of this study was to evaluate the role of multiple extremity injuries in terms of lethality, progression, and complications in multiple trauma patients and the actual influence of concomitant blood loss. The Extremity Severity Score (ESS) was developed as a central instrument for this purpose.
Material & methods
This investigation is a retrospective single center study at a Level I trauma center over the period 2008–2019. The study cohort was identified as patients who met an Injury Severity Score (ISS) of at least 16. People who were < 16 years old at the time of the accident, patients who were primarily treated in other hospitals or transferred directly from the trauma bay, and cases without a Revised Injury Severity Classification Score 2 (RISC2) were excluded. Similar to the calculation of the ISS or NISS, the three most severe limb injuries (including the bony pelvis, corresponding to the ISS region of the extremities) were squared and added together to calculate the ESS. The study cohort was divided into the groups ESS ≥ 16 and ESS < 16 and these were examined with regard to the primary endpoint of lethality and several secondary endpoints. In addition to the univariate analysis of the data set, a logistic regression model was calculated.
Results
Out of 3.101 cases 1.227 patients and 5.824 extremity injuries met the inclusion criteria. Both unadjusted lethality and Standardized Mortality Rate (SMR) were not significantly different for the EES 0.05). Patients in both groups died most frequently from Traumatic Brain Injury (TBI) (72.9%/47.4%), followed by exsanguination (9.8%/19.3%) and Multi Organ Failure (MOF) (6.8%/17.5%). More patients in the ESS ≥ 16 group died of exsanguination (4.6% vs. 23.1%, p = 0.007), while patients in the ESS < 16 group died more frequently of TBI (77.0% vs. 30.8%, p = 0.002). For the secondary endpoints, there were significantly more surgical interventions (2.5 vs. 7.6, p ≤ 0.001), an increased blood transfusion rate (20.3% vs. 50.6%, p ≤ 0.001) and longer ICU (8.9 d vs. 12.1d, p ≤ 0.001) and total hospital stay (8.9 d vs. 12.1 d, p ≤ 0.001) for the ESS ≥ 16 group.
Conclusion
In this study multiple severe extremity injuries did not influence lethality but the clinical course. ATLS is right for the first moment. However, treating more extremity injuries requires more resources. The result of comparable lethality can only be achieved, if a hospital can provide these resources for this vulnerable patient group. Patients with injuries to several extremities therefore still require special attention
Hepatic resection for ovarian cancer in Germany: a nationwide epidemiological study based on DRG (diagnosis-related groups) data (2020–2024)
Background
Hepatic metastases from epithelial ovarian cancer (EOC) represent an advanced disease stage, yet national guidelines provide no standardized recommendations for liver resection. This study aimed to describe the national trends and patterns of hepatic resections in EOC patients based on German hospital billing data (DRG system) from 2020 to 2024.
Methods
A retrospective epidemiological analysis was conducted using InEK (Institute for Hospital Remuneration Systems) datasets from 2020 to 2024. We extracted data related to diagnosis ICD code C56 (malignant neoplasm of the ovary) and OPS codes for hepatic resections (5-501.0, 5-502.0, 5-502.2, 5-502.3, 5-502.4, 5-502.5, 5-502.6). Case numbers, procedure frequency, hospital characteristics, and DRG classifications were analyzed.
Results
A total of 1,273 hepatic resections were performed in patients with ovarian cancer between 2020 and 2024. Annual case numbers ranged from 225 to 283, indicating stable surgical practice over time. Local excisions / atypical resections accounted for the majority (n = 1,165; 91%), while segmentectomies (n = 85; 6.7%) and major resections (n = 23; 1.8%) were less frequent but consistently performed. Age group analysis showed that the largest proportion of patients undergoing hepatic resection were aged 65–74 years (range 23%–27% yearly). followed by the 60–64 and 55–59 age groups. Younger patients (< 40 years) represented less than 5%. Most procedures (54.6%) were conducted in public hospitals with ≥ 1000 beds, with additional contributions from private non-profit (20.9%) and private for-profit (10.5%) institutions. A gradual increase in the share of private institutions was observed over time. The mean hospital stay varied by resection type: 16.7 days for local excisions, 17.1 days for segmentectomies, and 20.1 days for major resections.
