    Rethinking High-Grade Serous Ovarian Carcinoma: Towards Personalized Immunotherapy through Preclinical, Translational and Clinical Evaluation of Combined CD73 and PD-L1 Blockade

    Bakgrunn: Selv om høygradig serøs eggstokkreft (HGSOC) klassifiseres som en immunogen krefttype med lovende prekliniske resultater, er effekten av immunterapi beskjeden. Økende grad av persontilpasning og biomarkør-basert oppfølging kombinert med bedre prekliniske modeller for immunterapi kan øke behandlingsresponsen i HGSOC. Mål: Prosjektet skulle evaluere klinisk effekt i HGSOC av en ny kombinasjonsbehandling med to immunterapeutika, oleclumab og durvalumab (art. I). Biomarkører for behandlingsrespons og resistens skulle utforskes i blod (art. II). Videre skulle en humanisert musemodell for HGSOC med pasient-deriverte svulster (PDX) utvikles for at effekter i prekliniske funn og i pasientbehandling skulle gjenspeile hverandre bedre (art. III). Materialer og metoder: Oleclumab/durvalumab ble testet på 25 pasienter med tilbakefall av epitelial eggstokkreft i en klinisk utprøvingsstudie. Studien utforsket mulige immunhistokjemiske (IHC)-baserte biomarkører i svulstene (art. I). Pasientenes blod ble analysert med massecytometri (art. II). Humaniserte HGSOC PDX-mus ble behandlet med oleclumab/durvalumab. Musenes svulster og blod ble analysert henholdsvis med IHC og spektral strømningscytometri (art. III). Resultater: Den kliniske studien viste sykdomskontroll hos 27% av pasientene, 2,7 mnd. progresjonsfri overlevelse, og 8,4 mnd. totaloverlevelse. Ingen tumorbaserte biomarkører var assosiert med respons (art. I). Flere leukocytt-relaterte biomarkører med potensial for responsprediksjon og behandlingsovervåking ble identifisert i blod (art. II). I dyrestudiet ble resultatene fra den kliniske studien reprodusert, med alle mus som viste immun-ekskluderte svulster og progresjon (art. III). Konklusjoner: Kombinasjonen oleclumab/durvalumab var trygg, men viste begrenset effekt (art. I). De identifiserte leukocyttbaserte biomarkørene kan bidra til persontilpasning av immunterapi for HGSOC-pasienter etter validering av funnene (art. II). PDX-modellen danner en robust plattform for preklinisk testing av immunterapi i HGSOC (art. III).Background: Despite its designation as an immunogenic cancer and promising preclinical results, high-grade serous ovarian cancer (HGSOC) exhibits modest responses to both single-agent and combination immune checkpoint inhibitor (ICI) immunotherapy. Biomarker-based personalization and monitoring, as well as advances in preclinical modeling of HGSOC immunotherapy hold promise for improving ICI response. Aims: This project aimed to clinically evaluate the efficacy of a novel oleclumab/durvalumab ICI combination therapy in HGSOC (Paper I), identify blood-based biomarkers of response and resistance to said treatment (Paper II), and develop a cutting-edge humanized patient-derived xenograft (PDX) mouse model of HGSOC for improved preclinical result translation (Paper III). Materials & Methods: A clinical trial of combined oleclumab/durvalumab was conducted on 25 patients with relapsed epithelial ovarian cancer, along with immunohistochemical (IHC) biomarker exploration (Paper I). The patients’ blood samples were analyzed by mass cytometry (Paper II). Humanized HGSOC PDX mice were given oleclumab/durvalumab. Mouse tumors and blood were analyzed using IHC and spectral flow cytometry (Paper III). Results: Trial results showed a 27% disease control rate, 2.7-month progression-free survival, 8.4-month overall survival, and no response-associated IHC biomarkers (Paper I). Peripheral blood leukocyte analysis identified a diverse set of potential predictive, monitoring and response biomarkers (Paper II). The animal study replicated clinical trial results, with all mice exhibiting immune-excluded tumors and progression (Paper III). Conclusions: The oleclumab/durvalumab treatment was safe, but showed limited activity in HGSOC (Paper I). The proposed blood-based biomarkers may contribute to HGSOC immunotherapy personalization, though validation is needed (Paper II).     School achievement among youth receiving child welfare services: What role do social support and family cohesion play in explaining good school outcomes?

