Bayesian optimization of Laser-Wakefield Accelerators
Laser Wakefield Accelerators (LWFAs) are compact, high-gradient particle accelerators capable of producing ultra-relativistic electron beams over centimeter-scale distances. By driving large amplitude plasma waves with intense laser pulses, LWFAs can achieve accelerating fields orders of magnitude greater than those in conventional radiofrequency accelerators. Despite their potential for applications in medicine, industry, and ultrafast science, LWFAs remain limited by challenges in reproducibility, stability, and control. Their performance is highly sensitive to small changes in laser and plasma conditions, and they exhibit complex, nonlinear behavior. Traditional manual tuning and heuristic optimization strategies are insufficient to reliably access the high-dimensional parameter space and optimize multiple competing beam metrics such as energy, charge, and bandwidth.
This thesis addresses these challenges by developing and implementing machine learning-based optimization strategies, specifically Bayesian optimization (BO), for the systematic control and tuning of LWFAs. After reviewing the theoretical foundations of Bayesian optimization, a novel multi-objective, multi-fidelity optimization framework termed Trust-MOMF is introduced. This method enables sample-efficient exploration of complex, high-dimensional parameter spaces using data sources with varying accuracy. Unlike traditional approaches, this method supports continuous fidelities without requiring monotonic improvement across fidelity levels, making it especially suitable for systems where measurements are noisy, expensive, and inconsistent.
The performance of Trust-MOMF is first demonstrated in numerical simulations of LWFAs using particle-in-cell codes. The method shows an order of magnitude improvement in convergence times, substantially reducing the computational cost compared to standard multi-objective optimization techniques. The surrogate model learned during the optimization process is then inverted to enable energy tuning of the accelerator and uncover trade-offs between competing objectives such as bandwidth and efficiency.
In the second part of the thesis, this framework is applied to real-world LWFA experiments using ATLAS which is a petawatt-class laser system. The number of laser shots per configuration is interpreted as a fidelity variable, allowing the optimization to balance information gain with experimental cost. For the first time, multi-objective multi-fidelity Bayesian optimization is demonstrated in LWFA experiments, yielding high-quality electron beams with significantly fewer laser shots. Furthermore, the trained models are used to perform fine-tuning and a posteriori single-objective optimization, enabling reproducible energy tuning between 150 and 400 MeV by navigating an 8-dimensional parameter space.
Together, these results establish a generalizable and efficient approach for controlling LWFAs, enabling automated, data-driven operation tailored to specific user requirements. This work represents a significant step toward deploying laser-plasma accelerators in practical settings, including user facilities that require flexible and rapid re-optimization of beam parameters. It will further enable real-time tuning strategies, automated control, and eventual deployment of LWFAs in scientific user facilities
Analysis and extension of earthquake rupture dynamics
Geophysics is a broad field that explains phenomenons on earth from an interdisciplinary and data rich perspective. One of its modern ramifications is earthquake rupture dynamics, in which the material properties and the stress field are collapsed onto a compact support in which maximum shear deformation localizes -or the fault plane is defined-, and its contextualization in the framework of friction brings a mechanical understanding of the energy budget throughout the fault, and thus of the dynamic propagation of the rupture. This nonlinear behaviour is reflected in the source characteristics that are identified, interpreted, and validated against observables, such as on-fault asperities, supershear occurrence, backward, bilateral and unilateral propagation, among others. This fundamentally data- and physics-driven approach faces particular challenges in constraining the initial conditions governing fault stresses and strengths, as it is highly sensitive to these parameters.
This dissertation explores the extension of the initial conditions and assumptions used in rupture dynamic models through physics-driven methodologies. We first present a diffuse volumetric fault representation as an alternative to the traditional assumption of an infinitesimally thin fault representation. The study presents a 2D PETSc spectral element adaptation, se2dr, which adopts the stress glut method with a steady-state phase-field ansatz to reduce inherent spurious oscillations from the stress discontinuities inherent to the original stress glut method. The model successfully replicates planar interface results, while revealing dynamic complexities such as fault-oblique yielding within the volumetric fault zone. In a next step, we adopt a state-of-art dynamic rupture software SeisSol to investigate the dynamics of the 2021 Mw 7.4 Maduo earthquake. In this study we inform 3D dynamic rupture simulations, accounting for off-fault plasticity, with geodetically-inferred on-fault stress heterogeneities. The model can explain the event's complex kinematics and observations, in particular the mechanical viability of unilateral supershear propagation across this multi-segment fault system and its associated observational signatures. We further inform 3D dynamic rupture simulations, by coupling its initial conditions to the output of a long-term regional geodynamic model from pTatin3D. We develop a workflow to extract a fault geometry from the shear zone emergent from the evolving plastic strain in the long-term model. The study compares the effect of choices of the rheology and its associated stress, consistent with the fault geometry, on the dynamics of earthquake, the energy released, and its impact as an rupture arresting mechanism.
