    Diagnostic equipment and point-of-care tests in Norwegian emergency primary healthcare clinics: a cross-sectional study

    Introduction In recent years, the increasing availability of instruments and laboratory tests has significantly influenced the diagnostic work in primary healthcare. This study aimed to map the availability of diagnostic equipment and point-of-care tests (POCTs) in Norwegian emergency primary healthcare clinics and examine the demographic and organisational factors influencing their availability. Methods This cross-sectional study utilised the National Out-of-Hours Services Registry (NOOHR) survey across all Norwegian emergency primary healthcare clinics in early 2024. Clinic managers provided data on diagnostic equipment and POCTs. Clinics were categorised by population size, hospital proximity, co-location with general practitioner (GP) offices, and total equipment available. Results All 167 Norwegian emergency primary healthcare clinics responded. The median number of diagnostic equipment and POCTs was 17 (Interquartile range (IQR) 16–20). Clinics with the most equipment were often smaller, co-located with a GP office, and over 40 km from a hospital. Almost all clinics had a repertoire of CRP, urinary dipstick, haemoglobin, glucose, strep A test, SARS-CoV-2 rapid test, urinary HCG test, pulse oximeter, and ECG. Clinics far from hospitals more frequently had tests like D-dimer and troponin. Of the clinics with troponin tests, 69% had a low-sensitive POCT. Conclusion Norwegian emergency primary healthcare clinics are well equipped, especially if co-located with a GP office or far from a hospital. Further research is needed to enlighten how availability influences use and the possible impact on patient trajectories.     Fracture liaison service (FLS) is associated with lower subsequent fragility fracture risk and mortality: NoFRACT (the Norwegian capture the fracture initiative)

    Summary Subsequent fracture rates and associated mortality were compared before and after the introduction of fracture liaison service (FLS). In 100,198 women and men, FLS was associated with 13% and 10% lower risk of subsequent fragility fractures and 18% and 15% lower mortality. The study suggests that FLS may prevent fractures. Purpose Efficient fracture prevention strategies are warranted to control the global fracture burden. We investigated the effect of a standardized fracture liaison service (FLS) intervention on subsequent fracture risk and mortality. Methods The NoFRACT study was designed as a multicenter, pragmatic, register-supported, stepped-wedge cluster-randomized trial. The FLS intervention was introduced in three clusters with 4-month intervals starting May 2015 through December 2018 and included evaluation of osteoporosis and treatment in patients over 50 years with a low-energy fracture. Based on data from the Norwegian Patient Registry, patients with index fractures were assigned to the control period (2011–2015) or intervention period (2015–2018) depending on the time of fracture. Rates of subsequent fragility fractures (distal forearm, proximal humerus, or hip) and all-cause mortality were calculated. Results A total of 100,198 patients (mean age 69.6 years) suffered an index fracture of any type. During a maximum follow-up of 4.7 years, 11% (6948) of the women and 6% (2014) of the men experienced a subsequent fragility fracture, and 20% (14,324) of the women and 22% (8,326) of the men died. FLS was associated with 13% lower subsequent fragility fracture risk in women (hazard ratio (HR) 0.87, 95% confidence intervals (CI) 0.83–0.92) and 10% in men (HR 0.90, 95% CI 0.81–0.99) and 18% lower mortality in women (HR 0.82, 95% CI 0.79–0.86) and 15% in men (HR 0.85, 95% CI 0.81–0.89). Conclusion A standardized FLS intervention was associated with a lower risk of subsequent fragility fractures and mortality and may contribute to reduce the global fracture burden.publishedVersio

    Impact of Extrapulmonary Tuberculosis on the Health and Financial status of patients in high-burden, resource-constrained settings