    Bakgrunn: Skolen er en viktig arena for barn og unge i kontakt med barnevernet. Gode skoleutfall kan fungere som en beskyttende faktor for senere marginalisering. Det tenkes derfor som viktig å undersøke hvilke faktorer som kan fremme gode skoleutfall blant barnevernsbarn. Hensikt: Studiens formål var å undersøke skoleutfall blant barnevernsbarn, og hvorvidt generell populasjon og barnevernsgruppene hjelpetiltak og omsorgstiltak varierte i karakterer og fravær. Studien utforsket også om familiesamhold og sosial støtte, kunne bidra til å forklare gode skoleutfall blant barnevernsbarn. Metode: Datasettet bestod av 9059 ungdommer mellom 16 og 19 år som deltok i den kryss- seksjonelle studien ung@hordaland, som inngår i den longitudinelle studien Barn i Bergen. Svarprosenten var 53%. Kontakt med barnevernet ble definert som uavhengig variabel, og skoleutfall, operasjonalisert gjennom gjennomsnittskarakterer og fravær (registerdata), som avhengig variabel. Det ble videre undersøkt hvorvidt sosial støtte og familiesamhold modererte sammenhengen mellom barnevernskontakt og skoleutfall. Relevans: Funnene har relevans for barnevernstjenesten, skolen og andre aktører i velferdssystemet, som kan sette inn tiltak i miljøet rundt barnet for å fremme skoleutfall. Resultatene bidrar særlig til forskningen på gruppen hjelpetiltak. It is therefore important to investigate which factors can promote school achievement. We aimed to investigate school outcomes among children in the CWS, and whether there was a difference between children receiving in-home-services (IHS) and out-of-home services (OHS). The study also explored whether family cohesion and social support could contribute to explaining good school outcomes among children in the CWS. Methods: The data comprised 9059 individuals (16-19 years) who participated in the cross- sectional study ung@hordaland. Among these, 120 (1.3%) received IHS, while 163 (1.8%) received OHS. We examined the association between IHS/OHS groups and school achievement (grades and absence) with the general population (GP) as a reference group. We further explored possible interaction effect of Family Cohesion and Social Support on the relationship between IHS-status and school achievement. Findings: Both the IHS- and the OHS groups had significantly lower school grades compared to the GP. The results showed significantly higher school absence, measured in both hours and days, for the IHS group compared to the GP, but not for the OHS group. Social Resources and Family Cohesion operated as protective factors for the GP. The results were more inconclusive to what extent these factors could be protective for the CWS groups. Implications: The findings can be important for the CWS and other actors in the welfare system. The results contribute to the research on the IHS group.     Diversity and dynamics of microbial communities in Arctic permafrost- and Pingo systems with special focus on methane oxidizing bacteria

    Mikroorganismer er drivkraften bak alle økosystemer på jordkloden, og spiller helt sentrale roller i økosystemers funksjon. Grunnleggende innsikt i mikrobielle prosesser og interaksjoner er viktig for å forstå samspillet mellom det dynamisk kompleks av mikroorganismer som i samspill med de abiotiske miljø utgjør en funksjonell enhet. Dette gjelder også i de polare områdene. Klimaforandringer og økte atmosfæriske drivhusgasser, inkludert metan, har påvirket jordklodens økosystem helt siden det 18’de århundre. De mest dramatiske påvirkningene av klimaendringene skjer i de arktiske områdene siden de er svært følsomme for temperaturøkninger. Disse temperaturendringene fører til tining av permafrost, tilbaketrekning av isbreer og minkende havis. I områder med permafrost kan grunnvannet trenge sprekker i permafrosten via åpne pingo systemer i det høy arktiske landskapet, og dette er en del av de globale naturgassutslippene. Mikroorganismer er finnes naturlige i disse habitatene og kunnskap om deres tilstedeværelse og aktivitet er nødvendig for å bedre forstå effekten av klimaendringer. I permafrost er det lagret store mengder organisk karbon som mikroorganismer kan bryte ned, resultatet av disse prosessene er økt utslipp av karbondioksid (CO2) under aerobe forhold. Dette gjør at jordens mikrobiom spiller en avgjørende rolle i den biokjemiske prosessen, og spesielt i karbon syklusen i Arktis. Denne doktoravhandlingen har som mål å kartlegge, oppdage og tilføre ny kunnskap om mikroorganismene til stede i permafrostsystemer, med utgangspunkt i prøver fra Adventdalen, Svalbard. De mikrobielle samfunnene har blitt analysert ved bruk av høykapasitetssekvensering ved merking av 16S rRNA-genene fos å se på sammensetningen av det mikrobielle samfunnet i tillegg til metagenomanalyser for å få informasjon om hvilke prosesser og funksjoner som finner sted i disse økosystemene. Jeg har videre hatt et spesifikt fokusert på funksjonell