The next part of this dissertation explores potential insights gained from a global geodynamic context, to understand large-scale deformation and the ambient stress field, thereby providing a contextual mechanical framework to regional studies. Following this line of thought, we extract maps of erosional/non-depositional periods, or hiatus, that serve as proxies for vertical surface deflections induce by mantle convection, or dynamic topography. This study highlights the temporal and spatial variation of such hiatus surfaces across continents, offering a test for mantle flow retrodictions and an observational tool to support the identification mantle flow regimes. Finally, we analyze the role of the mantle flow as a driver for the horizontal stress field from an analytical, Couette-Poisseuille flow representation of the asthenosphere. This study provides a process-driven explanation for global stress patterns observed in stress indicators compiled in the World Stress Map. It emphasizes the importance of considering a global domain in geodynamic studies, and carries implications on the expected asthenospheric stress magnitudes
Bridging theory and observation: advancing cosmology and astrophysics through constrained simulations and machine learning
Galaxy clusters are the largest gravitationally bound and virialized structures in the Universe and powerful probes of both astrophysical processes and cosmological parameters. However, extracting precise information from galaxy clusters remains challenging due to their highly non-linear nature and the difficulty of connecting resolution and model-limited simulations with observations in a physically accurate and meaningful way. This thesis addresses these challenges by combining the use of constrained cosmological simulations and simulation-based inference using machine learning.
We first present a novel suite of constrained hydrodynamical simulations, the SLOW (Simulating the LOcal Web) project, designed to reproduce the large-scale structure of the local Universe on a one-to-one basis. We show how these simulations accurately replicate the positions and properties of over 46 observed galaxy clusters. We establish that the clusters’ masses, temperatures, X-ray luminosities, and SZ signals are matched with high fidelity, validating the use of constrained simulations based on peculiar velocities as tools for creating replicas of real cosmic structures present in our local Universe.
In a second step, we analyze the thermodynamic properties of these simulated cluster replicas, revealing that large-scale and intermediate radial profiles are well modeled. However, core regions show strong discrepancies, specially in the entropy and gas density values. We will discuss how these point to the necessity of improvements in resolution as well as improvements in the current implementation of galaxy formation models. Additionally, we will link the observed thermodynamic features of local clusters to the clusters’ mass assembly histories recovered in the simulations, establishing a connection between the formation path of clusters and their late-time cool-core/non-cool-core classification in observations.
Finally, we demonstrate that machine learning models trained on stacked thermodynamic profiles from high-resolution zoom-in simulations of galaxy clusters can recover both cosmological and astrophysical parameters with remarkable accuracy. Using the CAMELS-zoomGZ suite, which spans the 28-dimensional parameter space from the IllustrisTNG model, we achieve an unprecedented inference accuracy for cosmological parameters as well as astrophysical parameters. We will show how the inference remains robust under high noise levels and limited radial coverage. Moreover, we will explore how inference done using stacked galaxy cluster profiles outperforms the inference done with integrated quantities, positioning cluster profiles as a powerful tool for simulation-based inference.
Together, the results of this thesis provide a unified framework that connects the structure and evolution of galaxy clusters with the underlying physical parameters that govern them. By bridging simulations, observations, and machine learning-based inference, this work aims to lay the foundation for a new generation of precision cosmology using galaxy clusters as astrophysical and cosmological laboratories.Galaxienhaufen sind die größten gravitativ gebundenen und virialisierten Strukturen im Universum und bieten zahlreiche Möglichkeiten zur Untersuchung astrophysikalischer Prozesse und kosmologischer Parameter. Die präzise Gewinnung von Informationen aus Galaxienhaufen ist jedoch nach wie vor eine große Herausforderung, da ihre hochgradig nichtlineare Natur und die begrenzte Auflösung und Modellierungstiefe numerischer Simulationen eine physikalisch akkurate und sinnvolle Verbindung mit Beobachtungen erschweren. Diese Dissertation adressiert diese Problematik durch die Kombination von constrained kosmologischen Simulationen und simulationsbasierter Inferenz mittels maschinellen Lernens.
Im ersten Teil stellen wir eine neuartige Reihe hydrodynamischer \textit{constrained} Simulationen vor, das SLOW-Projekt (Simulating the LOcal Web), das darauf ausgelegt ist, die großräumige Struktur des lokalen Universums im Eins-zu-eins-Vergleich realitätsnah nachzubilden. Wir zeigen, dass diese Simulationen die Positionen und Eigenschaften von über 46 beobachteten Galaxienhaufen mit hoher Genauigkeit reproduzieren. Insbesondere stimmen die Massen, Temperaturen, Röntgenleuchtkräfte und SZ-Signale der simulierten Haufen sehr gut mit den Beobachtungen überein, was den Einsatz constrained Simulationen auf Basis von Eigengeschwindigkeiten zur Rekonstruktion realer Strukturen im lokalen Kosmos eindrucksvoll bestätigt.