    Tuberkulose (TB) er en av de store globale helseutfordringene og bidrar betydelig til sykelighet og dødelighet. Blant TB-tilfellene utgjør ekstrapulmonal tuberkulose (EPTB) en betydelig andel. Til tross for den høye sykdomsbyrden, vies EPTB mindre oppmerksomhet på grunn av dens relative ikke-smittsomhet og lavere dødelighetsrate sammenlignet med pulmonal tuberkulose (PTB). Rettidig diagnose og behandling av EPTB er utfordrende, hovedsakelig på grunn av lav sensitivitet i dagens diagnostiske metoder, noe som understreker behovet for bedre diagnostiske verktøy. Riktig helsehjelp kan forbedre pasientenes helse og forhindre høye økonomiske kostnader knyttet til sykdom. Med dette som utgangspunkt gjennomførte vi en prospektiv kohortstudie for å undersøke helsesøkingsveiene til presumptive EPTB-pasienter, diagnostisk forsinkelse og kostnadseffektiviteten av en nylig utviklet MPT64-antigen deteksjonstest for diagnostisering av EPTB. Vi analyserte også sykdommens innvirkning på helse-relatert livskvalitet og dens endring etter behandling. Til slutt undersøkte vi den økonomiske belastningen sykdommen påfører, fra symptomstart til behandlingens slutt, samt ulikheter i kostnader som pasientene pådrar seg for å få tilgang til helsehjelp. Våre resultater fra studie 1 viste at EPTB-pasienter opplever diagnostisk forsinkelse som varierer betydelig avhengig av ulike sykdomsmanifestasjoner: lymfadenitt, pleuritt, meningitt og andre (ascites og osteomyelitt). Forsinkelsene var til stede både på pasient- og helsesystemnivå. Faktorene som bidrar til disse forsinkelsene var forankret på individ-, samfunns- og helsesystemnivå. Til tross for flere besøk ved ulike helsefasiliteter, ble pasientene verken diagnostisert eller henvist til passende behandling. I motsetning til målene i Verdens helseorganisasjons End-TB-strategi var tilgang til helsetjenester ikke likt fordelt. Pasienter fra høyere sosioøkonomisk status (SES) var bedre beskyttet mot langvarige diagnostiske forsinkelser. Videre var forsinkelsene også avhengig av EPTB-manifestasjoner. I studie 2 var et hovedfunn at den nye MPT-64-antigentesten var mer kostnadseffektiv enn rutinemessig mikroskopi og Xpert-analyse. Ved å øke Xperts diagnostiske sensitivitet (79%) i scenarioanalyser, ble resultatene snudd til å vise den som mer kostnadseffektiv. Likevel er en slik høyere sensitivitet av Xpert primært knyttet til diagnosen av lymfadenitt. Dermed kan pålitelig diagnostisk behandling ikke oppnås med mindre flere diagnostiske tester tilpasses pasientenes behov. Våre resultater fra studie 3 viste at EPTB-sykdom påvirker helse-relatert livskvalitet i alle målte helsedimensjoner: mobilitet, egenomsorg, vanlige aktiviteter, smerte, ubehag, angst og depresjon. Sykdommen påvirket dermed både fysisk og psykisk helse. Videre analyse viste at disse helsedimensjonene ble ulikt påvirket av EPTBs manifestasjoner, med sterkere innvirkning fra meningitt og pleuritt enn fra lymfadenitt. Til tross for fullført behandling rapporterte en betydelig andel meningittpasienter (36%) fortsatt psykologiske helseproblemer. Faktorer som helsefasilitetene pasientene oppsøkte, påvirkning på arbeidsevne og SES var assosiert med redusert helse blant EPTB-pasienter. På grunn av sykdommen lider EPTB-pasienter ikke bare klinisk, men også økonomisk (studie 4). De totale sykdomskostnadene, som omfatter egenbetaling (OOP) for direkte og indirekte kostnader, varierte i henhold til pasientenes SES og sykdomsmanifestasjoner. Direkte kostnader utgjorde en høyere andel av inntekten blant pasienter med mer alvorlige sykdomsmanifestasjoner (meningitt, pleuritt, ascites og osteomyelitt) fra høyere sosioøkonomiske posisjoner (SEP). Hovedutgiftene rapportert som OOP var knyttet til diagnostikk, ikke-TB medisiner og konsultasjoner. For pasienter med mindre alvorlige sykdommer (lymfadenitt) fra lavere SEP, ble en stor del av inntekten brukt på indirekte kostnader, hovedsakelig knyttet til tap av arbeidsevne. Disse høyere kostnadene førte til katastrofale helseutgifter (CHE) og skjøv pasientene under fattigdomsgrensen. Som en del av studieprotokollen, tilbød vi en mer sensitiv MPT-64-test gratis til presumptive EPTB-pasienter i tillegg til rutinemessig behandling. Uten MPT-64-testen ville mange pasienter ikke blitt diagnostisert på grunn av rutinetestenes lave sensitivitet, noe som ville forlenget deres prediagnostiske fase og påført ytterligere indirekte kostnader. En passende og rettidig diagnose fremstår derfor som en avgjørende intervensjon for å redusere katastrofale helseutgifter ved å senke indirekte kostnader (mindre langvarig sykdom blant lymfadenittpasienter) eller direkte kostnader (tidlig diagnose før sykdommen forverres). Våre funn viser at EPTB-pasientenes belastninger var knyttet til sykdomsmanifestasjoner og SES. Sammenlignet med lymfadenitt pådro pasienter med mer alvorlige EPTB-manifestasjoner høyere totale kostnader (studie 4). Pasienter fra høyere SES hadde både mulighet til å betale mer og beskyttelse mot langvarige diagnostiske forsinkelser (studie 1) og lavere livskvalitet (blant pleurittpasienter i studie 3). Inntil ensartede finansielle beskyttelsesmekanismer og universell helsedekning er tilgjengelig for pasientene, kan ikke utfordringer med forsinket diagnose og behandling fullt ut løses.Tuberculosis (TB) is one of the major global health issues which significantly contributes to morbidity and mortality. Among