Behavior-based safety with paramedics
Accident statistics from the German Federal and Railroad Accident Insurance (UVB) revealed that volunteer and full-time paramedics are exposed to an increased risk of workplace accidents. This trend is evident in the accident figures reported by the German Red Cross (DRK), which have been rising steadily since 2011. In part, the increased risk and number of accidents can be attributed to the perception of potential risks and the associated safety behavior of employees. Behavior-Based Safety (BBS) is recognized as one of the most well-researched and effective methods for changing behavior in the field of occupational safety. We report on our research using BBS to improve safety behavior among paramedics of the DRK. Results indicate that behavioral observations and feedback led to improvements in safety compliance in two rescue stations of the DRK. Resources for and obstacles to the implementation of BBS practices in the work of paramedics are discussed

Evaluation of GaN Transistors for Grid-Connected 3-Level T-Type Inverters
This paper presents a complete workflow for the evaluation of GaN transistors in voltage source inverters. With the associated high switching speed of transistors based on GaN, it is important to consider some critical points in the design phase as well as in the measurement setup in order to be able to utilise and verify the advantages of GaN properly. For this reason, the presented circuit board’s design focuses on a minimised power loop inductance. Simulation models, an analytical approach and measurement results with the aim of determining this inductance are compared with each other. A good compliance results between the presented methods. Additionally, the description of a test bench is given, which enables the performance of the opposition method. This setup allows the measurement of the designed H-bridge’s arising losses and the GaN-transistor’s switching behaviour. In comparison to the conventional double pulse method, this approach enables results that are more accurate for determining losses

Palliative Versorgung und Therapiezieländerung bei Herzinsuffizienz im ambulanten Bereich
Hintergrund
Palliative Versorgung ist Teil der ganzheitlichen Behandlung bei fortgeschrittener Herzinsuffizienz. Wie diese Versorgung im ambulanten Bereich in Deutschland derzeit tatsächlich aussieht, ist unklar.
Methode
Wir führten eine Umfrage zur aktuellen palliativen Versorgungspraxis bei fortgeschrittener Herzinsuffizienz unter niedergelassenen Kardiolog:innen und Hausärzt:innen in Deutschland durch. Die Umfrage wurde von einer multiprofessionellen Projektgruppe der Deutschen Gesellschaft für Kardiologie – Herz- und Kreislaufforschung e. V. entwickelt und durch den Bundesverband Niedergelassener Kardiologen e. V. online durchgeführt.
Ergebnisse
An der Studie nahmen 235 Personen teil. Eine Mehrheit der Befragten gab an, häufig oder immer mit Patient:innen mit fortgeschrittener Herzinsuffizienz über ihre Therapieziele gesprochen zu haben. Hausärzt:innen gaben wesentlich öfter als Kardiolog:innen an, immer oder häufig Aufgaben der palliativen Grundversorgung zu übernehmen. Keine:r der befragten niedergelassenen Kardiolog:innen, jedoch 35,1 % der Hausärzt:innen gaben an, bei Patient:innen mit fortgeschrittener Herzinsuffizienz häufig oder immer eine spezialisierte ambulante Palliativversorgung (SAPV) verordnet zu haben. Über 90 % der Kardiolog:innen gaben an, Patient:innen selten oder nie weiter zu betreuen, wenn diese nicht mehr selbst in die Praxis kommen konnten.
Konklusion
Nur wenige Niedergelassene in Deutschland überweisen ihre Patient:innen mit fortgeschrittener Herzinsuffizienz an spezialisierte palliativmedizinische Versorgungsstrukturen. Palliative Grundversorgung und die Involvierung der SAPV werden wesentlich häufiger von Hausärzt:innen als von niedergelassen Kardiolog:innen übernommen
The Distribution Logistics of Covid-19 Vaccines in Germany: A Case Study
Purpose of this paper: The global rollout of COVID-19 vaccines has been one of the most significant public health measures in modern history. The logistics systems of the vaccination campaigns were key to both their effectiveness and efficiency. This paper examines the logistical framework conditions of the German COVID-19 vaccination campaign and analyses vaccine distribution in ultra-cold supply chains in order to contribute to a better understanding of the design of challenging logistics systems in the field of tension between public and private actors.
Design/methodology/approach: We use a case study approach to look at the COVID-19 vaccination campaign in Germany where the federal structure gives the 16 federal states (the units of analysis in the case study) a decision-making authority in health-related issues that was also reflected in the various logistics systems that were implemented for the campaign. Primary data was obtained through in-depth interviews and enriched with secondary data from newspapers and other sources. The cases were analysed through the theory lens of the collaborative governance framework.
Findings:
Federalism leads to braking effects where a rapid, coordinated and technically excellent response would be required. Although the vaccination campaign was an overall success, some logistical processes lacked GDP (Good Distribution Practice) approval, so the start was a bit bumpy. The opportunity to develop standard procedures for similar situations in the future was not properly utilised, reducing preparedness to a level below what was possible.
Value: This case study is one of the few empirical works that deal with the logistical systems and processes behind vaccination campaigns. In particular, it sheds light on the interplay between public and private actors in a federal government structure.
Research limitations/implications:
Primary data could only be obtained from four of the 16 federal states. This is mainly due to the fact that dealing with the COVID-19 pandemic in Germany is still an open issue and an unparalleled political minefield.
Practical implications:
The research shows that standardised, GDP-approved and configurable logistics processes together with a strategic inventory of equipment, infrastructure and consumables are key elements to be prepared for similar situations in the future