Im zweiten Schritt analysieren wir die thermodynamischen Eigenschaften dieser simulierten Haufenrepliken. Dabei zeigt sich, dass die Profile auf großen und mittleren Skalen gut modelliert werden. In den Kernregionen treten jedoch deutliche Abweichungen auf, insbesondere bei Entropie- und Gasdichtewerten. Dies unterstreicht die Notwendigkeit verbesserter Auflösung und einer weiterentwickelten Modellierung der Galaxienentstehung. Darüber hinaus stellen wir einen Zusammenhang zwischen den beobachteten thermodynamischen Eigenschaften lokaler Galaxienhaufen und deren Masseaufbau-Historien in den Simulationen her, wodurch wir eine Verbindung zwischen der Entstehung eines Haufens und seiner späteren Einordnung als Cool-Core- oder Non-Cool-Core-System aufzeigen.
Abschließend demonstrieren wir, dass maschinelles Lernen, trainiert auf gemittelten thermodynamischen Profilen aus hochaufgelösten Zoom-in-Simulationen von Galaxienhaufen, sowohl kosmologische als auch astrophysikalische Parameter mit bemerkenswerter Genauigkeit rekonstruieren kann. Mithilfe der CAMELS-zoomGZ-Simulationsreihe, welche den 28-dimensionalen Parameterraum des IllustrisTNG-Modells abdeckt, erreichen wir eine bislang unerreichte Inferenzgenauigkeit. Wir zeigen, dass diese Inferenz auch bei starkem Rauschen und begrenzter radialer Abdeckung stabil bleibt. Zudem analysieren wir, wie die Inferenz basierend auf gemittelten Profilen den Ansatz auf Basis integrierter Größen deutlich übertrifft. Somit zeigen wir, dass Galaxienhaufenprofile eine besonders leistungsstarke Datengrundlage für simulationsbasierte Inferenz bilden.
Insgesamt liefert diese Arbeit einen kohärenten Rahmen, der die Struktur und Entwicklung von Galaxienhaufen mit den zugrundeliegenden physikalischen Parametern verbindet. Durch die enge Verknüpfung von Simulationen, Beobachtungen und maschinellem Lernen legt diese Dissertation das Fundament für eine neue Generation präziser Kosmologie, welche den astrophysikalischen und kosmologischen Informationsgehalt von Galaxienhaufen vollständig ausschöpft
Chitinase A and Cytolysin A: important virulence factors of Salmonella enterica, secreted by type 10 secretion systems
Salmonella enterica is a foodborne pathogen which is spread by contaminated food or water. The infection by Salmonella leads to millions of illnesses and death cases per year. It is a facultative intracellular pathogen, which is able to invade into host cells and to survive and proliferate within the cells by translocation of effector proteins and secretion of toxins. During invasion, a membrane compartment emerges, which is restructured to a Salmonella-containing vacuole (SCV). While non-typhoidal serovars typically result in self-limiting gastroenteritis, typhoidal serovars have the capability to spread from the initial intestinal infection site to secondary sites within the human body. This leads to systemic disease and long-term bacterial persistence. S. enterica harbors several virulence factors crucial for colonizing the host and infiltrating diverse host target cells. We first focused on the pathogenicity factor chitinase A (ChiA). It was known that ChiA functions as an enzyme, breaking down chitin by cleaving its β1-4 glycosidic bonds. Our hypothesis was that ChiA also plays an important role for S. enterica virulence in a chitin-free environment such as the human host. It was our aim to elucidate the effect of ChiA on Salmonella invasion into human cells and further to clarify the effect of transcriptional regulators and other proteins encoded in the chiA operon on ChiA regulation and secretion. We found that the deletion of chiA leads to reduced invasion into human host target cells, comparable with data of other studies, published at about the same time. Intriguingly, our studies revealed that ChiA-mediated invasion occurs specifically in polarized human intestinal epithelial cells, whereas non-polarized or non-intestinal epithelial cells did not show reduced invasion phenotypes. In addition, we found that both cell-associated and free mucus lead to ChiA expression and secretion. This finding aligns with other studies showing that host cell glycans, which have structures similar to mucus, are targets of Salmonella chitinases. Furthermore, we unraveled the regulation of ChiA expression by a transcriptional regulator (STM0017) that we named ChiR. Finally, we showed that ChiA is actively secreted by a type 10 secretion system (T10SS) encoded within the chiA operon. Former studies have shown that the