TB cases, a significant proportion is attributable to extrapulmonary tuberculosis (EPTB). Despite such a high disease burden, there is less attention on EPTB amid its relative non-transmissibility and lower mortality rates than pulmonary tuberculosis (PTB). The timely diagnosis and treatment of EPTB remain a challenge mainly due to the low sensitivity of the current diagnostic methods, highlighting the need for better diagnostic tools. The appropriate healthcare can have an impact by improving patients' health status and preventing high financial costs due to illness. With this information in mind, we conducted a prospective cohort study to study the healthcare-seeking pathways of the presumptive EPTB patients, diagnostic delay and the cost-effectiveness of a newly developed MPT64-antigen detection test to diagnose EPTB patients. We also analysed the impact of illness on the health-related quality of life (HRQoL) and its change following the TB treatment. Finally, we studied the financial burden of illness from the onset to the completion of treatment and inequality in the costs incurred by patients to access healthcare. Our results in study 1 indicated that EPTB patients face a diagnostic delay that significantly varies based on various disease manifestations: lymphadenitis, pleuritis, meningitis and others (ascites and osteomyelitis). The diagnostic delay was both at patient and health system levels. The factors associated with these delays were embedded at the individual, society, and health system levels. Despite attending several health facilities (HFs) multiple times, patients were neither diagnosed nor referred to the appropriate healthcare. Unlike goals set in the World Health Organization’s end-TB strategy, healthcare access was not equally available to all patients. Patients from higher socioeconomic status (SES) were protected from prolonged diagnostic delays. Moreover, diagnostic delays also varied by the EPTB manifestations. In study 2, one main finding was that the new MPT-64 antigen detection test was more cost-effective than the routine microscopy and the Xpert assay. By increasing Xpert’s diagnostic sensitivity (79%) during scenario analyses, the cost-effectiveness results switched to presenting it as more cost-effective. However, such a higher sensitivity of the Xpert is only associated with the diagnosis of lymphadenitis. Thus, reliable diagnostic care may not be possible unless multiple diagnostic tests according to patients’ needs are implemented. Our results in study 3 showed that EPTB illness impacts the HRQoL of EPTB patients across all measured dimensions of health: mobility, self-care, usual activities, pain, discomfort, anxiety & depression. Thus, the disease affected not only physical health but also psychological health. Further analysis showed that these dimensions of health were also variedly affected by the manifestation of EPTB disease: a stronger impact by meningitis and pleuritis than lymphadenitis. Despite completion of treatment, a large proportion of meningitis patients (36%) reported psychological problems in their health. Factors like the HF attended by patients, impact on working capacity and the SES were associated with the decrease in health status of EPTB patients. Due to illness, EPTB patients suffer not only clinically but also financially (Study 4). The total cost of illness, comprising out-of-pocket (OOP) direct and indirect costs, varied according to patients’ SES and the disease manifestations. Direct costs constituted a higher proportion of total income among patients with more severe disease manifestations (meningitis, pleuritis, ascites and osteomyelitis) from higher socioeconomic positions (SEP). The main reported OOP expenditures were higher diagnostics, non-TB medication, and consultation costs. On the other hand, the income of less severe (lymphadenitis) patients among the low SEPs was largely consumed by higher indirect costs due to a greater impact of their lost working capacity. Not only did the higher costs turn out to be catastrophic health expenditures (CHE) for EPTB patients, but they also pushed patients below the poverty line. As part of our study protocol, in addition to routine care, we provided a more sensitive MPT-64 test for EPTB diagnostic test free of cost to presumptive EPTB patients. Otherwise, many patients could not be diagnosed due to the relatively low sensitivity of routine diagnostic tests. Eventually, their pre-diagnostic phase could continue, leading to additional indirect costs. Thus, an appropriate and timely diagnosis appears to be a game-changer intervention in alleviating the CHE by reducing indirect costs (reducing prolonged illness among less severe lymphadenitis) or direct costs (early diagnosis before illness aggravates in severity). Our findings show that EPTB patients’ bearings were linked to their disease manifestations and SES. Compared to lymphadenitis, patients with more severe EPTB manifestations incurred higher total costs (Study 4). Patients from the higher SES who could spend more were also protected from a prolonged patient-level diagnostic delay (Study 1) and lower quality of life (among pleuritis patients in Study 3). Unless uniform financial protection mechanisms and universal health coverage are available to patients, delayed diagnosis and treatment challenges cannot be completely resolved.Doktorgradsavhandlin

