Global Training in Inflammatory Bowel Disease: Addressing Growing Challenges in IBD
No full textInflammatory Bowel Disease (IBD) is increasing in incidence globally, with low- and middle-income countries (LMICs) facing a rapid surge in disease burden. However, the availability of structured training programs for IBD care remains highly unequal. This review evaluates the current global landscape of IBD workforce training, with an emphasis on identifying gaps and proposing solutions to enhance multidisciplinary preparedness. The IBD care team extends beyond gastroenterologists and includes surgeons, radiologists, pathologists, IBD nurses, dietitians, and mental health professionals. Yet, structured training pathways and credentialing mechanisms are scarce outside of high-income countries. Key barriers include limited access, financial constraints, inadequate infrastructure, and lack of standardized curricula. Emerging digital platforms, simulation-based education, and role-specific training frameworks offer promising strategies. Developing globally harmonized curricula, enhancing funding models, and fostering international collaborations are essential to build a resilient and equitable IBD workforce. Addressing these disparities is critical to improving global IBD outcomes
Prosecuting Nihilistic Violent Extremism: An Examination of Federal and State Charges Against 764 and Related Networks
No full textIn April 2025, U.S. authorities arrested two alleged leaders of 764, a 21-year-old U.S. citizen residing in Greece and a 20-year-old U.S. citizen from North Carolina. They were arrested on charges of operating an international child-exploitation enterprise, in connection with the online terrorist network ‘764’. US Attorney General Pamela Bondi described the group as “one of the most heinous online child exploitation enterprises we have ever encountered – a network built on terror, abuse, and the deliberate targeting of children”. The arrests marked a significant development in federal efforts to disrupt and dismantle the network, while simultaneously underscoring the scale of the problem. In December 2025, the FBI reported that it was actively conducting more than 350 investigations connected to 764 and other nihilist violent extremist (NVE) groups, while the number of victims is estimated to reach into the thousands globally and new cases continue to emerge. Internationally, Canada is the first country to officially designate 764 as a terrorist entity under its Criminal Code thereby granting authorities expanded legal tools to disrupt their activities. In the absence of a similar U.S. designation, prosecutors are limited in the types of charges that can be applied to offenders. Atomic Offsets: Nuclear Policy Shifts for Arctic States in the Wake of Moscow’s Special Military Operation
No full textThis article examines how Russia’s 2022 invasion of Ukraine has intensified nuclear reliance among Arctic states. Applying a new framework of “conventional-nuclear vulnerability,” it analyzes the nuclear policy shifts of Sweden, Finland, and Russia. The findings show that Finland and Sweden pursued NATO membership to secure nuclear guarantees, while Russia adopted riskier nuclear signaling and lowered thresholds for nuclear use. These developments highlight the growing centrality of nuclear deterrence in Arctic security, underscoring the challenges for U.S. and NATO policymakers in balancing assurance and deterrence in a volatile regional environment.

A.C.T.S.: Accessing Care for Trafficking Survivors
No full textHuman trafficking has been a persistent societal problem with significant costs for states, the nation, and across the world. Victims struggle to recognize that they have endured having been trafficked and society lacks the awareness and understanding that is necessary to help. While many organizations have implemented programs aimed at helping victims escape and recover, victims often remain unaware of what resources are available.
Advancements in technology may allow victims a bridge to connecting with services and could offer assistance to law enforcement and prosecutors who are working to hold trafficking offenders accountable. Many younger victims today feel comfortable using technology and the use of AI could help to build trust and rapport which could be transferred to advocates.
Research indicates victims often do not know about local resources. Research also indicates youth victims are more likely to report their traffickers than other members of society. Since most victims of trafficking have access to phones, developing a mobile application to connect them to services and anonymously report offenders may help victims receive help sooner. This app may allow law enforcement and prosecutors to hold offenders accountable.
From writing grant proposals to the launch of the app it is expected to take approximately 2 years. Regular monitoring and analysis of data collected by the app will be necessary. The overall cost of this project is expected to be between 1,421,834 for both development and the first fiscal year after launch. Funding should come from grants and donations.
Quantitative analysis of referrals and prosecutions will determine the success of the app. This project is limited to youth victims in the Omaha area, but it is recommended this pilot program be expanded geographically and demographically. If successful, this app could offer a solution to help victims, educate society, and sentence offenders
An Aquatic Treadmill Alters Lower Limb Walking Dynamics in Typically Developing Children and Children with Cerebral Palsy
No full textThis block-randomized crossover study investigated how a speed-modulated aquatic treadmill (AT) impacts the walking biomechanics of pediatric gait. Eight cerebral palsy (CP) and fifteen typically developing (TD) children walked at normal, slow, and fast treadmill speeds in AT and dry treadmill (DT) conditions. The joint angles of participants were calculated from inertial measurement units to derive sample entropy (SE) measures that quantified the regularity or complexity of motion. A hierarchical statistical model revealed that the CP group had lower SE values for the hip, knee, and ankle joints in the AT and at slower than faster treadmill speeds. Only the SE values of the knee and ankle joints were impacted for the TD group. The lower SE values suggest improved regularity for participants at slower speeds and in the AT environment. This study highlights the potential of AT to improve the walking biomechanics of children with CP in acute exposure, but further work is needed to investigate the AT condition as a gait rehabilitation environment