secretion of cargo proteins by T10SS in Gram-negative bacteria is facilitated by specified peptidoglycan hydrolases. They showed that these peptidoglycan hydrolases accumulate within the periplasm by traversing the inner membrane with the help of holins. Subsequently, the hydrolases cleave the peptidoglycan layer which enables the secretion of proteins through the bacterial cell wall. However, the final release mechanism through the bacterial outer membrane is currently unknown and needs further investigations in the future. In a second study, we focused on cytolysin A (ClyA), a barely described toxin known to be encoded only in strictly human-adapted typhoidal Salmonella serovars such as Typhi and Paratyphi A. ClyA is a pore-forming toxin and its homologue in Escherichia coli is well-described regarding its pore formation characteristics and regulation of expression. Using S. enterica serovar Paratyphi A, we demonstrated that ClyA represents a mainly intracellularly induced toxin that is exclusively expressed and secreted from bacteria residing within the SCV. Since its expression within the SCV correlates with the activity of a T10SS, which is responsible for the secretion of typhoid toxin, we hypothesized that the same secretion system also secretes ClyA. Using a TtsA peptidoglycan hydrolase mutant, we showed that ClyA secretion is completely abolished in this T10SS deficient mutant, thereby verifying our hypothesis. Once secreted, ClyA exhibit specific cytolytic activities towards macrophage-like THP-1 and U937 cells, but lacked activity towards epithelial cells such as HeLa, CaCo-2 and HT29-MTX cells. We postulate that during infection, ClyA is mainly produced within infected epithelial cells and aids Salmonella infection by lysing attracted macrophages after its release from epithelial producer cells. However, this assumption and the mechanistic details regarding the susceptibilities of different cell types need to be investigated in further studies.
In summary, this work elucidates the expression, regulation, and secretion of ChiA and ClyA, two important yet barely described virulence factors of S. enterica. Additionally, it demonstrates the relevance of two similar but slightly different T10SS and their impacts on the pathogenicity of S. enterica
Die 1-Stunden-Plasmaglukosekonzentration im oralen Glukosetoleranztest als Prädiktor für den Glukosestatus 5 Jahre postpartum
Hintergrund und Zielsetzung der Arbeit
In früheren Arbeiten verschiedener Gruppen zeigte sich eine Überlegenheit der 1-Stunden-Plasmaglukose (1h-PG) aus dem oralen Glukosetoleranztest (oGTT) bei der Prädiktion von Diabetes mellitus Typ 2 (T2D) in der Allgemeinbevölkerung. Nüchtern- (NPG) und 2-Stunden-Plasmaglukose (2h-PG) schnitten schlechter ab. So wurde eine neue T2D-Risikogruppe mit norm-wertigen NPG- und 2h-PG-Werten, aber erhöhter 1h-PG ≥ 155 mg/dl (NGT-1h-path) beschrieben, die durch die aktuellen Prädiabeteskriterien nicht erfasst wird. Gestationsdiabetes mellitus (GDM) weist ebenfalls auf ein erhöhtes Risiko für einen späteren T2D hin. Welche Frauen nach einem GDM aber besonders gefährdet sind, ist noch nicht ausreichend definiert. Im Rahmen dieser Arbeit sollte deshalb einerseits untersucht werden, ob die 1h-PG aus einen postpartalen oGTT hin-sichtlich ihrer prädiktiven Eignung im postpartalen T2D-Risikoscreening den NPG- und 2h-PG-Werten überlegen ist. Andererseits sollte betrachtet werden, ob eine routinemäßige Mitbestimmung der 1h-PG im postpartalen oGTT die Trennschärfe des Tests erhöhen könnte – also keine Überlegenheit dieses Werts, sondern seine zusätzliche Bestimmung als Ergänzung.
Methodik
Die Auswertungen der vorliegenden Arbeit erfolgten auf Basis von longitudinalen Daten des 5-Jahres-Follow-Ups von 131 Frauen der PPSDiab Studie. Als primärer Endpunkt wurde die Manifestation von T2D oder das Auftreten eines fortgeschrittenen Prädiabetes (kombiniert abnormale Nüchternplasmaglukose (IFG) und gestörte Glukosetoleranz (IGT)) beim 5-Jahres-Follow-Up untersucht. Für NPG, 1h-PG und 2h-PG wurden jeweils binär logistische Regressionsmodelle erstellt und Sensitivitäts- und Spezifitätsanalysen durchgeführt. Die NGT-1h-path Kohorte wurde im Ver-gleich zu NGT-Probandinnen mit normwertiger 1-PG < 155 mg/dl (NGT-1h-norm) und den Prädiabetespopulationen bezüglich klinischer und metabolischer Charakteristika verglichen. Außerdem wurde die Prävalenz von pathologischer Glukosetoleranz (PGT) innerhalb der beiden NGT-Kohorten zum Zeitpunkt 5 Jahre postpartum untersucht.