    Effects of Physiotherapy Interventions for adult patients with Parkinson’s Disease – A Systematic Review

    Background: Parkinson's Disease (PD) stands out as one of the most widespread neurological disorders resulting in physical disabilities and movement impairments. A multidisciplinary approach involving physical therapies alongside pharmacological and neurological treatments is recommended. We aimed to evaluate the effectiveness of physiotherapy interventions on balance, functional mobility, quality of life, and activities of daily living compared to medication in patients with Parkinson's disease (PD). Additionally, we sought to assess the differences in outcomes between physiotherapy interventions implemented in high-income and upper-middle-income countries versus those in low-income and lower-middle-income countries (LICs and LMICs). Methods: Electronic searches, supplemented with bibliographic searches, were done in MEDLINE (Ovid), Physiotherapy Evidence Database (PEDro), and the Cochrane Library. Two independent reviewers (FM and GM) conducted the search, focusing on published randomized controlled trials (RCTs). A narrative synthesis was performed to summarize the findings. Results: A total of 6 studies were selected from 583 articles. Four trials showed that there was a significant effect in improving quality of life after physiotherapy intervention. Four studies assessing activities of daily living showed significant improvement after physiotherapy intervention as compared to baseline. Positive outcomes were observed for balance and functional mobility following treadmill and conventional gait training. The lack of relevant data in the context of LICs and LMICs made it challenging to draw any conclusions about the effectiveness of interventions in these settings. Conclusion: This systematic review revealed significant improvements in balance, functional mobility, quality of life, and activities of daily living among patients with Parkinson's Disease who underwent physiotherapy interventions in compared with medication.     Horisontale fusjoner og vertikal produktdifferensiering

