Tommy Orange’s Spider Symbolism as It Connects to Lacanian Psychoanalysis
The characters in Tommy Orange’s There There represent a collective “Urban Indian” experience that is made up of both hardship and perseverance. While some characters fall into the traps set for them by society and generational trauma, others find it within themselves to resist those traps and identify with their true “self”. By applying Lacanian psychoanalysis to the charter development seen in the novel, Lacan’s theories of “lack” and the “mirror stage” can be identified as playing a major role in the character’s choices and behaviors. This is specifically evident in Orange’s repetition in the use of spiders, webs, and even bumps on the skin of characters containing spider legs, as a metaphor that symbolizes and signifies a character’s emergence from the traps of generational trauma and finding the “self”
Prescreening for Dementia in the Older Adult Surgical Population: A Systematic Review
In the older adult surgical population, individuals with dementia are at higher risk for postoperative delirium (POD) which has been associated with a decline in patients’ postoperative function, longer hospital length of stay (LOS), and increased morbidity and mortality (Kassahun, 2018). Although the neuropathology of POD remains not fully understood, the pathology of dementia and Alzheimer’s disease (AD) in particular, has been highly researched. Alterations in dementia-associated biomarkers are seen up to a decade before clinical signs and symptoms are evident resulting in difficulty predicting POD in the older adult population (Olsson et al., 2016). Amyloid-ß 1-42 (Aß42), a dementia-associated biomarker most commonly seen in AD, that aggregates in the brain forming amyloid-ß plaques (Hall & Guyton, 2011). Research on Aß42 suggested that reduced cerebrospinal fluid (CSF) Aß42 concentration reflects the accumulation of Aß42 in the brain and seen before clinical emergence of the disease (Fagan, 2006). The purpose of this systematic review was to determine whether the preoperative CSF concentration of Aß42 in non-dementia older adult surgical patients predicts the incidence of POD. A comprehensive literature review was completed using CINAHL and PubMed/MEDLINE investigating POD and possible relationships with dementia and dementia-associated biomarkers. The PRISMA framework was used to identify eligible studies. Descriptive data tables were completed that reported pertinent data and study outcomes. Individual studies were assessed using the Critical Appraisal Skills Programme (CASP) checklist. A cross-study analysis table was created to compare outcomes across the studies. The findings of this systematic review are inconclusive but suggest that CSF Aß42 may be a potential independent predictor of POD in the older adult surgical population
Does Oral Chlorhexidine Mouth Care Prior to Intubation Impact Ventilator-Associated Pneumonia? A Systematic Review
Background: Chlorhexidine gluconate (CHG) is a broad-spectrum antiseptic agent that has become widely used for mouth care in intubated patients. Many studies have found it to be effective in the prevention of ventilator-associated pneumonia (VAP) when used after intubation; however, there is very limited research exploring the proper time to initiate CHG.
Purpose: The purpose of this systematic review was to determine if the use of oral care with CHG prior to intubation impacts the incidence of VAP. Methods: The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) was used to guide the selection process of articles and the Critical Appraisal Skills Programme (CASP) was used to critically appraise the randomized control trials (RCTs) selected for this systematic review. Four randomized RCTs met inclusion criteria.
Results: Three of the four RCTs which met inclusion criteria, Houston et al. (2002), DeRiso et al. (1996), and Lin et al. (2015), showed an improvement in VAP rates with the use of preintubation CHG in cardiac surgery patients. Only one RCT, the Munro et al. (2015) study, showed no benefit; this was the only study that included non-cardiac surgery patients.
Conclusion: Based on the results of this systematic review, it can only be recommended that cardiac surgery patients receive CHG prior to or after intubation; however, more research needs to be done to determine the most effective dosing, frequency, and CHG application procedure. In addition, further study exploring the safety of administering CHG prior to intubation in noncardiac surgery patients is needed
The Effect of Green Space on the Physiologic Function of the Adult: A Systematic Review
Environmental greenness is often associated with improved psychological outcomes, but the use of green space as a protective factor for maintaining physiologic health is understudied. However, growing evidence exists on the benefit of greenness on physiologic health. The purpose of this systematic review was to evaluate the effect of green space on the physiologic function of the adult. Cohort studies were searched for that had all elements of inclusion criteria. Six final studies were included in this systematic review utilizing PRISMA guidelines and CASP tool for cohort studies. Data from the studies was collected and a cross study analysis was conducted to compare all studies and assess for themes in study outcomes. Results of this review demonstrate that green space has a protective effect on physiologic health. Areas with higher levels of greenness are associated with lower prevalence of central obesity, diabetes mellitus, and self-reported rates of cardiovascular disease and stroke as well as decreased rates of mortality from cancers, and kidney and respiratory diseases. Implications of this study include the importance in understanding risk factors for development of disease. Awareness of a patient’s environment that includes natural spaces should be identified as a potential risk factor for the development of cardiovascular illness, obesity, and diabetes mellitus