Ergebnisse
Die 1h-PG erwies sich in den logistischen Regressionsmodellen als ein signifikanter Prädiktor für T2D/kombinierte IFG+IGT beim 5-Jahres-Follow-up, verlor allerdings in einem kombinierten Modell mit NPG die Signifikanz. Bei der Prädiktion von T2D/kombinierte IFG+IGT war die 1h-PG mit dem Grenzwert von ≥ 155 mg/dl im Vergleich zu NPG und 2h-PG durch eine höhere Sensitivität und eine geringere Spezifität ausgezeichnet. Als Cut-off mit optimalem Verhältnis von Sensitivität und Spezifität wurde für die 1h-PG ein Grenzwert von ≥ 166 mg/dl ermittelt. Die NGT-1h-path Kohorte war im Vergleich zu NGT-1h-norm durch signifikant schlechtere Werte des Matsuda Index (4,58 (2,99 – 5,90) vs. 6,67 (4,42 – 8,02), p < 0,01), Disposition Index (215,09 (163,26 – 291,80) vs. 276,46 (197,69 – 338,78), p < 0,05) und der Fläche unter der Glukosekurve (294,50 (281,44 – 311,25) vs. 236,63 (214,06 – 249,63), p < 0,001) charakterisiert. Im Vergleich zu IGT und IFG waren die metabolischen Veränderungen von NGT-1h-path geringer ausgeprägt. NGT-1h-path und NGT-1h-norm unterschieden sich signifikant bezüglich der Prävalenz von PGT fünf Jahre postpartal (34,4% versus 14,3%, p < 0,05). Ein NGT-1h-path Status war mit einem 3,8-fach erhöhten Risiko (1,22 – 11,66, p < 0,05) für PGT beim 5-Jahres-Follow-Up verbunden.
Schlussfolgerung
In den vorliegenden Analysen konnte keine Überlegenheit der 1h-PG bei der Prädiktion von T2D/kombinierte IFG+IGT nachgewiesen werden, die einen Ersatz der 2h-PG durch die 1h-PG ermöglichen würde. Eine zusätzliche Mitbestimmung der 1h-PG in einem 3-Punkt oGTT könnte aber eine verbesserte Risikostratifizierung nach GDM ermöglichen.Objective
Previous investigations of various research groups showed a superior ability of the 1-hour plasma glucose concentration (1h-PG) obtained from the oral glucose tolerance test (oGTT) in predicting future type 2 diabetes (T2D) in the general population. In comparison, fasting (NPG) and 2-hour plasma glucose concentration (2-PG) performed worse. Additionally, a 1h-PG ≥ 155 mg/dl in sub-jects with normal fasting and 2h-PG (NGT-1h-path) reveals a currently undetected group of indi-viduals with an increased risk for T2D. Gestational diabetes (GDM) is related to an increased risk for future T2D as well. Currently, indications of women exhibiting a particularly high T2D risk are still subject to current research. Therefore, the object of the current analysis was to evaluate if the 1h-PG obtained from a postpartum oGTT examination is superior to NPG and 2h-PG regarding the postpartum T2D risk stratification. Furthermore, it was to be investigated whether an addi-tional determination of the 1h-PG in the postpartum oGTT displays an added value for the post-partum T2D risk screening in relation to an improved selectivity of the risk stratification.
Methods
In the current investigation longitudinal data of 131 women after GDM from the PPSDiab study were analyzed and T2D or advanced prediabetes (combined impaired fasting glucose (IFG) plus impaired glucose tolerance (IGT)) at five years postpartum was evaluated as the primary end-point. Binary logistic regression models as well as sensitivity and specificity analysis were per-formed for NPG, 1h-PG and 2h-PG. NGT-1h-path and NGT-subjects with 1h-PG < 155 mg/dl (NGT-1h-norm) as well as prediabetic subjects were compared regarding clinical and metabolic charac-teristics. Furthermore, the prevalence of pathological glucose tolerance (PGT) five years postpar-tum was analyzed.