    Konkurranse er en avgjørende faktor for å sikre effektiv ressursbruk og velfungerende markeder. Konkurransemyndighetene har hovedansvaret for å sikre god og effektiv konkurranse. En viktig del av deres arbeid er fusjonskontroll. Konkurransemyndighetene har som oppgave å kontrollere foretakssammenslutninger for å forhindre fusjoner som i betydelig grad hindrer effektiv konkurranse. En viktig del av fusjonsanalysen er produktdifferensiering. Det teoretiske rammeverket for produktdifferensiering tar i stor grad utgangspunkt i horisontal differensiering. Følgelig vet vi mindre om fusjoner med vertikal differensiering. Formålet med denne oppgaven er å undersøke hvordan vertikal differensiering påvirker konkurranseanalyser av horisontale fusjoner. Jeg ønsker å sammenligne effektene jeg finner med teorien for horisontal differensiering. I min analyse benytter jeg en modell med priskonkurranse i et oligopolmarked med tre vertikalt differensierte bedrifter. Modellen tar utgangspunkt i rammeverket til Gabszewicz et al. (2019). Jeg finner at en fusjon mellom to bedrifter som ikke er direkte naboer, ikke påvirker bedriftenes priser. Ved en fusjon mellom to bedrifter som er direkte naboer øker prisene. Videre finner jeg at prisøkning er størst desto mindre differensiering det er mellom de fusjonerte bedriftene, i likhet med hva vi vet om fusjoner med horisontal differensiering. Jeg finner også at den vertikale rangeringen av produktene til de fusjonerte bedriftene i større grad påvirker endringen i bedriftenes priser, sammenlignet med hva grad av differensiering gjør. Når jeg beregner UPP for de tre fusjonene for bedriftene med produkter av typen høy-, middels- og lavkvalitet, finner jeg resultater som i stor grad samsvarer med hva jeg ellers finner i analysen for den vertikalt differensierte modellen. Jeg finner at den av de fusjonerte bedriftene med lavest kvalitet har størst insentiv til å øke deres priser ved en fusjon. Johnson og Rhodes (2021) undersøker også fusjoner i markeder med vertikal differensiering, men hensyntar bedriftenes mulighet til å endre deres produktsortiment etter en fusjon. Artikkelen til Johnson og Rhodes (2021) antyder at resultatene jeg finner trolig vil kunne påvirkes ved tilføyelse av de to effektene produktmikseffekten og kostnadssynergier. I hvilken retning disse effektene vil trekke resultatene er derimot vanskeligere å trekke slutninger om. Funnene jeg gjør i denne oppgaven er interessante, ettersom enkelte av funnene er i kontrast til fusjoner med horisontal differensiering.     Freshwater transport in the East Greenland Current in a regional, high-resolution model

    Freshwater transported into the interior Nordic Seas can increase the stratification of the interior basins, potentially suppressing the formation of dense water masses feeding the lower limb of the Atlantic Meridional Overturning Circulation. The freshwater pathway ontheEast Greenland shelf east of the Fram Strait remains the most significant uncertainty in observational studies, and our understanding of the freshwater transport into the interior Nordic Seas is limited due to few observations. In this thesis, I use output from a regional high-resolution model to construct a closed freshwater budget of the East Greenland Current system and to identify freshwater pathways. I also identify eddies in the vicinity of Jan Mayen to quantify whether eddies contribute significantly to freshwater diversion from the East Greenland Current into the interior Nordic Seas. The closed freshwater budget is constructed with four boundaries, where the northern boundary extends from the coast of Greenland to a distance 200 km east of the East Greenland shelfbreak at 76N, and the eastern boundary has an eastward distance of 200 km from the shelfbreak. Eddies are detected using the Okubo-Weiss parameter and sea surface height contours and tracked using an overlapping method. I estimate that freshwater transports in the northern and southern boundaries are consistent with previous observational studies. I identify two major freshwater pathways along the eastern boundary, the Jan Mayen region and the southern Iceland Sea. These are located in the Jan Mayen region and the southern Iceland Sea, and account for ∼27% and ∼71% of the eastward freshwater transport, respectively. With the identified eddies in the Jan Mayen region, I found that anti-cyclonic eddies contribute to freshwater transport into the interior Nordic Seas and offshore of the East Greenland shelf. The Northern Jan Mayen Ridge captures anti-cyclonic eddies, resulting in the area being a hotspot of eddy activity and freshwater transport toward the interior basins. The freshwater budget indicate that the regional high-resolution model considered in this study reproduces the observed freshwater transports through the Fram Strait in the north and the Kögur transect in the south. My results also show the significant freshwater transport contribution from eddies, which demonstrate the importance of using eddy resolving ( 1km resolution) models to estimate the freshwater budget.Masteroppgave i meteorologi og oseanografiGEOF399MAMN-GEO

