    Guidelines for the Optimal Assessment of Airway to Predict Difficult Intubation

    Managing the airway is the cornerstone of anesthesia care. However, difficult airway or intubation is not clearly defined. A major complication with a difficult airway is a cannot intubate, cannot ventilate (CICV) situation, which is reported to be as high as 10%. In addition, according to the American Society of Anesthesiologists (ASA) Closed Claim Study, Adverse respiratory events are the most common type of injury, with difficult intubation [DI] and ventilation contributing to most of the cases. Twenty-eight percent of all anesthesia deaths are related to a CICV situation. As anesthesia evolves, basic airway assessments were developed and include mallampati (MP), thyromental distance (TMD), upper lip bite test (ULBT), and interincisor distance (IID). However, not one basic airway assessment accurately predicts a DI. The problem is inappropriate or inadequate airway management because a difficult airway can lead to an emergency, such as an anoxic brain injury, respiratory compromise, or even cardiac arrest. The DNP project aims to implement a standardized guideline for the preoperative assessment tool, the LEMON law, as the standard of care for preoperative airway assessment in patients undergoing surgical operations requiring endotracheal tube (ETT) intubation. Theoretical implementation plan at a medical center in the Midwest utilizing a direct supervision anesthesia model to enact the Plan-Do-Check-Act (PDCA) model to evaluate the effectiveness and outcomes relating to the prediction of difficult airway, the prevalence of hypoxia, and effect on patients.     Evidence-Based Practice Guidelines for the Surgical Patient with Obstructive Sleep Apnea

    Obstructive Sleep Apnea (OSA) is a medical condition which many people may be affected by but may be unaware of the presence of the condition. The incidence of OSA has increased in direct correlation with the rising rate of obesity in the general population. While chronic conditions may arise if OSA goes untreated, patients with OSA also have an increased risk for acute complications following surgical procedures such as airway obstruction, hypoxia, brain damage, and death. Many anesthetic medications administered during surgery exacerbate the pathological consequences of OSA, predisposing patients to adverse respiratory events during the recovery period following a procedure. As advancements in medicine and the methods of anesthesia delivery continue to be made, there are certain techniques which can be included in the care plan of a patient with OSA to mitigate the risks associated with the disease. This project aims to create evidence-based practice guidelines (EBP) for the care of patients with OSA who may be at an increased risk for developing respiratory complications following surgical procedures. It was developed utilizing The Iowa Model Revised: Evidence-Based Practice to Promote Excellence in Healthcare. Although many of the postoperative complications seen in patients with OSA are preventable, updated guidelines for the care of this patient population have not been published since 2014.     Feasibility of an Integrated Online Fantasy Sports Platform to Promote Meditation Activity in a Male Undergraduate Population

    Given the growing global burden of mental health disorders, the use of smartphone-based mindfulness meditation applications has skyrocketed. However, such applications have failed to attract a substantial base of male users and suffer from poor attrition and adherence rates across genders. It was proposed this may be due to a lack of basic psychological need satisfaction (BPNS) as outlined by self-determination theory (SDT). It was theorized that online fantasy sports could compensate for possible shortcomings of conventional meditation apps in inspiring consistent usage by men. Using a randomized control trial design, this study tested an eight-week, gamified meditation intervention linking the mechanics of fantasy sports to weekly meditation goals (termed “mindful fantasy sports”) in an all-male sample (n = 24). A secondary aim of the study was to determine if mindful fantasy sports were more intrinsically motivating than conventional meditation apps by measuring BPNS. The current findings showed participants in the mindful fantasy sports groups meditated significantly more often and for a significantly longer cumulative duration than peers in the control condition. Additionally, those playing mindful fantasy sports reported higher degrees of BPNS, specifically higher competence satisfaction. These findings support the notion that mindful fantasy sports can be effective in the promotion of meditation activity in a male population. Thus, mindful fantasy sports could be a promising mental health tool with potential for scalability and wide dissemination

    The Effects of Road Salt on Larval Gray Treefrog (Hyla Versicolor) Development and Plastic Responses to Predation Threat

    The extensive use of road salt has increased the salinity of aquatic ecosystems, leading to negative impacts across trophic levels and potential interactions with other biotic stressors. Predation threat can alter tail morphology, slow larval growth, and accelerate metamorphosis in gray treefrogs (Hyla versicolor). The present study aimed to examine the effects of road salt on H. versicolor growth and development and explore potential impacts on plastic predation responses. Larvae were reared in 30 outdoor mesocosms under no-salt-added (110 mg Cl-/L), low (230 mg Cl-/L), and high (1,000 mg Cl-/L) chloride concentrations, with and without a dragonfly larvae (Pachydiplax longipennis) predator. Caged predators were fed five H. versicolor tadpoles daily to induce kairomone production and generate antipredation responses. Salinity and predation facilitated developmental and morphological changes in H. versicolor larvae and induced interactive effects on tail morphology. Salinity induced greater larval growth rates and increases in tail and organ size. Predation did not affect larval growth or any measures of tail morphology, but also increased organ size. While both stressors decreased time to metamorphosis, they produced opposing results, with higher salinity inducing larger froglets and predation inducing smaller froglets. Interactive effects on tail morphology were found, indicating that plastic predation responses were altered at high salt concentrations. This study has important implications for how increases in environmental salinity levels will affect H. versicolor as morphological and developmental changes in larvae can have greater impacts on fitness post-metamorphosis in the terrestrial environment

    An Analysis of Fish Biodiversity and Ecosystem Function in the Scioto River Watershed Since the 1990s

    Biodiversity is a critical aspect for ecosystem conservation. High levels of biodiversity increase trophic structure and nutrient flow, which are vital for ecosystem function. Rivers and their tributaries comprise watersheds where small, first-order streams flow into second-order streams and so on as streams get larger and, presumably, their communities more diverse. Historically, this was so but fish communities have declined in Ohio due to anthropomorphic changes to watersheds. The Clean Water Act of 1972 aimed to increase water quality by assessing ecosystem function. In Ohio, the Ohio EPA utilizes fish to assess water quality and has developed a large database on fish abundance and distribution. Biodiversity and environmental stability in the Scioto River Watershed were determined from these data. Species Richness (S) was significantly greater in higher-order streams (p \u3c 0.05), except between fourth and fifth order streams. Likewise, Shannon-Weiner Diversity Index (H’) significantly increased with higher-order streams (p \u3c 0.05), except for third and fifth-order streams (p=1.000), and fourth and fifth-order streams (p = 0.103). The Index of Biotic Integrity (IBI) scored the highest in fourth-order, then third-order, fifth-order, second-order, then first-order streams. The total number of species (S) increased over time in third-order and the combination of fourth and fifth-order streams since the 1990s (p \u3c 0.05), and H’ increased in first, third, and the combination of fourth and fifth order streams (