Results
In the logistic regression models, the 1h-PG was a significant predictor for T2D/combined IFG+IGT, but lost significance in a combined model with NPG. In comparison to NPG and 2h-PG, the 1h-PG with the cut-off of ≥ 155 mg/dl showed a superior sensitivity, but a decreased specific-ity in predicting T2D/combined IFG+IGT. For the 1h-PG, a threshold from ≥ 166 mg/dl was deter-mined as the optimal cut-off regarding sensitivity and specificity. NGT-1h-path significantly dif-fered from NGT-1h-norm in insulin sensitivity (Matsuda index (4.58 (2.99 – 5.90) vs. 6.67 (4.42 – 8.02), p < 0.01)), disposition index (215.09 (163.26 – 291.80) vs. 276.46 (197.69 – 338.78), p < 0.05), and area under the glucose curve (294.50 (281.44 – 311.25) vs. 236.63 (214.06 – 249.63), p < 0.001). In comparison to IFG and IGT the metabolic parameters of NGT-1h-path were only slightly reduced. NGT-1h-path exhibited a higher prevalence of PGT five years postpartum than NGT-1h-norm (34.4% vs. 14.3%, p < 0.05) and showed a 3,8-fold increased risk (1.22 – 11.66, p < 0.05) for PGT at five years follow up.
Conclusion
To estimate the risk for future T2D/combined IFG+IGT, there was no benefit in a shortening of oGTT time by determining the 1h-PG instead of 2h-PG. However, an additional measurement of 1h-PG in routine oGTT might improve the postpartum T2D risk stratification after GDM

Mit der jährlich steigenden Anzahl von Kindern, die durch ART gezeugt wurden, rückt die langfristige und kurzfristige Gesundheit dieser Personen zunehmend in den Fokus. Aktuelle Studien äußern Bedenken hinsichtlich einer beeinträchtigten vaskulären Gesundheit bei ART-Kindern. Diese beinhalten eine generalisierte endotheliale Dysfunktion, eine erhöhte arterielle Steifigkeit und einen erhöhten Blutdruck. Im Gegensatz dazu konnten mehrere Studien diese Befunde nicht bestätigen und keine vaskulären Veränderungen bei ART-Kindern im Vergleich zu spontan konzipierten Kontrollen nachweisen. Eine verminderte vaskuläre Gesundheit trägt wesentlich zur Entwicklung von CVD bei, die eine der führenden Todesursachen unter den nicht übertragbaren Krankheiten darstellen. Eine frühzeitige und präzise Risikostratifizierung von Individuen mit potenziell erhöhtem kardiovaskulären Risiko ist daher von großer Bedeutung. Das Ziel der Forschung in den Artikeln „Vascular Health in Adults Born After Using Assisted Reproductive Technologies" und „Subjects Conceived through Assisted Reproductive Technologies Display Normal Arterial Stiffness" war eine umfassende Untersuchung der vaskulären Gesundheit von ART-Nachkommen im Vergleich zu spontan konzipierten Altersgenossen. Zusätzlich war die Bewertung der PWV als Maß für die globale arterielle Steifigkeit sowie des zweidimensionalen Speckle Trackings der Aorta abdominalis und der Karotiden – eine neuartige Methode zur Bewertung der regionalen arteriellen Steifigkeit – von Interesse.
Beide Single-Center-Studien, deren Probanden vor allem aus der Metropolregion München stammten, hatten das Ziel einer präzisen und umfassenden Bewertung des Gefäßsystems. Dies umfasste die Erhebung anthropometrischer Variablen, der Vorerkrankungen, des Schwangerschaftsverlaufs und der Geburt, des mütterlichen Bildungsniveaus, der körperlichen Untersuchung sowie der Ernährungsqualität. Die Endothelfunktion wurde mithilfe des EndoPAT™ 2000-Geräts bewertet. Der brachiale Blutdruck, der zentrale Blutdruck und die PWV wurden mit einem oszillometrischen Blutdruckmessgerät aufgezeichnet. Die cIMT beider Arteriae carotis communis wurde sonografisch bestimmt. Darüber hinaus wurde nach einer Fastenperiode von ≥4 h ein laborchemisches, kardiometabolisches Risikoprofil erstellt. Das zweidimensionale Speckle Tracking der Aorta abdominalis und der Karotiden erfasste die regionale arterielle Steifigkeit von ART-Teilnehmern und spontan konzipierten Altersgenossen. Die globale arterielle Steifigkeit wurde mittels PWV gemessen.
Die Ergebnisse der Studie „Vascular Health in Adults Born After Using Assisted Reproductive Technologies“ zeigten keine statistisch signifikanten Unterschiede in der endothelialen Funktion, dem brachialen Blutdruck, dem zentralen Blutdruck, der PWV, der cIMT und dem Lipidprofil zwischen jungen Erwachsenen, die nach ART und solchen, die spontan gezeugt wurden. Die weitergehende Unterteilung in die Untergruppen der ART-Behandlungen zeigte ebenfalls keine statistisch signifikanten Unterschiede in den untersuchten Gefäßparametern zwischen den Kohorten der IVF-, ICSI- und spontan gezeugten Gruppen.
