    Extra-gonadal germ cell tumour – what about the testis!

    Extra-gonadal germ cell tumours (EGGCT) are rare. Therefore further investigations of the testis is aimed at sourcing a possible primary origin of gonadal tumour. Over the years, various case series on EGGCT have been reported questioning its true nature as in a majority of them, a primary source is found in the testis, thus representing a metastatic gonadal tumour. The testis pathology could be either a true germ cell foci, an intra-tubular epithelial neoplasia or an area of fibrosis, indicating a „burnt out tumour‟. We report a 39-year-old male who underwent laparotomy and excision of a retroperitoneal tumour. Histopathological examination revealed retroperitoneal lymph node of mixed germ cell tumour origin. Clinical and ultrasound examination of bilateral testis was normal. The patient refused orchidectomy or a testicular biopsy. He underwent four cycles of bleomycin, cisplatin, and etoposide with no evidence of tumour recurrence on follow up and remains disease free after 12 months of diagnosis. A literature review of EGGCT, its relation and factors relating with future testicular tumour is presented

    Dramatic response of iris and retinal neovascularisation following usage of intravitreal ranibizumab as pre-operative adjunct in vitrectomy for advanced proliferative diabetic retinopathy

    A 37-year-old Malay woman presented with progressive deterioration in vision and was diagnosed with advanced proliferative diabetic retinopathy with neovascular glaucoma. Intravitreal ranibizumab injection (an anti-vascular endothelial growth factor) was administered prior to vitrectomy. Slit lamp assessment 2 days post-injection revealed significant regression of both iris and retinal neovascularisation. This resulted in adequate reduction of intra-ocular pressure prior to surgery. In addition, the regression of retinal vessels reduced the risk of intra-operative haemorrhage, thus aiding the surgical excision of the fibrovascular membranes. Periodic post-operative assessment in the first 3 weeks showed minimal inflammation and no recurrence of vitreous haemorrhage. This case illustrates that intravitreal ranibizumab has a role as an adjunct therapy prior to diabetic vitrectomy to significantly reduce the risk of intra-ocular haemorrhage

    Swelling behaviors and characterization of oil palm empty fruit bunch-graft-poly (Acrylamide) superabsorbent polymer composites

    The development on a class of technologically advanced materials like superabsorbent polymer composites (SAPC) involved multidisciplinary research started when its potential is recognized. Oil palm empty fruit bunch-graft-poly (acrylamide) (OPEFB-g-PAAm) SAPC was synthesized via simultaneous crosslinking and graft copolymerization of acrylamide (AAm) monomer onto OPEFB backbone with N,N’-methylenebisacrylamide (MBA) and ammonium persulphate (APS) acts as crosslinker and initiator, respectively, along with neutralization using solution polymerization method. Systematically, variables that affect the swelling behaviors of OPEFB-g-PAAm SAPC were optimized. The effects of initiator, crosslinker and filler concentration as well as solvent quantity and the degree of neutralization on water absorbency of OPEFB-g-PAAm SAPC were studied. Maximum absorbency of OPEFB-g-PAAm SAPC was achieved at 589.06 g/g when the amount of APS and MBA were 1.5 wt.% and 0.15 wt.%, respectively, and the OPEFB loading was at 10 wt.%. The optimum amount of solvent used was 15 mL and the degree of neutralization was 20%.     Mapping of faults in the Libyan Sirte Basin by magnetic surveys

    Magnetic surveys were carried out in Farigh area which is located in the eastern part of the Libyan Sirte basin. Interpretation of the onshore magnetic anomaly of this area, suggests that the high total magnetization may be caused by an intrusive body. Analysis of the magnetic power spectra indicates the presence of four sub-anomalies at depths of 340 m, 1400 m, and 2525 m which is probably related to the igneous rocks. The presence of igneous rock as basement at depth of 4740 m was confirmed by drilling. Assuming that all rock magnetization in the area is caused by induction in the present geomagnetic field, it strongly suggests that the causative structure has a remnant magnetization of declination (D) = −16° and inclination (I) = 23°. Based on pseudogravity data, the total horizontal derivative map shows high gradient values in NW-SW trends related to the structures in the eastern part of the Sirte basin. The 3D Euler deconvolution map derived from gravity data clearly indicates the location of igneous body in the study area as well as its tectonic trends and depth, which is estimated at 350 m to 1400 m below the surface. Depth of gravity anomalies at 1400 m to 2525 m is considered as anomalies in between shallow and deep. Anomaly at depth of approximately 4740 m below the surface is interpreted as basement rock. Geologically, the magnetic survey shows that the source of anomaly is a mafic igneous rock of Early Cretaceous age.     Kesan penambahan protein soya terhadap pengerasan & kekuatan mampatan simen brushit-HA