Conclusion
Hepatic resection for ovarian cancer is performed regularly in Germany, primarily at high-volume centers, suggesting a growing clinical acceptance of aggressive cytoreduction strategies despite the absence of formal recommendations. Our findings underline the need for prospective, multicenter studies and guideline updates addressing hepatic metastases in ovarian cancer
Immunogenicity of a trivalent haemorrhagic fever vaccine candidate against Sudan virus, Marburg virus and Lassa virus in an mpox vaccine
A multivalent vaccine targeting high-consequence infectious diseases in Sub-Saharan Africa (SSA), which are linked to high mortality, morbidity and overlapping clinical manifestations, would significantly improve health security and economic stability in this region. Trivalent vector vaccines were devised to deliver digitally optimized versions of Orthoebolavirus, Orthomarburgvirus glycoproteins (GPs) and a Lassa mammarenavirus (LASV) nucleoprotein (NP) by a single Modified Vaccinia Ankara (MVA) known to protect against mpox virus (MPXV) along with a matched DNA vaccine. Three immunizations in mice and Hartley guinea pigs with MVA only or a DNA prime followed by two MVA administrations induced comparable levels of binding antibodies and LASV-specific T-cells, respectively. While DNA priming mitigated MVA-specific antibody responses, GP- and NP-specific antibodies developed already after a single MVA vaccination. Although a post-outbreak Ebola virus vaccine is available, outbreaks by other filoviruses, annual LASV epidemics and increased incidence of MPXV infections support the rationale for an MVA-based trivalent haemorrhagic fever vaccine for endemic and high-risk human populations in SSA
Transformation der Innenstädte. Wirtschaftliche Performance von Mixed-Use Immobilien. Eine empirische Analyse
Diese Studie analysiert die Auswirkungen von Mischnutzung (Mixed-Use) in innerstädtischen Immobilien auf Immobilienpreise, insbesondere in Berlin und Frankfurt am Main. Die zentrale Frage der Studie ist, ob eine höhere funktionale Diversifizierung von innerstädtischen Immobilien zu einem Preisauf- oder Preisabschlag führen kann. Für die Analyse wurden tatsächlich getätigte Transaktionen von gemischt genutzten Immobilien mittels linearer und nicht-linearer Hedonik untersucht. Insgesamt flossen 1.100 Transaktionen in Berlin und Frankfurt a. M. im Zeitraum von neun respektive sechs Jahren in die Analyse ein, deren Nutzungsmischung mittels eines skalaren Diversifizierungsindex abgebildet wird. Außerdem wurde untersucht, inwiefern die Preiswirkungen von lokalen Marktbedingungen und der jeweiligen Stadtstruktur abhängen. Die Ergebnisse zeigen deutliche Unterschiede im Einfluss der funktionalen Diversifizierung auf die Immobilienpreise zwischen den beiden Städten: In Frankfurt a. M., das durch eine kompakte, monozentrische Stadtstruktur mit einem dominanten Bürosektor geprägten Markt gekennzeichnet ist, kann ein ausgeprägter und statistisch robuster Preisaufschlag für Gebäude mit hoher Nutzungsdiversifizierung identifiziert werden. Dies deutet darauf hin, dass in Frankfurt am Main eine gezielte Diversifizierung innerhalb eines Objektes als wertsteigernd wahrgenommen wird – vermutlich aufgrund der begrenzten Flächenverfügbarkeit und der effizienzorientierten Nachfrage am Markt. Im Vergleich zeigt sich im polyzentrischen Berlin mit einer historisch gewachsenen Mischnutzungsstruktur ein geringerer Preiseffekt für Nutzungsdiversifizierung. Zugleich zeigt sich in Berlin eine andere Preisdynamik, da auch bereits moderate Diversifizierung als positiv und wertsteigernd wahrgenommen wird. Unterhalb dieser stadtspezifischen Diversifizierungsgrade für einen positiven Preiseffekt zeigt die empirische Analyse ebenfalls negative Preiszusammenhänge auf, die sich in der Tendenz bis zum Diversifizierungsgrad für Single-Use Objekte fortsetzen. Diese Ergebnisse verdeutlichen, dass Diversifizierung kontextabhängig von Investoren wahrgenommen wird und eine erfolgreiche Mixed-Use Strategie die städtebaulichen und marktspezifischen Strukturen individuell berücksichtigen muss. Ferner liefert diese Studie eine Indikation dafür, wie Mischnutzung durch Investoren in den unterschiedlichen Stadttypologien wahrgenommen wird. Da bisher nur zwei Städte untersucht wurden, muss diese Interpretation eher als plausible Hypothese anstatt als Beleg gesehen
werden. Richtig bleibt jedoch, unabhängig von der dahinterliegenden Begründetheit, dass eine differenzierte Planung erforderlich ist, um Marktpotenziale für Mixed-Use-Gebäude optimal zu nutzen und positive Preisimpulse zu generieren
Development of a Novel Homogeneous Liposome-Based One-Step Assay for SARS-CoV-2 Antibody Detection in Human Serum Based on Fluorescent Liposomes and Complement Activity
Monitoring antibodies in patient serum enables the diagnosis of infectious and chronic diseases, the assessment of an individual’s immune status, and possible protection against infection and could thus be a ubiquitous tool for pandemic preparedness and personalized medicine. Advancing from the traditional enzyme-linked immunosorbent assay (ELISA), a homogeneous assay format was developed that simplifies assay procedures and enables a wash-free one-step performance. SARS-CoV-2 was chosen as the model virus, and its Spike protein-derived receptor binding domain (RBD) was covalently coupled to fluorescent liposomes. The binding of patient antibodies triggered the complement system, led to liposome lysis, and allowed quantitative fluorescent detection. The liposome assay was optimized with respect to liposome lipid composition, RBD coverage and surface chemistry, incubation conditions, and pretreatment of a standardized complement source. A proof-of-principle was demonstrated through artificially supplemented anti-RBD antibodies and full titrations with known positive sera. Testing of 37 SARS-CoV-2 negative sera and 28 sera from individuals with SARS-CoV-2 (breakthrough) infections resulted in a specificity of 95%, sensitivity of 93% and an excellent correlation (R2 = 0.82, Spearman r = 0.90) with antibody titers determined in an ELISA approved for diagnostic use. Finally, the liposome assay showed a good correlation to a pseudovirus neutralization test (pVNT) (R2 = 0.72, Spearman r = 0.84), similar to the diagnostic ELISA. As the new liposome assay does not require any wash steps and can be easily adapted to other viral targets by changing the surface antigen, it provides a new avenue for high-throughput immunodiagnostics