In der Studie „Subjects Conceived through Assisted Reproductive Technologies Display Normal Arterial Stiffness“ zeigte sich, dass die regionale und globale arterielle Steifigkeit bei den ART-Teilnehmern und ihren spontan konzipierten Altersgenossen statistisch nicht signifikant unterschiedlich war. Eine Subanalyse von Personen ≥10 Jahren, einschließlich einer Korrelationsanalyse, wurde durchgeführt, um den Einfluss des Alters auf die untersuchten Parameter zu evaluieren. Auch hierbei konnten keine statistisch signifikanten Unterschiede in der regionalen und globalen arteriellen Steifigkeit zwischen beiden Gruppen festgestellt. Interessanterweise war der brachiale systolische Blutdruck bei den ART-Teilnehmern tendenziell höher war als bei den Kontrollen. In den Korrelationen zwischen den beiden Gruppen zeigten sich keine statistisch signifikanten Unterschiede.
Insgesamt konnten beide Studien keine Beeinträchtigung der Gefäßfunktion bei ART-Nachkommen nachweisen. Es fanden sich keine Anzeichen für ein erhöhtes Risiko arterieller Steifigkeit im zunehmenden Alter bei ART-Nachkommen, da in den gemessenen Parametern keine statistisch signifikanten Unterschiede zu spontan konzipierten Kontrollen festgestellt wurden. Zukünftige Studien mit multizentrischem Design und größeren Stichprobengrößen sind erforderlich, um eine präzise kardiovaskuläre Risikostratifizierung der erwachsenen ART-Bevölkerung zu ermöglichen. Dies schließt auch eine Evaluation einer möglicherweise erhöhten kardiovaskulären Morbidität aufgrund verfrühter Gefäßalterung im Lebensverlauf ein.With a yearly increasing number of children born after using ART, the long- and short-term health of these individuals has become a subject of great interest. Recent studies have raised concerns about impaired vascular health in ART children, including generalized endothelial dysfunction, increased arterial stiffness, and elevated blood pressure. In contrast, several studies have failed to validate these findings and could not detect vascular modifications in ART children compared to spontaneously conceived controls. Decreased vascular health contributes significantly to the development of CVD, one of the leading causes of death among non-communicable diseases. Therefore, early and precise risk stratification of individuals with potentially increased morbidity seems beneficial. The objective of research in the articles “Vascular Health in Adults Born After Using Assisted Reproductive Technologies” and “Subjects Conceived through Assisted Reproductive Technologies Display Normal Arterial Stiffness” was to conduct an extensive investigation into the vascular health of ART offspring compared to spontaneously conceived peers. Additionally, the evaluation of PWV as a measure for global arterial stiffness as well as two-dimensional speckle tracking of the abdominal aorta and both common carotid arteries, a novel method to assess regional arterial stiffness, was of particular interest.
Both single-center studies, whose participants primarily came from the greater Munich area, aimed for a precise and comprehensive evaluation of the vascular system. This included the assessment of anthropometric variables, prior medical history, course of pregnancy and birth, maternal educational level, physical examination, and diet quality. Endothelial function was assessed using the EndoPAT™ 2000 device. Brachial blood pressure, central blood pressure and PWV were recorded via an oscillometric blood pressure device. The cIMT of both common carotid arteries was determined sonographically. In addition, a cardiometabolic risk profile after a fasting period of ≥4 h was executed. Two-dimensional speckle tracking of the abdominal aorta and both common carotid arteries assessed regional arterial stiffness in ART subjects and spontaneously conceived peers. Global arterial stiffness was measured via PWV.
The study “Vascular Health in Adults Born After Using Assisted Reproductive Technologies” showed no statistically significant differences in endothelial function, brachial blood pressure, PWV, cIMT, or blood lipid levels between ART and spontaneously conceived young adults. Furthermore, no statistically significant differences in vascular function were found among the IVF, ICSI, and spontaneously conceived subgroups.
The results of the study “Subjects Conceived through Assisted Reproductive Technologies Display Normal Arterial Stiffness” suggest no statistically significant difference in regional and global arterial stiffness between the ART subjects and their spontaneously conceived peers. A sub-analysis of subjects aged ≥10 years, including a correlation analysis, was performed to assess the influence of age on the studied parameters. No statistically significant differences in regional and global arterial stiffness were found between the two groups. Interestingly, brachial systolic blood pressure tended to be higher in ART subjects compared to controls. No statistically significant differences were observed in the correlations between the two groups.