gruppe av bakterier som kan omdanne metan til CO2. Det ble også oppdaget og gjennomført vellykket isolering av nye psykrofile metanoksiderende bakterier (MOB) i sedimenter fra Lagoon pingo i Adventdalen. Dette er et verdifullt bidrag til økt kunnskap om de ulike bakteriene i slike kuldetilpassete habitater. resultatene vil ha betydning for videre studier og kunnskap om mikrobiell økologi og klimaendringer. For å studere sammensetningen av det mikrobielle samfunnet i permafrostjord og pingo-sedimenter blir det i avhandlings artikkler (I, II og III) brukt høykapasitets sekvenseringsmetoder rettet mot 16S rRNA-genet. I artikkel I presenteres den mikrobielle samfunnsprofilen og metabolske funksjonen til holocene (ung alder) permafrostkjerner fra Adventdalen, Svalbard. Metagenomikk ble benyttet til å hente ut informasjon om metabolske prosesser i permafrostjorden. Det totale samfunnsgenomet ble studert og ulike gener og mulige metabolismespor (sykluser) ble undersøkt. Permafrost er et statisk og rikt habitat for mikroorganismer. Tining av permafrosten gir tilgang til organiske karbon som har vært bundet i tusenvis av år og frigjør naturlige klimagasser. I denne avhandlingen viste permafrost jord fra holocene alder en dominans av bakterier, og vi fant liten variasjon mellom prøvene. I permafrosten dominerte Actinobacteriota (40-50 %) og Chloroflexi (25 %), etterfulgt av Pseudomonadota (10 %). De metabolske funksjonene ble studert ved bruk av metagenomanalyse, og metabolismer som viser omsetting av karbon, nitrogen, svovel ble påvist sammen med stressrespons gener og gen for nedbrytning av aromatiske forbindelser ble identifisert. I artikkel II beskrives den mikrobielle sammensetningen av sedimenter fra Lagoon Pingo i Adventdalen som ble samlet inn om våren. Disse resultatene blir videre koblet til vanngeokjemi, sedimentært totalt karbon- og nitrogeninnhold. Det ble tatt ut prøver til inkubasjonsforsøk, men det ble ikke påvist utslipp av metangass til tross for at vannet var overmettet med biogent metan. Strukturen i de mikrobielle samfunnene samsvarte med det som tidligere er rapportert fra kjemolitotrofe, oligotrofe og kjemoheterotrofe miljøer som hydrotermiske skorsteiner i havdypet, næringsfattige innsjøer, ferskvannssjøer og jordsmonn. Pingo-habitatet fremhever færre ekstreme forhold enn permafrostsedimentene i artikkel II, fordi det kontinuerlige innsiget av grunnvann gjennom pingo danner et islokk som beskytter sedimentene mot lave temperaturer og gjør at de aldri fryser. Sedimentene i Lagunen Pingo har en kontinuerlig tilførsel av metan og svovel. Til tross for dette ble det likevel ikke påvist noen metanproduserende arker i disse mikrobielle samfunnene. Noe som indikerer at metankilden må være knyttet til grunnvannets strømningsvei under pingo strukturen. Videre var de forventede metabolske aktivitetene basert på slektssammensetning i sedimentene; metanoksidasjon av Methylobacter sp., svovelreduksjon av Desulfocapsa sp. og svoveloksidasjon av Thiobacilllus sp. Pingoer er et relativt nytt habitat for studier av mikrobiell samfunns-sammensetning, og kunnskapen om de mikrobielle populasjonene koblet til disse systemene er minimal. Vi har fortsatt svært begrenset informasjon om metanoksiderende bakterier som lever i kalde miljø. Vi har satt opp mange anrikningskulturer med inokulum fra pingo sedimenter hvor vi har benyttet CH4 som eneste karbon kilde. Vi har observert stor aktivitet i disse primære anrikningene, men det har vært vanskelig å få rene kulturer fra disse anrikningene som er viktig for å kunne studere isolatenes fysiologi. Vi overvant disse utfordringene i artikkel III, og lyktes med å isolere og dyrke en psykrofil metanoksiderende bakterie fra Lagoon Pingo. Oppdagelsen av denne ny Methylobakterier svalbardensis sp.nov-stammen LS7T4AT, som var anriket fra Lagoon Pingo, representerer et viktig tillegg av nye isolater og som kan øke vår forståelse av bakterier i kalde miljøer. Undersøkelser av denne nye stammen ved hjelp av metagenomikk har gitt innsikt i dens funksjonelle gener og metabolisme. Til sammen har disse funnene utviklet vår forståelse av strukturen til mikrobielle samfunn og deres antatte metabolske roller i permafrost og periglaciale strukturer, kjent som Pingos. Denne avhandlingen utforsker også MAGS fra anrikningsbasert metagenomikk av andre pingoer, og avslører tilstedeværelsen av Methylomonadaceae, inkludert Methylobakterie- og Burkholderia-familiene, som er kjent for metanoksidasjonsgener. Methylobakterier finnes utbredt i arktiske permafrost og sedimenter og kan derfor foreslås som en modellbakterie for å studere metanoksidasjon og utslippsrater i Arktis i et klima i endring.Microorganisms regulate biological processes in various environments, including the Polar regions. Climate change and increased greenhouse gases have impacted Earth’s ecosystem since the 18th century, with the Arctic being particularly sensitive