    Kajian ini meliputi penyediaan simen kalsium fosfat yang terdiri daripada campuran serbuk brushit (DCPD) dan hidroksiapatit (HA) yang dihasilkan secara sintesis dalam makmal. Larutan penimbal digunakan sebagai fasa cecair dan dicampurkan dengan campuran serbuk DCPD dan HA bagi menghasilkan simen tulang secara in situ dengan nisbah cecair kepada serbuk (L/P) antara 0.26 – 0.36 mL g-1. Sodium alginat dan protein soya ditambahkan ke dalam simen kalsium fosfat dan dikaji kesannya terhadap simen yang dihasilkan. Keputusan kajian menunjukkan simen kalsium fosfat dapat dihasilkan pada nisbah L/P 0.32 mL g-1 dengan penambahan 1%bt sodium alginat dan 1%bt protein soya dengan kebolehsuntikan yang maksimum (100%) dan tempoh pengerasan selama 26 min. Penambahan protein soya juga telah meningkatkan kekuatan mampatan simen tulang daripada 2.90 MPa (tanpa protein) kepada 3.98 MPa pada 1.5%bt protein. Simen kalsium fosfat-protein soya yang dihasilkan juga menunjukkan sifat kebioaktifan. Pertumbuhan apatit berlaku pada hari ke-3 selepas rendaman di dalam larutan simulasi badan (SBF) dan membesar dengan perlanjutan tempoh rendaman

    Penulisan ini adalah berkaitan perkembangan kreativiti dan pengurusan organisasi seni persembahan pentas iaitu teater moden yang merangkumi perancangan rekabentuk, sumber manusia dan kreativiti dalam sesuatu pementasan. Pengurusan seni persembahan mempunyai peranannya tersendiri dalam menyumbang kepada sesuatu seni pementasan yang berkesan. Ia secara langsung melibatkan aktiviti rekabentuk yang harus dilalui oleh pereka set dan pasukan dalam menghasilkan sesebuah rekaan dan binaan set teater yang mantap. Kajian ini dilakukan di Istana Budaya, Kuala Lumpur pada 2006. Kaedah yang digunakan dalam kajian ini adalah kualitatif (temubual). Hasil penyelidikan ini dapat membantu memgumpulkan data serta menganalisisnya demi mengutarakan hujah-hujah dan jawapan yang tepat dan berfeadah. Hasil dari kajian ini mendapati bahawa pereka set dan pasukan dalam produksi berperanan penting bagi menghasilkan sesebuah produksi yang mantap dan bermutu.Objektif utama artikel ini ialah untuk mengenalpasti psikologi pemikiran dan dorongan kerja yang wujud antara pereka set dan ahli pasukan dalam organisasi produksi teater di Istana Budaya. Artikel ini juga untuk mengenalpasti pemantauan dalam rekaan dan binaan set antara pereka set dan ahli pasukan dalam organisasi produksi teater di Istana Budaya. Penyelidik telah mengenalpasti tiga aspek penting yang harus diberi perhatian oleh pereka set dan pasukan produksi dalam proses menghasilkan rekaan dan binaan set. Penggunaan carta morfologi untuk setiap peringkat akan membantu bagi menghasilkan keberkesanan proses rekaan dan binaan set untuk sesebuah persembahan. Penggunaan templet petak lukisan yang berupa storyboard dapat membantu menghasilkan kejelasan visual setiap proses bagi semua peringkat produksi yang akan dipersembahkan.