In conclusion, both studies indicate that there is no impairment in vascular function in ART offspring. There is no evidence suggesting an increased risk of arterial stiffness with advancing age, as no statistically significant differences in parameters of arterial stiffness between ART subjects and spontaneously conceived peers were observed. Future studies with a multicenter design and greater sample sizes are required for a precise cardiovascular risk stratification of the adult ART population. This should also include an evaluation of potential increased cardiovascular morbidity due to premature vascular aging over the life course
Die Impfbereitschaft des Gesundheitspersonals in stationären und ambulanten Einrichtungen während der COVID-19-Pandemie
Hintergrund: Die COVID-19-Pandemie stellt vor allem das Gesundheitspersonal in stationärer sowie in ambulanter Pflege vor neue Herausforderungen. Besonders sie sind einem hohen Infektionsrisiko ausgesetzt und stellen selbst ein großes Risiko für eine Infektionsübertragung auf die Pflegebedürftigen dar. Zusätzlich belasten strenge Isolationsmaßnahmen und neue Vorgehensweisen zur Infektionsbekämpfung die ohnehin schon durch den seit Jahren herrschenden Personalmangel stark beanspruchten Mitarbeiter.
Ziel der Studie: Welche Assoziationen können zwischen psychischen Erkrankungen und psychologischen Einflussfaktoren auf die Impfbereitschaft bei Gesundheitspersonal in der stationären und ambulanten Pflege in Bayern festgestellt werden?
Methoden: Neben soziodemografischen Daten wurden psychosoziale Determinanten, sowie Assoziationen zur Impfbereitschaft untersucht. Die Datenerhebung fand von März 2020 bis Februar 2023 im Rahmen des Bayerischen ambulanten COVID-19 Monitors statt (Deutsches Register klinischer Studien DRKS 26039). Neben soziodemographischen Faktoren wurden Symptome von Depression (PHQ-9) und Burn-out (MBI), sowie psychologische Einflussfaktoren auf die Impfbereitschaft (5C) ermittelt. Es wurde eine multivariate Regressionsanalyse mit schrittweiser Selektion der Variablen durchgeführt.
Ergebnisse: Insgesamt gingen n=220 Studienteilnehmer in die Analyse ein. Burnout-Symptome, sowie depressive Symptome beim Gesundheitspersonal wirkten sich negativ auf deren Impfbereitschaft aus. Ein höheres Bildungsniveau hatte einen positiven Einfluss auf die Bewertung von impfspezifischen Informationen. Strukturell wahrgenommene Hürden und ein geringeres Verantwortungsgefühl gegenüber anderen, waren Gründe für eine geringere Impfbereitschaft. Das Vertrauen in den Impfstoff war umso geringer, je häufiger das Gesundheitspersonal mit infizierten und pflegebedürftigen Menschen in Kontakt kam.
Schlussfolgerung: Die vorliegende Studie zeigt, dass eine Vielzahl von gruppenspezifischen psychosozialen Faktoren die Impfbereitschaft des Gesundheitspersonals während der COVID-19-Pandemie beeinflussen kann. Vor allem wird der Bedarf an gezielten Maßnahmen zur Verbesserung der Burnout-Prävention für das Gesundheitspersonal deutlich. Es ist daher dringend erforderlich, gezielte Interventionen umzusetzen, um rasch einen Effekt auf die noch laufende Pandemiesituation, sowie zukünftige mögliche Pandemien zu erzielen.Background: Since the COVID-19 pandemic started, health care workers in inpatient and outpatient care has been confronted with unknown obstacles. Especially health care workers in nursing homes were at a higher risk to be exposed to the virus and unintentionally contributed to the infection process. In addition to that, they had to cope with strict isolation measures, new strategies of infection control and therefore a higher workload, even though most of them already had to deal with burnout and depression before the COVID-19 pandemic.
Aim of the study: What associations can be found between mental disorders and psychological factors influencing the readiness to vaccinate among health care workers in inpatient and outpatient care facilities in Bavaria?
Methods: From the data of a register study running since March 2021, 220 eligible study participants were selected to represent the healthcare staff in inpatient and outpatient facilities throughout Bavaria (Germany). In addition to sociodemographic data, psychosocial determinants related to vaccination intention were investigated. A multivariate regression analysis with stepwise variable selection was performed.
Results: A total of n=220 study participants were included in the analysis. Burnout symptoms and depressive Syndromes among health workers negatively impacted vaccination intention, while higher education level positively impacted the evaluation of vaccination information. Structurally perceived barriers and a lower sense of responsibility towards others were reasons for lower intention to get vaccinated. Confidence in the vaccine was lower the more frequently healthcare workers came into contact with infected people and those in need of care.
Conclusion: The present study shows that a variety of group-specific psychosocial factors may influence healthcare workers' readiness to vaccinate during the COVID-19 pandemic. Most importantly, it highlights the need for targeted interventions to improve burnout prevention for healthcare workers, especially concerning depersonalization. The results detected associations between psychological determinants and vaccination readiness in this important professional group of the health sector. It is therefore urgent to implement targeted interventions to quickly achieve an effect on the ongoing pandemic situation as well as future possible pandemics