to temperature change. Significant impacts on geological features like thawing permafrost, retreat of glaciers, and loss of sea ice have been frequently reported. Terrestrial cold seeps release methane gas through permafrost via open system pingos in the High Arctic environment and are part of the global natural gas emissions. Microorganisms are natural inhabitants in these niches, and knowledge about their activity and interactions is needed to understand global climate change. Microbially-driven degradation of organic matter in permafrost releases methane (CH4) and carbon dioxide (CO2) under aerobic conditions. Therefore, the soil microbiome is crucial in driving the global biogeochemical processes, especially the carbon cycle in the Arctic. This PhD thesis has aimed to shed light on some of the black boxes and add new information about the microbiome in permafrost systems in Adventdalen, Svalbard. The microbial communities have been analyzed using metabarcoding of the 16S rRNA gene for community analyses and metagenomics for a deeper functional analysis. I have specifically targeted the methane-oxidizing bacteria that can convert methane to CO2. Then, novel psychrophilic methanotrophic bacteria from the pingo methane seep sediments were successfully enriched and isolated, thereby broadening the understanding of bacteria from extreme habitats. These findings significantly impact our understanding of microbial ecology and climate change. To study the microbial community composition of permafrost soil and pingo sediments, high throughput sequencing methods targeting the 16S rRNA gene were used in Paper I, Paper II, and Paper III to illuminate the unknown microbial community composition. In Paper I, insights into the microbial profile and metabolic functions of Holocene (young age) permafrost cores from Adventdalen Valley, Svalbard, are presented. Permafrost is a static and rich habitat for microorganisms; its thawing will give access to the organic carbon trapped for thousands of years and release natural GHGs. In this thesis, Holocene age permafrost sediments from Adventdalen Valley showed the dominance of Bacteria, and no significant changes with depth were seen. The dominating phyla were Actinobacteriota (40-50%) and Chloroflexi (25%) in permafrost, followed by other Pseudomonadota (10%). The putative metabolic functions were studied using metagenomic analysis, and critical pathways like carbon-nitrogen and sulfur, stress response and degradation of aromatic compounds, and molecular repair mechanisms were identified. Paper II shows the microbial composition of sediments from Lagoon Pingo collected during spring, which links it with water geochemistry, sedimentary total carbon, and nitrogen contents. Samples were taken for incubation experiments, but no methane gas was detected despite the water being super-saturated with biogenic methane. The structure of the microbial communities corresponded with those reported from chemolithotrophic, oligotrophic, and chemoheterotrophic environments like deep-sea hydrothermal vents, nutrient-deficit lakes, and freshwater lakes and soils. The pingo habitat supports fewer extreme conditions than the permafrost sediments in Paper II because the continuous seepage of groundwater through the pingo forms an ice lid that protects the sediments from low temperatures so that the sediments never get frozen. The Lagoon Pingo sediments have a continuous supply of methane and sulfur. Still, no methanogens were found in the microbial communities, indicating that the source of methane must be associated with the groundwater flow path beneath the pingo. Furthermore, predicted metabolic activities based on the genus composition found in the sediments were methane oxidation ability by Methylobacter sp., sulfur reduction by Desulfocapsa sp., and sulfur oxidation by Thiobacilllus sp. Pingos are a new study area for microbial community composition, so knowledge about their indigenous microbial populations is minimal. This study, therefore, opens new routes for examining these microbial-rich habitats with rich geochemistry. We still have very limited information about methanotrophs living in cold environments. It has been difficult to get into pure cultures, and studying their physiology has been a significant barrier in the research field. We overcame these challenges in Paper III and successfully targeted and cultivated pure psychrophilic methanotrophs from Lagoon Pingo. The discovery of a novel strain of the Methylobacter svalbardensis sp.nov strain LS7T4AT, enriched from the methane seep at the Lagoon Pingo, marks an important addition of novel isolates and in our understanding of bacteria in extreme environments. Investigating this new strain using metagenomics of the pure culture has provided insights into its functional genes and pathways Together, these findings have evolved our understanding of the structure of