    School mathematic's political economy for all

    Jeg viser i oppgaven at den nyliberale politiske økonomien vekselvirker med og opprettholder matematikk for alle som begrunnelse for skolematematikken på ulike måter. Innenfor nyliberal politisk økonomi betraktes utdanning som produksjon av humankapital og viktig for økonomisk vekst, og matematikkresultatene i PISA og TIMMS regnes som viktige indikatorer på landets konkurransekraft. Matematikk for alle begrunner skolematematikkens innslag i skolen samtidig som denne økonomiske rollen ikke reflekteres i slagordet. Jeg har vist at skolematematikken har røtter i kommersielt-administrative behov og dermed er institusjonalisert i den kapitalistiske moderniteten. Fagets historiske, epistemologiske og pedagogiske karakteristika innebærer at skolematematikken speiler og medvirker i naturalisering og reproduksjon av nyliberal rasjonalitet og tankegang. Mens matematikk for alle gir lovnader om inklusjon, bidrar skolematematikken til sortering og eksklusjon ved å tilordne verdier gjennom skolens kredittsystem. Lærere og forskere er en del av skolen som nyliberal institusjon, og slagordene som er knyttet til begrunnelser for faget opprettholdes ved hjelp av ideologiske fantasier.     El trabajo de los profesores de ELE con la ansiedad lingüística en la escuela secundaria noruega

    Fra fenomenet “Foreign Language Anxiety” (eller språkangst på norsk) ble omtalt for første gang i 1985, har det vært omfattende forskning om elever i L2 i skolesammenheng. Også i norske studier viser det seg at man hovedsakelig har forsket på elevperspektivet, men at lite er forsket på lærernes arbeid med språkangst. Derfor bidrar denne studien med ny informasjon om opplevelsene og arbeidet til lærerne i spansk på norske ungdomsskoler med språkangst. Vi forsket på fire lærere fra ulike skoler, om deres opplevelser med fenomenet, strategiene de bruker for å jobbe med det, deres innflytelse som lærer i dette, og til slutt hvorfor de jobber med språkangst på denne måten. Dataene ble samlet inn gjennom semistrukturerte intervjuer, som deretter ble transkribert og delt inn i koder og kategorier. Resultatene viser at, til tross for fenomenets viktighet i spansk som fremmedspråk, hadde tre av lærerne ikke hørt ordet før. Derimot tolker vi deres assosiasjoner om konseptet som noe som handler om frykten for å bruke språket muntlig i spansk som fremmedspråk. På denne måten jobber lærerne implisitt med språkangsten gjennom aktiviteter i små grupper, et fokus på å maksimisere muntlig produksjon og gjennom en variasjon av vurderingssammenhenger. I tillegg anser de et trygt klassemiljø som essensielt for at elevene føler seg mer komfortable og mindre redde, og at motivasjon fremstår som en viktig faktor. På en annen side snakket bare en av lærerne implisitt om elevens autonomi, som har vist seg å være en viktig affektiv faktor i arbeidet med språkangst. Disse resultatene indikerer at det bør eksistere ressurser for lærere, både i offentlige dokumenter og gjennom bedre lærerutdanning, for å jobbe eksplisitt med språkangst. During this process, fault block crests become prone to gravity collapse, particularly where weak sediments or sedimentary rocks are involved. The Mort landslide studied here is a large-scale slide formed by gravity collapse of a major Jurassic North Sea rift fault block at the time when local rift topography approached 1.5–2 km. From seismic data, this 24 km3 large marine slide is seen to stretch ∼15 km along the major Snorre rift fault, making it one of the largest mapped North Sea rift landslides. It is characterized by a single basal detachment (the Mort detachment) on which Triassic-Jurassic sediments moved up to 2–2.5 km. Well-developed detachment corrugations at the 100-m to kilometer scale reveal dip-slip sliding toward 100–110° (ESE), parallel to the slip direction of the underlying Snorre rift fault. The slide is broken up by normal faults that detach onto the low-angle Mort detachment, but in general, stratigraphic layering is well preserved. It shows a complex pattern where folds within the slide are upright with wavelengths up to 2 km, attesting to the soft nature of the slide. A prominent set of folds are ESE-plunging and mimics the corrugated geometry of the underlying Mort detachment. Other folds trend subparallel to the slide and probably formed as a result of differential movements during sliding. The toe zone shows evidence of layer back-rotation and thrusting, resting on the upper Jurassic Heather Formation. Postdated by uppermost Jurassic claystones of the Draupne Formation, the slide must have formed toward the end of the rift maximum around 155 Ma, at a time when the surface had reached its maximum relief of 1.5–2 km. The implications of such a gravitational collapse are exhumation of older (e.g., reservoir) rocks on fault block crests and possibly a change in local fluid migration pathways from local basins to those crests.