microbial communities and their putative metabolic roles in permafrost and periglacial structures, known as Pingos. This research has perfected experimental methods for targeting methanotrophs from low temperatures, and the novel psychrophilic methane oxidiser will significantly expand our understanding of bacteria present in extreme environments, especially in climate-sensitive habitats. This thesis also explores MAGS from enrichment-based metagenomics of other pingos, revealing the presence of Methylomonadaceae, including the Methylobacter and Burkholderia families, known for methane oxidation genes. Methylobacter is constantly found in Arctic sediments and can be proposed as a model bacterium for studying methane oxidation and emission rates in a changing climate.Doktorgradsavhandlin

    Identification and Participation in the United States: A Panel Matching Analysis of Voter ID Laws’ Effect on State-level Turnout

    The United States has a long and controversial history with voter suppression. Has voter suppression returned in the form of voter ID laws? My research question is "to what degree do voter ID laws affect turnout in the United States". This topic is crucial as it addresses the broader implications of voter ID laws on democratic participation and equity in voter access. I look at state-level data of voter turnout in the United States from 2000 to 2022, examining presidential and midterm elections. The theoretical framework I use is rational choice institutionalism, which posits that institutions help shape the behavior of actors - which in this case is voters. To answer my research question and test my hypotheses, I use panel matching and two-way fixed effects regression. The analysis reveals that implementation of any type of voter ID law depresses turnout by somewhere between -2.7 to -4 percent, with no significant difference in depression attributable to the strictness of these laws. I find that the depressing effects of voter ID laws is disproportionately affecting Black people – but only by -0.10 percent more than other racial groups. My findings also include mixed evidence regarding the long-term depressive effects of voter ID laws on turnout. These results suggest that states should reevaluate implementing voter ID laws, or simultaneously introduce measures to mitigate the depressive effects these laws have on voter participation.MasteroppgaveSAMPOL350MASV-SAP

    Timing of progesterone treatment to prevent preterm birth in pregnancies with a short cervix: A population-based historical cohort study

    Introduction: Randomized trials have shown that progesterone treatment in mothers with a short cervix may reduce the risk of preterm birth, but the optimal time window for treatment remains unknown. We aimed to investigate progesterone treatment for the prevention of preterm birth by gestational age at diagnosis and initiation of treatment. Material and Methods: This was a population-based historical cohort study of 1162 mothers with singleton pregnancies diagnosed with a cervix <20 mm from 16 to 31 gestational weeks receiving progesterone treatment (n = 390) or no preventive treatment (n = 772). Data were collected from the Medical Birth Registry of Norway from 2014 to 2020 and linked to national health registries providing demographic, diagnostic, and prescription information. Risks of preterm birth <28, <34, and <37 gestational weeks were compared between mothers with and without progesterone treatment in the full study sample and in three periods of gestational age at diagnosis (16–21, 22–27, and 28–31 weeks) using log-binomial regression analyses. Results: The absolute risk of preterm birth <28 gestational weeks was 0.8% in mothers treated with progesterone and 3.4% in mothers who did not receive treatment (adjusted relative risk (aRR) 0.25, 95% confidence interval (CI) 0.08–0.81). The strongest protective association was observed in mothers diagnosed from 16 to 21 weeks (aRR 0.13, 95% CI 0.02–0.98). Preterm birth <34 weeks occurred in 8.7% of mothers in the progesterone group and 11.1% in the untreated group (aRR 0.80, 95% CI 0.54–1.17), and the relative risk reduction associated with treatment diminished with increasing gestational age at diagnosis: aRR 0.27 (95% CI 0.08–0.96) from 16 to 21 weeks; aRR 0.68 (95% CI 0.38–1.23) from 22 to 27 weeks; and aRR 1.30 (95% CI 0.71–2.39) from 28 to 31 weeks. There was no difference in the risk of birth <37 weeks in mothers treated with progesterone (23.1%) and untreated mothers (22.3%), and the risk estimates were similar in the three periods of gestational age at diagnosis. Conclusions: Compared to no treatment, progesterone treatment is associated with a reduced risk of preterm birth <28 gestational weeks in pregnancies with a short cervix. The preventive effect of treatment may extend to 34 weeks if treatment is initiated early in the second trimester.publishedVersio
